Hunminjeongeum Phonetics (I): Phonetic and Phoniatric Consideration for Explanation of Designs of Middle Vowel Letters (훈민정음 음성학(I): 중성자(홀소리) 제자해에 대한 음성언어의학적 고찰)

  • Hunminjeongeum was made by the Great King Sejong, and composed of 17 consonant and 11 vowel letters. All the 28 letters were made according to the shape of vocal organ or space at the point of articulation for each letters. This review article focused on phonetic and phoniatric consideration for explanation of the designs of the middle vowel letters, especially three main vowel letters [ • (天, heaven), ㅡ (地, earth), ㅣ (人, human)] using video-fluoroscopic evaluation as well as computed tomography scanning, etc. During articulating / • / sound, a ball-like space at frontal portion of the oral cavity was found, tongue was contracted, and sound was deep (舌縮而聲深). During /ㅡ/ sound, a flat air space between oral tongue and hard palate was created. Tongue was slightly contacted neither deep nor shallow (舌小縮而聲不深不淺). During /ㅣ/ sound, tongue was not contacted and Sound is light (舌不縮而聲淺). Tongue was moved forward making longitudinal oro-pharyngeal air space. So, I'd like to suggest that we had better change the explanation drawing from a philosophical modeling to a more scientific modeling from real vocal tract space modeling during articulating middle vowels of Hunminjeongeum.

The Effect of Short-term Lumbar Stabilization Exercise for Lumbar Muscle Strength and Postural Balance on Chronic LBP (단기간의 요부안정화 운동이 만성요통환자의 근력과 자세균형에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the effect of short-term lumbar stabilization exercise to patients suffering from chronic low back pain(CLBP) on the strength of their lumbar flexor and extensor and the postural balance of them and to suggest effective treatment duration for them. METHODS: Study subjects were 20 patients with CLBP who's been reported the pain for more than 12 weeks. For these subjects, muscle strength of both extensor and flexor of lumbar and the balancing ability were measured before and after the intervention. Lumbar stabilization exercise was composed of matt, ball and sling exercise. The intervention was applied for total 12 sessions for 4 weeks (3 times a week) and each session was for 40 minutes. RESULT: For muscle strength of both extensor and flexor of lumbar, there was significant differences after the application of lumbar stabilization exercise(p<.05). For balancing ability, stabilization shows significant differences in all of the positions except PC(eyes closed on pillows)(p<.05). Distribution of weight shows no significant differences but it was generally distributed within normal range after the exercise and Fall index shows significant differences between before and after the exercise(p<.05). CONCLUSION: These results could have positive effect on deciding different periods of therapeutic intervention.

Effects of vibration resistance exercise on strength, range of motion, function, pain and quality of life in persons with tennis elbow

  • Objective: The purpose of this study was to investigate the effects of vibration resistance exercise (VRE) in patients with tennis elbow on strength, range of motion (ROM), function, pain and quality of life (QOL). Design: Randomized controlled trial. Methods: Twenty-seven participants were randomly assigned to either the VRE group (n=9), weight resistance exercise (WRE) group (n=9) or control group (n=9). Each group underwent hot compress for 15 minutes and electrotherapy for 15 minutes. The VRE group underwent exercises using a vibrating ball for 5 minutes. The WRE group underwent exercises using dumbbells for 5 minutes. All participants were evaluated on wrist extension strength using a hand-held dynamometer, grip strength level through an electric dynamometer, and ROM through a smartphone goniometer application before and after intervention. Patient-rated tennis elbow evaluation, Visual Analogue Scale and Short Form 8 were measured by questionnaires. Results: There were showed significant differences among the VRE, WRE and control group in wrist extensor muscle and hand grip strength, ROM, elbow function, pain and QOL after intervention (p<0.05). The VRE group showed a greater significant improvement in wrist extensor muscle strength and elbow function compared with the others (p<0.05). The VRE and the WRE groups had a more significant improvement in wrist extensor muscle and hand grip strength, ROM and pain compared with the control group (p<0.05). However, there were no significant differences in QOL among the three groups. Conclusions: Combining VRE with thermotherapy and electrotherapy appears to be more effective in improving wrist extension strength, ROM, elbow function, QOL in tennis elbow patients.

  • From 1958 to October 1977, 294 cases of acquired heart disease were operated. There were 68 cases of pericardium, 3 trauma, 2 foreign body, one cardiac thrombus, 3 atrial myxoma, 2 left atrial and 1 right atrial, 2 Budd-Chiari syndrome, and 214 valvular heart disease. Out of 214 cases of valve operation 73 valves were replaced in 64 patients. Male to female ratio was 1.46: 1. The youngest age was 14 years in male and 18 in female. The oldest was 54 years in male and 52 in female. Fifty-five cases of single valve were replaced, consisting of 47 mitral and 8 aortic valves. There were 9 double valve replacement cases which consist of 7 mitral and aortic and 2 mitral .and tricuspid valves. Six varieties of prosthetic valves, 3 ball and 3 disc types and 3 kinds of xenograft tissue valves were utilized. Beall, BjSrk-Shiley and Starr-Edwards prosthetic valves and Hancock valves were used mainly. For single valve 34. 5% and for double valve replacement 44% mortality were noted. There were 23 operative deaths out of 64 patients, over all mortality rate of 36.9%. Mortality for mitral valve replacement was 29.5%. But in recent 12 consecutive cases one death occurred, showing 8.3% mortality. In earlier days thrombocyte anti-adhesive drug dipyramidole-persantin-aspirin and/or SP 54 were adminstered. But in recent cases after heparinization, coumadin and Persantin were prescribed routinely.

The Effect of Exercise Program on Pain of Patients with Chronic Low Back Pain (운동프로그램이 만성요통 환자의 통증에 미치는 효과)

  • Purpose : This study was to find the effects of conservative therapy and sling therapy program of strength exercise and Gymnic Ball therapy program of stabilizer exercise and to find the effects of pain relieve through visual analogue scale(VAS) in chronic low back pain patients (male; 18, female;22). Methods : The measurement of pain was performed to both group by VAS from pre treatment to one to seven weeks. Results : 1. By the result of one-way ANOVA, Both group was the statistical significance in VAS during seven weeks(p<.05). 2. By the result of paired t-test, Both group was not statistical significance between the values of pre treatment and treatment after six and seven weeks but was statistical significance one week to seven weeks(p<.05). Conclusion : It was concluded that low back pain would be relieved by the progressive exercise and education for low back pain, and that the interventions would be helpful to prevent additional injuries by increasing the flexibility and muscular endurance.

Comparison of Effects of Abdominal Stabilization Exercise with Stabilizer and GymBall on Respiratory Muscle Activation (스태빌라이저와 짐볼을 이용한 배 안정화 운동이 호흡근 활성도에 미치는 영향 비교)

  • PURPOSE: The purpose of this study was to identify the effect of abdominal stabilization exercise with Stabilizer and Gymball. METHODS: The participants were divided randomly into 2 groups each of 20 participants in the gymball exercise group and 20 participants in the exercise group. The changes of respiratory muscle activity between the two groups were measured three times a week for 4 weeks. The activity of rectus abdominis and internal oblique was measured using surface EMG to measure respiratory muscle activity. The training was repeated five sets, and one set was repeated 6 times with one 10 second exercise and 5 second rest. RESULTS: The results of the study showed that there was a significant difference in the amount of respiratory accessory muscle activation, inspiratory pressure, respiratory minute volume at the experimental group with stabilizer and control group with gymball exercise. Between-group difference was statistically significant only for rectus abdominis muscle activation. CONCLUSION: Both the stabilizer and the gymball exercises were effective in improving the respiratory rate but there was no difference between the two groups. Also, when comparing activity of muscle in both group, both groups were effective in improving and more effective than improvement of muscular activity of stabilizer in muscle of stabilizer.

Patellar Tendinitis in the adult Volleyball Players (성인 배구 선수에서 발생하는 슬개건염)

  • Purpose: To estimate the prevalence and incidence of patellar tendinitis, to discover the patellar tendinitis in top level male volley ball players. Materials and Methods: Thirteen members of the male adult volleyball players were included in this study. From history and physical examination, 3 of the 13 players had patellar tendon pain associated with activity and were diagnosed as patellar tendinitis at the time of the study. Investigators were blinded about the injury status of the players. For all players, data such as age, volleyball experience, amount of training and competition hours were recorded. Results: Of the thirteen cases, two spikers complained pain on the lower pole of the patella, one setter on the upper pole. Conclusion: This findings suggest that patellar tendinitis in the volleyball players are influenced by the amount of the practice and their position. In most cases, the supervised control of the practice will reduce the overuse injuries in the volleyball players.

Intermittent Bleeding Method after Replantation o the Distal Phalanx (원위 수지 재접합술 뒤 간헐적 실혈 요법)

  • Purpose: To evaluate the effect of intermittent bleeding method in the distal phalanx replantation. Materials and Methods: From January 2007 through June 2009, authors have replanted 117 cases of distal phalangeal amputation in adults at Soo Hospital and Chonbuk National University Hospital. Cases of zone II were 60 cases and zone III 57 according to Allen classification. Male to female ratio was 8.7:1.3. The most common cause was machinery injury in the factory, 98 cases(83.8%), next one was belt injury of the machine, 11 cases(9.4%) and others, 8(6.8%). At least one digital artery and digital nerve were anastomosed under the operating microscope, but vein was impossible to anastomosis as unable to find out in the zone II and III. After anastomosis of one or more digital arteries and nerves, heparine(6,000-10,000 units) was kept to intravenous injection for 24 hours and at the same time fish mouth incision in 2-3 millimeter diameter was made in the distal radial and ulnar margin of the replanted distal phanlanx. From the first 30 minutes to an hour after replantation, incision site was swabbed with heparinized cotton ball for 5 minutes in every 30 to 40 minutes to make sure perfusion for 24 hours, every an hour at the second day, every two hours at the postoperative third to fifth day. Results: 92 cases(78.6%) was completely survived at average postoperative third week follow-up and satisfied with preservation of the finger nail, digit length, good range of motion of the distal interphalangeal joint and acceptable sensibility at average 1.2 years follow-up. Conclusions: Intermittant bleeding method in replantation of crushed distal phalanx impossible to anastomosis of vein at zone II and III of Allen classification was regarded as one of the notable salvage procedure.

Seasonal Variation of Total Body Fat in Men (사람의 피부두겹 및 총지방량에 관한 연주 -제1편 총지방량의 계절적 변동-)

  • Skinfold thickness measurement and total body fat calculation were made in summer (July) and winter (December or January) On 70 medical students (age: 20 yr),8 national team basketball players (age: 20 yr), and 9 middle-age men. Skinfold thickness measurements were made on 4 sites, namely, back, arm, waist and abdomen. The mean skinfold thickness (mm) of the 4 sites was substituted into the following formulae. For adult of 20 years old: % Fat=0.911x+8.1, and for middle-aged men % Fat=1.199x+1.41. In young medical students and ball players body weight decreased, mean skinfold thickness increased in winter season. As a result total body fat .(% body weight) increased in winter In middle-aged men both body weight and mean skinfold thickness increased in winter and resulted in an increase in the total body fat. The detailed data are as follows: 1. In medical students summer: winter values were: body weight, 59.7 :58.9 kg; mean skinfold thickness, 7.85 : 8.12 mm; and total body fat, 15.0 : 15.5% body weight (P<.30). 2. In national team basketball players summer: winter values were: body weight, 73.5 : 69.1 kg; mean skinfold thickness, 7.2 : 7.5 mm; total body fat, 11.6 : 12.1% (P:NS). 3. In middle-aged men summer vs winter values were: body weight, 61.5 : 63.0 kg; mean skinfold thickness, 10.3 : 11.8 mm; total body fat, 17.2 : 18.0% (P:NS). 4. Skinfold thickness on back showed no seasonal variation and on abdomen the thickness increased in winter. 5. It was concluded that the predominant factor in increasing total body fat in winter is the decrease in body exercise in the winter time.

  • Purpose: This study was conducted to test the impact of The Dynamic Visual-Motor integration training has effect on the visual perception reaction velocity. Dynavision were used to measure data from the participating 24 students(K college). Method : The participants were the 24 students of 'K' College in Busan in there twenties. They were divided into the The Dynamic Visual-Motor integration training group and the control group. To know if the Dynamic Visual-Motor integration training has effect on the visual perception reaction velocity, the Dynamic Visual-Motor integration training was implemented triweekly for 4 weeks. In Dynamic Visual-Motor integration training the ball should be grasped with one hand and threw by an arm. Only the balls threw beyond the objective point were counted. The visual perception reaction velocity and the number of response were measured before and after experiment by Dynavision. Result : Firstly, the visual perception reaction velocity was increased in Dynamic Visual-Motor integration training group compared with control group. Secondly, the number of response was also increased in Dynamic Visual-Motor integration training group compared with control group. Conclusion : As a result of The Dynamic Visual-Motor integration training has an effect on the visual perception reaction velocity and the number of response. The Dynamic Visual-Motor integration training seems to be effective for cerebral apoplexy patient who has visual perceptional disability or cerebral palsy child in training for visual perceptional development or daily living activities development. Study participated by more detailed and practical patients in hospital is needed.
