Establishment of Healthcare Delivery System through Improvement of Health Insurance System (건강보험수가제도 개선을 통한 의료전달체계 확립방안)

  • Establishing a healthcare delivery system is key to building a cost-effective healthcare system that can prevent the waste of healthcare resources and increase efficiency. Recently, the rapid increase in the national medical expenditures due to the aging of the population and the increase in chronic diseases has raised the question about the sustainability of the healthcare system including the health insurance system. This is why we need to reform the medical delivery system, including the function setting of medical institutions. Accordingly, gradual and practical efforts based on the recognition of reality are needed for solving the problems and improving the medical delivery system. The first effort is to secure policy measures to establish functions and roles of medical institutions which are the basis of the healthcare delivery system, and a systematic medical use system for appropriate medical use. This approach can be achieved through a reasonable health insurance schemes. Without reasonable reform efforts, it will be difficult for Korea's health care system to develop into a system that can provide cost-effective and high-quality medical services that the people want.

  • The present study examined a moving bed biofilm reactor (MBBR) bioreactor on a laboratory scale for simultaneous removal of atrazine, organic carbon, and nutrients from wastewater. The maximum removal efficiency of atrazine, chemical oxygen demand (COD), total phosphorus (TP) and total nitrogen (TN) were 83.57%, 90.36%, 90.74% and 87.93 respectively. Increasing salinity up to 40 g/L NaCl in influent flow could inhibit atrazine biodegradation process strongly in the MBBR reactor.Results showed that MBBR is so suitable process for efficiently biodegrading of atrazine and nitrogen removal process was based on the simultaneous nitrification-denitrification (SND) process.

Verification of the Possibility of Convergence Medical Radiation Shielding Sheet Using Eggshells (계란 껍데기를 이용한 융합 의료방사선 차폐시트의 가능성 검증)

  • In order to manufacture a lightweight medical radiation shielding sheet, a new shielding material was studied. We tried to verify the possibility of a shielding material by mixing egg shell powder, which is thrown away as food waste at home, with a polymer material. Existing lightweight materials satisfy eco-friendly conditions, but there are difficulties in the economics of shielding materials due to the cost of the material refining process. This study aims to solve this problem by using egg shells, which are household waste. A 3 mm-thick shielding sheet was fabricated using HDPE, a polymer material, and particle distribution within the cross-section of the shielding sheet was also verified. The shape of the particles was rough and there were voids between the particles, and the average weight per unit area was 1.5 g/cm2. The shielding performance was around 20% in the low energy area and 10% in the high energy area, showing the possibility of a low-dose medical radiation shielding body.

Quality attributes and shelf-life of freshly cut beef coated with waste feather keratin-ginger starch composite enriched with avocado peel polyphenolic-rich extract

  • The utilization of coatings composed of bio-based materials in the processing and preservation of meat presents an environmentally conscious, secure, cost-effective, and superior method for prolonging the storage life of meat while also preserving its nutritional value. In this study, changes in physical, chemical, and microbiological characteristics of freshly cut beef coated with distilled water (control) and keratin-starch composites (K-S) functionalized with 0.0-, 0.2-, 0.6-, and 1.0-mL avocado peel polyphenolic-rich extract (APPPE) kept at 4℃ for 12 days were evaluated periodically at 3-day interval using standard techniques. Keratin was extracted from waste feathers, while starch was obtained from ginger rhizomes. Following a 12-day storage period, beef coated with APPPE-enriched K-S composites exhibited a significant (p<0.05) improvement in shelf life by minimizing deteriorative changes in pH and color (as determined by metmyoglobin level) in addition to inhibiting oxidative changes in lipids (as determined by TBARS level) and proteins (protein carbonyl level) in comparison to control and K-S composite without APPPE. Furthermore, microbial growth was significantly (p<0.05) suppressed in meat coated with K-S composite functionalized with APE at 0.6 and 1.0 mL compared to the control. The study suggested that APPPE-enriched K-S composite could offer an eco-friendly and safe food preservation technique for fresh meat.

Affection of Citric Acid Production from Tapioca Alcoholic Distillery Waste by Using the Cell of Aspergillus niger (Tapioca주정증류 폐기물에서 Aspergillus niger 균주의 구연산 생산에 미치는 영향에 관한 연구)

  • Tapioca alcoholic distillery waste was utilized as dual purposes to produce citric acid and to reduce the amount of waste to be treated. Primarily an attempt was made to optimize the process conditions by Aspergillus niger in shake bath. The effects of pH, temperature, nitrogen and phosphorus sources on citric acid production were investigated. Maximum concentration of citric acid was made at temperature of $30^{\circ}C$ and pH of 4.3, while maximum cell dry weight was obtained at $35^{\circ}C$. The addition of methanol or ethanol to culture medium promoted citric acid production remarkably, but the addition of $NH_4NO_3,\;KH_2PO_4$ and Manganese as mineral source decreased the acid production.

  • A column-switching semi-micro HPLC method with fluorescence detection was developed for the direct analysis of rebamipide in human plasma. Plasma was filtered through a 0.45 $\mu\textrm{m}$ membrane filter and 5 ${\mu}\ell$ of the filtrate was directly injected onto the pre-column. After elution of the plasma proteins to waste, the retained rebamipide and internal standard(ofloxacin) were transferred to a C18 semi-microcolumn (5$\mu\textrm{m}$, 150 ${\times}$2.0mm) where they were separated using acetonitrile-1.4% acetic acid (40:60, v/v) as mobile phase. Effects of Wet Cupping (Al-Hijamah) on Cholesterol in a Sudanese Population

  • Background: Wet cupping (WC) is an efficient and cost-effective technique for removing metabolic waste from the bloodstream via the skin. The study aimed to examine the effect of WC on cholesterol levels including total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) in a Sudanese population. Methods: In this prospective cross-sectional study, 30 participants undergoing regular WC therapy were enrolled. Blood samples collected twice: pre-WC therapy (case group) and 10-14 days afterward (controls). Results: Of the participants, 56.67% were male and 43.33% were female, aged 24-69. Pre-WC TC and LDL-C levels were significantly higher than the post-WC control group (p = 0.001). Conversely, HDL-C levels decreased significantly in the pre-WC cases compared to controls (p = 0.001). No significant sex-based difference in mean cholesterol levels (p > 0.05). Conclusions: After WC, males and females experienced significant reductions in TC and LDL-C, and significant increase in HDL-C.

Kinetics for Citric Acid Production from the Concentrated Milk Factory Waste Water by Aspergillus niger ATCC 9142 (Aspergillus niger ATCC 9142 세포에 의해 농축된 우유공장폐수로부터 구연산생산에 대한 동력학 연구)

  • The waste water from a milk factory was investigated for possibility of use to the production of citric acid by cells of Aspegillus niger ATCC 9142. The addition of $Mn^{2+},\;Fe^{2+}\;and\;Cu^{2+}$ ions to waste increased citric acid production steadily, but addition of metal ion $Mg^{2+}$ decreased the citric acid production. The amount of produced citric acid by Aspegillus niger ATCC 9142 with addition 50 g/1 and 100 g/1 of reducing sugar in milk factory waste water were 7.2 g/1 and 16.5 g/1 respectively. Mathematical model was simulated for their predictability of cell growth, citric acid production and substrate consumption rate and coincided with experimental data.
