  • The purpose of this study was to analyze characteristics related to the activity of Daily Living (ADL) and dementia among the elderly people who have Medicaid. The cross-sectional descriptive survey study was a nationwide randomization sampling among the population of elderly families who have Medicaid. The data were collected during the month of October, 1999 and total sample was 1,027 elderly people. There were major findings according to the studies. In the results of the ADL assessment most of elderly people were within the 24 to 45 point range. Also, 63.3% of elderly people who made 45 points do not need help when performing daily activities according to the 15 areas of activity components, and 4.9% of these people couldn't do their daily activities. The results of the Dementia assessment were 70.6% of elderly people were in the normal range, 21.7% have a mild case, and 2.8% have severe case of dementia. These were found by using instruments for mental states, which simplified to items of detection of early dementia. In the result of these tests, there was a significantly positive correlation between ADL and degree of dementia with the pearson correlation coefficients. As a result of these studies, the author recommend to strengthen function and organization of public health like a visiting nurse center for elderly people who are over 65 years old. In addition, the government should apply early detection and management system for dementia in the community continuously and cost-effectively, especially for elderly people who live alone and are vulnerable elderly as our priority.

  • Objectives: Although increased reactive oxygen species (ROS) is caused by stress accelerates collagen degradation, there was no data on the relationship between stress and urinary hydroxyproline (Hyp) and proline (Pro), a good marker of collagen degradation. The purpose of this study was to evaluate the relationship between depression, anxiety, and stress (DAS) and concentrations of urinary Hyp and Pro. Methods: 97 hospital employees aged 20 to 58 were asked to fill out comprehensive self-administrated questionnaires containing information about their medical history, lifestyle, length of the work year, shit-work and DAS. Depression Anxiety Stress Scale (DASS) was applied to evaluate chronic mental disorders. Urine samples were analyzed using High Performance Liquid Chromatography (HPLC) with double derivatization for the assay of hydroxyproline and proline. Results: The mean value of Hyp and Pro concenturation in all subjects was $194.1{\pm}113.4\;{\mu}mol/g$ and $568.2{\pm}310.7\;{\mu}mol/g$. DASS values and urinary Pro concentrations were differentiated by sex (female > male, p < 0.05) and type of job (nurse > others, p < 0.05). In the stepwise multiple linear regressions, urinary Hyp and Pro concentrations were influenced by stress (Adjusted $r^2$ = 0.051) and anxiety and job (Adjusted $r^2$ = 0.199), respectively. Conclusions: We found that stress and anxiety were correlated with urinary Hyp and Pro concentrations. To identifying a definite correlation, further study in large populations will be needed.

  • This study was designed to identify a more effective method of nursing interview in gathering informations by comparing the two, the structured and unstructured methods. May through October, 1976, 40patients on psychiatric wards of Korea University Hospital and National Mental Hospital, Seoul, were sampled and interviewed on their admission/within one week of admission. Both methods were used simultaneously by different interviewers ; 48 hour interval were kept between the two approaches of interview. Contents of informations gathered were grouped according to the criteria developed by the researcher, frequency, percentile score and the means were scored, and the two methods were compared; the amount of information gathered, number of questions used, time spent and the general response towards the methodologies. Results of the study are summarized as follows ; 1. 1. 5 times as many informations were gathered through the structured interview. (1366 ; 849) 2. 68.32% of informations gathered through the unstructured interview and 42.46% of the structured interview revealed to be overlapping; through structured interview, more unique informations (57.54%) were gathered. 3. The average of 33.25 minutes were consumed for the structured interview while 95, 50 minutes for the unstructured. Much higher time consumption is revealed in the unstructured. 4. Majority of patients showed positive response towards both approaches of interview (90% each), however, interviewers responded to experience of certain degree of tension (stress) in the unstructured method (35%) 5. The average of 0.88 question were used to gather one information in the structured while 1.87questions for the unstructctured were used. 6. The average of 0.97 minutes were consumed to gather one information in the structured while 4.50minutes for the unstructured. 7. The average of 1.11 minutes were consumed for one question for the structured while 2.41 minutes for the unstructured. The results of the study revealed that the structured interview were more effective in gathering informations; a larger quantity of informations were gathered in shorter time. Recommendations for further in investigations are as follows ; 1. Comparative analysis of informations gathered through the two methods by experienced professional nurse- interviewers is recommended. 2. Comparative analysis of the quality of informations is recommended. 3. Comparative analysis of the interviewing process (interactions) in relation to the characteristics of patients'health problems is recommended.

  • The aim of this study is to present the basic datum for the promotion of effectiveness and improvement of nursery tasks, by grasping the situation of operating room nurses tasks and the degree of their fatigue. For these researches, 70 nurses out of the operating rooms of 3 university-affiliated hospitals-two in Seoul and one in Pusan, were chosen at random by a non-probability sampling method. These researches were done from April 14 to April 26 in 1997 by questionairing method. The questionaire was composed of 30 items, which asks the examinees of their physical, mental, and neurosensory symptoms, with 10 items respectively. The reliability of the research instrument was turned out very high with Cronbach's ${\alpha}=.9376$. The datum were electronically processed using Statistics Program for Social Sciences(SPSS). The analysis of datum in this study has a general character, in which the demosociological character and the special ex-officio character was calculated by frequency and percentage. The situation of tasks in operating rooms was calculated by frequency and percentage. The fatigue of operating room nurses was calculated by average and standard deviation. To compare the fatigue with regard to the character of operating room nurse's tasks, t-test and F-test(ANOVA) were used after the character of variations, and the variations at the level of P<.05 which might have some meaning was verified after the fact with Duncan's Multiple Range(DMR). The results of this research are as follows : 1) The nurses working in operating rooms show their fatigue in three fields-in Group I physical symptoms 3.28, in Group III neurosensory symptoms 2.85, and in Group II mental symptoms 2.73, which shows I Dominant type (general type). 2) They complain, in Group I they are "feeling the heavy legs," 3.28 and in Group II they "occasionally forget soon what to do," 3.09. and in Group III, they "feel lumbago," 3.47, which is the highest rate of the three. The highest rate results from the character of their tasks, in which they have to move rapidly the heavy appliances and do their jobs standing many hours, especially wearing heavy radiation protector. 3) As to transportation, subway using group feel the greatest fatigue, 3.18(F=4.315, P=.008). 4) As to department, nurses in the orthopedic's surgery part feel the greatest fatigue, 3.26(F=2.040, P=.050). 5) As to the change of physical symptoms, the group answering that they found physical abnormality after working in operating rooms show the greatest fatigue, 3.12(t=-3.13, P=.003). 6) At to the general circumstances, the group answering that they receive insufficient consideration on their physical abnormality in their department, show the greatest fatigue, 3.10(F=3.200, P=0.47). 7) As to the relation with superior officers, the group answering that their superior officer has an impetuous temperament, show great fatigue, 3.11(F=4.855, P=.011). 8) As to the time of feeling fatigue, the fatigue reaches the highest point 1~2 hours after operations, 3.04(F=2.703, P=.046). 9) When they feel fatigue after scrub nurse duties, they feel the greatest fatigue 2 hours after the duties, 3.09( F=2.841, P=.038). 10) As to the operation instruments, when they use complex instruments borrowing from the outside in addition to the basic instruments, their fatigue becomes the greatest, 3.09(F=7.831, P=.000). 11) As to the kind of operation, when they participate in orthopedic's surgery operations, they feel the greatest fatigue, 3.18(F=4.362, P=.000). With the above results, it is proved that the degree of operating room nurses' fatigue is considerably high. So it may be concluded that the measure for lessening the fatigue should be find immediately, not on the level of personal matters but on the level of hospital nursing administration.

  • As contemporary society has become more complicated, specialized, and segmented, people are experiencing more diverse types of stress. In particular, while several factors associated with job stress have been examined among nurses, who belong to a professional group, the existing research has made no quantitative assessments of stress that reflect temporal differences in individuals. Therefore, the aim of this study is to understand the effects of job stress on alpha-amylase with regard to the working hours of nurses, to assess the variations in jobs stress over time, and provide basic data to improve the quality of nursing services. Ninety nurses working in three shifts in general, emergency, and intensive care wards of a university hospital in D City participated in this study. Salivary alpha-amylase (SAA) was extracted and analyzed at two-hour intervals from 07:00 to 15:00 from nurses on the day shift and from 23:00 to 07:00 from those working the night shift. The SAA level was highest between 23:00 and 01:00 for nurses in general wards ($mean{\pm}S.D.\;39.00{\pm}14.88$) and between 11:00 and 13:00 for those in both intensive care units and emergency wards ($mean{\pm}S.D.\;67.50{\pm}62.93$ and $mean{\pm}S.D.\;39.67{\pm}35.96$, respectively). The characteristic variation in SAA was significant between 23:00 and 01:00 (p < 0.01) and for those in their fifties or older (p < 0.01). The activation ratio of alpha-amylase, a stress reactant, showed an increase when the sympathetic nervous system was activated by mental stress; in addition, job stress was manifested with the effect of awakening at different time segments and at different ages among the nurses. With the aim of raising the level of service based on the nurses maintaining their mental health, it is necessary to focus sharply on the time segment for critical control and to conduct repetitive studies to determine the divisions of eustress critical values as well as to expand the population.

  • 1. The purpose of this study. The purpose of this study is to plan and investigate short-term nursing education of two-years associate degree program to produce middle-level professional nurses which are needed by-society and nation. Current nursing education in Korea is divided into four years degree program, three years diploma program Even (though) there are differences in the aims of their education, the curriculums are not much different between the education for producing leaders which is its basic purpose and training middle-level professional nurses. Therefore the purpose of associate degree program lies in minimizing the waste of time and finance which are invested for long-term education for middle-level professional nurses. And also this coincide with the policy of national technical training and definite supply of nurse manpower according to health policy for effective role and ability of nurse. 2. The method of study. This is based on the study of literature, research on the actual condition and investigation of opinion- through questionnaire. L) The study of literature: Domestic and foreign literatures for two years associate degree program were studied and investigated. 2) Research on the actual condition : Current three years nursing education program was collected and analysed. 3) Investigation of opinion. The problem of curried nursing education system and the possibility of two years associate degree program were investigated through questionnaire. 3. The result of the study. 1) The trend of recent nursing education. a. The aims of nursing in past chiefly taking care of physical disease of patient has recently changed to nursing of character including physical, mental, socio-economic, educational and psychological condition. b. For the performing systematic and effective nurse's duty according to her role, the-change of educational system which is classified as a range of education the period of education and certificate after graduation has been enforced or fulfilled. c. Nursing education also has a trend to become a collage or two years associate degree program which can get same legal protection as other educational institutions whose basic purpose is education. Attached nursing school to hospital is getting disappeared because of disadvantage of educational system. 2) Problems. Depending upon research on actual condition of current 3 years nursing education program. a. There are too many subjects. b. Contents of education could be doubled because major subjects are subdivided in detail. c. The credits for graduation are too heavy comparing to the period of study or the ability of students. (The necessary credits are 150.8 for three years according to actual investigation 4. There is no certain standard in organizing curriculum therefore there are too much differences between schools. 4. Basic Plan. The plan for two years associate degree program in nursing education depending on demand of professional nursing field of society is based on following items. 1) Training middle-level professional nurse lay emphasis on liberal arts and basic major field. 2) Liberal arts are divided into required and optional subjects and students could take courses by choice. 3) Major subjects are compound together by fields and they become the sciences of nursing Ⅰ,Ⅱ,Ⅲ,Ⅳ and every items has its educational purposes and contents major study includes laboratory practice and clinical experience. 4) The required credits for graduation are to which means 17-18 credits a semester. The above has been planned to solve the problems of current three years nursing education program.
