This study contained the clinicostatistical analysis of the frequency in relation to sex, age, locations, chief complaints, duration, radiographic findings, recurrence, teeth, and treatment methods. The results were as follow : 1. Of a total of 128 benign odontogenic tumors, ameloblastomas(57 cases; 44.5%) and odontomas (44 cases ; 34.4%) mostly occupied. The other types of lesions were 8 calcifying odontogenic cysts, 7 benign cementoblastomas, 4 myxomas, 3 adenomatoid odontogenic tumors, 2 calcifying epithelial odontogenic tumors, 2 ameloblastic fibro-odontomas, and 1 odontogenic fibroma. 2. In age and sex distribution, benign odontogenic tumors occured slightly more often in males(53.9%) than females(46.1%) and the majority of cases(79.7%) were found during 2nd, 3rd, and 4th decade. 3. There was a predilection for mandibular lesions(mandible-maxilla ratio, 2.6 : 1). 4. The most common chief complaint was swelling(29.7%) and in respect to duration, the cases less than 1 year(50.0%) mainly appeared. 5. There were 7 cases(13.0%) of recurrence on ameloblastoma and there was no recurrence in the others. 6. In Ameloblastoma It commonly occured during 3rd and 4th decade(59.6%) and mean age was 30.2 years. The majority of cases were occurred in mandible(96.5%) , especially mandibular molar and angle area(71.9%). The most common chief complaint was swelling(47.4%) and in respect to duration, the cases less than 1 year(52.6%) mainly appeared. In relation to teeth, there were resorption of root(52.6%), displacement of teeth(31.6%), and in relation to impacted teeth(43.9%). There was higher recurrence rate in the cases by conservative treatment(14.7%) than radical treatment(10.0%). As regards radiographic findings, conservative treatments were prevalent in the cases of unilocular type(85.7%) as compared with multilocular type(48.5%). and there was higherrecurrence rate in the cases of multilocular type(18.2%) than unilocular type(4.8%). As regards the type of treatment in relation to age, conservative treatments were prevalent in patients younger than 20 years of age. 7. In Odontomas It commonly occured during 2nd decade(50.0%) and in maxillary anterior teeth(40.9%). The most common chief complaint was delayed retention and permanent impaction of teeth(72.7%), and most frequently associated with impacted teeth(79.5%).

  • This study was conducted to evaluate the quality in medical records by analyzing its completeness through setting up the level of record on the patient's past history and through examining the actual medial records. Targeting the information on the patient's past history in interns' records, residents' records and nurses' records toward 403 inpatients who were admitted first in 2004 at an university hospital due to stomach cancer. We analyzed whether the charts were recorded or not, recording level, the satisfaction with the expectant level of the records in the hospital targeted for a research and the level of agreement. The results were as follows; first, as for the rate of recording those each items, they were high in the chief complaint & present illness and the past illness history. Depending on the group of recorders, the recording rate showed big difference by items. Second, as a result of measuring the level after dividing the recording level of items for the patient's past history from Level 1 to Level 4 by each item, the admission history, the past illness history, and the family history were about Level 3, and the smoking history, the medication history, the chief complaint & present illness, the drinking history and allergy were about Level 2. In the admission department, it was excellent in the interns' records for the medical department. Third, as a result of its satisfactory level by comparing the expect level of a record and the actual record by item in information on the patient's past history, which was expected by the medical-record committee members of the hospital targeted for a study. And forth, we analyzed the level of agreement with Kappa score in the level of 'Yes' or 'None' related to the corresponding matter in Level 1, in terms of information on the past history in the intern's record, the resident's record, and the nurse's record. The level of agreement in the resident's record & the nurse's record, and in the intern's record & the resident's record was from "excellent" to "a little good". There were differences in the level of completeness and in reliability for the information on the past history by the recorder group or by the admission department. The encounter process that was performed by the admission department or the recorder group, indicated the result that was directly reflected on the quality of medical records, thus it was required further study about the medical record documentation process and quality of care. The items that showed the high recording rate quantitatively were rather low, consequently we'd should develop the tool for the qualitative inspection and evaluate the medical records further. And the items were needed to be detailed in the record level were rather low, and hence there needed to be a documentation guideline and education by the clinical departments.

  • We report a rare case of gastric cancer presenting with a gastrocolic fistula during ramucirumab and paclitaxel combination therapy that was successfully managed with colonic stenting. A 75-year-old man was admitted to our hospital with the chief complaint of melena. Esophagogastroduodenoscopy revealed a large ulcerated tumor in the lower stomach, judged by laparoscopy as unresectable (sT4bN1M0). After four cycles of first-line chemotherapy with S-1 plus oxaliplatin, the patient showed disease progression, and second-line therapy with ramucirumab and paclitaxel was started. At the end of the third cycle, the patient had gastric antral stenosis, which necessitated the placement of a gastroduodenal stent. When the patient complained of diarrhea 10 days later, esophagogastroduodenoscopy revealed a fistula between the greater curvature of the stomach and the transverse colon. The fistula was covered by double colonic stenting, with a covered metal stent placed within an uncovered metal stent, after which leakage from the stomach to the colon stopped.

  • The author studied on the 115 cases of the ameloblastoma which had been diagnosed with biopsy during the period of 1962 to 1994 at the Kyungpook National University Hospital, Pusan National University Hospital, Youngnam University Hospital, Keimyung University Hospital, Catholic Medical School Hospital, and Maryknol Hospital. This study contains the statistical analysis of the clinico-pathological findings such as sex, age, location, chief complaint, duration, treatment method, size, recurrence and impacted teeth in relation to radiographic findings. The results were as follows : 1. The incidence rate was 60% in male and 40% in female. 2. At the time of diagnosis, the age of the patients ranged from 9 to 69 years old (average 31.4 years old). The cases of 73% were in the 2nd, 3rd and 4th decades of life. 3. The majority of cases, 88.7% occurred in the mandible, especially 59% in the mandibular angle area. 4. The most prevalent chief complaint of swelling was 56.5%. 5. As regards duration, the cases less than 12 months appeared 49.5%, and average duration is 30.45months. 6. Unilocular radiographic findings showed 43.5%, Multilocular radiographic findings appeared 56.5%. 7. Conservative treatment was performed 40.0%, and radical treatment was employed 60.0%. 8. There was the variation of lesional sizes between 2.0cm and 15cm, average size 6.19cm. 9. Recurrent cases were 28.7%, and impacted teeth were shown 30.43%. 10. The radiographic finding has no relationship with the factors of age, location, chief complaint, recurrence and impacted teeth, however there is slight indication that the below 30-year-old patient tend to be related to the multilocular type. 11. The multilocular radiographic findings were shown more frequently in the groups of females, over average of 30.45 months, over average size of 6.19cm and radical treatment.

  • Objectives : Fatigue is a common symptom experienced by everyone. Nevertheless, clinicians have a tendency of ignoring it since fatigue itself is not considered a distinct disease. Actually, some limited research about chronic fatigue syndrome has been made within the country, but in reality, the probability of getting this syndrome is still considered very low due to the strict diagnosis standard. Therefore, there are tremendous numbers of patients who do not get enough attention from clinicians for their fatigue symptoms only because technically they do not belong to the syndrome. Therefore, a basic statistical database must be compiled and patient management programs must be developed. To accomplish this, we conducted this study by measuring degree of fatigue, clinical characteristics and processes of Oriental medical treatment of fatigue patients. Methods : The objects of this study were selected from the new patients who entered the tonification Clinic in Kynnghee Oriental Medical Center between August 11, 2000 and October 7, 2000. Their main complaint was fatigue and they did not suffer from any physical or mental problem either historically or at the time of the study. The objects were divided into two groups based on duration of fatigue; fatigue under 6 months is considered as acute fatigue and fatigue for longer than 6 months is chronic fatigue. The prepared survey sheet for measuring fatigue degree was distributed to the patients with their consent. The patients were divided again into three subgroups : the fIrst group went through 1st test and constitution test after tonification clinic; the second one went into constitution test skipping Ist measuring test; the third one went into only tonification clinic with neither 1st measuring test nor constitution test. Results : The total number of object patients was 47 and 80% of them were considered as 'fatigue patients' by the Chalder scale. Among all patients, 29.5% requested treatment for chronic fatigue, which is over 6 months. The average of scale II for all patients was 14.8, which indicates moderate fatigue. The averages of scale II-1, II-2, II-3 were respectively 7.5, 5.9, and 3.7 so the most common complaint was physical fatigue. When compared scale II based on occupations, student group scored 6.9 and office man group scored 8.5 in scale II-1, physical fatigue, but it was not significant. Conclusions : Numerous number of patients have come to Oriental medical centers or hospitals in Korea. Therefore, deeper statistical research and follow-up-monitoring are reqnired in the Oriental medical academic world. In this study, among all patients who entered the tonification Clinic in Kyunghee Oriental Medical Center, 29.5% requested treatment for chronic fatigue, which is over 6 months. This kind of statistical report is the first time trial in the Oriental medical academy world. Through these steps, more objective treatment can be made and standards of prognosis assessment can be established.tablished.
