Urine production is vital for the removal of certain waste products produced by metabolism in the body and for the maintenance of homeostasis in the body. The kidneys produce urine by the following three precisely regulated processes: filtration, reabsorption, and secretion. Urine is composed of water, certain electrolytes, and various waste products that are filtered out of the blood through the glomeruli. The physical features of urine are evaluated carefully to detect any abnormal findings that may indicate underlying diseases in the genitourinary system. A change in urine color may indicate an underlying pathological condition, although many of the causes of abnormal urine color are benign effects of medications and foods. A characteristic and specific odor may be the result of a metabolic disease rather than a concentrated specimen or a simple urinary tract infection. Although transient changes in urine output and nocturia are usually benign conditions, persistent abnormal findings require further workup, with a thorough medical history taking. This article presents many of the conditions that physicians may encounter and will help them in the diagnosis and in establishing a treatment plan.
Climate change is a global emergency. Consequently, current global targets to combat the climate crisis include reaching net-zero carbon emissions by 2050 and keeping global temperature increases below 1.5 ℃. In 2014, the healthcare carbon footprint was 5.5% of the total national footprint. Gastrointestinal endoscopy (GIE) has a large carbon footprint compared to other procedures performed in healthcare facilities. GIE was identified as the third largest generator of medical waste in healthcare facilities for the following reasons: (1) GIE is associated with high case volumes, (2) GIE patients and relatives travel frequently, (3) GIE involves the use of many nonrenewable wastes, (4) single-use devices are used during GIE, and (5) GIE is frequently reprocessed. Immediate actions to reduce the environmental impact of GIE include: (1) adhering to guidelines, (2) implementing audit strategies to determine the appropriateness of GIE, (3) avoiding unnecessary procedures, (4) using medication rationally, (4) digitalization, (5) telemedicine, (6) critical pathways, (7) outpatient procedures, (8) adequate waste management, and (9) minimizing single-use devices. In addition, sustainable infrastructure for endoscopy units, using renewable energy, and 3R (reduce, reuse, and recycle) programs are necessary to reduce the impact of GIE on the climate crisis. Consequently, healthcare providers need to work together to achieve a more sustainable future. Therefore, strategies must be implemented to achieve net-zero carbon emissions in the healthcare field, especially from GIE, by 2050.
Styela clava tunic is generated in large amounts as a waste from S. clava processing plants and causes environmental problem. Although biological activities of S. clava were reported by many investigators, study on S. clava tunic was little. In this study, therefore, tyrosinase inhibition and antioxidative activities of extracts from S. clava tunic using different solvent were investigated for recycling of the fishery waste. Among extraction methods tested, autoclaved extraction (25.7%) and hot water extraction (18.2%) appeared to be effective for extraction. The highest total phenolic content was 46.6 mg/g in autoclaved extract while the highest flavonoid content was 23.0 mg/g in chloroform extract. All extracts possessed tyrosinase inhibition activity and the inhibition activity was concentration-dependent. Inhibition concentration ($IC_{50}$) against tyrosinase activity was $0.36{\times}10^4$ mg/ml in ethanol extract, $0.11{\times}10^3$ mg/ml in acetone extract and 0.27 mg/ml in n-butanol extract. Among extracts tested, hot water and autoclaved extracts displayed higher antioxidative activity than organic solvent extracts. Therefore, our data suggest that extract from S. clava tunic may potential candidate for cosmetic product with whitening effect and medicine for diseases caused by various oxidative stresses.
느타리버섯의 생산단가를 절감할 목적으로 한의원 등에서 폐기처리 되고 있는 한방슬러지를 수거하여 느타리 버섯 배지의 개발을 행하였다. 한방재료의 일반성분을 검토한 결과 조회분 함량은 홍화대와 지리오갈피가 11.6%와 10.1%를 나타내어 대조구인 폐면의 9.6% 보다 높게 나타났다. 조지방 함량은 모든 시험구에서 대조구인 폐면보다 높게 나타났다. 조단백 함량은 폐면배지에서는 9.8%로 검출되었으나 야생 쑥, 지리오갈피, 한방슬러지 및 홍화대에서는 각각 14.9%, 13.9%, 13.4% 및 11.5%로 대조구보다 높게 나타났다. 한방슬러지, 야생쑥 및 지리오갈피의 pH는 5.27~5.72 범위로 폐면이 나타낸 pH 5.70과 거의 같은 수준이었다. 느타리버섯의 균사 성장에 미치는 각 한방재료의 첨가농도는 3% 및 6%의 첨가보다 9%의 첨가구에서 느타리버섯 균사의 성장이 양호하였다. 그러나 한방재료의 첨가는 $P.$$tolaassi$의 생육 및 $Trichoderma$$sp.$의 생육에는 크게 영향을 미치지 못하였다. 폐면배지에 9%의 한방슬러지를 첨가하여 실시한 현장 실험결과 버섯의 균사생장은 폐면배지에서 보다 약 2~3일 정도 빠르게 나타났고, 버섯의 수확량은 10~15% 정도 증수되었으며 또한 버섯의 대 및 갓 등이 균일하고 우수하였다.
방사선 진료기술의 발전에 따라 의료분야에서 발생하는 방사성폐기물이 급속히 증가하고 있다. 최근의 경향을 보면, 갑상선암 진료 목적의 I-131을 비롯하여 PET/CT 조영제로 사용하는 F-18, 핵의학검사에 폭 넓게 적용하는 Tc-99m 등의 방사성동위원소 사용이 일반화 되고 있다. 사용과정에서 이러한 핵종에 오염된 방사성폐기물이 발생하게 되는데, 일정기간 보관한 후에 방사능이 규제해제(Clearance) 수준으로 감쇠하면 소각 등의 방법으로 자체처분하게 된다. 국제원자력기구(IAEA)에서는 $10{\mu}Sv/y$의 개인선량 및 1man-Sv/y의 집단선량과 함께 핵종별 농도에 근거하여 방사성폐기물의 규제해제기준을 제시하였다. 이 연구에서는 IAEA의 핵종별 방사능농도기준에 따른 규제해제 시점을 확인하기 위하여, Tc-99m과 방사화학상유사한 성질을 가지고 있는 Re-186 관련 방사성폐기물을 수집하여 방사능을 측정하였다. Re-186은 반감기가 3.8일로 방사성의약품으로는 비교적 길고, 베타선 및 감마선을 방출하여 방사성의약품 치료와 영상에 모두 사용된다. Re-186 사용과 관련하여 발생하는 오염된 일회용장갑(Poly Glove)의 방사능측정을 위하여 다중파고분석기(Multi Channel Analyzer; MCA)를 이용하였으며, 이를 위하여 표준물질을 제작하여 MCA를 교정한 이후 감마방사능 측정절차에 따라 수행하였다. 측정결과를 근거로 방사능 감쇠 유도식을 산출하여 이론식과 대비하여 고찰하였는바, Re-186 핵종의 유도반감기(3.6일)는 이론적 반감기(3.8일)에 비해 다소 짧은 것으로 나타났다. 따라서 측정결과에 근거한 유도반감기를 적용한다면, 다소 줄어든 기간 동안 Re-186 핵종 폐기물을 보관하였다가 자체처분을 할 수 있을 것으로 확인하였다. 이 연구 결과는 현재 추진하고 있는 국제표준화기구(International Organization for Standardization) 국제표준에 포함될 예정이다.
최근 우리나라 기술발전과 글로벌화 경향에 맞추어, 의료기기산업도 국제적인 의료기기관리제도에 적합하도록 체질을 개선해 가고 있으며, 국제적인 규제수준에 도달할 수 있도록 완화의 가닥을 잡고 있다. 또한 최근의 입법 경향을 보면 의료기기 산업의 각종 규제는 최소한의 범위에 머물고 새로운 기술의 결합 내지 융합에 개방적인 자세를 취하고 있다. 결합 의료기기에 활용된 의료기술은 대부분이 인체나 환경에 위해성이 없는 의료기술일 가능성이 크므로, 이렇게 위해성이 제로에 가까운 의료기기를 1등급으로 분류하여 위해성이 거의 없는 의료기기와 같이 취급함으로써 발생하는 불필요한 행정력과 시간의 낭비를 최소화할 수 있다. 이를 통하여 의료기기 취급자 특히 판매업자에 포함되는 사업자의 영역이 넓혀지므로, 사업자들의 사업의지를 고양할 수 있으며, 국민의 의료기기에 대한 선택권과 접근권이 제고될 수 있다. 본 논문에서는 결합 의료기기의 중고 수리 및 유통질서를 확립하고 공정한 거래를 촉진하기 위한 제반의 법률을 조사하고 규제를 개선하는 방향을 제시한다.
현재 우리나라의 의료전달체계는 1989년 이후 1,2,3차 진료체계로 구분하여 지역 간 진료과목별 균형유지와 더불어 의료자원의 낭비를 방지하고 있으나 의료시설의 대도시 집중에 따라 지역 간 의료서비스의 불균형이 발생되고 있다. 따라서 본 연구에서는 중소도시인 진주시를 대상으로 접근성과 편리성이 강조되는 1차 의료서비스 질을 공간적 분포 특성에 근거하여 평가하였다. 그 결과 1차 의료시설 모두 공간적 분포에 대한 응집지수가 0~0.25로서 강한 응집도를 나타냈다. 필수진료과목의 경우 외과와 산부인과의 응집도가 높았으며 기타 진료과목은 성형외과와 안과의 응집도가 가장 높게 나타났다. 이를 바탕으로 병원의 위치와 이용자층과의 상관성 분석을 수행한 결과, 필수진료과목의 경우 병원의 위치와 이용자층간의 상관관계는 약하게 나타났으며 기타 진료과목 중 한의원의 경우 응집도는 높았으나 이용자층이 많은 곳에 비교적 양호하게 분포되어 접근성이 가장 우수한 것으로 분석되었다. 하지만 한의원을 제외한 나머지 기타진료과목은 이용자층과 의료시설의 위치가 상이하여 접근성 측면에서 문제점이 발생되었다. 이러한 분석 결과를 토대로 1차 의료서비스를 이용하는 이용자층을 고려한 입지선정의 개선대책 마련이 필요하다고 판단된다.
Dopamine is an effective pressor for the treatment of shock and hypotension when patients do not respond to plasma volume expansion. Two dopamine intravenous delivery systems are currently available in Korea. The objective of this study was to compare dopamine premixed with prefilled system in terms of supply costs (preparation costs + personnel time), contamination rates and convenience. Time-and-motion studies were conducted to determine the time and costs associated with preparation and administration of the two systems. They were analyzed and compared by Mann-Whitney test. To evaluate the contaminaton rates of the two systems, both systems were prepared in an open environment similar to that of practical situations. Premixed and compounded solutions were then filtered by $0.22{\mu}m$ membrane filters, which were cultured at $37^{\circ}C$ for 10 days and their contents were visually checked for bacterial contamination. The convenience of the two systems was compared by itemized user assessments on preparation, dose calculation, admixture, administration and disposal of waste matters. They were analyzed by Wilcoxon's signed rank test and 100 part percentage. It was found that the preparation costs $(mean{\pm}SD)$ for premixed and prefilled systems were $271.70\pm293.55\;Won$ (Korean currency) and $1521.04\pm510.63\;Won$, respectively. The preparation time $(mean{\pm}SD)$ for premixed system was $68.10\pm35.69\;sec.$ while at for prefilled system was $154.03\pm50.06\;sec.$ (n=59 each, p<0.001). No bacterium was observed in the samples of both systems (n=20, each). User assessments indicated that the premixed system was more convenient than the prefilled system except for the item of dose calculation (n=24, p<0.001). Subjective evaluations have proven that the use of the dopamine premixed system resulted in increased efficiency of intravenous preparation by allowing personnel to devote more time to other labor-intensive duties and lower total medical costs.
Objectives: Emergency medical personnel (EMPs) are pre-hospital emergency responders who are at risk of exposure to infections and may also serve as a source for the transmission of infections. However, few studies of infection control have specifically addressed EMPs in the Republic of Korea (hereafter Korea). The goal of this study was to assess the current status of infection prevention and control programs (IPCPs) for EMPs in Korea. Methods: A cross-sectional survey was conducted to quantitatively assess the resources and activities of IPCPs. A total of 907 EMPs in five metropolitan cities completed a structured questionnaire from September 2014 to January 2015. The data were analyzed using descriptive statistics, multi-response analysis, and the chi-square test. Results: The mean age of the participants was $34.8{\pm}15.1years$. IPCPs were found to have weaknesses with regard to the following resources: the assignment of infection control personnel (ICP) (79.5%), hand hygiene resources such as waterless antiseptics (79.3%), the use of paper towels (38.9%), personal protective equipment such as face shields (46.9%), and safety containers for sharps and a separated space for the disposal of infectious waste (10.1%). Likewise, the following activities were found to be inadequately incorporated into the workflow of EMPs: education about infection control (77.5%), post-exposure management (35.9%), and the decontamination of items and spaces after use (88.4%). ICP were found to have a significant effect on the resources and activities of IPCPs (p<0.001). The resources and activities of IPCPs were found to be significantly different among the five cities (p<0.001). Conclusions: IPCPs for EMPs showed some limitations in their resources and activities. IPCPs should be actively supported, and specific IPCP activities for EMPs should be developed.
Purpose: The negative pressure isolation ward is a key facility in preparedness and response to infectious diseases. For the sustainable operation of the facility, appropriate facility improvement is required. The experience of medical staff responding to infectious diseases in the COVID-19 pandemic provides effective informations for facility planning. Methods: The post occupancy evaluation (POE) was conducted by interviewing medical staff who is working on Nationally designated negative pressure isolation ward in general hospital. Floor plan analysis was conducted before field surveys for identifying facility characteristic and spatial composition. After that, field surveys were conducted at 3 hospitals, and interviews and fieldwork were conducted together. Results: It is necessary to increase the standard size of ward area from 15m2 to 20m2. The size of the doffing room has to be planned for accommodation of two or more people. Equipment storage, clean storage and waste storage also should be properly planned. There were almost no problems with the circulation in the ward. There was not enough space for medical staff. Implications: For a sustainable and safe negative pressure isolation ward planning, it is necessary to exploit learning from the medical staffs who have many experiences of coping with infectious diseases.
