Development of a Scale to Measure Self-Care for Korean Patients with Chronic Obstructive Pulmonary Disease (만성 폐쇄성 폐질환자의 자가관리측정도구 개발)

  • Purpose : The objective of this study was the development and validation of a scale to measure the self-care of patients with chronic obstructive pulmonary disease(COPD) in Korea. Method: Self-care scale was developed based on the self-care activities patients had to carry out in order to manage their COPD. The original scale contained 34 items rated along a five-point Likert scale and was reviewed by 18 professional nurses and 10 Korean patients with COPD for content validity. Subsequently, patients with COPD were asked to complete this 23-item scale and further tests were done with the 125 useable responses. Result: Factor analysis identified eight factors-'maintaining a clean air way', 'taking medication', 'support from family', 'preventing infection', 'managing symptoms', 'breathing exercising', and 'taking in nutrition'. The internal consistency of the total scale was Cronbach's α=0.7226. These eight factors explained 60.8% of total variance. There was correlation among Korean Self-Care Scale score, administration level, and knowledge level but there was no correlation to patients' satisfaction with medical services. Conclusion: The 23 item questionnaire positively identified 8 areas defined important for COPD patients. Further studies are required to see how these can be integrated into patient education.

A Study for the Improvement of a Fundermental Nursing Practice Course (기본간호학 실습교과과정 개선에 관한 연구)

  • It was reported that nursing students had most difficulties in clinical competency due to their short of knowledge and skills in nursing care. Environmental difference between laboratory in nursing school and hospital was another difficulty to them. This implicated that more preclinical competency education is needed in fundermental nursing to achieve skills of care and the laboratory environment of nursing school had to be improved. The purpose of this study was to examine contents, total lecture and practicum house of the fundermental nursing education in nursing schools. It was also examine the difference of contents between hospital in-service education and nursing school. It's study is a descriptive study with study sample of 56 nursing professors who teach fundermental nursing in colleges and universities. Structured Questionnaires was used in collecting data. The result showed that practicum hours total 96.8 hours which was 59% of funderm nursing course. The education regar medication account for large numbers of h Aseptic technique, elimination care, vital oxygen related care were the next common contents. When compared with hospital in-service education, 76% of the education were the same. This suggested that pre-clinical education in fundermental nursing needed to be enforced in its contents & education hours to establish clinical practice centered nursing education.

Emergency Nurses' Perception and Performance of Tuberculosis Infection Control Measures (응급실 간호사의 결핵감염관리에 대한 인식과 수행)

  • Purpose: The purpose of this study was to investigate the perception and performance of tuberculosis infection control measures among emergency department nurses. Methods: A structured questionnaire was administered to a convenience sample of 227 emergency department nurses from 20 hospitals in B City. Results: The mean perception score was 4.52 out of a possible 5, and the mean performance score was 3.31 out of a possible 5. There were big differences between the perception and performance score on the items regarding wearing a gown or mask, while there were small differences on the items regarding dealing with patients' articles or medication. Conclusion: Emergency nurses showed high perception and low performance score on the items related to facilities and regulation such as visitation restriction or patient isolation. There was a positive correlation between perception and performance score. Conclusion: For emergency nurses, perception is higher than performance of tuberculosis infection control measures. The findings indicate the importance of providing continuing education for emergency nurse professionals seeking to update and increase their knowledge and to develop evidence-based nursing protocols on tuberculosis infection control in emergency department.

Analysis of Nursing Records for Elderly Patients with Abdominal Pain in the Emergency Medical Center (응급의료센터에 내원한 복부통증 노인 환자에 대한 간호기록 분석)

  • Purpose: This study was done to analyze nursing assessment and nursing care for pain in the electronic nursing records for the elderly patients with abdominal pain visiting the Emergency Medical Center. Methods: This study is a descriptive study based on nursing records from January to December 2015. A total of 1155 records for elderly patients with abdominal pain were gathered. Results: The mean age of elderly patients whose records were analyzed was 75.2 years. Analysis of nursing records regarding pain management showed that semi-urgent severity (93.7%), direct emergency room visits (58%), and 6.01 hours of emergency room stay (6.01 hours)were the most frequently documented characteristics of the elderly patients with pain complaints. Recording time of nursing assessment for abdominal patients was 1.01 hour; the average pain intensity was 3.97. The mostly used nursing intervention for abdominal pain was medication (65.1%). There was no record of non-pharmacological pain nursing interventions. Conclusion: The results of this study showed that improving knowledge and nursing practice for pain management is much of necessity. In particular, development of the non-pharmacological nursing interventions for pain is needed. Further research is also imperative to develop and evaluate record systems for pain management that can be used in the emergency room.

  • Objectives : This study analyzes herbs combinations and primary virtues that constitute the prescriptions of Gyeji-tang, Mahwang-tang, and Daecheongryong-tang to effectively analyze medication preparations described in "Shanghanlun" using database, and test the results of the data structure and effectiveness of queries. Methods : This study enters "Sanghanjapbyeongronyeonggudaeseong" as the original text into the database, and even uses one provision as independent knowledge by dividing provisions in accordance with the content. Results : Five tables and 12 queries were created and used for data input and analysis. Using the types of Herbs included in the prescriptions, this study manages to search for the same or supplemented prescriptions, and the primary virtues of the prescriptions were collected and compared. Conclusions : Subject to the combination of herbs and pathology for the prescriptions described in "Shanghanlun" utilizing the database, this study found relevant texts and combinations of herbs and pathologies, and using this, the time required for theoretical research between prescriptions could be shortened.

Antiepileptic Drugs in Children : Current Concept

  • An epileptic seizure is defined as the transient occurrence of signs and/or symptoms due to abnormally excessive or synchronous neuronal activity in the brain. The type of seizure is defined by the mode of onset and termination, clinical manifestation, and by the abnormal enhanced synchrony. If seizures recur, that state is defined as epilepsy. Antiepileptic drugs (AEDs) are the mainstay of treatment. Knowledge about initiating and maintaining adequate AEDs is beneficial for the clinician who treats children with epilepsy. This article will delineate the general principles for selecting, introducing, and discontinuing AEDs and outline guidelines for monitoring adverse effects. In general, AED therapy following a first unprovoked seizure in children is not recommended. However, treatment should be considered after a second seizure. In children and adolescents, if they are seizure-free for at least 2 years, attempts to withdraw medication/s should be made, taking into account the risks vs. benefits for the individual patient. The decision on when and what AED to use should be tailored according to the patient. For optimal treatment, the selection of adequate AEDs can be achieved by considering the precise definition of the patient's seizure and epilepsy syndrome. Continuous monitoring of both therapeutic and adverse effects is critical for successful treatment with AEDs.

A case report of urinary incontinence improved by Gyejigabuja-tang. (계지가부자탕(桂枝加附子湯)을 이용하여 호전된 요실금 치험 1례)

  • Objective : The purpose of this case report is to offer an unprecedented target for Gyejigabuja-tang by analyzing a case in which the symptoms were improved by the herbal decoction therapy, and to accumulate a fundamental knowledge in herbal medicine therapy for urinary incontinence. Method : We studied a patient's chart with the patient's consent and displayed patient's timeline and adherence in a figure according to the CARE guideline. The progress of urination symptoms were assessed with the frequency of urination and how long the patient can stand urine. The accompanied lower limb pain was estimated with Numerical Rating Scale(NRS) Results : The patient's adherence was good during the medication period and symptoms related with urination and accompanied lower limb pain were improved with no specific side effects. NRS of the lower limb pain decreased from 7 to 2, the frequency of urination also improved from over 10 times to 6~8 times per day, and the patient can stand urine from less than 1 second to 2~3 minutes. Conclusions : Gyejigabuja-tang was effective for an urinary incontinence patient which corresponds to the provision No.20 in Shanghanlun.

Review of Clinical Researches for Herbal Medicine Treatment on Functional Abdominal Pain in Children. (소아 기능성 복통의 한약치료에 대한 임상연구 동향 - 중의학 논문을 중심으로 -)

  • Objectives The purpose of this study is to evaluate the efficacy and safety of herbal medication for the treatment of functional abdominal pain in children by analyzing randomized controlled trials conducted in China. Methods Literatures from China National Knowledge Infrastructure published up to 23 November, 2020 were searched. Then they were collected and analyzed by their publication year, demographic information, treatment methods used, duration of illnesses, duration of treatments, follow-up periods, outcome measurement and adverse events. Results A total of 27 studies were selected for the analysis. In all studies, the total efficacy of the herbal medicine treatment group was reported to be higher than that of the control group. The most frequently used medical herb was Root of Glycyrrhiza uralensis Fischer (甘草). The most frequently used classes of herbal medications are Qi regulating medicine (理氣藥), Qi tonics (補氣藥), Aromatic wet medicine (芳香化濕藥), and Digestant (消食藥). The adverse events rate for the herbal medicine treatment group were significantly lower than those in the Western medicine treatment group. Conclusions This study showed that herbal medicine treatment can be effective and safe option for treating pediatric functional abdominal pain. However, additional well-designed clinical studies are needed to solidify the findings.

Adherence to antiretroviral therapy and associated factors among HIV-positive adolescents in Sub-Saharan Africa: a systematic review

  • Purpose: Although long-term viral suppression and antiretroviral therapy (ART) success depend on sustained adherence, adolescents' adherence rates are suboptimal. Optimal adherence is influenced by various factors. Since Sub-Saharan Africa is home to over 80% of adolescents living with human immunodeficiency virus (HIV) and considering their unique characteristics and susceptibility to poor adherence, it is crucial to provide updated knowledge on adherence rates and their determinants among this population. This review aims to present up-to-date data on adherence rates and associated factors among HIV-positive adolescents in Sub-Saharan Africa. Methods: A systematic review was conducted following the PRISMA guidelines. The PubMed and Scopus databases were used to identify documents corresponding to the study's objectives. Eleven studies were included in this review after being selected from among all studies that were found online from 2017 to 2023. Results: The reported adherence rates ranged from 55% to 86%. In total, 32 factors were found to be related to adherence among HIV-positive adolescents in Sub-Saharan Africa. These included 12 adherence-facilitating factors and 20 adherence-inhibiting factors. The most often mentioned factors affecting adherence were advanced World Health Organization clinical stage (i.e., stage IV), ART dose and regimens, a lack of support, and violence victimization. Conclusion: Our findings can help healthcare providers collaborate with HIV-positive adolescents to improve ART adherence and ensure the best possible health outcomes.

The Study of DNA Mutations of Phenylketonuria in Koreans (한국인에서의 페닐케톤뇨증의 유전자변이에 대한 고찰)

  • Purpose : Phenylketonuria(PKU) is an inborn error of metabolism and a genetic disorder resulting from a deficiency of phenylalanine hydroxylase(PAH) and decreased activity of tetrahydrobiopterin(BH4).In this study the correlation between the DNA mutation and clinical manifestations was investigated and PAH DNA mutations were compared bewteen Asian and Caucasian populations. Methods : DNA was isolated from peripheral leukocytes. The PAH gene was amplified by Polymerase Chain Reaction(PCR) and the sequence was analyzed with Multiplex Ligation-dependent Probe Amplification(MLPA). Results : We characterized the PAH gene of 102 independent Korean patients with PKU. PAH nucleotide sequence analysis revealed 44 different mutations, including 10 novel mutations comprising 9 missense mutations(N207D, K95del, A447P, G344D, P69S, S391I, A202T, G103S, and I306L) and 1 novel splice-site variant mutation(IVS10-3C>G). R243Q was the most prevalent mutation in this study. A259T has not previously been reported in Asian populations, but we found that this mutation had a frequency of 10.1% in our study. Furthermore, the genotypes of $BH_4$ responsive patients were analyzed and were divided into two groups: $BH_4$ medication-only group and $BH_4$ medication with diet therapy group. In the $BH_4$ medication-only group and $BH_4$ medication with diet therapy group, R241C was the most common mutation. Conclusion : Novel mutations in the PAH gene of PKU patients are still being discovered. Additional information as to the frequency of mutations in the tetrahydrobiopterine responsive gene is also accumulating. We anticipate that knowledge of these PKU gene mutations will assist the diagnosis, genetic counseling, and therapeutic treatment of PKU patients in future.
