PURPOSE. The aim of this study was to observe stress concentration in the implant, the surrounding bone, and other components under the pull-out force during the crown removal. MATERIALS AND METHODS. Two 3-dimensional models of implant-supported conventional metal ceramic crowns were digitally constructed. One model was designed as a vertically placed implant ($3.7mm{\times}10mm$) with a straight abutment, and the other model was designed as a 30-degree inclined implant ($3.7mm{\times}10mm$) with an angled abutment. A pull-out force of 40 N was applied to the crown. The stress values were calculated within the dental implant, the abutment, the abutment screw, and the surrounding bone. RESULTS. The highest stress concentration was observed at the coronal portion of the straight implant (9.29 MPa). The stress concentrations at the cortical bone were lower than at the implants, and maximum stress concentration in bone structure was 1.73 MPa. At the abutment screws, the stress concentration levels were similiar (3.09 MPa and 3.44 MPa), but the localizations were different. The stress at the angled abutment was higher than the stress at the straight abutment. CONCLUSION. The pull-out force, applied during a crown removal, did not show an evident effect in bone structure. The higher stress concentrations were mostly observed at the implant and the abutment collar. In addition, the abutment screw, which is the weakest part of an implant system, also showed stress concentrations. Implant angulation affected the stress concentration levels and localizations. CLINICAL IMPLICATIONS. These results will help clinicians understand the mechanical behavior of cement-retained implant-supported crowns during crown retrieval.
본 증례는 치조골의 흡수가 심한 두 명의 완전무치악 환자에서 서로 다른 임플란트 시스템을 이용한 하악의 미니-임플란트 지지 피개의치에 대한 증례보고이다. 두 환자는 하악 의치의 유지력 부족으로 인한 의치 사용시 불편감 및 동통을 주소로 호소하였다. 각각의 환자는 4개의 미니 임플란트를 이공사이에 판막의 거상없이 식립하였다. 첫번째 증례는 미니볼 어태치먼트와 일체형으로 된 미니-임플란트(Slimline, Dentium, Seoul, Korea)를 식립하고 구강내 직접법으로 어태치먼트를 연결하여 즉시하중을 시행하였다. 두번째 증례는 분리형으로 로케이터 어태치먼트와 연결되는 미니-임플란트(LODI, Zest Anchors, Escondido, CA, USA)를 이용하였으며, 임플란트 식립 8주 후 간접법으로 어태치먼트를 연결하였다. 최종 의치 장착 후 6개월 정기검진까지 미니볼과 로케이터 어태치먼트를 이용한 하악의 미니-임플란트 지지 피개의치 증례에서 환자의 전반적인 만족도가 개선되었으며 주목할 만한 합병증 없이 만족할 만한 임상적 결과를 얻었다. 단 추후 지속적인 유지관리 및 장기적인 평가가 더 필요하리라 사료된다.
무치악 환자의 임플란트 보철 치료는 지지, 유지, 안정성, 발음 등의 측면에서 더 우수한 결과를 보인다. 임플란트를 이용한 무치악 환자의 치료는 다양하며, 크게 가철성과 고정성으로 나눌 수 있다. 가철성 치료에는 임플란트 유지 및 지지 피개의치가 있으며, 고정성 보철치료에는 ceramo-metal 보철물과 임플란트 하이브리드 보철물(implant hybrid prosthesis)이있다. 치료계획의 수립은 잔존 치조제, 연조직, 악간 관계, 환자의 경제적 상황 등을 종합적으로 고려하여 수립해야 한다. 임플란트를 이용한 하이브리드 보철물은 고정성 치료를 통해 환자에게 적절한 교합력을 제공하고 심리적인 안정감을 주는 동시에 적절한 연조직의 회복을 통한 심미적인 결과를 얻을 수 있다. 본 증례는 73세 여환으로 상악 Kennedy Class I 부분 무치악과 하악 무치악 상태로 내원하였다. 상악에는 우측 견치부터 좌측 제1소구치까지 7개의 잔존치를 이용한 가철성 국소의치, 하악은 양측 측절치와 제1소구치 부위에 4개의 임플란트 고정체를 이용한 하이브리드 보철물로 최종 치료 진행하였으며 치료 후 기능적, 심미적으로 만족스러운 결과를 얻었기에 이를 보고하고자 한다.

Purpose: The aim of this study was to assess the influence of kilovoltage- peak (kVp) and the metal artifact reduction (MAR) tool on the detection of buccal and lingual peri-implant dehiscence in the presence of titanium-zirconia (Ti-Zr) and zirconia (Zr) implants in cone-beam computed tomography (CBCT) images. Materials and Methods: Twenty implant sites were created in the posterior region of human mandibles, including control sites (without dehiscence) and experimental sites (with dehiscence). Individually, a Ti-Zr or Zr implant was placed in each implant site. CBCT scans were performed using a Picasso Trio device, with variation in the kVp setting (70 or 90 kVp) and whether the MAR tool was used. Three oral radiologists scored the detection of dehiscence using a 5-point scale. The area under the receiver operating characteristic (ROC) curve, sensitivity, and specificity were calculated and compared by multi-way analysis of variance (α=0.05). Results: The kVp, cortical plate involved (buccal or lingual cortices), and MAR did not influence any diagnostic values (P>0.05). The material of the implant did not influence the ROC curve values(P>0.05). In contrast, the sensitivity and specificity were statistically significantly influenced by the implant material (P<0.05) with Zr implants showing higher sensitivity values and lower specificity values than Ti-Zr implants. Conclusion: The detection of peri-implant dehiscence was not influenced by kVp, use of the MAR tool, or the cortical plate. Greater sensitivity and lower specificity were shown for the detection of peri-implant dehiscence in the presence of a Zr implant.
To assess the new multichannel cochlear implant by radiography in Stenver's projection, because MRI generates artifacts, inducing an electrical current and causing device magnetization. CT is relatively expensive and the metal electrodes scatter the image. Multichannel cochlear implant insertion using the multichannel cochlear implant device. Patients underwent postoperative radiography of their implants. The radiographs were obtained in a Stenver's. The insertion depth of the implant was measured on the radiographs and the results were correlated with the surgical results of insertion depth and with audiometric tests. Patients a correct inserted electrode was found, while in patient complications concerning the electrode were noticed. Radiographs in the Stenver's projection are sufficient for the postoperative assessment of the multichannel cochlear implant device and an exact evaluation of the insertion depth.
In this report, the results of a systematic study on the effects of polycrystalline silicon gate depletion on the reliability characteristics of metal-oxide semiconductor field-effect transistor (MOSFET) devices were discussed. The devices were fabricated using standard complimentary metal-oxide semiconductor (CMOS) processes, wherein phosphorus ion implantation with implant doses varying from $10^{13}$ to $5{\times}10^{15}cm^{-2}$ was performed to dope the polycrystalline silicon gate layer. For implant doses of $10^{14}/cm^2$ or less, the threshold voltage was increased with the formation of a depletion layer in the polycrystalline silicon gate layer. The gate-depletion effect was more pronounced for shorter channel lengths, like the narrow-width effect, which indicated that the gate-depletion effect could be used to solve the short-channel effect. In addition, the hot-carrier effects were significantly reduced for implant doses of $10^{14}/cm^2$ or less, which was attributed to the decreased gate current under the gate-depletion effects.
The purpose of this study is to compare the biocompatibility of commercial purity Ti, Ti-6AI-4V and Ti-6AI-7Nb alloy specimens with and without surface treatment in mouse abdominal connective tissue in vivo. Each metal was implanted into specific abdominal subdermal tissue site of female mouse. After 4 weeks, the implants were removed and abdominal tissues were fixed, dehydrated and embedded in glycol methacrylate resin. And the tissues were histologically prepared for microscopical evaluation. It was characterized by the presence of connective tissue with fibrous capsule surrounding the implant. The fibrous tissue surrounding the implant was studied to determine the biocompatibility of implanted metals. The average thickness of the fibrous capsule formed around the implant was much thinner for the hydrogen peroxide added hydrochloric acid solution-treated specimen than for the others. The results of this evaluation indicate that modification of the surface properties of titanium and titanium alloy implants changes the biological properties in the abdominal connective tissue. In conclusion, these observations suggest that the proper surface treatment performed in the study is effective for the improvement of biocompatibility.
Purpose: Surface alterations of titanium discs following instrumentation with either a nylon brush or a metal brush were evaluated. Methods: A total of 27 titanium discs with 3 surface types (9 discs for each type), including machined (M) surfaces, sandblasted and acid-etched (SA) surfaces, and surfaces treated by resorbable blast media (RBM), were used. Three discs were instrumented with a nylon brush, another 3 discs were instrumented with a metal brush, and the remaining 3 discs were used as controls for each surface type. Surface properties including the arithmetic mean value of a linear profile (Ra), maximum height of a linear profile (Rz), skewness of the assessed linear profile (Rsk), arithmetic mean height of a surface (Sa), maximum height of a surface (Sz), developed interfacial area ratio (Sdr), skewness of a surface profile (Ssk), and kurtosis of a surface profile (Sku) were measured using confocal microscopy. Results: Instrumentation with the nylon brush increased the Ra, Sa, and Sdr of the M surfaces. On the SA surfaces, Ra, Sa and Sdr decreased after nylon brush use. Meanwhile, the roughness of the RBM surface was not affected by the nylon brush. The use of the metal brush also increased the Ra, Sa, and Sdr of the M surface; however, the increase in Sdr was not statistically significant (P=0.119). The decreases in the Rz, Sz, Ra, Sa, and Sdr of the SA surfaces were remarkable. On the RBM surfaces, the use of the metal brush did not cause changes in Ra and Sa, whereas Rz, Sz, and Sdr were reduced. Conclusions: Titanium surfaces were altered when instrumented either with a nylon brush or a metal brush. Hence, it is recommended that nylon or metal brushes be used with caution in order to avoid damaging the implant fixture/abutment surface.
Statement of problem. Surface alteration of the implant screws after function may be associated with mechanical failure. Theses metal fatigue appears to be the most common cause of structural failure. Purpose. The purpose of this study was to evaluate surface alteration of the implant screws after function through the examination of used and unused implant screws in SEM(scanning electron microscope). Materials and methods. In this study, abutment screws(Steri-oss, 3i), gold retaining screw(3i) and titanium retaining screw(3i) were retrieved from patients. New, unused abutment and retaining screws were prepared for control group. Each of the old, used screws was retrieved with a screwdriver. And retrieved implant complex of Steri-oss system was prepared for this study. Then, SEM investigation and EDS analysis of abutment and retaining screws were performed. And SEM investigation of cross-sectioned sample of retrieved implant complex was performed. Results. In the case of new, unused implant screws, as maunfactured circumferential grooves are regularly examined and screw thread are sharply remained. Before ultrasonic cleansing of old, used implant screw, a lot of accumulation and corrosion products were existed. After ultrasonic cleansing of old, used implant screws, circumferential grooves as examined before function were randomly deepened and scratches increased. Also, dull screw thread was examined. More surface alterations after function were examined in titanium screw than gold screw. And more surface alteration was examined when retrieved with driver than retrieved without driver. Conclusions. These surface alteration after function may result in the screw instability. Regularly cleansing and exchange of screws was recommended. We recommend the use of gold screw rather than titanium screw, and careful manipulation of the driver.
목 적 : 두경부 치료계획 CT영상에서 dental implant로 인한 metal artifact 발생 시 O-MAR(Metal artifact Reduction for Orthopedic Implants)(ver. 3.6.0, Philips, Netherlands)를 적용할 수 있을지 여부를 평가하고자 한다. 대상 및 방법 : 모든 CT영상은 Brilliance Big Bore CT(Philips, Netherlands)에서 관전압 120kVp, 2mm 두께로 촬영하였으며, O-MAR를 이용하여 Metal artifact reduction 후 전산화치료계획장비(Eclipse ver 10.0.42, Varian, USA)로 원본영상과 비교, 분석하였다. O-MAR의 기본적인 성능 테스트를 위해 Metal artifact가 발생하지 않은 영상과 발생한 영상에서 O-MAR 적용시, HU 변화를 검증하기 위해 원통형 팬텀과 cerrobend 막대, 불균질 팬텀을 이용하여 실험하였다. 각각의 원본 영상과 O-MAR 적용 영상에서 관심영역 내 HU 변화를 측정하였다. 이를 바탕으로 본 연구의 주목적인 dental implant로 인한 metal artifact 발생 영상을 재현하기 위해 팬텀을 제작하여 사용하였고, 실제 임상 환자 영상에 O-MAR를 적용한 영상과 원본 영상 그리고 artifact 부분을 보정한 영상의 선량 분포를 SNC Patient(Sun Nuclear Co., USA)로 비교하였다. 결 과 : 두경부에서의 metal artifact를 재현한 원본 영상과 O-MAR 적용영상의 선량 분포를 비교한 결과 gamma passing rate 는 2 mm / 2% 기준으로 99.8%, 일치를 보였다. 실제 임상 환자 영상을 바탕으로 O-MAR 적용 전후 영상과 density corrected CT 영상에 동일한 조건으로 치료 계획을 수립하여 선량 분포를 비교한 결과는 98.5% 일치로 비교적 높은 gamma passing rate를 보였다. 전체적인 선량 분포 차이는 모두 2% 이내로 팬텀 실험과 실제 임상 환자 영상 실험에서 비슷한 결과로 나타났다. 하지만 선량 편차가 적더라도 국소적으로 집중되어 있는 것은 문제의 소지가 될 가능성이 있다. 화질 개선 면에서는 모든 실험에서 O-MAR 적용영상이 원본에 비해 개선됨을 알 수 있었으나, 두경부 metal artifact를 재현한 팬텀 영상 air cavity 내에서 최대 HU 값이 상승하는 경우가 생겼고, 환자 영상에서는 air cavity가 tissue로 잘못 보정되는 경우 또한 발견할 수 있었다. 결 론 : 업체에서 제시한 사용제한 사항인 피부 근처와 저밀도 영역이 공존하는 두경부에서 O-MAR의 사용 가능성을 확인해 본 결과, 원본의 왜곡과 보정이 동시에 일어났다. 심지어 팬텀 실험보다 더 심한 artifact가 생긴 환자의 경우 air cavity가 tissue로 잘못 보정되는 경우도 발생하였다. 결과적으로 아직까지는 O-MAR 알고리즘이 air cavity와 photon starvation artifact를 정확히 구분하지 못하는 것으로 보인다. 선량 측면에서의 영향은 임상에서 배제될 만큼 큰 차이를 보이지는 않았다. 임상에서 원본과 O-MAR 적용 영상을 비교하며 작업한다면 contouring, artifact 보정작업, DRR 화질 개선 등에 도움을 받을 수 있을 것으로 사료된다.
