A qualitative study on the case managers' experiences of implementing Assertive Community Treatment model in Korea (한국에서의 ACT(Assertive Community Treatment) 적용에 대한 사례관리자의 경험에 관한 연구)

  • The attention of case management currently has been highlighted and its importance has been highly emphasized in mental heath field. At this point, We conducted a qualitative study on the experience of case managers from the Suwon Community Mental Health Center where it has implemented the ACT model. Through the result of this study, we verified feasibility of ACT model into Korea. and it has shown that assertive service and team approach has been a critical ingredients of ACT and it extended case manager's ability and brought improvement in relationship between clients and them. the clients has brought a positive changes. However case managers have experienced the dilemma between experts initiatives and clients dependency, and they empathised the importance of the familism and mental health system in Korea. In order to implement ACT extensively, we suggested several implications.

Phenomenological Study on the Life of People with Schizophrenia (조현병을 가진 사람의 삶에 대한 현상학적 연구)

  • This study aimed to understand the meaning and essence inherent in the life experiences of people with schizophrenia. We approached with phenomenological method, and the research participants were 10 adults, aged 20 or older, who were diagnosed with schizophrenia. As a result, we found four essential themes. The participants with schizophrenia lived a life with special constraints of being thrown to the reality in which they experienced continuing suffering. Nevertheless, their daily lives were maintained on the basis of their own feelings and needs. In this study, we confirmed that they should be understood and treated in the mental health field with the perspective that they are not 'patients' nor a pathological outcome, but a 'human' with schizophrenia.

The evaluation of the appropriateness of resource allocation in a community health center (보건소 기능의 중요도에 따른 자원배분의 적절성 평가)

  • This study was performed to evaluate the appropriateness of resource allocation based on the ranking of health center function. Through the Delphi processes, health center functions were ranked in order of importance as follows; planning and research, followed by health education, health promotion, management of chronic diseases, health screening tests, welfare activities, mental health services, medical personnel management, medical services, prevention of communicable diseases, maternal and infant health services, housekeeping, management of oral hygiene, nutrition services, surveillance for community health services, family planning, and administration of the health center. In relation to the above priorities, the allocation of manpower was not appropriate. Even though the expert groups emphasized on functions such as planning and research, health education, and health promotion, they inputted more personnel for administration of a health center, maternal and infant health services, and medical services which were evaluated with lower importance. The budget allocation showed the same trends as the above. Although the functions such as planning and research, health education, and health promotion, and management of chronic diseases were evaluated highly, the budget was allocated accordingo to the the results of the former fiscal year rather than on the importance of function. However the budget for nutrition services, surveillance for the community health services, family planning, and administration of a health center was allocated according to priority. Based upon the above findings, community health center should be given the opportunity to make their own ranking of health center function and to allocate their resources including personnel and budget in order to improve the responsibilities and roles of the community health center.

Effects of Solution-focused Group Counseling on School Attitude, Self-esteem and Hope in School Maladjusted Adolescents (해결중심 집단상담이 학교부적응 청소년의 학교생활태도와 자아존중감 및 희망에 미치는 효과)

  • Purpose: This study was designed to examine the effects of solution-focused group counseling on the school attitude, self-esteem and hope in school maladjusted adolescents. Methods: The research design of this study was a non-equivalent control group pre-post test design. Thirty middle school students referred to a community mental health center in G city for school maladjustment from a school and welfare center were assigned to the experimental group and thirty middle school students in care for school maladjustment at a welfare center were assigned to the control group. The solution-focused group counseling for the experimental group was conducted once a week for 90 minutes during 6 weeks. The data were collected from December 10, 2003 to May 23, 2004. The data was analyzed by x2-test, t-test and paired t-test using the SAS program. Results: 1) School attitude increased significantly after solution-focused group counseling (t=6.036 p=.001). 2) Self-esteem increased significantly after solution focused group counseling (t=2.705, p=.010). 3) Hope increased significantly after solution-focused group counseling (t=3.671, p=.001). Conclusion: Solution-focused group counseling appears to be a very effective intervention for school maladjusted adolescents by increasing self-esteem, positive school attitude, hope and coping with problems.

Application of Concept Mapping in Program Planning for the Mental Disorders: Can be Achieved Consensus Expected Outcomes of the Mental Disorders and Community Psychiatric Rehabilitation Center Employees through Client Participation? (정신장애인을 위한 프로그램 기획에의 컨셉트 맵핑(concept mapping) 적용 : 클라이언트 참여를 통해 사회복귀시설 종사자와 정신장애인의 기대성과 합의를 이룰 수 있는가?)

  • This study apply concept mapping to realize of client participation and self-determination in social welfare program for the mental disorders. They are relatively easily marginalized in decision-making process of their program. But realization of client participation and self-determination is directly connected with effect of service. For this reason, we confirmed the applicability of concept mapping in program planning that support client participation. Case of this study is social welfare program of B community psychiatric rehabilitation center located in the A city. This program is community interchange service for the mental disorders. Interchange type is to have a food with the mental disorder and the solitary elderly. We took advantage of the concept mapping to derive the outcomes that are expecting the mental disorders and mental health social workers. Concept mapping was proceeding in six steps; preparation stage ${\rightarrow}$ idea collection stage ${\rightarrow}$ structuralization stage ${\rightarrow}$ analysis stage ${\rightarrow}$ interpretation stage ${\rightarrow}$ application stage. Participants were a total of 25 people including the mental disorders and community psychiatric rehabilitation center employees. The participants produced 42 statements. Sorting results, the mental disorders produced 6 clusters; community psychiatric rehabilitation center employees produced 3 clusters. The mental disorders classified better detail than community psychiatric rehabilitation center employees. Two group were found gap of expected outcomes each other, went narrowed it. They agreed 3 expected outcomes finally. We identified empirically the usefulness of concept mapping to realize self-determination and program participation.

Effects of the Upright Body Type Excercise on Lung Capacity and Depression of People with Mental Illness (바른체형운동 지도가 정신장애인의 우울감 및 폐활량에 미치는 효과)

  • The purpose of this study was to investigate the effects of the regular Upright Body Type Excercise on posture, muscle strength of leg, lung capacity and depression of people with mental illness. The subjects of this study were recruited from B mental health center (5 years and more mental illness patients, n=19) located in P city. For the exercise group, They were carried out the Upright Body Type Excercise Program during 8 weeks (60 min/time, 3 times/week). They were randomly divided into two groups. Exercise group (n=10) and Control group (n=9). And also, they were agreed with consent forms before the experiments. Research results were as follows. Through the upper body type exercise, there was significantly difference in PEF and FEV1/FVC in the trained group. And also, there was much improved in depression level in the trained group. The change of melancholy feeling before and after the program was not statistically significant. However, because of limitations of sampling size due to the peculiarities of the program participants, in consideration of the limit of statistical validation exists clearly, intended to better understand the subjective experience of attendees, qualitative analyzing(qualitative research) was carried out in parallel. It was conducted a deep interview only person accepted among program participants, thematic analysis, subject analysis tasks to be subdivided by classifying by considering the semantic units of what participants expressed, was thus carried out. It found that the degree of melancholy feeling of mental disorders who participated in the Upright Body Typed Exercise Program was reduced. The course of the experience of change in depression appered the three subjects and six sub-themes such as "the start of the change", "interest of the program", "recognition of the need of the body’s health", "physical health promotion", "recovery of physical function", "negative change of habits (attitude)", "reduction of sense of depression", "confidence that it is possible to", "hope for the future". Therefore, upper body type exercise is much helpful in lung capacity and mental health of people with mental illness. So, this type of exercise mostly needed in the people with mental illness group than the other group for the quality of life.

Health Inequalities among the Elderly : Mediation Effect of Social Participation Between Educational Level Inequalities and Self-rated Health·Depression (노인의 건강불평등 : 교육불평등에 따른 건강불평등에 대한 사회참여의 매개효과)

  • This research attempts to explain the influence of educational level inequalities on self-rated health and depression of the elderly. Also, we are focusing whether there is a mediating effect of social support between educational level inequalities and self-rated health depression of the elderly. The data was collected from July, 30 to August, 15, 2009. 631 persons who live in Gangnam-Gu area over 60 years of age were recruited. Frequency, percentage, mean, standard deviation and multiple regression were employed using SPSS 12.0. The result of this study shows that educational level inequalities have a influence on the self-rated health and depression. It is also verified that social participation variable has a partial mediating effect between educational level inequalities and mental health(self-rated health and depression). This study carried out a positive linear relationship between educational level and health: the higher education, the better the health. And also, the results present the importance of developing adequate intervention programs for the elderly having low educational level to improve social participation and to enhance mental health(self-rated health and depression).

Health Care Needs and Health Problems of the Subjects in a Health Center (보건소 방문간호 대상자의 건강문제와 방문간호 요구)

  • In order to investigate the basic data for the visiting nursing care plan of Health Center in Korea, the questionnaire survey in regard to health care needs and health problems of the subjects with visiting nursing care was carried out on 131 subjects with visiting nursing care. The results were as follows : 1. The subjects consisted of 38.2% in male and 61.8% in female. 77.1% of the subjects had no job. 2. In the health problem, the subjects symptoms were 31.2% of hypertension, 20.6% of arthritis, and 19.1% of diabetes and other chronical illness. Utilization of medical care facilities were 61.8% of private clinics or general hospitals and 29.0% of Health Center. 3. 10 areas of health care needs that subjects wanted were disease management(19.5%), demand for welfare concerns(15.7%), health promotion and disease prevention(14.2%), information for medical institution(12.3%), health management for the aged(10.8%), hospice care(8.0%), prevention of dementia(8.0%), care for mental hygiene(6.7%), management for drinking, smoking and drug abuse(3.3%), home care nursing and rehabilitation nursing(1.2%).

The Effectiveness of Brief Family Education Programs for Schizophrenic Outpatients and their Families in a Community Social Welfare Center. (퇴원한 정신분열병 환자와 가족에 대한 지역사회복지관에서의 단기가족교육 프로그램의 효과)

  • This study verified the effectiveness of brief family education programs for schizophrenic outpatients and their families in a community social welfare center, using a pre - post test control group design. The results of this study verified that the program was effective to improve knowledge about schizophrenia, reduce feelings of rejection and anxiety about patients, and help the patients to recognize their family emotional support. Therefore, it was found in this study that the brief family education program in the community social welfare center had a positive impact on schizophrenic outpatients and their families' treatment and rehabilitation. From the results of this study, it can be seen that this program positively effects the treatment and rehabilitation of schizophrenic patients and the stability of their families. The program can also be easily utilized as a community resource. Furthermore, the results of this study can be utilized as fundamental support to suggest the need for mental health programs in community social welfare centers. This study suggested the importance of increasing the number of mental health professionals working in this arena. There is also a need to create additional supportive programs for schizophrenic outpatients and their families. Follow up care also needs to be provided to families after completing family education programs. Continuous study is needed in these areas.

Institutional Ethnography on Organization of Case Management Practice at a Mental Health Center (정신보건센터 사례관리 실천의 조직화에 관한 제도적 문화기술지)

  • This study is a research that investigated organization of case management practice at a mental health center. For organization research of case management practice, this study applied the research method of Institutional Ethnography, and analyzed 'work knowledge' and 'texts' data which were obtained through field work that lasted 3 months with 11 study participants. The analysis is not interpretation of the researcher, but it is work that assembles 'work knowledge' of field workers until they reach discovering institutions that organize case management practice. As a result of this study, case management practice of the mental health center was being standardized as 'a service that is biased towards symptom management without comprehensive and continual resources links' by new public management theory, bio-medicine discussion, 'standardization policy', 'treatment rate improvement policy', 'performance-based budget system', 'performance evaluation system' and 'continual linkage policy', mediating texts. This is a different outcome from usefulness of case management that is claimed to support politically.
