의료서비스에서 고객지향요인이 서비스가치와 고객만족에 미치는 영향 (A Study on Effects of Customer Orientation Factors in Relation to Medical Services on the Values of the Services and Customer Satisfaction)

  • The purpose of this study is to investigate the effects of customer orientation factors in relation to medical services on the values of the service and customer satisfaction and loyalty. Those factors include patient safety and, as found by previous studies, specialization, explanation of what to be medically examined and customer orientation itself. Based on these pervious studies, this study surveyed customers of 7 general hospitals located in Busan to empirically identify relations between customer orientation factors of medical services and the values of the services and customer satisfaction. Results of the study can be summarized as follows. First, this study is very meaningful in that it established a basic theory of patient safety as one of the above customer orientation factors, and tried to empirically demonstrated the theory by applying it to medical services. Second, another of the factors, specialization was found positively affecting the values of medical services, but not affecting customer satisfaction. Customers are likely to choose specialized medical institutions even at higher cost when they undergo an accident or disease. Nevertheless, in factors, whether medical service providers are specialized is not influencing customer satisfaction. This is because medical institutions are failing to properly make recognized their specialization to customers who want to receive specialized medical services. Third, another of customer orientation factors, that is, explanation of what to be medically examined was found not having positive effects on the values of medical services and customer satisfaction. This is probably because enough time was not given for the explanation or because the explanation itself was not provided enough. Fourth, medical service providers' customer orientation was found positively influencing the values of medical services and customer satisfaction. In other words, it seems that customer-centered attitudes and behaviors of medical service providers had positive effects on customers' perception of medical services. Fifth, another of the factors, that is, patient safety was found positively affecting the values of medical services and customer satisfaction. This is probably because medical services' accurate diagnoses and reliable services had positive effects on customers' perception of medical services. Sixth, customers' perceived values of medical services were found having positive effects on customer satisfaction and loyalty. This suggests that the values of medical services are an antecedent variable that directly influences customer satisfaction and loyalty. Seventh, customer satisfaction was found positively affecting customer loyalty. This suggests that customer satisfaction is an antecedent variable of customer loyalty. In conclusion, this study showed that in relation to medical services, customer orientation factors' significant influences on the values of the services and customer satisfaction requires continuous efforts for raising customers' perceived qualities of medical services.

Developing a Best-Evidence Pre-employment Medical Examination: An Example from the Construction Industry

  • The Dutch construction industry has introduced a compulsory preemployment medical examination (PE-ME). Best-evidence contents related to specific job demands are, however, lacking and need to be gathered. After the identification of job demands and health problems in the construction industry (systematic literature search and expert meeting), specific job demands and related requirements were defined and instruments proposed. Finally, a work ability assessment was linked to the instruments' outcomes, resulting in the modular character of the developed PE-ME. Twenty-two specific job demands for all Dutch construction jobs were identified, including kneeling/squatting, working under time pressure, and exposure to hazardous substances. The next step was proposing self-report questions, screening questionnaires, clinical tests, and/or performance-based tests, leading to a work ability judgment. "Lifting/carrying" is described as an example. The new modular PE-ME enables a job-specific assessment of work ability to be made for more than 100 jobs in the Dutch construction industry.

Current Status of Patient Safety Regulations, Guidelines and Support Mechanisms in Korean Hospitals

  • Purpose: This study was conducted to investigate patient safety regulations and guidelines in order to understand their current status, and to examine support measures to improve patient safety in Korean hospitals. Methods: The participants were the safety officers from hospitals with 200 or more beds and 112 hospitals responded to the online survey. The questions covered patient safety regulations, the performance level of patient safety activities, patient safety incident reporting systems, the dedicated professional, training, support mechanisms, and expectations of reporting systems. Results: Among preventative measures, fall prevention and hand hygiene were reported to be most widely practiced (92% and 91%, respectively). Time-out for invasive procedures showed a relatively low practice rate at 70%. Among patient care activities, transfusion, surgery and sedation, medication, and infection management were performed by 84, 74, 93 and 93% of the hospitals, respectively. Patient safety activities included patient safety committee, patient safety cooperation between decision-making bodies, patient safety workshops, seminars, lectures, and training for employees. Conclusion: Patient safety regulations and guidelines have not yet been sufficiently prepared, and a public institution such as a certification authority is of crucial importance to enforce these guidelines.

201Tl의 생산과정에서 발생한 방사성 폐기물의 제염 효율 평가 (Evaluation of the Decontamination Efficiency of Radioactive Wastes Generated during the Production of 201Tl)

  • This study was conducted for the purpose of efficient radioactive waste disposal and management. Experiment was evaluated the decontamination efficiencies of the four types decontamination materials(Water, Alcohol, Decontamination Water, Decontamination Gel) with radioactive wastes generated during radio-pharmaceutical production process at Korea Institute Radiological and Medical Sciences(KIRAMS). The radioactive waste sample used in experiment is a lead plate of the fume hood that was disposed in April, 2019. In the experimental method, radioactive waste was measured before and after decontamination using a HPGe semiconductor detector and Gamma survey meter. The measured values before and after decontamination were evaluated for decontamination efficiency as a percentage. As a result, it was confirmed that a lot of specific activity and surface dose rate was removed from the radioactive wastes. In particular, when decontamination water was used, most of the radioactivity of radioactive wastes was removed. Considering these results, if decontamination water is used in decontamination of radioactive waste, decontamination efficiency equivalent to the disposition criteria can be expected with just one decontamination treatment. In addition, in the case of water and alcohol, only on decontamination was effective in approximately 75% and 95%. Otherwise, when decontamination gel was used, it was confirmed that the largest deviation occurred among all experimental results.

의료용 방사성폐기물 자체처분을 위한 방사능 측정 및 평가 (Measurement and Estimation for the Clearance of Radioactive Waste Contaminated with Radioisotopes for Medical Application)

  • 의료분야에서 방사선 진료기술의 발전에 따라 방사성폐기물의 수량은 급속히 증가하고 있다. 방사성폐기물에는 주로 PET/CT에 사용하는 $^{18}F$을 비롯하여 핵의학검사에 사용하는 $^{99m}Tc$ 등과 같이 반감기가 매우 짧은 방사성동위원소가 함유되어 있다. 이를 처분하기 위하여 국제원자력기구(IAEA)는 개인선량($10{\mu}Sv/y$) 및 집단선량(1 man-Sv/y)과 핵종별 농도에 근거하여 각각 폐기물의 규제해제기준을 제시(IAEA Safety Series No 111-P-1.1, 1992 및 IAEA RS-G-1.7, 2004)하였다. 이 연구에서는 IAEA 기준에 따른 방사능농도를 측정하기 위하여, $^{18}F$, $^{99m}Tc$, $^{123}I$, $^{125}I$$^{201}TI$ 관련 방사성폐기물을 수집하고 측정용기를 준비하였다. 그리고 MCA를 이용한 감마방사능 측정, 감마계수기를 이용한 감마방사능 측정, 베타입자 방출 핵종의 방사능 측정방법 및 절차를 수립하고, 표준물질을 제작하여 교정하였다. 측정결과를 근거로 방사능 감쇠 유도식을 산출하였으며, 이를 이론식과 대비하여 고찰하였다. 이 연구 결과는 ISO 표준으로 추진할 예정이다.

The Food Safety of Superfine Saengshik Processed by Top-down Technique in Mice

  • Saengshik is an uncooked and powdered functional food composed of various edible plants, and has been consumed widely due to its health benefits and convenient uptake. Recently, superfine ground saengshik, which contains a certain extent of nanoscale particles, has been commercialized to enhance efficacy, but its safety has not been determined. This study was conducted to evaluate the food safety of superfine saengshik (SS) through general toxicity examination after oral uptake in mice compared to conventional fine saengshik (FS). The SS particle size distribution was 0.479-26.303 f.1m in diameter, with about 68.92% of particles with a diameter < $0.955{\mu}m$. From our safety evaluation, the number of white blood cells (WBCs) and biochemical values in the serum fell into the normal range, and the weight of organs showed no significant difference between FS and SS groups. Histological observation of the liver, small intestine and large intestine did not show any abnormal or pathological findings under light microscopy. Our results suggest that oral intake of SS is not harmful to mice in terms of general toxicity.

Efficacy and Safety of Laser Surgery and Transurethral Resection of the Prostate for Treating Benign Prostate Hyperplasia: a Network Meta-analysis

  • Background: Traditional meta-analyses or systematic reviews of randomized controlled trials (RCTs) have been used to compare laser surgeries and transurethral resection of the prostate (TURP) for benign prostate hyperplasia (BHP), but they cannot provide a hierarchy regarding efficacy and safety of treatment. Objective: We therefore performed a network meta-analysis (NMA) to compare and create hierarchies for efficacy and safety of TUPR and laser surgeries for BPH. Materials and Methods: We searched for reports of RCTs published up to April 25, 2015. After methodological quality assessment and data extraction, we performed an NMA to compare TURP and laser surgeries for BPH. Results: We ranked the treatments of TURP and laser surgeries for BPH. For IPSS at 6 months, holmium laser resection of the prostate (HoLRP) ranked the first-best and at 12 months, holmium laser enucleation of the prostate (HoLEP). For Qmax at 6 and 12 months, HoLEP ranked the first-best; for operative time it was TURP; for cathedral removal time, diode laser enucleation of the prostate (DiLEP) ranked the first-best. Conclusions: Although TURP is considered the gold standard for treating BPH, it is not better in terms of efficacy and safety compared with the laser surgery. Our NMA created hierarchies for the 9 types of surgery in terms of efficacy and safety, which should help clinicians choose the best approach for the individual patient.
