Both immune reaction and hypersensitivity reaction are occurred by the same mechanism, the antigen and antibody reaction. The favorable result of this reaction towards a host is called clinically an immune reaction and the opposite results is called an hypersensitivity reaction. Type IV hypersensitivity reaction is a delayed type which is related to the cellular immune reaction and a contact hypersensitivity is included in this type. Various dental materials such as metal (mercury, nickel, chrome, cobalt), resin and eugenol are etiologic substances. Patch test kit is composed of test substance with a controlled concentration which respond only to a susceptible patient and an aluminum chamber, and etiologic substances for hypersensitivity can be easily and comfortably found just by applying the kit to the patient's skin. In this case report, the patch test was performed to a patients with oral lichen planus and the allergen, restorative material was found. After removal of the matching restoration from the patient's mouth, the symptom was improved.

Pseudotumors are not uncommon complications after total hip arthroplasty (THA) and may occur due to differences in bearing surfaces of the head and the liner ranging from soft to hard articulation. The most common causes of pseudotumors are foreign-body reaction, hypersensitivity and wear debris. The spectrum of pseudotumor presentation following THA varies greatly-from completely asymptomatic to clear implant failure. We report a case of pseudo-tumor formation with acetabular cup aseptic loosening after revision ceramic-on-metal hip arthroplasty. The patient described herein underwent pseudotumor excision and re-revision complex arthroplasty using a trabecular metal shell and buttress with ceramic-on-polyethylene THA. Surgeons should be aware of the possibility of a pseudotumor when dealing with revisions to help prevent rapid progression of cup loosening and implant failure, and should intervene early to avoid complex arthroplasty procedures.
PURPOSE. Many dentists use desensitizing agents to prevent hypersensitivity. This study compared and evaluated the effect of two desensitizing agents on the retention of cast crowns when cemented with various luting agents. MATERIALS AND METHODS. Ninety freshly extracted human molars were prepared with flat occlusal surface, 6 degree taper and approximately 4 mm axial length. The prepared specimens were divided into 3 groups and each group is further divided into 3 subgroups. Desensitizing agents used were GC Tooth Mousse and $GLUMA^{(R)}$ desensitizer. Cementing agents used were zinc phosphate, glass ionomer and resin modified glass ionomer cement. Individual crowns with loop were made from base metal alloy. Desensitizing agents were applied before cementation of crowns except for control group. Under tensional force the crowns were removed using an automated universal testing machine. Statistical analysis included one-way ANOVA followed by Turkey-Kramer post hoc test at a preset alpha of 0.05. RESULTS. Resin modified glass ionomer cement exhibited the highest retentive strength and all dentin treatments resulted in significantly different retentive values (In Kg.): GLUMA ($49.02{\pm}3.32$) > Control ($48.61{\pm}3.54$) > Tooth mousse ($48.34{\pm}2.94$). Retentive strength for glass ionomer cement were GLUMA ($41.14{\pm}2.42$) > Tooth mousse ($40.32{\pm}3.89$) > Control ($39.09{\pm}2.80$). For zinc phosphate cement the retentive strength were lowest GLUMA ($27.92{\pm}3.20$) > Control ($27.69{\pm}3.39$) > Tooth mousse ($25.27{\pm}4.60$). CONCLUSION. The use of $GLUMA^{(R)}$ desensitizer has no effect on crown retention. GC Tooth Mousse does not affect the retentive ability of glass ionomer and resin modified glass ionomer cement, but it decreases the retentive ability of zinc phosphate cement.

The treatment objective of mandibular fracture is a return to normal function. According to Champy, a rigid fixation of mandibular angle fracture is performed by using 4 or 6 holes titanium miniplates on the external oblique ridge of mandible. However, the limitations of metal plate such as hypersensitivity, interference with the cranio-facial growth of growing child, secondary bone resorption around the plate, foreign body reaction, declination of primary callus formation, and bone atrophy due to the lack of normal stress reaction of the bone have been reported. Recently, biodegradable miniplate has been introduced and used as an alternative to the metal plate despite its lower strength than that of the titanium plate and the side effect caused by the resorption in the body. In this study, 61 patients diagnosed as mandibular angle fracture and treated from Jan. 1998 to Dec. 2004 in our department have been reviewed. Metal plate fixation was used in 50 patients and biodegradable plate fixation in 11 patients on the external oblique ridge around the fractured mandibular angle according to the principle of monocortical osteosynthesis by Champy. We compared the incidence of side effects and the degree of bony union at the mandibular inferior border in two different fixation methods. In conclusion, we have found that one miniplate regardless of matter could provide enough strength to grasp bony fragments of the tension site and compress the inferior border of mandible without any complications.
Purpose: Traditionally, titanium miniplate has been used for rigid fixation of mandible fractures. This study evaluated the usefulness and stability of biodegradable plate and screw for treatment of mandible fractures. Methods: In this study, 61 patients(92 areas) diagnosed as mandible fracture in the last 2 years have been reviewed. We used titanium plate and screw in 32 patients, and biodegradable plate and screw($INION^{(R)}$) in 29 patients. Stability of plates and screws, bony healing process and its side effects were observed by clinical and radiographic assessment. Results: In the titanium material group, one of malocclusion, two of mouth opening limitation, three of pain, three of palpation were shown. The plate of six patients involved in these complications were removed. In the biodegradable group, two of mouth opening limitation, two of pain, one of localized wound infection were shown and one plate was removed secondarily. Conclusion: There was no statistical difference between two groups in bony healing and complication rates. Biodegradable implants show efficient stability during initial bone healing and low side effects in long-term follow up periods.
Phytochelatins play an important role in heavy metal detoxification in plants as well as in other organisms. The Arabidopsis thaliana mutant cad1-3 does not produce detectable levels of phytochelatins in response to cadmium stress. The hypersensitivity of cad1-3 to cadmium stress is attributed to a mutation in the phytochelatin synthase 1 (AtPCS1) gene. However, A. thaliana also contains a functional phytochelatin synthase 2 (AtPCS2). In this study, we investigated why the cad1-3 mutant is hypersensitive to cadmium stress despite the presence of AtPCS2. Northern and Western blot analyses showed that expression of AtPCS2 is weak compared to AtPCS1 in both roots and shoots of transgenic Arabidopsis. The lower level of AtPCS2 expression was confirmed by RT-PCR analysis of wild type Arabidopsis. Moreover, no tissue-specific expression of AtPCS2 was observed. Even when AtPCS2 was under the control of the AtPCS1 promoter or of the cauliflower mosaic virus 35S promoter (CaMV 35S) it was not capable of fully complementing the cad1-3 mutant for cadmium resistance.
Background : The human being and the bacteria have accomplished a balance and have coexisted each other during long time. Probiotics have evolved with the human host to exist and the result they have operated profitably to human host. So it requires that the functions of probiotics are expounded in view of Traditional Korean Medicine. Aim : Suggest the functions of probiotics in view of Traditional Korean Medicine. Method : The author's research has been performed to review the related references. Results : Probiotics assist the absorption of the lactose, proteins and minerals and product several kinds of vitamins, organic acids. Probiotics suppresses the growth of noxious bacteria and the production of harmful substances or gases. They absorbed and discharge the bile acid, and thus help us maintain the optimal level of blood cholesterol concentration. They can reinforce the immune response of the mucous membrane and control the hypersensitivity immune reaction such as asthma, atopy on the other hand. Probiotics have right functions as above and so can be applied widely in treatment of various disease and symptom. Conclusion : Considering the functions of probiotics in view of Traditional Korean Medicine, they participate in our spleen-earth-system (digestion and synthesis) and liver-wood-system (regulation of digestion, metabolism, internal secretion etc.), assist the function of lung-metal system(respiration and regulation of water metabolism) and regulate wi-chi (reinforce/control immune system). Consequently, hereafter there would be a necessity of control a circumstance in treatment of various diseases under these categories that probiotics should be able to do their right functions inside the human body.
As a material of metal-ceramic prosthesis, nickel as a form of Ni-Cr alloy has been used for many dental prostheses in many cases. However, several problems in use of the alloy have been revealed (ex : tissue stimulation, skin allergy, hypersensitivity, cytotoxicity and carcinogenecity). Little is known about nickel with respect to the relationship between Ni-prosthesis and gaining of Ni-resistance in oral microorganisms. The present study was undertaken to check wheather use of Ni-prosthesis leads to occurrence of Ni-resistant microorganisms. So this study may suggest the possible relationships between the oral microorganisms and nickel-resistance in oral environment. Bacteria were isolated from the gingival crevicular fluid on the pateints wearing Ni-Cr prosthesis. The isolated bacteria were tested for their Ni-resistance in nickel containing media at different concentration from 3mM to 110mM. E. coli HB101 was used as control. The Ni-resistant bacteria were isolated and biochemically identified. The Ni-resistant bacteria were tested several bio-chemical, molecular-biological tests. Performed tests were ; measuring the growth curve, antibiotic test, growth ability test in liquid media, isolation of the chromosome and plasmid, digestion of DNA by restriction enzyme, electrophoresis of chromosome and plasmid DNA, identification of Ni-resistant genes by the DNA hybridization. The results were as follows: 1) The bacteria isolated from gingival crevicular fluid on the patients wearing Ni-Cr alloy pros-thesis showed nickel-resistance. 2) The isolated microorganisms grew at nickel containing media of high concentrations (60mM-110mM). 3) Based on the biochemical tests, the isolated microorganisms were identified as Enterococcus faecalis(13 cases), Klebsiella pneumoniae(1 case) and Enterobacter gergeviae(1 case). 4) Enterococcus faecalis expressed not only nickel resistance but also the multi-drug resistance to several antibiotics ; chloramphenicol, kanamicin, streptomycin, lincomycin, clindamycin. However, all strain showed the sensitivity against the tetracycline. 5) DNA hybridization result suggest that there is no homology between the previousely known gene of nickel resistance in Klebsiella pneumoniae and chromosomal DNA of Enterococcus faecalis.

연구목적: 변연적합도는 보철수복물의 성공을 좌우하는 중요한 요소 중의 하나이다. McLean과 von Fraunhofer등에 의하면 120 ${\mu}m$ 이하를 임상적으로 허용할 수 있다고 했는데, 이를 넘는 정확하지 않은 변연은 지각과민을 일으키거나 이차 우식을 일으키고, 치태 축적을 용이하게 하여 보철물의 수명을 단축시키게 된다. 이에 최근에 우리나라에 소개되어 임상적으로 사용되고 있는 CAD-CAM 전부도재 수복물 중 $LAVA^{(R)}$ 시스템 (3M ESPE, St. Paul, MN)과 $EVEREST^{(R)}$ 시스템 (KaVo Dental GmbH, Biberach, Germany)으로 제작한 지르코니아 전부 도재관들이 도재전장 금속주조 수복물과 비교해 보아 코핑 상태일 때와 도재축성 후 각각 변연적합도의 차이가 있는지를 살펴보고자 하였다. 재료와 방법: 상악 제1소구치 덴티폼 치아를 high speed diamond bur 로 교합면 2.0 mm, 순설면 1.0 mm, 그리고 변연은 chamfer margin을 6도 측면 경사 절삭 기계로 균일하게 제작해서 이를 인상 채득하여 금속 지대치를 제작하였다. 이를 덴티폼 모형상에 장착한 후 전악인상을 채득하여 $LAVA^{(R)}$ 수복물 코핑, $EVEREST^{(R)}$ 수복물 코핑, 그리고 도재전장 금속주조 수복물 코핑을 각각 8개씩 제작하였다. 이들 코핑을 염색제를 섞은 Fit-checker $II^{(R)}$ (GC Cor., Tokyo, Japan)를 이용하여 금속 지대치에 접합시킨 뒤, $Microhiscope^{(R)}$ system (HIROX KH-1000 ING Plus, Seoul, Korea)을 이용하여 300배율로 관찰하면서 변연 간격을 ${\mu}m$단위로 측정하였다. 도재축성 후에도 마찬가지 방법으로 모든 수복물의 변연간격을 측정하여 통계 처리를 하였다. 결과: 코핑 상태에서의 평균 변연간격 값은 $EVEREST^{(R)}$ 에서 $52.00{\pm}11.94\;{\mu}m$, $LAVA^{(R)}$ 는 $ 56.97{\pm}10.00\;{\mu}m$, 그리고 도재 전장 금속 주조 수복물은 $97.38{\pm}18.54\;{\mu}m$를 보였고, 도재축성 후에 평균 변연 간격값은 $EVEREST^{(R)}$ 는 $61.69{\pm}19.33\;{\mu}m$, $LAVA^{(R)}$ 는 $70.81{\pm}12.99\;{\mu}m$, 그리고 도재 전장 금속 주조 수복물은 $115.25{\pm}23.86\;{\mu}m$를 보였다. 결론: 1. $LAVA^{(R)}$ 와 $EVEREST^{(R)}$ 수복물은 도재 전장 금속 주조 수복물과 비교하여 각각 코핑상태일 때와 도재 축성 후 모두 유의성 있는 (P < .05) 차이를 보이는 우수한 변연 적합도를 나타내었다. 2. $LAVA^{(R)}$ 와 $EVEREST^{(R)}$ 수복물 간의 평균 변연 간격 비교에서 코핑 상태일 때와 도재 축성 후에 모두 유의성 있는 차이는 없었다 (P > .05). 3. $LAVA^{(R)}$과 $EVEREST^{(R)}$ 그리고 PFM 은 각각 코어 상태일 때와 비교하여 도재 축성 후 변연 간격에서 약간 증가되었으나 유의성 있는 차이를 보이지는 않았다 (P > .05).
