Effects of Image-Use Medication Education on Knowledge, Self-Efficacy, and Misuse of Medication in Elderly Inpatients (이미지 활용 약물교육이 노인환자의 약물지식, 복약자기효능감 및 복약오류에 미치는 효과)

  • Purpose: This quasi-experimental study was done to develop image-use medication education for older inpatients and to evaluate the effects on their knowledge, self-efficacy, and misuse of medication. Methods: Fifty nine elders (experimental group - 30, and control group - 29) received medication education once a week for 3 weeks. Data were collected before (pretest), right after (post 1), and 4 weeks after the program finished (post 2). Drug Use Knowledge Scale, Self-efficacy for Appropriate Medication Use Scale, and Drug Misuse Scale were used. Analysis included descriptive statistics, $x^2$ test, repeated measured ANOVA. Results: Findings showed significant differences in knowledge of medication and drug misuse between groups according to time(pretest, post 1, and post 2). At post 1 and post 2, knowledge and self-efficacy levels were significantly higher and misuse scores were lower in the experimental group compared to the control group. Conclusion: Findings indicate that elder-tailored medication education consisting of group education and individual guidance with images is effective and practical for medicine safety in elderly inpatients. Moreover, it could lead to a healthier life for elders, even elders with multiple chronic diseases and taking several medications.

Medication Status and the Effects of a Medication Management Education Program for the Elderly in a Community (지역사회 노인의 약물복용실태와 약물관리 프로그램의 효과)

  • Purpose: This study investigated medication status by examining the effects of a medication management education program on the knowledge of medications and medication misuse behaviors in the elderly in a local community. Methods: This study used a non-equivalent control group quasi-experimental design. For the study, 116 subjects were assigned to the control group and another 116 subjects were assigned to the experimental group. The medication management education program consisted of 1:1 education, practice in medication management, consultation, and discussion. Data were analyzed using the SPSS 21.0 program. Results: Statistically significant differences were found between the experimental and control groups in terms of their knowledge of medications and medication misuse behaviors. Conclusion: The results indicate that the medical management education program is effective in improving the knowledge of medications and decreasing medication misuse behaviors. Therefore, this education program can be used as an intervention to improve the medication behaviors of the elderly in local communities.

Experiences of Medication Misuse and Abuse Behaviors in Elderly People with Chronic Disease in Community (지역사회에 거주하는 만성질환 노인의 약물 오·남용행위 경험)

  • The purpose of this study was to understand the essential meanings in experiences of medication misuse and abuse behavior in elderly people with chronic disease in community. The participants were 10 patients with chronic disease over 65 ages who were selected by convenience sampling. Data were collected through individual in-depth interviews from February to May 2014, and were analyzed using Colaizzi (1978)'s phenomenological analysis method. As a results, the essential meanings and structure of medication misuse and abuse behavior experience in elderly people with chronic disease were derived into five clusters of themes as follows: 'Burden on the medications', 'Anxiety about the side effect of the medications or the disease exacerbation', 'There is no alternative other than the medications', 'Expectations of the medications effect for my criteria', 'Lack of understanding on the correct medication use'. The results of this study contributed to promoting a deeper understanding of medication misuse and abuse behaviors in elderly people with chronic disease, and provided for the basic data of strategic development for the prevention of medication misuse and abuse behaviors from the perspective of elderly people with chronic disease in community.

Evaluation of a Medication Self-management Education Program for Elders with Hypertension Living in the Community (지역사회 고혈압 노인의 약물 자기관리 교육 프로그램의 효과)

  • Purpose: The purpose of this study was to examine the effect of a medication self-management education program on medication awareness, communication with health care provider, medication misuse behavior, and blood pressure in elders with hypertension. Methods: The research design for this study was a non-equivalent control group quasi-experimental design. Participants were 23 elders for the control group, and 26 elders for the experimental group. The experimental group participated in the medication self-management education program which included the following, verbal education, 1:1 consultation, practice in medication self-management, and discussion over 5 sessions. Data were analyzed using the SPSS 18.0 program. Results: There were statistically significant differences between the experimental and control group for medication awareness, medication misuse behavior, and communication with health care providers. However, no significant difference was found between the two groups for blood pressure. Conclusion: The results indicate that the education program is effective in improving medication awareness and communication with health care providers and in decreasing medication misuse behavior. Therefore, it is recommended that this education program be used as an effective intervention for improving medication self-management for elders with hypertension.

A Convergence Factors Associated with Medication Adherence among the Elderly in the Community (지역사회 노인의 약물복용이행과 관련된 융합 요인)

  • This study is a descriptive investigation study to identify the Convergence factors of the subjective health status, communication with medical personnel and drug misuse on medication adherence. The subjects of this study were 179 senior citizens who used community elderly welfare centers. Data were collected from January 13, 2020 to January 17, 2020. Data analysis was performed using frequency, percentage, mean, standard deviation, t-test, ANOVA. Post-Hoc Test Was performed using Scheffe test, Pearson's correlation coefficients and multiple regression analysis. The regression equation of this study explained 24% of medication adherence. Drug misuse(𝛽=-4.32, p=<.001) was the factor that had the greatest influence on the medication adherence, followed by the presence or absence of chronic disease (𝛽=-3.04, p=.003), marital status (𝛽=2.64, p=.009), and communication with medical personnel (𝛽=2.26, p=.025 ) in that order. Therefore, it is necessary to establish a convergence system of medication adherence for the elderly using community welfare centers.

Factors Associated with Drug Misuse Behaviors among Polypharmacy Elderly (다약제 복용 노인의 약물 오용 행위에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to investigate factors associated with drug misuse behaviors among polypharmacy elderly. Methods: This was a cross-sectional survey. Participants consisted of 116 polypharmacy elderly who were taking 5 or more medications each day. Data were collected via face to face interviews. Data were analyzed using the PASW 18.0 program. Data concerning predisposing factor (knowledge, benefit), enabling factor (communication with health care provider), and need factor (perceived health status, number of disease) were collected. Results: The total mean score of drug misuse behaviors among polypharmacy elderly was 3.04 out of 10 points. Communication with health care provider, perceived health status, and knowledge were found to be significantly correlated with drug misuse behaviors. In stepwise multiple regression analysis, a total of 42% of the variance in drug misuse behaviors was accounted for communication with health care provider, perceived health status, and knowledge. Conclusion: Therefore, education program for improving communication with health care provider, and knowledge should be designed and provided for polypharmacy elderly.

Factors Affecting Medication Errors and Medication-related Educational Needs of Community dwelling Older Adults (지역사회 거주 노인의 약물오용 영향요인: 약물지식 및 복약관리 교육요구를 중심으로)

  • Purpose: The purpose of this study was to investigate the factors affecting medication errors and the medication management educational needs of community-dwelling older adults. Methods: From February 20 to February 23, 2017, 150 elderly people aged 65 or older were surveyed using a structured questionnaire. Results: A total of 85.7% of the older adults were taking medication, but their drug knowledge was found to be low. The medication error rate was 24.9%, and the score for medication management education requirement was 3.61 out of a possible 5points. Factors affecting medication errors were perceived health status and knowledge of medication, and their explanatory power was 43% in total. Conclusion: It was concluded that nursing intervention is needed to reduce older adults' medication errors and to increase their knowledge of medication. Additionally, groups of older adults with high medication errors should be intensively educated, and when developing a medication management education program, the contents of the sub-areas and items in which the participants' needs were high should be reinforced.

Development of an RFID Goggle to Deliver Medication Information to the Blinds (시각장애인 의약품 정보 안내를 위한 RFID 고글 개발)

  • Recent development of ubiquitous technology prompted its application for the blinds. In this work, we developed an RFID goggle equipped with a voice guide for the blinds who have difficulties in obtaining medication informations. When a blind, wearing our RFID goggle, tags the RFID tag attached to a drug, the RFID goggle announces the already saved medication information. We used STM32 as MCU and MFRC523 for RFID chipset in the reader. Our miniaturized hardware supported ISO 14443A protocol, and the standardized conformance test was performed. When the blinds wear our RFID goggle, they can conveniently access to the medication informations, and thus the drug misuse cases may decrease.

  • Objectives : Focusing on Wujutong's critiques for Xiebaisan misuse, this study investigates how Xiebaisan and its constituent drugs have historically been used by medical doctors and analyzes the disputes in the controversial context with a summary. Methods : The contents of Xiebaisan prescription were collected and examined from the past literature and medical database, and the issues related to the debate were examined by analyzing the contents. Results : In the results of exploring the opinions of medical doctors, in general, Xiebaisan and Sangbaipi, a major medicinal drug, were seen to play a role in the work of reinforcing and reducing. In particular, Ye Tianshi was used with cold and pungent medicine to Xiebaisan as a method of releasing heat with pungent medicine for acute fever in spring, and at the same time, care was taken to avoid depletion of secretion in the stomach. Conclusions : Wujutong's critique is that in early symptoms of viral infection, the misuse of Xiebaisan seems to emphasize the adverse effects of the illness. It is hoped that further research on Xiebaisan incremental medication will be conducted in the future to help clinical applications of Xiebaisan system medication.

Inhaler Competency and Medication Adherence in Older Adults and Adults with Obstructive Lung Disease (폐쇄성 폐질환 노인 환자와 성인 환자의 흡입제 사용 숙련도와 투약이행도)

  • Purpose: The aims of current study were to assess the inhaler competency and medication adherence, and to identify association of inhaler competency with medication adherence in patients with obstructive lung disease. Methods: We did a secondary analysis of the Hanyang Obstructive Pulmonary Evaluation data in a single institution from June 2014 to April 2015 after an approval of Institutional Review Board. A total of 150 patients with asthma or chronic obstructive lung disease participated in the study. Inhaler competency was evaluated accuracy in each step for using metered dose inhaler. Medication adherence was calculated using actually dispensed doses based on the prescribed inhaler doses. Results: Older adults (${\geq}65$) had lower competency in using inhaler (66.7 vs 83.3, z=-4.52, p<.001) and poorer medication adherence (67.7 vs 91.8, $x^2=14.06$, p<.001) than adults (<65). Inhaler competency was associated with medication adherence (p=.26, p=.001). Surprisingly, more than 50% of patients were current smokers. Conclusion: Inhaler competency and medication adherence were lower in older adults with obstructive lung disease than those in adult-age patients. Therefore, an individual education program for older patients should be developed to improve the rates of proper use of inhalers. Nursing management for obstructive lung disease should focus on developing behavioral intervention strategies for smoking cessation.
