전이성 골종양의 역학적 연구 (An Epidemiologic Study of Metastatic Bone Tumor)

  • 1988년부터 1993년까지 최근 6년간 고신의료원에서 전이성 골종양으로 진단받고 치료받았던 환자중 조직학적으로 확진되었던 417명의 환자를 대상으로 후향적 조사를 통하여 연령별, 성별, 골전이 부위별 분포와 원발성 암에 따른 전이의 양상을 분석하여 다음과 같은 결과를 얻었다. 1. 골전이를 가장 잘 일으키는 원발성 암은 폐암(29.5%), 위암(15.3%), 유방암(11.3%), 자궁경부암(5.3%), 간암(4.8%)등의 순이었으며, 원발 부위 미상의 전이성 골종양은 7.7%였다. 2. 연령 분포는 40대 이후에서 발생한 경우가 85.3%로 대부분을 차지하였으며, 평균 연령은 54.8세였다. 남자의 경우 50대(39.4%), 60대(27.5%), 40대(14.3%)의 순이었으며, 여자의 경우는 50대(31.9%), 40대(21.7%), 60대(20.5%)의 순으로 발생하였다. 3. 골전이 부위별 분포는 척추(25.2%), 골반골(16.0%), 늑골(15.3%), 대퇴골(14.5%), 두개골(8.3%), 상완골(5.5%), 견갑골(5.1%)등의 순이었으며, 골전이의 분포 양상은 체간골이 75.8%로 가장 많았고, 그 다음으로 근위부의 장관골이 19.8%, 슬관절과 주관절 이하의 원위 골격계가 4.4%의 순으로 나타났다. 척추 중에서는 흉추(42.1%), 요추(39.1%), 경추(13.2%), 천추(5.6%)의 순으로 나타났다. 4. 골전이 부위별 분포는 원발성 암의 종류에 관계없이 다발성 전이(73.1%)가 단발성 전이(26.9%) 보다 많았다. 5. 원발성 종양에 따른 분포 양상은 폐암의 경우 연령 분포는 50대, 60대, 40대의 순이었고, 남녀비는 2.3 : 1이었으며, 골전이는 척추, 골반, 대퇴골의 순으로 발생하였다. 위암의 경우 연령 분포는 50대, 60대, 40대의 순이었고, 골전이 부위는 척추, 대퇴골, 골반의 순이었다. 유방암의 경우 연령 분포는 40대, 50대, 30대의 순이었고, 골전이 부위는 척추, 늑골, 골반의 순이었다. 6. 원발 부위 미상의 전이성 골종양의 경우는 60대가 가장 많았으며, 30대부터 70대까지 분포되어 있었다. 골전이 부위는 척추, 대퇴골, 골반골, 늑골, 견갑골의 순이었으며, 조직학적으로는 선암, 편평세포암이 많았다.

Metastatic hepatocellular carcinoma in the maxilla and temporal bone: a rare case report

  • Hepatocellular carcinoma (HCC) is a common, primary malignant liver disease that usually metastasizes to the lungs, followed by the abdominal lymph nodes and brain. However, extrahepatic metastasis to the maxillofacial area is uncommon and predominates in the mandible, so HCCs in the maxilla or temporal bone from a primary hepatic lesion are extremely rare. We present a case of HCC in the maxilla and temporal bone in a 52-year-old male, which was first suspected to be a squamous cell carcinoma after computed tomography but was confirmed as a metastasis related to his primary HCC after fine-needle aspiration biopsy followed by immunohistochemical analysis.

Clinical experience with $^{18}F$-fluorodeoxyglucose positron emission tomography and $^{123}I$-metaiodobenzylguanine scintigraphy in pediatric neuroblastoma: complementary roles in follow-up of patients

  • Purpose: To evaluate the potential utility of $^{123}I$-metaiodobenzylguanine ($^{123}I$-MIBG) scintigraphy and $^{18}F$-fluorodeoxyglucose ($^{18}F$-FDG) positron emission tomography (PET) for the detection of primary and metastatic lesions in pediatric neuroblastoma (NBL) patients, and to determine whether $^{18}F$-FDG PET is as beneficial as $^{123}I$-MIBG imaging. Methods: We selected 8 NBL patients with significant residual mass after operation and who had paired $^{123}I$-MIBG and $^{18}F$-FDG PET images that were obtained during the follow-up. We retrospectively reviewed the clinical charts and the findings of 45 paired scans. Results: Both scans correlated relatively well with the disease status as determined by standard imaging modalities during follow-up; the overall concordance rates were 32/45 (71.1%) for primary tumor sites and 33/45 (73.3%) for bone-bone marrow (BM) metastatic sites. In detecting primary tumor sites, $^{123}I$-MIBG might be superior to $^{18}F$-FDG PET. The sensitivity of $^{123}I$-MIBG and $^{18}F$-FDG PET were 96.7% and 70.9%, respectively, and their specificity were 85.7% and 92.8%, respectively. $^{18}F$-FDG PET failed to detect 9 true NBL lesions in 45 follow-up scans (false negative rate, 29%) with positive $^{123}I$-MIBG. For bone-BM metastatic sites, the sensitivity of $^{123}I$-MIBG and $^{18}F$-FDG PET were 72.7% and 81.8%, respectively, and the specificity were 79.1% and 100%, respectively. $^{123}I$-MIBG scan showed higher false positivity (20.8%) than $^{18}F$-FDG PET (0%). Conclusion: $^{123}I$-MIBG is superior for delineating primary tumor sites, and $^{18}F$-FDG PET could aid in discriminating inconclusive findings on bony metastatic NBL. Both scans can be complementarily used to clearly determine discrepancies or inconclusive findings on primary or bone-BM metastatic NBL during follow-up.

  • Bisphosphonate inhibits the function of osteoclast, so they are widely used for multiple myeloma, Paget's disease, metastatic malignant bone disease, and severe osteoporosis. This drug is very effective for preventing severe complication of osteoporosis, some unpredictable complication occurred such as esophageal malignancy, atypical fracture of femur, and osteonecrosis of the jaw. Bisphosphonate related osteonecrosis of the jaw (BRONJ) is closely related with invasive, open bone surgery like tooth extraction. BRONJ associated with dental implant is rare, however, as the use of bisphosphonate increase, BRONJ cases with dental implant are increasing. In this article, we will describe the considerations during dental implant treatment for patient under bisphosphonate therapy.

Clinicopathologic Features and Prognosis of Osteosarcoma in Turkish Adults

  • Background: Osteosarcomas are the most common solid malignancies of bone. In the last two decades there have been no concrete developments in their systemic treatment. In this trial we aimed to present our osteosarcoma patient clinical and demographic outcomes. Materials and Methods: Patients treated and followed up for osteosarcoma in Ankara Numune Education and Research Hospital from 2002 to 2012 were reviewed retrospectively. Results: A total of 21 patients (15 male, 6 female) were diagnosed with osteosarcoma. The disease was located at extremities in 76% and in 14% was metastatic at the time of diagnosis. Median disease free survival (DFS) was 36 months in non-metastatic patients and median progression free survival (PFS) was 2 months in metastatic patients (p<0.0001). Median overall survival (OS) was 80 months and 4 months, respectively (p=0.012). There were no survival differences in terms of presentation with pathological fracture, tumor size, tumor grade, alkaline phosphatase and lactate dehydrogenase level and type of chemotherapy regimen. Conclusions: Tumor site and stages are the most important prognostic factors for osteosarcoma. Extremity primary tumors have beter survival rates than non-extremity tumors. As a result of the use of effective chemotherapy the long term survival rates have improved from 10-20% to 60-70% in the last decades but we need more active agents, especially for metastatic cases.

Prognostic factors in breast cancer with extracranial oligometastases and the appropriate role of radiation therapy

  • Purpose: To identify prognostic factors for disease progression and survival of patients with extracranial oligometastatic breast cancer (EOMBC), and to investigate the role of radiation therapy (RT) for metastatic lesions. Materials and Methods: We retrospectively reviewed the medical records of 50 patients who had been diagnosed with EOMBC following standard treatment for primary breast cancer initially, and received RT for metastatic lesions, with or without other systemic therapy between January 2004 and December 2008. EOMBC was defined as breast cancer with five or less metastases involving any organs except the brain. All patients had bone metastasis (BM) and seven patients had pulmonary, hepatic, or lymph node metastasis. Median RT dose applied to metastatic lesions was 30 Gy (range, 20 to 60 Gy). Results: The 5-year tumor local control (LC) and 3-year distant progression-free survival (DPFS) rate were 66.1% and 36.8%, respectively. High RT dose (${\geq}50Gy_{10}$) was significantly associated with improved LC. The 5-year overall survival (OS) rate was 49%. Positive hormone receptor status, pathologic nodal stage of primary cancer, solitary BM, and whole-lesion RT (WLRT), defined as RT whose field encompassed entire extent of disease, were associated with better survival. On analysis for subgroup of solitary BM, high RT dose was significantly associated with improved LC and DPFS, shorter metastasis-to-RT interval (${\leq}1month$) with improved DPFS, and WLRT with improved DPFS and OS, respectively. Conclusion: High-dose RT in solitary BM status and WLRT have the potential to improve the progression-free survival and OS of patients with EOMBC.

Treatment and Outcomes of Ewing Sarcoma in Turkish Adults: A Single Centre Experience

  • Background: Ewing sarcoma is a small round cell tumor arising from soft tissue and bone that predominantly affects children and adolescents. The most unfavorable prognostic factor is the presence of distant metastasis at the time of diagnosis. Materials and Methods: The records of 26 Ewing sarcoma patients (14 male, 12 female) were re-evaluated retrospectively. Results: The median age was 26.5 (19-42) years. Eight patients (31%) showed a primary tumor in their extremities, 8 (31%) in the thorax, 4 (15%) at the vertebra, 4 (15%) in the head and neck, and 2 (8%) in the abdomen. Five patients (19%) had distant metastasis at diagnosis. The median progression-free survival was 72 months and 10 months in localized and metastatic disease, respectively (p=0.005). The overall survival rate was 19 months in metastatic disease, and the 5-year overall survival rate was 64% in localized disease (p=0.006). Patients who had localized disease in the extremities and were under age 30 had a favorable prognosis. Conclusions: Although Ewing sarcoma is a tumor affecting children and adolescents, it may be seen in adults, where the prognosis is generally worse. Although it is a highly malignant tumor, it is possible to achieve improved survival with combined modality treatments.

폐에 발생한 선편평상피세포암종;1례 보고 (Adenosquamous Carcinoma of the Lung - One Case Report -)

  • Adenosquamous carcinoma of the lung is a rare disease entity and its clinical characteristics, treatment, and prognosis are poorly described. Stage I disease are amenable to operation as in bronchogenic carcinoma of non-small cell type, but stage III adenosquamous carcinoma shows highly aggressive behavior with rapid progression. We have experienced a case of stage III adenosquamous carcinoma of the lung in a 38-year-old male with a large mass in the right upper lobe with multiple metastasis in the mediastinal lymph nodes. Interestingly, all of the metastatic lymph nodes proved to be adenous component histologically. After right pneumonectomy and extensive lymph node dissection, the patient had received one cycle of chemotherapy. However, clinical signs of distant metastasis to bone and brain were discovered within 5 months postoperatively.

경골에 단독으로 발생한 Rosai-Dorfman병 (Rosai-Dorfman Disease as a Solitary Lesion of the Tibia)

  • Rosai-Dorfman병은 특발성 양성 조직구 증식성 질환으로 결절형과 결절외형으로 분류되는 질환이며 골 침범은 매우 드물다. 증례는 32세 남자로 좌측 하퇴의 통증을 주소로 내원하였으며 림프절병증은 동반하지 않았다. 임상 및 영상학적으로 전이암, Langerhans 세포 조직구증 등이 의심되어 병변에 대한 절개 생검 시행하였고, 조직 검사 결과 Rosai-Dorfman병으로 진단되어 골소파술 시행하였다. 국내에서 Rosai-Dorfaman병이 사지에 단독으로 발생한 증례는 없기에 저자들은 이를 문헌 고찰과 함께 보고하고자 한다.

Bone Metastasis from Gastric Cancer: The Incidence, Clinicopathological Features, and Influence on Survival

  • Purpose: To evaluate the incidence, clinicopathological characteristics, treatment outcomes, prognostic factors, and survival of gastric cancer patients with bone metastases. Materials and Methods: Of 4,617 gastric cancer patients who were treated between 2001 and 2013, 176 patients with bone metastases were analyzed. Results: The incidence of bone metastasis was 3.8%. The most common histopathological subtype was adenocarcinoma (79%) with poor differentiation (60.8%). The median interval from the diagnosis to bone metastasis was 11 months. The median survival time after bone metastasis was 5.4 months. Factors that were associated with longer median survival times included the following: isolated bone metastasis (P=0.004), well-differentiated tumors (P=0.002), palliative chemotherapy (P=0.003), zoledronic acid treatment (P<0.001), no smoking history (P=0.007), and no metastatic gastric cancer at the time of diagnosis (P=0.01). On the other hand, high levels of lactate dehydrogenase (LDH) (hazard ratio [HR]: 1.86; P=0.015), carcinoembryonic antigen (CEA) (HR: 2.04; P=0.002), and carbohydrate antigen (CA) 19-9 (HR: 2.94; P<0.001) were associated with shorter survival times. In multivariate analysis, receiving zoledronic acid (P<0.001) and performance status (P=0.013) were independent prognostic factors. Conclusions: Smoking history, poor performance status, poorly differentiated adenocarcinoma, and high levels of LDH, CEA, and CA 19-9 were shown to be poor prognostic factors, while receiving chemotherapy and zoledronic acid were associated with prolonged survival in gastric cancer patients with bone metastases.
