Treatment Planning and Dosimetry of Small Radiation Fields for Stereotactic Radiosurgery (Stereotactic Radiosurgery를 위한 소형 조사면의 선량측정)

  • The treatment planning and dosimetry of small fields for stereotactic radiosurgery with 10 MV x-ray isocentrically mounted linear accelerator is presented. Special consideration in this study was given to the variation of absorbed dose with field size, the central axis percent depth doses and the combined moving beam dose distribution. The collimator scatter correction factors of small fields $(1\times1\~3\times3cm^2)$ were measured with ion chamber at a target chamber distance of 300cm where the projected fields were larger than the polystyrene buildup caps and it was calibrated with the tissue equivalent solid state detectors of small size (TLD, PLD, ESR and semiconductors). The central axis percent depth doses for $1\timesl\;and\;3\times3cm^2$ fields could be derived with the same acuracy by interpolating between measured values for larger fields and calculated zero area data, and it was also calibrated with semiconductor detectors. The agreement between experimental and calculated data was found to be under $2\%$ within the fields. The three dimensional dose planning of stereotactic focusing irradiation on small size tumor regions was performed with dose planning computer system (Therac 2300) and was verified with film dosimetry. The more the number of strips and the wider the angle of arc rotation, the larger were the dose delivered on tumor and the less the dose to surrounding the normal tissues. The circular cone, we designed, improves the alignment, minimizes the penumbra of the beam and formats ball shape of treatment area without stellate patterns. These dosimetric techniques can provide adequate physics background for stereotactic radiosurgery with small radiation fields and 10MV x-ray beam.

The Usefulness of Endobronchial Ultrasonogram for Peripheral Lung Lesion (폐주변부 병변의 진단시 기관지 초음파(Endobronchial Ultrasonogram)의 유용성)

  • Background: Endobronchial ultrasonogram (EBUS) has increased the diagnostic yield of a bronchoscopic biopsy of peripheral pulmonary lesions (PPL). This study evaluated the diagnostic yield of EBUS-guided transbronchial biopsy (TBB) and the visibility of EBUS PPL. Methods: Between August 2007 and November 2008, 50 patients (32 men and 18 women, median age, 61.1${\pm}$10 yrs; range, 16 to 80 yrs) whose PPL lesions could not be detected with flexible bronchoscopy were enrolled in this study. Among the 50 patients, 40 cases were malignant lesions (adenocarcinoma 25, squamous cell carcinoma 10, small cell carcinoma 5) and 10 cases were benign lesions (tuberculoma 7, fungal ball 1, other inflammation 2). Results: The mean diameter of the target lesion was 35.4${\pm}$4.3 mm. Of the 50 patients examined, the overall diagnostic yield by EBUS-TBLB was 46.0% (23/50). The visualization yield of EBUS was 66.0% (33/50). A definitive diagnosis of PPL localized by EBUS was established using EBUS-TBLB in 69.6% (23/33) of cases. The diagnostic yields from washing cytology and brushing cytology from a bronchus identified by EBUS were 27.0% and 45.4% respectively. The diagnostic yields reached 78.7% when the three tests (washing cytology, brushing cytology and EBUS-TBLB) were combined. The visualization yield of EBUS in lesions <20 mm was significantly lower than that in lesions ${\geq}$20 mm (p=0.04). The presence of a bronchus leading to a lesion (open bronchus sign) on the chest CT scan was associated with a high visualization yield on EBUS (p=0.001). There were no significant complications associated with EBUS-TBLB. Conclusion: EBUS-TBLB is a safe and effective method for diagnosing PPL. The lesion size and open bronchus signs are significant factors for predicting the visualization of EBUS.

  • Purpose: The purpose of this article is to identify and understand the complications of RTSA and to review the current methods of preventing and treating this malady. Materials and Methods: Previous constrained prostheses (ball-and-socket or reverse ball-and-socket designs) have failed because their center of rotation remained lateral to the scapula, which has limited of the motion of the prostheses and produced excessive torque on the glenoid component, and this leads to early loosening. The Grammont reverse prosthesis imposes a new biomechanical environment for the deltoid muscle to act, thus allowing it to compensate for the deficient rotator cuff muscles. Results: The clinical experience does live up to the lofty biomechanical concept and expectations: the reverse prosthesis restores active elevation above $90^{\circ}$ in patients with a cuff-deficient shoulder. However, external rotation often remains limited and particularly in patients with an absent or fat-infiltrated teres minor. Internal rotation is also rarely restored after a reverse prosthesis. Failure to restore sufficient tension in the deltoid may result in prosthetic instability. Conclusion: Finally, surgeons must be aware that the results are less predictable and the complication/revision rates are higher in revision surgery than that in the first surgery. A standardized monitoring tool that has clear definitions and assessment instructions is surely needed to document and then prevent complications after revision surgery.

Effects of Stabilization Exercise with and without Respiratory Muscle Training on Respiratory Function and Postural Sway in Healthy Adults (호흡근훈련 유무에 따른 안정화 운동이 건강한 성인의 호흡 기능과 자세 동요에 미치는 영향)

  • Purpose : Stabilization exercise and respiratory muscle training are used to train trunk muscles that affect postural control and respiratory function. However, there have been no studies that combine stabilization exercise and respiratory muscle training. The purpose of this study is to investigate effects of stabilization exercise with and without respiratory muscle training on respiratory function and postural sway. Methods : Fifteen healthy adults were recruited for this experiment. All the subjects performed stabilization exercise with and without respiratory muscle training. For stabilization exercise with respiratory muscle training, the subjects sat on a gym ball wearing a stretch sensor. The subjects inspire maximally as long as possible during lifting one foot off the ground, alternately for 30 seconds. The stretch sensor was placed on both anterior superior iliac spine (ASIS), and the stretch sensor was used to monitor inspiration. For stabilization exercise without respiratory muscle training, the subjects sat on a gym ball and lifted one foot off the ground, without respiratory muscle training. Kinovea program used to investigate postural sway tracking during exercise. The maximum inspiratory pressure (MIP) and maximum expiratory pressure (MEP) were measured using a spirometer to investigate changes of respiratory muscle strength before and after exercise. A paired t-test was used to determine significant differences postural sway tracking, MIP, and MEP between stabilization exercise with and without respiratory muscle training. Results : There were significantly lower a distance of postural sway tracking during stabilization exercise with respiratory muscle training, compared with stabilization exercise without respiratory muscle training (p<.05). The MIP and MEP were significantly increased after stabilization exercise with respiratory muscle training compared with before stabilization exercise with respiratory muscle trianing (p<.05). Conclusion : The results of this study suggest that stabilization exercise with repiratory muscle training would be recommended to improve postural control and respiratory muscle strength.

Long-Term Survival Benefit of the Bronchial Arterial Embolization for Patients Presenting with Non-Traumatic Hemoptysis in a District Emergency Center (권역 응급의료센터에 내원한 비외상성 객혈 환자에서 기관지 동맥 색전술의 장기 생존 효과)

  • Background : This study was conducted to evaluate the survival benefit of the bronchial arterial embolization (BAE) for patients presenting with non-traumatic hemoptysis. Methods : The clinical data were retrospectively collected from the medical records and the Order Communicating Systems (OCS). The information dealing with death was collected from national death certificates. After enrolled patients were divided with two group such as BAE group (patients who were managed with BAE) and non-BAE group (patients who were managed with conservative modality), the survival benefit of BAE was estimated during the observational period of 24 months through using the Kaplan-Meier survival graph and the Cox-proportional hazard regression analysis. Results : The number of total cases was 272. Of these, BAE group involved 63 and non-BAE group involved 209. 69 cases had the malignant pulmonary lesions, 149 cases had non-malignant chronic lung lesion such as the mycobacteria infection, fungus ball, or bronchiectasis (BE), and 54 cases had the other pathologic conditions. For each sub-groups such as 'malignant lung lesion' group, 'non-malignant chronic lung lesion' group as well as about all cases, the adjusted hazard ratios (HRs) of BAE for death was not significantly different compared to the conservative management. But the adjusted HRs as to underlying causes such as 'malignant lung lesion' group and 'the other conditions' group increased significantly compared to 'non-malignant chronic lung lesion' group. Conclusion : There was no significant survival benefit by BAE procedure on survival in patients presenting with non-traumatic hemoptysis.

Molecular and Morphologic Identification of Spirometra ranarum Found in the Stool of African Lion, Panthera leo in the Serengeti Plain of Tanzania

  • The present study was performed with morphological and molecular analysis (cox1 and nad1 mitochondrial genes) to identify the proglottids of spirometrid tapeworm found in the stool of an African lion, Panthera leo, in the Serengeti plain of Tanzania. A strand of tapeworm strobila, about 75 cm in length, was obtained in the stool of a male African lion in the Serengeti National Park ($34^{\circ}$ 50' E, $02^{\circ}$ 30' S), Tanzania, in February 2012. The morphological features of the adult worm examined exhibited 3 uterine coils with a bow tie appearance and adopted a diagonal direction in the second turn. The posterior uterine coils are larger than terminal uterine ball and the feature of uteri are swirling rather than spirally coiling. The sequence difference between the Spirometra species (Tanzania origin) and S. erinaceieuropaei (GenBank no. KJ599680) was 9.4% while those of S. decipiens (GenBank no. KJ599679) differed by 2.1% in the cox1 and nad1 genes. Phylogenetic tree topologies generated using the 2 analytic methods were identical and presented high level of confidence values for the 3 major branches of the 3 Spirometra species in the cox1 gene. The morphological and molecular findings obtained in this study were nearly coincided with those of S. ranarum. Therefore, we can know for the first time that the African lion, Panthera leo, is to the definitive host of this tapeworm.

Analysis of Patients with Hemoptysis in a Tertiary Referral Hospital

  • Background: This study attempted to investigate the main causes of hemoptysis, the type of examinations used for diagnosis, the treatment modalities and outcomes. Methods: A retrospective study was conducted on the medical records of 221 patients admitted to the Chonnam National University Hospital, between January 2005 and February 2010, with hemoptysis. Results: Bronchiectasis (32.6%), active pulmonary tuberculosis (18.5%), fungus ball (10.8%), and lung cancer (5.9%) accounted for most causes of hemoptysis. Computed tomography scan was the most sensitive diagnostic test when employed alone, with positive yield of 93.2%. There were 161 cases of conservative treatment (72.9%), 42 cases of bronchial artery embolization (BAE) (19.0%), and 18 cases of surgery (8.1%). Regarding the amount of hemoptysis, 70 cases, out of 221 cases, were mild (31.5%), 36 cases moderate (16.2%), and 115 cases massive hemoptysis (52.0%). Most of the patients were treated conservatively, but if there was more bleeding present, BAE or surgery was more commonly performed than the conservative treatment ($p{\leq}0.0001$). In the multivariate model, severe hemoptysis and lung cancer were independently associated with short-term recurrence. BAE was independently associated with long-term recurrence, and lung cancer was associated with in-hospital mortality. The overall in-hospital mortality rate was 11.3%. Conclusion: Hemoptysis is a common symptom with a good prognosis in most cases. However, patients exhibiting massive bleeding or those with malignancy had a poorer prognosis. In-hospital mortality was strongly related to the cause, especially in lung cancer.

A Study on a Morphological Identification of Pinelliae Tuber, Typhonii Flagelliformis Rhizoma and Arisaematis Rhizoma (반하(半夏), 수반하(水半夏) 및 천남성(天南星)의 형태(形態) 감별에 관한 연구)

  • Objectives : A morphological classification among Pinelliae Tuber, Typhonii Flagelliformis Rhizoma and Arisaematis Rhizoma was made through microscopic observation. Method : The slice of the tested material made by paraffin section technique was colored with Safranine Malachite Green contrast methods, and then observed and photographed by olymphus-BHT. Result : 1. Pinelliae Tuber is like a ball with a $1{\sim}1.5cm$ diameter, with a concave apex and pockmarked root trace around. 2. Typhonii Flagelliformis Rhizoma is somewhat oval or like a cone, with a $4{\sim}8mm$ diameter, a $7{\sim}15mm$ height, a projected round apex, and a pin-pointed back. 3. Arisaematis Rhizoma is somewhat flat round with a $2{\sim}2.5cm$ diameter, a little than Pinelliae Tuber, and its apex has incomplete lobopodium and root trace. 4. Fundamental tissues of Pinelliae Tuber, Typhonii Flagelliformis Rhizoma, and Arisaematis Rhizoma have starch grains and mucilage cells, and their mucilage cells include the raphides of calcium oxalate. Conclusion : Internal forms of Pinelliae Tuber, Typhonii Flagelliformis Rhizoma, and Arisaematis Rhizoma are difficult to distinguish from one another, but Pinelliae Tuber and Arisaematis Rhizoma can be distinguished by their external forms and sizes, and Pinelliae Tuber and Typhonii Flagelliformis Rhizoma were distinguished by the forms of apex.

NMR Studies on Turn Mimetic Analogs Derived from Melanocyte-stimulating Hormones

  • Oligomers with $\alpha$-aminooxy acids are reported to form very stable turn and helix structures, and they are supposed to be useful peptidomimetics for drug design. A recent report suggested that homochiral oxa-peptides form a strong eight-member-ring structure by a hydrogen bond between adjacent aminooxy-acid residues in a $CDCl_3$ solution. In order to design an $\alpha$-MSH analog with a stable turn conformation, we synthesized four tetramers and one pentamer, based on $\alpha$-MSH sequence, and determined the solution structures of the molecules by two-dimensional NMR spectroscopy and simulated annealing calculations. The solution conformations of the three peptidomimetic molecules (TLV, TDV, and TLL) in DMSO-$d_6$ contain a stable 7-membered-ring structure that is similar to a $\gamma$-turn in normal peptides. Newly-designed tetramer TDF and pentamer PDF have a ball-type rigid structure that is induced by strong hydrogen bonds between adjacent amide protons and carbonyl oxygens. In conclusion, the aminooxy acids, easily prepared from natural or unnatural amino acids, can be employed to prepare peptidomimetic analogues with well-defined turn structures for pharmaceutical interest.

Electron Microscopic Study on the Effects of the Kainic Acid in the Duck Retina (Kainic Acid가 미성숙 오리 망막에 미치는 영향에 관한 전자현미경적 연구)

  • In order to identify the effects of the kainic acid in the retina kainic acid (120 nmol/$5{\mu}l$) were injected through the pars plana of the ciliary body into the duck eye ball at 7th day after hatching. The animals were sacrificed by decapitation on the 1st, 4th and 10th day after injection of the kainic acid. The specimen was processed for the light and electron microscopes respectively. The results obtained were as followings 1. The inner nuclear, inner plexiform and ganglion cell layers of the retina showed marked changes in all of the groups. 2. The cells in the inner nuclear layer showed marked degenerative changes of the organelles including the pyknoses and destruction of the nucleus on the 1st day after injection. These destructive changes of the cells were progressively disappeared until the 10th day after injection. 3. After injection of the kainic acid the neurites of the inner plexiform layer were swollen at the first, and thereafter those changes were gradually disappeared until the 10th day. 4. In the ganglion cell defected by the kinic acid, the enlargement and segmentation of the endoplasmic reticulum and pyknosis of the nucleus were-observed in all the groups, although the lipid droplets were only shown in the 4th day. From the above results, the evidence suggests that kainic acid gives to the toxic effect on the nuurons of the retina with exception of the photoreceptors.
