대사증후군 관리를 위한 직장기반 식생활 중재 프로그램의 효과 (Effect of a Worksite-based Dietary Intervention Program for the Management of Metabolic Syndrome)

  • Objectives: To investigate the effect of a worksite-based dietary intervention program for the management of metabolic syndrome (MS) among male employees. Methods: A dietary intervention program combining individual and environmental approach was implemented targeting white-collar employees at a worksite located in Seoul for 10 weeks. Out of 104 employees having agreed to participate in the program, those having three or more out of five components of MS and having two components, including a waist circumference component were classified into "the high risk group" (n=41) and received group nutrition education and individual nutrition counseling three times each. The rest of the study subjects were considered as "the low risk group" (n=63). The food environment at the worksite, where both the high and low risk groups were exposed, was changed to promote healthy eating. Physical data including MS components were collected and a questionnaire on dietary behaviors was administered before and after the intervention. The data from the high risk group (n=17) and the low risk group (n=20), excluding the subjects ineligible for or failed to complete the study (n=67), were analyzed. The difference before and after intervention was tested for significance by Wilcoxon signed-rank tests. Results: Weight, body mass index (BMI), waist circumference, blood pressure, HDL-cholesterol, and HbA1c and the healthy dietary practice score improved significantly after intervention in the high risk group. The median number of MS components decreased significantly from 3.0 to 1.0 in the high risk group. In the low risk group, only HbA1c significantly decreased. Conclusions: The 10-week worksite-based dietary intervention program combining individual and environmental approach was found to be effective for managing MS of male employees.

Association between Egg Consumption and Metabolic Disease

  • The effect of high egg intake on metabolic syndrome (MetS), a major risk factor for cardiovascular disease (CVD), has not been clearly elucidated. This study was conducted to review the literature related to egg consumption and the risk of metabolic disease as well as to examine the association between high egg intake and MetS in Korean adults. A literature review was conducted using published papers in PubMed and EMBASE through December 2017. We have reviewed 26 articles, which were associated with egg consumption and metabolic diseases, and found that the results were controversial. Therefore, we analyzed data from 23,993 Korean adults aged 19 yrs and older. MetS was defined based on criteria from the Adult Treatment Panel III. Egg consumption of 4-6 times/wk and 1 time/day were significantly associated with reduced prevalence of MetS (Odds ratio (OR)=0.82; 95% Confidence interval (CI)=0.71-0.95 for 4-6 times/wk, OR=0.83; 95% CI=0.69-0.99 for 1 time/day) compared to those who consumed eggs less than once monthly. However, consuming two or more eggs per day was not associated with MetS. As for the components of MetS, an egg intake of once daily decreased the prevalence of abdominal obesity and an intake of 2-7 eggs weekly was shown to prevent a reduction in the high-density lipoprotein cholesterol levels. This study suggests that while consuming eggs 4-7 times weekly is associated with a lower prevalence of MetS, consuming two or more eggs daily is not associated with a reduced risk for MetS.

배추김치 섭취와 대사증후군 발생률과의 관련성 : 한국인유전체역학조사사업의 10년 추적조사 결과 (Association of Korean fermented cabbage kimchi consumption with an incidence of metabolic syndrome: 10-year follow-up results of the Korean Genome and Epidemiology Study)

  • 본 연구에서는 한국인유전체역학조사사업에서 수집한 2001 ~ 2002년 기반조사 자료로부터 건강한 한국 성인 3,560명의 배추김치 섭취빈도에 따른 일반적 특성, 식품군 섭취빈도와 영양소 섭취량을 분석하였고, 이들 중 2,259명을 대상으로 10년 추적조사에서 대사증후군과 그 구성 요소의 발생률 위험도를 분석하였다. 기반조사에서의 배추 김치 섭취빈도에 따라 1일 기준으로 '1회 미만 섭취군', '1 ~ 2회 섭취군', '3회 섭취군'으로 대상자를 분류하였으며, 세 집단 간 성별에 따른 유의적인 차이는 없었고, 남녀 모두 연령, 교육수준, 소득수준, 거주지역에서 유의적인 차이가 있었다. 배추김치 섭취빈도에 따라 식품군 섭취빈도는 연령, 교육수준, 소득수준, 거주지역, 음주여부와 에너지 섭취량으로 보정한 후 남녀 모두에서 3회 섭취군의 밥류와 두류의 섭취빈도가 다른 두 군의 섭취빈도보다 높았으며, 육류의 섭취빈도는 낮았다. 배추김치 섭취빈도가 높을수록 남녀 모두 에너지 섭취량이 많았으며, 대부분의 영양소 섭취량도 많았다. 10년의 추적기간 동안 대상자 2,259명 중 933명이 대사증후군을 나타냈으며, 배추김치 섭취빈도에 따라 남자의 경우에는 대사증후군 발생률 위험도에 유의적 차이가 없었으나, 여자의 경우 교란변수인 연령, 교육수준, 소득수준, 거주지역, 음주여부, 에너지 섭취량으로 보정한 후에도 HR이 0.63 (95% CI: 0.47 ~ 0.86)으로 유의적으로 감소한 것으로 나타났다. 결론적으로 지역사회기반 코호트의 10년 추적조사 결과 성인 여자의 경우 배추김치를 1일 3회 섭취하는 집단은 1회 미만 섭취하는 집단에 비해 대사증후군 발병 위험이 낮은 것으로 나타났다.

  • The tumour lysis syndrome (TLS) is a group of metabolic abnormalities caused by rapid and unexpected release of cellular components into the circulation as a result of massive destruction of rapidly proliferating malignant cells. It usually develops in patients with hematologic malignancies like acute lymphoid leukemia, non-Hodgkin and Burkitt's lymphoma after initiation of chemotherapy or may, rarely, occur spontaneously. Though TLS is seldom observed in relation to solid tumours, there have been reports of connections with examples such as lung, liver, breast, gastric carcinomas. The clinical manifestations of TLS include hyperuricemia, hyperkalemia, hyperphosphatemia and hypocalcemia. These indications if untreated lead to life-threatening complications such as acute renal failure, cardiac arrhythmias, seizures, and eventually death due to multiorgan failure. Therefore early detection of TLS is of vital importance. This can be accomplished by identification of high risk patients, implementation of suitable prophylactic measures andmonitoring of the electrolyte levels in patients undergoing chemotherapy.

영양서비스 개발을 위한 대구지역의 인슐린저항성증후군 패턴의 인구학적 특성 분석 (Patterns of Insulin Resistance Syndrome in the Taegu Community for the Development of Nutritional Service Improvement Programs)

  • The clustering of insulin resistance with hypertension, glucose intolerance, hyperinsulinemia, increased triglyceride and decreased HDL cholesterol levels, and central and overall obesity has been called syndrome X, or the insulin resistance syndrome(IRS). To develop a nutrition service for IRS, this study was performed to evaluate the prevalence of each component of the metabolic abnormalities of IRS and analyze the clustering pattern of IRS among subjects living in the Taegu community. Participants in this study were 9234(mean age ; M/F 48/40yrs);63.5% were men, 24.4% were obese, 13.3% had hypertension. 3.7% had hyperglycemia, and 32.4% had hyperlipidemia. The IRS was defined as the coexistence of two or more components among metabolic abnormalities; obesity, hypertension. hyperglucemia and hyperlipidemia. The prevalence of IRS in Taegu was 19.2%(M/F:20.8%/16.4%), the clustering of these fisk variables was higher in advanced age group. Among the subjects of IRS having two of more diseases, 75.6% were obese, the pattern were similar in men and women. The younger, the higher the prevalence of obesity associated clustering patterns. The prevalence of obesity associated patterns among the hyperglycemia associated clustering patterns was 44.5%. The samples of the representative clustering patterns were obesity and hyperlipidemia (8.0%), hypertension and hyperlipidemia(3.2%), hypertension, obesity and hyperlipiemia(3.1%), hypertension and obesity(2.3%), and hyperglycemia and hyperlipidemia(0.8%). The clustering of obesity and hyperlipidemia until 50 year old groups, and the clustering of hypertension and hyperlipidemia in the 60 and 70 age groups were the most prevalent. We concluded that insulin resistance syndrome was a relatively common disorder in the Taegu community, and prevalence and the characteristics of the intervention strategies for IRS are desired, an effective improvement will be achieved.

Dynamic Energy Balance and Obesity Prevention

  • Dynamic energy balance can give clinicians important answers for why obesity is so resistant to control. When food intake is reduced for weight control, all components of energy expenditure change, including metabolic rate at rest (resting energy expenditure [REE]), metabolic rate of exercise, and adaptive thermogenesis. This means that a change in energy intake influences energy expenditure in a dynamic way. Mechanisms associated with reduction of total energy expenditure following weight loss are likely to be related to decreased body mass and enhanced metabolic efficiency. Reducing calorie intake results in a decrease in body weight, initially with a marked reduction in fat free mass and a decrease in REE, and this change is maintained for several years in a reduced state. Metabolic adaptation, which is not explained by changes in body composition, lasts for more than several years. These are powerful physiological adaptations that induce weight regain. To avoid a typically observed weight-loss and regain trajectory, realistic weight loss goals should be established and maintained for more than 1 year. Using a mathematical model can help clinicians formulate advice about diet control. It is important to emphasize steady efforts for several years to maintain reduced weight over efforts to lose weight. Because obesity is difficult to reverse, clinicians must prioritize obesity prevention. Obesity prevention strategies should have high feasibility, broad population reach, and relatively low cost, especially for young children who have the smallest energy gaps to change.

Interactions between the $FTO$ rs9939609 polymorphism, body mass index, and lifestyle-related factors on metabolic syndrome risk

  • Whether the $FTO$ polymorphisms interact with environmental factors has not yet been evaluated in associations with metabolic syndrome (MS) risk. The present study investigated the association of the $FTO$ rs9939609 genotypes, body mass index (BMI), and lifestyle-related factors including smoking, alcohol drinking, physical activity, and diet with MS incidence. A population-based prospective cohort study comprised 3,504 male and female Koreans aged 40 to 69 years. At the beginning of the study, all individuals were free of MS and known cardiovascular disease. Incident cases of MS were identified by biennial health examinations during a follow-up period from April 17, 2003 to April 15, 2009. Pooled logistic regression analysis was applied to obtain relative odds (RO) of MS with its 95% confidence interval (CI). After controlling for potential MS risk factors, we observed no association between the rs9939609 genotypes and MS incidence. In analysis stratified by BMI, however, carriers with the $FTO$ risk allele whose BMI is $29kg/m^2$ or greater showed an approximately 6-fold higher RO (95% CI: 3.82 to 9.30) compared with non-carriers with BMI less than $25kg/m^2$. In particular, the association between the rs9939609 variants and MS risk was significantly modified by high BMI (P-value for interaction < 0.05). Such significant interaction appeared in associations with central obesity and high blood pressure among the MS components. Because carriers of the $FTO$ risk alleles who had BMI of $29kg/m^2$ or greater are considered a high risk population, we suggest that they may need intensive weight loss regimens to prevent MS development.

The Association Between Circulating Inflammatory Markers and Metabolic Syndrome in Korean Rural Adults

  • Objectives: This study was performed to investigate the associations between the metabolic syndrome (MetS) and inflammatory markers. Methods: This cross-sectional analysis was performed using data from 1578 Koreans aged 40-69 years residing in a rural area. We investigated associations between MetS and circulating high sensitivity C-reactive protein (hs-CRP), white blood cells (WBC) and adiponectin. MetS was defined using the criteria proposed by the National Cholesterol Education Program Adult Treatment Panel III (ATP-III). Results: Increased WBC counts and hs-CRP levels and decreased adiponectin levels were observed in subjects with MetS. WBC, hs-CRP and adiponectin levels linearly deteriorated with an increase in the number of MetS components (all ptrend <0.005). Finally, adjusted odds ratios (ORs) for the risk of MetS by increase/decrease in 3 inflammatory markers were calculated by multivariate logistic regression analyses. In terms of changes in inflammation markers, in men, the adjusted ORs (95% confidence interval) were 1.15 (1.01-1.31) for WBC, 1.64 (1.02-2.64) for hs-CRP, and 0.19(0.08-0.45) for adiponectin, whereas corresponding adjusted ORs (95% Cls) in women were 1.27 (1.15-1.40), 0.98 (0.67-1.42), 0.09 (0.04-0.18), respectively. Conclusions: Serum adiponectin levels and WBC counts were found to be strongly associated with MetS in both sexes. However, hs-CRP lost its significance after adjusting for BMI and other inflammatory markers in women. This study shows that inflammatory response is associated with MetS in the Korean population. Further prospective studies are necessary to confirm the contribution made by inflammatory markers to the development of MetS.

폐경이 대사증후군에 미치는 영향 (The Effects of Menopause on the Metabolic Syndrome in Korean Women)

  • 본 연구는 국민건강영양조사 2012년도 원시자료를 이용한 이차분석 연구로서, 연구 목적은 폐경이 대사증후군 유병율 및 구성요소에 미치는 영향을 확인하는 것이다. 층화집락복합표본추출로 선정된 총 8,058명 중 만 19세 이상 성인 여성이며 월경 및 폐경에 대한 정확한 정보를 제공한 2,746명을 대상으로 하였다. 연구결과, 폐경 여부에 따른 대사증후군 구성요소의 평균 차이는 모두 통계적으로 유의하였다. 대사증후군 유병율은 폐경 전 여성 13.2%, 폐경 후 여성 42.7%이었다. 폐경후 대사증후군에 이환될 확률은 4.88배(95% CI=3.888-6.126) 증가하고, 40세를 기준으로 나이가 5세 증가할 때마다 대사증후군에 이환될 확률은 각각 3.15배(95% CI=1.862-5.331), 4.159배(2.558-6.761), 5.971배(3.955-9.016), 9.52배(6.591-13.749) 증가하였다. 연구 결과를 바탕으로 폐경 후 여성의 대사증후군 예방 및 관리를 위한 중재 프로그램이 개발 및 적용되어야 할 것이다.

CACNA1A Gene Polymorphism is Associated with Hypertension in Korean Population

  • High blood pressure (BP) is the most frequent risk factor among metabolic syndrome components. The control of hypertension is very important to prevent the cardiovascular risk in metabolic syndrome. The dysfunction of calcium channel is responsible in the regulation of the vascular muscle contribution to hypertension. Calcium channel, voltage-dependent, P/Q type, alpha-1A subunit (CACNA1A) gene is located in brain and known to control the intracranial hypertension. In this study, we investigate whether the polymorphisms of CACNA1A gene is associated with hypertension. The 49 CACNA1A genotypes were determined using the Affymetrix Genotyping chip array in 92 hypertension and 279 control individuals from a Korean population. Logistic and multiple regression models were employed to analyze the genetic contributions of polymorphisms. Out of 49 polymorphisms, six SNPs (rs12611029, rs16035, rs7259944, rs10419472, rs17777900, and rs4926294) showed a significant association with hypertension in three alternative models (codominant, dominant, and recessive models; P<0.05 after adjusting for age and sex). Our results suggest that the CACNA1A gene may be associated with hypertension in the Korean population.
