Design and Implementation of Medical Image Information System (의료 화상 정보 시스템의 설계 및 구현)

  • In this paper, MIlS (Medical Image Information System) has been designed and implemented using INGRES RDBMS, which is based on a client/server architecture. The implemnted system allows users to register and retrieve patient information, medical images and diagnostic reports. It also provides the function to display these information on workstation windows simultaneously by using the designed menu-driven graphic user interface. The medical image compression! decompression techniques are implemented and integrated into the medical image database system for the efficient data storage and the fast access through the network.

Fast Medical Volume Decompression Using GPGPU (GPGPU를 이용한 고속 의료 볼륨 영상의 압축 복원)

  • For many medical imaging systems, volume datasets are stored as a compressed form, so that the dataset has to be decompressed before it is visualized. Since the decompression process takes quite a long time, we present an acceleration method for medical volume decompression using GPU. Our method supports that both lossy and lossless compression and progressive refinement is possible to satisfy variable user requirements. Moreover, our decompression method is well parallelized for GPU so that the decompression takes a very short time. Finally, we designed that the decompression and volume rendering work in one framework so that the selective decompression is available. As a result, we gained additional improvement in volume decompression.

  • For the effective management of medical images, it becomes popular to use computing machines in medical practice, namely PACS. However, the amount of image data is so large that there is a lack of storage space. We usually use data compression techniques to save storage, but the process speed of machines is not fast enough to meet surgical requirement. So a special hardware system processing medical images faster is more important than ever. To meet the demand for high speed image processing, especially image compression and decompression, we designed and implemented the medical image CODEC (COder/DECoder) based on MISD (Multiple Instruction Single Data stream) architecture to adopt parallelism in it. Considering not being a standard scheme of medical image compression/decompression, the CODEC is designed programable and general. In this paper, we use JPEG (Joint Photographic Experts Group) algorithm to process images and evalutate the CODEC.

  • A multimedia remote ultrasound image transfer system for tole-medical services was implemented. The image data of the ultrasound scanner system of the slave is compressed using image compression system. It is transferred to the master PC via ISDN UNI(User-Network Interface) and decompressd by image decompression system. The total system is composed of three parts; first, the image capture card which transfers bidirection- al data between ultrasound scanner system and PC, second, image compression and decompression card, finally, ISDN TA(Terminal Adaptor) card for transferring the image. This system has a easy user interface because it is executed on the basis of MS-WINDOWS. So it is capable of serving medical services at a remote place.

Delayed Unilateral Soft Palate Palsy without Vocal Cord Involvement after Microvascular Decompression for Hemifacial Spasm

  • Microvascular decompression is a very effective and relatively safe surgical modality in the treatment of hemifacial spasm. But rare debilitating complications have been reported such as cranial nerve dysfunctions. We have experienced a very rare case of unilateral soft palate palsy without the involvement of vocal cord following microvascular decompression. A 33-year-old female presented to our out-patient clinic with a history of left hemifacial spasm for 5 years. On postoperative 5th day, patient started to exhibit hoarsness with swallowing difficulty. Symptoms persisted despite rehabilitation. Various laboratory work up with magnetic resonance image showed no abnormal lesions. Two years after surgery patient showed complete recovery of unitaleral soft palate palsy. Various etiologies of unilateral soft palate palsy are reviewed as the treatment and prognosis differs greatly on the cause. Although rare, it is important to keep in mind that such complication could occur after microvascular decompression.

Surgical Management Options for Trigeminal Neuralgia

  • Trigeminal neuralgia is a condition associated with severe episodic lancinating facial pain subject to remissions and relapses. Trigeminal neuralgia is often associated with blood vessel cross compression of the root entry zone or more rarely with demyelinating diseases and occasionally with direct compression by neoplasms of the posterior fossa. If initial medical management fails to control pain or is associated with unacceptable side effects, a variety of surgical procedures offer the hope for long-lasting pain relief or even cure. For patients who are healthy without significant medical co-morbidities, direct microsurgical vascular decompression [MVD] offers treatment that is often definitive. Other surgical options are effective for elderly patients not suitable for MVD. Percutaneous retrogasserian glycerol rhizotomy is a minimally invasive technique that is based on anatomic definition of the trigeminal cistern followed by injection of anhydrous glycerol to produce a weak neurolytic effect on the post-ganglionic fibers. Other percutaneous management strategies include radiofrequency rhizotomy and balloon compression. More recently, stereotactic radiosurgery has been used as a truly minimally invasive strategy. It also is anatomically based using high resolution MRI to define the retrogasserian target. Radiosurgery provides effective symptomatic relief in the vast majority of patients, especially those who have never had prior surgical procedures. For younger patients, we recommend microvascular decompression. For patients with severe exacerbations of their pain and who need rapid response to treatment, we suggest glycerol rhizotomy. For other patients, gamma knife radiosurgery represents an effective management strategy with excellent preservation of existing facial sensation.

Posterior Floating Laminotomy as a New Decompression Technique for Posterior Cervical Spinal Fusion Surgery

  • Objective : In the cervical spine, many surgical procedures have been developed to achieve optimal results for various disorders, including degenerative diseases, traumatic injury, and tumor. In this study, we report our experience and follow-up results with a new surgical technique for cervical spine entitled posterior floating laminotomy (PFL) in comparison with conventional laminectomy and fusion (LF). Methods : Data for 85 patients who underwent conventional LF (n=66) or PFL (n=19) for cervical spine disorders between 2012 and 2019 were analyzed. Radiological parameters, including cervical lordosis (CL), T1 slope (T1S), segmental lordosis (SL), and C2-7 sagittal vertical axis (SVA), were measured with lateral spine X-rays. Functional outcomes, comprising the modified Japanese Orthopaedic Association (mJOA), neck disability index (NDI), and visual analog scale (VAS) scores, were also measured. For the patients who underwent PFL, postoperative magnetic resonance image (MRI) was performed in a month after the surgery, and the degree of decompression was evaluated at the T2-weighted axial image, and postoperative computed tomography (CT) was conducted immediately and 1 year after the operation to evaluate the gutter fusion. Results : There was no difference in CL, T1S, SL, and C2-7 SVA between the groups but there was a difference in the preoperative and postoperative SL angles. The mean difference in the preoperative SL angle compared with that at the last follow-up was -0.3° after conventional LF and 4.7° after PFL (p=0.04), respectively. mJOA, NDI, and VAS scores showed significant improvements (p<0.05) during follow-up in both groups. In the PFL group, postoperative MRI showed sufficient decompression and postoperative CT revealed gutter fusion at 1 year after the operation. Conclusion : PFL is a safe surgical method which can preserve postoperative CL and achieve good clinical outcomes.

Improvement of Image Sensor Performance through Implementation of JPEG2000 H/W for Optimal DWT Decomposition Level

  • In this paper, a particular application of digital photos, remote sensing, remote shooting air moving, high-resolution and high compression of medical images required by remote shooting of JPEG2000 standard applied in the field of hardware design, production was implemented. JPEG2000 standard for image compression using the software implementation of the processing speed is very slow compared to conventional JPEG disadvantages, and also the standard of JPEG2000 DWT (Discrete wavelet transform) to improve the level of compression for image data if processing speed is a phenomenon that has degraded. In order to solve these JPEG2000 compression / decompression groups were designed and applied. In this paper, the optimal JPEG2000 compression / reservoir hardware by changing the level for still image compression, faster computation speed and quality has shown improvement.
