Recent studies suggest that most meniscal cysts can be treated surgically by arthroscopic management of the meniscal tear and arthroscopic cyst evacuation. But arthroscopic cyst decompression may sacrifice a substantial amount of meniscal tissue that is not torn in order to expose the "stalk" of the cyst. Nowadays, the trend is changing as preserving the involved meniscus to prevent from inevitable degenerative changes after meniscectomy. The purpose of this report is to describe a new surgical technique that minimizes loss of meniscal tissue in hopes of maximizing residual meniscal function. We experienced 10 patients with meniscal cysts that were consisted of four lateral cysts and six medial cysts. Menisci were torn in all cases. Arthroscopic partial meniscectomy and decompression of cysts were performed in 9 cases, and arthroscopic partial meniscectomy and open cystectomy in 1 case. The procedures were consisted of injection of the methylene blue into the cyst, partial meniscectomy of the meniscal tear until the dye was seen in orifice of the cyst, and decompression of cyst through cystic opening. This article serves to confirm the relationship between torn menisci and cysts, to re-evaluate the occurrence ratio of the meniscal cyst on the medial to lateral meniscus, and to assess the efficacy of arthroscopic partial meniscectomy and decompression of cyst as a potentially meniscal sparing procedure.

Purpose : To analyze the clinical result of the arthroscopic decompression of meniscal cyst and meniscus resection or repair of meniscus tear. Materials and Methods : From April 1994 and October 2001, 19 patients with diagnosis of meniscal cyst associated with tears of the meniscus were treated by arthroscopic meniscal resection or repair with decompression of the cyst. The mean age was 39.8 years(range, 22-58years). The follow-up period ranged from 3 months to 36 months with an average of 18 months. Seven of 19 patients had tenderness over the joint line with palpable mass. Treatment consists of arthroscopic resection or repair of meniscal tear with decompression of the cyst through transmeniscal approach. Open excision of cyst was performed in one case. Clinical evaluation was performed using Lysholm knee score and Tegner activity. All cases were executed proper treatment using arthroscopy. Results : Twelve cysts involved the lateral meniscus$(64\%)$ and seven cysts were on medial cyst$(36\%)$. Most of lateral meniscal cysts were located in anterior one-third and medial meniscal cyst were on posterior one-third. Meniscal tear were observed in seventeen cases$(89.5\%)$ and most tears were horizontal$(79\%)$. Preoperative symptom disappeared and no cyst recurrences were observed at last follow-up(mean follow-up: 18 months). Conclusion : Meniscal cysts involved lateral side in $64\%$ and most of them were associated with meniscus tear$(89.5\%)$ which consists of mainly horizontal component$(79\%)$.

Meniscal cyst is an uncommon disease of knee joints, arising less frequently from medial meniscus than lateral. Most of they are accompanied with horizontal tear of meniscus. This is the case of 24 year old man who had 2 separate medial meniscal cysts with longitudinal tear on posterolateral side of medial meniscus without trauma. Treatment consisted of arthroscopic decompression of cysts and meniscal repair.

The meniscal cyst of knee joint is a rare disease, and the discoid meniscal cyst has not been reported in Korea. We report 3 cases of the discoid meniscal cyst confirmed by MRI and arthroscopy. In physical examination, all cases have tenderness of the knee joint. One has palpable mass of knee joint and the other one has limitation of knee motion with knee flexion contracture of 10 degrees and positive McMurray test at external rotation of the knee. All cases are complete discoid meniscus, which are, one medial discoid meniscus and two lateral discoid meniscus through MRI. We confirmed horizontal tear of meniscus in all cases and the location of meniscal cysts are anterior hem of meniscus in one and body of meniscus in two. We could get excellent results in all 3 cases that return to normal knee range of motion.

Extraarticular synovial chondromatosis is a very rare disease which affects the hands, feet, and wrists most commonly. In cases of involvement around large joints, the tissues around knee are usually involved. It arises from tendon sheath, capsular tissue and bursae. It is an idiopathic process in which the synovial cells undergo the metaplasia into cartilage cells. Chondroid matrix of the cartilaginous nodules calcify and ossify to be the osteochondroma. On simple radiograms before calcification and/or ossification of the chondroid tissues the cartilaginous nodules look normal on radiograms. Therefore MRI is needed to establish the diagnosis. We report a case of extraarticular pan-peri-meniscal synovial chondroma around right knee, initially suspected as a ganglion cyst which clinically mimicked a large rounded lesion or a cystic lesion on MRI.

Medial knee joint pain is a common problem in the field of orthopedics. In these patients, a high resolution ultrasound examination can reveal medial collateral ligament (MCL) bursitis, meniscal cyst, degeneration changes to the MCL and meniscal protrusion etc. Prolotherapy is effective in these patients. The author performed prolotherapy for MCL bursitis of the knee joint, and confirmed the disappearance of the bursitis using high resolution ultrasound.

Ganglion cysts have been described in all joints. In the knee, although the finding of meniscal degenerative cysts is fairly common, a few cases have been reported concerning ganglion cysts of anterior cruciate ligament. However, ganglion cysts of posterior cruciate ligament are quite rare. We experienced two cases of ganglion cyst in the posterior cruciate ligament, which were treated successfully with arthroscopic debridement.
