  • The development of flexor digital tendon of the hand was studied by electron microscopy in human fetuses ranging from 9 mm to 260 mm crown rump length. The primordium of tendons was first identified as discrete collection of mesenchymal cells at 25 mm fetus. Synovial sheath formation had commenced by 40 mm fetus and was complete by 70 mm fetus. Cell junction or adhesion sites at all ages were noted between the tendon cells. When dilatation of the synovial cavity occurred, two types of synovial cells were observed. A-type cells had numerous vesicles and large vacuoles. In contrast, B-type cells were characterized by abundant rough endoplasmic reticulum and well-developed Golgi complex. By $150mm{\sim}260mm$ fetuses, a mojority of the synovial cells were type B. The most remarkable difference between the synovial cells of full-term fetus and adult was the larger amount of collagen fibers in the latter. The vascular buds were first observed between the individual fibril bundles in the interfascicular space at 150 mm fetus. At 25 mm fetus, collagen fibrils were first noted within narrow cytoplasmic recesses which were continued with the extracellular space. Collagen fibrils were filled in almost entire extracellular space at 150 mm fetus. Besides collagen fibrils in the extracellular space small elastic fibers were also identified and followed in their development.

  • Administration of local anesthetics or morphine through epidural space has the effect of curbing postoperative increases in endocrine hormone. Other benefits include improving metabolic reaction and eliminating postoperative pain. However, repeated administration of local anesthetics through epidural space causes tachyphylaxis, and the unstable blockade of sensory nerve resulting in insufficient analgesia. Morphine has excellent postoperative analgesic effect, but complications including: itching, nausea, vomiting, urinary retention and respiratory depression may be associated with its administration. Sixty patients that fall into the category of ASA class I and II were randomly selected for the purpose of the experiment. Thirty patients were give 4 mg of morphine and the rest, 4 mg of morphine plus 80 ml of 0.25% bupivacaine administered through epidural space with the Baxter infuser. Analgesic effect was satisfactory in both groups. On the day of operation, the effect was stronger in group I (P<0.05) and on postoperative second day, group II showed better analgesic effect (P < 0.05). Group II had more patients who complained of itching (P < 0.05). Other complications were statistically insignificant. The findings indicate that administration of morphine through epidural space for postoperative pain management is an effective procedure. Baxter infuser was found to be very instrumental in pain control while reducing the chance of complications.

  • Recently, the Korean medical system changed with diversification & specialization and the field of medical service is also specialized in these days. And healing environment is proposed as a general concept in the hospital architecture. There is a trend that a establishment of women's hospital is increasing as specialized type. In difference from general hospital, they are providing specialized and integrated services in response to the women's disease and demand. Particularly, Outpatient department space in women's hospital has been changed in terms of area, shape and program. However, there used to be not enough research efforts of responding these changes. This study includes the Outpatient department of s women's hospital in the form of small and medium sized building over a certain level in Korea. And the methods of the study adopted the theoric examination based concept and features of women's hospital, the abstraction of distinctions in the Outpatient department through the analysis of the drawing, and the analysis of distinctions in the Outpatient department of women's hospital with the structural space classified and the analysis of space relation. Therefore, the purpose of this is to study investigates and analyzes the spatial characteristics of the Outpatient department space in women's hospital through a analysis of case study. To conclude, the space composition can be classified into the entry space, the clinic space for patients, the support space of patients' check-up, the reception and waiting space, the rest and convenience space, and the link space in the Outpatient department of women's hospital. The level space can account for the waiting space type, the clinic office type, the type based on the relation between the waiting room and the clinic office with its features. As a result, the results will be applied for the planning & design for the future women's hospital.

  • Purpose: It is recently reported that the increasing noise in the hospitals has caused psychological and physiological stress problems with patients, and medical staffs. This study intends to investigate and analyze the noise levels in the sub stations in comparison with those in the wards in general. This study tries also to find some alterative solutions to the immediate problems. Methods: Noise measurements are conducted in advance prior to analyzing their results at two general hospitals with more than 900 beds, in the comprehensive nursing service wards and in the main/ sub stations located in the general wards Results: Although the noise level in the comprehensive nursing service wards has been slightly lower than that in the general wards, this result is over the recommended noise levels. Therefore it is recommended that efforts should be made to low down the noise level as an alternative and to replace the aged carts as well. Implications: The comprehensive nursing service wards are required to take measures against the various noise sources.

  • Abstract In a conventional digital sector scan system in the ultrasound medical imaging, the sampling space is in the polar coordinates while the display space is in the cartesian coordinates, which necessitates a coordinate transformation process resultion process resulting the overall system very complex and bulky. In this paper we propose a new architecture of the Digital-Scan-Converter (DSC) for the ultrasound sector scan system in which sampling space is the same as the display space, so the data are displayed as they are acquired without any interpola- tion process required. To implement the above real time DSC. two frequency synthesizes with very high switching time and a low-pass filter are required. The former determines the precise location of the data points and the latter fills the gap betw- een pixels in the horizontal direction.

  • Continuous epidural blockade is a widely accepted, useful technique for providing anesthesia and analgesia. But there have been several anecdotal reports of complications such as: abnormal position, knotting, shearing of the catheter, etc. We experienced a case of shearing of the end of an epidural catheter which was difficult to remove from epidural space of a 39-year-old patient. Ultrasonogram proved to be very effective to confirm the presence and position of the retained catheter. We surgically removed the retained catheter as it could potentially lead to infection of the epidural space. We advocate the ultrasonogram as an effective procedure to confirm and locate retained epidural catheter.
