  • Modern treatment of diabetes with hypoglycemic agents have been resulted the Pathological changes. Therefore, the pattern identifications of Korean traditional medicine also have to change. The aim of this study is to learn about the recent change of the pattern identifications on diabetes mellitus. Chinese traditional medicine is more free than us in medical activities because of the combination of Western and Eastern medical treatment and so there has been much published studies. Therefore, 35 papers that were searched from 2003 to 2010 in China by the keyword '糖尿' & '消渴' through the date base system of Kisti & Cnki were selected and analyzed. 35 review papers were composed of 28 observing academically and 7 clinical research studies. The combination of Western and Eastern Medical treatment has been effective than Western Medical treatment alone and the stasis(瘀血) acted as an important etiology on Diabetes mellitus. In the treatment of diabetes the stasis should be considered consistently from the onset.

  • Bell's palsy is an isolated facial paralysis of sudden onset caused by a neuritis of the seventh nerve within the facial canal. It occurs often in the adult man with a history of recent exposure to local cold, such as sleeping next to an open window, or in some cases it occurs after infections of the nasopharynx or masticator spaces. Especially, this neuropathy have linked with the major collagen disorders (diabetes mellitus). A segmental demyelination develops rapidly, with vascultitis in microinfarcts and ischemia to the nerve segment. The authors experienced about the bizarre neurological symptom of Bell's palsy after inferior alveolar nerve block anesthesia and TMJ dislocation in diabetic mellitus. The early and correct consultation with the multiple medical and dental departments was important to prevent the inadequate care & medicolegal problems.

  • Purpose: The purpose of this study was to identify the effects of a cognitive behavioral stress management program on perceived stress, coping strategies, and cortisol in people with Diabetes Mellitus Type 2. Method: Patients with type 2 diabetes were recruited from of a public health center in an urban area. Thirty-five participants (experimental group=17, and control group=18) enrolled. The experimental group received small group-based cognitive behavioral stress management training for eighty minutes per session, once a week for eight weeks. Data were collected at baseline and ninth week for perceived stress, affect-oriented coping, problem-oriented coping, and cortisol. The data were analyzed using SPSS 12.0. Results: The experimental group had significantly less perceived stress (t=-9.82, P<.001) and cortisol (t=-2.14, p=.040) than the control group. No significant group differences were found in affect-oriented coping (t=-.43, p=.673) or problem-oriented coping (t=1.40, P<.170). Conclusion: These results suggest that a cognitive behavioral stress management program can have positive effects on perceived stress and cortisol in patients with diabetes mellitus type 2. Further research with a larger sample and for a longer period is needed to expand our understanding of the effects of the program for patients with diabetes mellitus type 2.

  • Objectives : There is a need to develop a data quality management algorithm to improve the quality of healthcare data using a data quality management system. In this study, we developed a data quality control algorithms associated with diseases related to hypertension in patients with diabetes mellitus. Methods : To make a data quality algorithm, we extracted the 2011 and 2012 discharge damage survey data from diabetes mellitus patients. Derived variables were created using the primary diagnosis, diagnostic unit, primary surgery and treatment, minor surgery and treatment items. Results : Significant factors in diabetes mellitus patients with hypertension were sex, age, ischemic heart disease, and diagnostic ultrasound of the heart. Depending on the decision tree results, we found four groups with extreme values for diabetes accompanying hypertension patients. Conclusions : There is a need to check the actual data contained in the Outlier (extreme value) groups to improve the quality of the data.

당뇨병증 골절렛트의 골치유 과정에서 맥동성 초음파가 미치는 영향 (The effects of Pulsed Ultrasound on the bone healing process in Fracture Model of Diabetes Mellitus Rat)

  • Purpose: The study to pulsed ultrasound effects of diabetes mellitus fracture model in rat. Methods: We used 36 Sprague-Dawely rats which were divided into 3 groups; the animals were divided into group of 4 rats each 4th, 14th and 28th days. All animal was induced diabetes mellitus model to used streptozotocin with 45 mg/kg. Pulsed wave were divided into $50\;mW/cm^2$ and $100\;mW/cm^2$. Results: T-ALP value was significantly change from group II, III on 14th, 28th days (p<0.05). Especially T-ALP value to between groups was significantly change from group II on 14th days (p<0.05). Osteocalcin value was significantly change from group II on 28th days(p<0.05). At fracture site, osteoblast, osteoclast expression was observed from 4th days after treatment and it reached its maximum intensity at 28th days. At fracture site, $TGF-{\beta}_1$ expression was observed from 4th days after treatment and it reached maximum intensity at 14th days. Conclusion: According to this study, diabetes mellitus fracture model to the more effective is divided into $50m\;W/cm^2$ pulsed ultrasound.

심장외막지방두께와 당뇨와의 연관성 연구 (The Relationship Between Epicardial Adipose Tissue and Diabetes Mellitus)

  • The incidence of cardiovascular disease increases rapidly after 40's. The thickness of the epicardial adipose tissue was measured to analyze the risk factors affecting the thickness change. We present the cut off value for the epicardial adipose tissue thickness for high impacted variables. For the study, 547 patients underwent echocardiography, epicardial adipose tissue thickness, body mass index, abdominal subcutaneous fat thickness and diabetes mellitus were used as analytical variables. As a result, multiple regression analysis of age group showed that diabetes mellitus was highly influential in all age group, so we could predict the cut off value for the epicardial adipose tissue thickness for diabetes mellitus. As a result of ROC curve analysis, cut off value of 7.44 mm was obtained with sensitivity of 79.6% and specificity of 60.6% for diab etes variab le. Logistic regression analysis b ased on this value showed that the risk for diab etes increased 6 times with each increase in the thickness of the epicardial adipose tissue. Among the various obesity indexes, epicardial adipose tissue is used as an important index especially to prevent the risk of cardiovascular disease. Therefore, we suggest that the use of echocardiography as a routine screening method for the prevention of cardiovascular disease and metabolic syndrome, which is rapidly increasing in patients over 40's age, will enable more effective monitoring.   • Objective:The Herbal Remedy for Diabetes Mellitus-01 (HRDM-01) is composed of 11 species of medicinal plants. HRDM-01 is used as anti-hyperglycaemic agent. Anti-hyperglycaemic agents in western medicine often have hepatotoxicities. Therefore, we investigated safety of HRDM-01, especially in liver functions. Methods:We investigated the effects of HRDM-01 on liver function measuring serum AST, ALT and LDH levels and histopathological changes of liver tissue using photomicroscope. In addition, we also investigated the effects on serum lipid levels such as total cholesterol, HDL-cholesterol and triglyceride. Results:In our experiment, single injection of streptozotocin elevated levels of AST and ALT in serum. But LDH level was not affected. In addition, our animal model showed elevated levels of total cholesterol and triglyceride in serum. 30 days treatment with HRDM-01 lowered serum AST level. Serum levels of ALT and LDH did not affected. In addition, HRDM-01 lowered serum triglyceride level significantly. In histopathological observation, we did not find any abnormal changes in all experimental groups. Conclusions:Briefly, HRDM-01 did not elevate serum levels of ALT, LDH, total cholesterol and did not affect histopathological changes in liver tissues. Moreover, HRDM-01 lowered serum AST, triglyceride, which are elevated by induction of diabetes mellitus. These results suggest the safety of HRDM-01 in diabetic treatment.

한국인 당뇨병에 관한 조사연구 (Studies on diet theraphy of diabetes mellitus among Koreans)

  • The incidence of diabetes mellitus among Koreans as shown an increasing tendency recently, probably due to the various factors such as the improving living conditions. The majority of people are ignorant or indifferent to the nature, progress and prognosis of diabetes mellitus in Korea. 107 cases of diabetes mellitus which had been admitted to Severance Hospital between January and August, 1971 were Studied. Of these 107 cases, 22 cases were interviewed thoroughly during their hospital stay and the response to their diet therapy was carefully checked. 1) Of the 107 cases 69 cases were male and 38 cases were female; the sex ratio was 18:1. The age of the onset of the disease was as follows: 2 cases were under 20 years of age; 20 cases (18.7%) were under 40 years of age and 85 cases (79.5%) were over 40 years of age. Juvenile diabetes was less frequent when compared with developed countries. 2) Patients complaints and symptoms on admission, complications of sickness, and duration of sickness until the female discharge were also studied. We found that the incidence of tuberculosis complication in diabetes melltius was alarming (13. 8%). 3) In most cases, the control of diabetes was inadequate and diet practisis by the patients was also very poor even when they had known of the diabetes mellitus for a considerable period of time. During hospitalization 75 cases (70.0%) were controlled by diet and oral medications alone. Only 16 cases (15.0%) needed insulin injection, the remaining 16 cases required both diet control and insulin injection. 4) In general, patients received hospital diet satisfactorily. Only a few cases complained of difficulties with milk intake because of no previous dietary experience or of excessive meat orfish because they preferred vegetables and fruit. 5) Patients responded well to the dietitians interviews in the hospital but follow up study and care were poorly organized after discharge from the hospital. 6) The diet exchange 1ist published by the Korean Diabetic Association was not well received by the patients or the general puplic because it is not inexpensive and detailed instructions were not given at the time of discharge from the hospital.

데이터마이닝 기법을 이용한 당뇨 발생 예측모형 개발 (A Development of a Predictive Model Using the Data Mining Technique on Diabetes Mellitus)

  • As prior studies indicate that chronic diseases are mainly attributed to health behavior, preventive health care rather than treatment for illness needs to improve health status. Since chronic conditions require long-term therapy, health care expenditures to treat chronic diseases have been substantial burden at national level. In this point of view, this study suggests that the health promotion program should be based on Knowledge Based System Using Data Mining Technique, we developed a predictive model for preventive healthcare management on diabetes mellitus. Generally, in the outbreak of diabetes mellitus there is a difference in lifestyle and the risk factors according to gender. So we developed a predictive model in accordance with gender difference and applied the Logistic Regression Model based on Data Mining process. The result of the study were as follow. The lift of the last predictive model was an average 2.23 times(male model : 2.13, female model 2.33) more improved than in the random model in upper 10% group. The health risk factors of diabetes mellitus are gender, age, a place of residence, blood pressure, glucose, smoking, drinking, exercise rate. On the basis of these factors, we suggest the program of the health promotion.
