국내 한방병원의 IRB 및 임상시험 실태조사 (Current Status of Institutional Review Boards and Approvals of Clinical Research in Oriental Medical Hospitals in Korea: A Survey)

  • Objective: To investigate institutional review boards and approved clinical trials of Oriental medical hospitals in Korea Methods: We e-mailed 16 Oriental medical hospitals a survey consisting of questions that addressed two topics, institutional review boards and the clinical trials they approved. The first part included questions about whether each hospital had an institutional review board, that board's staffing and education, reviews, and expedited reviews. The clinical trials portion covered the number and types of approved clinical trials, treatment methods, whether an investigator or a sponsor initiated the trials, diseases, time frame, and clinical trial fee. Results: We received a response to our e-mail from 14 hospitals, all of which had an institutional review board. The average number of institutional review board members was 13.57. Of these Oriental medical hospitals, 70% and 49% said that they regularly educated their institutional review board staff and clinical trial, investigators, respectively. 79% of the hospitals claimed to conduct regular reviews, and 50% said that they did so monthly. The number of approved clinical trials increased sharply from 11 in 2005 to 102 in 2008. 68% of these clinical trials were randomized, and the most-used treatment methods were herbal medicine (35%) and acupuncture (29%). The most common target diseases were circulatory (19%), urinogenital (14%), and musculoskeletal (13%) disease. Conclusions: Despite the rapid increase in clinical research in oriental medical hospitals, many more efforts including raising IRB quality, varying research diseases and increasing clinical trials in the hospitals located in non-metropolitan area, should be made.

간호사의 팔로워십에 따른 직무만족, 조직몰입 및 간호업무수행 (Effects of the Followership on Organizational Commitment, Job Satisfaction, Nursing Task Performance)

  • Purpose: This study was designed to find out the fact that the followership types of nurses would effect on job satisfaction, organizational commitment and nursing task performance. Methods: The sample consisted of 219 nurses working in 2 medical institutions. SPSS program was used for descriptive statistics, ANOVA, chi-square test, Duncan test, Pearson Correlation, and Multiple Regression. Results: In the followership type, effective follower was the largest as 47.0% and passive follower was 26.9%. Nurses who had a effective followership style had the highest level of organizational commitment, job satisfaction and those who had a passive followership style had the lowest level of organizational commitment, job satisfaction, and nursing performance. The followership was significantly related to the organizational commitment, job satisfaction and nursing task performance. The active involvement had a positive effect on the organizational commitment, job satisfaction and nursing task performance. The critical thinking had a positive effect on the job satisfaction. Conclusion: These results demonstrate that the most effective followership is effective follower. And organizational managers must take into account followership types when they develop the strategies for organizational effectiveness.

시계열 개입 분석을 이용한 환자의뢰제도의 개입효과 평가 (An Evaluation of a Patient Referral System using Intervention Analysis)

  • The purpose of this study was to introduce the methodology of intervention analysis with time series data and to investigate the influence of the patient referral system on medical care utilization in Kangwha county. The data were obtained at the Kangwha Medical Inurance Society and we analysed the material based on the outpatient care fee. The results were as fellows: 1. The average outpatient care utilization in the hospital decreased by 41.7% due to the patient referral system. 2. The utilization of the health instituation increased by 278.8 persons per month due to the patient referral system. 3. The patient referral system did not influence the total outpatient are utilization. The methodology of intervention analysis, which detected the effect of intervention, will be helpful to the study of public health area.

일본의 노인보건시설에 대한 연구 (Study of the Geriatric Health Care Facility in Japan)

  • The objective of the present study is to review of the system, type of care and utility of the Geriatric Health Care Facility(GHCF) in Japan. Geriatric Health Care Facilities in Japan were started with subsidies from the Ministry of Health and Welfare in 1987 to encourage return of the elderly from hospitals to their homes rather than other destinations such as nursing homes or hospitals. Concerning to the type of care, there is the difference between GHCF and other geriatric care facilities(i.e., geriatric hospital and nursing home). GHCF provides both medical and nursing care. The following services are available for the GHCF's user's. As institutional care services, rehabilitation training, ADL exercise, nursing care and management of medicine, are available. For the out-patients, supplying meals, taking a bath, and rehabilitation services, are also available. The medical treatment fee at the facilities is about US $ 1,500 per month. Expenses for meals, daily necessities, shall be borne by the recipient, Those expenses are about US $ 360 per month. In anticipation of the coming of the aging society, the Goverment must be formulated consecutively several vital policies of measures, especially GHCF, for the elderly in the future few years.

QALY를 이용한 가정간호서비스의 비용효용분석 (A Cost-Utility Analysis of Home Care Services by using the QALY)

  • Purpose: The aim of this study was to analyze economical efficiency of home care service by comparing a cost-utility ratio(CUR) between home care and hospitalization. Method: The analytic framework of this study was constructed in 5 stages: Identifying the analytic perspectives, measurement of costs, measurement of utility, analysis of CUR, and sensitivity test. Data was collected by reviewing medical records, home care service records, medical fee claims, and other related research. Result: The mean of the annual total cost was 23,317,636 Won in home care and 73,739,352 Won in hospital care. QALY was 0.389 in home care and 0.474 in hospital care, so CUR was 299,712,545 QALY in home care and 777,841,266 QALY in hospital care. Conclusion: The findings affirmed that home care had an economical efficiency in the aspect of utility compared to hospitalization. Therefore, the findings of this study can be used to develop a governmental health policy or to expand the home care system. In addition, the cost-utility analysis framework and process of this study will be an example model for cost-utility analysis in nursing research. Therefore, it will be used as a guideline for future research related to cost-utility analysis in nursing.

NIC과 연계된 산부인과 환자 간호중재에 대한 한국 건강보험 수가체계와 미국 ABC 코드체계와의 수가 비교 분석 (Korean and United States: Comparison of Costs of Nursing Interventions)

  • Purpose: This study was performed to compare the costs of nursing interventions implemented for the obstetrical and gynecological patients using Korean Reimbursement System and ABC codes system developed in the US for costing out interventions performed by health care professional. Methods: First, the narrative data on nursing interventions were extracted from electronic medical record system of a tertiary university and mapped with Nursing Intervention Classification (NIC) by two researchers until 100% consent was reached. Narrative nursing interventions mapped with NIC were then remapped with ABC codes system using the electronic program developed in the research. The mapping data were analyzed with real numbers, frequency, percentage, mean, and standard deviation. Results: More nursing interventions were mapped with ABC codes than Korean reimbursement system. Total of 97 different types of narrative interventions could be mapped with NIC, 43 NIC interventions could be reimbursed by ABC code but only 16 NIC interventions were reimbursed by Korea Reimbursement System. Conclusion: Korean medical insurance fee system needs amendment to include more comprehensively interventions performed by nurses which are very important to patient outcomes. Further study is needed to develop strategies to costing out nursing interventions.

간호사가 지각하는 간호조직문화유형이 직무만족, 조직몰입에 미치는 영향 (Effects of the Organizational Culture on the Job Satisfaction and Organization Commitment)

  • Purpose: This study aimed to examine the impact of Organizational Culture on job satisfaction and organizational commitment in nursing organization. Methods: The sample consisted of 264 nurses of 2 tertiary medical institutions. The instruments used in this study were the nursing organizational culture questionnaire (Han, 2001), job satisfaction scale (Paula, 1978), and Organizational Commitment scale (Mowday et al., 1979). The data were analyzed by descriptive statistics, one-way ANOVA, t-test, Pearson Correlation. Coefficient, and multiple regression using the SPSS Win 14.0 program. Results: The dominant organizational culture of nursing organization was rank-oriented culture. The mean score of job satisfaction and organizational commitment was 3.28 and 3.40 respectively. Organizational commitment and Job satisfaction were positively correlated with affiliative-oriented culture, innovative-oriented culture, and task-oriented culture. Affiliative-oriented and task-oriented culture were major variables influencing on job satisfaction and affiliative-oriented culture was a major variable influencing organizational commitment. Conclusion: Organizational culture, especially affiliative-oriented culture had a significant influence on nurses' job satisfaction and organizational commitment. Therefore, we have to develop strategies to enhancing the affiliative-oriented culture to improve the job satisfaction and organizational commitment of nurses.

구조방정식 모형을 이용한 치과 임플란트 환자의 만족과 재이용 의사에 미치는 영향요인 분석 (Associated factors which influence satisfaction of dental implant patients and intention to revisit using structural equation model)

  • Objectives : This study is to confirm the influencing factors on patients satisfaction and intention to revisit the implant service clinic by structural equation model. Methods : We surveyed 250 implant patients visiting 6 dental clnics(hospitals) in Busan, Changwon, Gimhae city and questionnaire was for four weeks in July 2012, a total 209 patients filled out the questionnaires within July and the complete data were analyzed. we were used structural equation model for analysis to confirm influencing factors on implant patients' satisfaction and intention to revisits. Results : This study finds that factors influencing on implant patients' satisfaction and intention to revisit are reasonable medical fee, medical skill treatment of dentist, dental hygienist and other support staff, administrative procedure, and there existed between influencing factors and the patient's treatment satisfaction as well as intention to revisit. In order to satisfy the implant patients, dentist and hygienists should try to provide high quality of treatment and dental services. Conclusions : The results of this study provide managerially important messages to the managers of dental clinics in order to develope management strategies for new customers.

종합병원 일반병동 간호행위의 활동기준원가분석 (Activity-Based Costing Analysis of Nursing Activities in General Hospital Wards)

  • Purpose: The purpose of this study was to explore the relationship between cost and revenue for inpatient nursing activities in general wards. Methods: Data were collected from 12 medical-surgical wards in one general hospital from January 1 to December 31, 2010. The nursing activities were categorized into 2 groups according to nursing service payment type in terms of the Korea health insurance system. Descriptive statistics were used to identify nursing activities and nursing activity costs. Results: Of 140 nursing activities identified as performed in general wards, payment for 69 items was included in nursing management fees. The percentage of each cost for the nursing units was 90% for labor, 4% for materials, and 6% for operating expenses. The cost for medical support nursing service accounted for 38% of costs and nursing management fees, 62%. The average profit and loss was -237,257,000 won. The cost recovery rate for nursing service was only 44%. Conclusion: The results indicate a need to measure the economic value of nursing activities performed in general wards and use it as a basis for establishing an adequate reimbursement system for nursing service.

  • Although most of Korean PT professors and clinical lecturers has been thought the importance of the PT student's clinical practice, there has got a lot of training problems in clinical field. As the result, of our survey study pointed out trouble of the (1) training duration (2) training credits (3) training evaluation (4) training workbook (5) training fee (6) training hospital numbers and (7) training quality etc, in Korean today's situation. So we suggest the Internship-training system for above half a year after student has got a PT licence, as a desired training model.
