The Proposal of Segmentation Algorithm for the Applying Breast Ultrasound Image to CAD (유방 초음파 영상의 CAD 적용을 위한 Segmentation 알고리즘 제안)

  • The objective of this paper is to design segmentation algorithm for applying the breast ultrasound image to CAD(Computer Aided Diagnosis). This study is conducted after understanding limits, used algorithm and demands of CAD system by interviewing with a medical doctor and analyzing related works based on a general CAD framework that is consisted of five step-establishment of plan, analysis of needs, design, implementation and test & maintenance. Detection function of CAD is accomplished by Canny algorithm and arithmetic operations for segmentation. In addition to, long computing time is solved by extracting ROI (Region Of Interests) and applying segmentation technical methods based morphology algorithm. Overall course of study is conducted by verification of medical doctor. And validity and verification are satisfied by medical doctor's confirmation. Moreover, manual segmentation of related works, restrictions on the number of tumor and dependency of image resolution etc. was solved. This study is utilized as a support system aided doctors' subjective diagnosis even though a lot of future studies is needed for entire application of CAD system.

  • Background: Injection therapy around the distal biceps tendon insertion is challenging. This therapy may be indicated in patients with a partial distal biceps tendon tear, bicipitoradial bursitis and tendinopathy. The primary goal of this study was to determine the accuracy of manually performed injections without ultrasound guidance around the biceps tendon. Methods: Seven upper limb specialists, two general orthopedic specialists, and three orthopedic surgical residents manually injected a cadaver elbow with acrylic dye using an anterior and a lateral infiltration approach. After infiltration the cadaveric elbows were dissected to determine the location of the acrylic dye. Results: In total, 79% of the injections were localized near the biceps tendon. Of these injections, 20% were localized on the radius near the bicipitoradial bursa. In total, 53% of the performed infiltrations were injected by anterior and 47% by lateral approaches. Of the injections near the distal biceps (79%), 47% were injected by an anterior and 53% by a lateral approach. Of the injections on the radius (20%), 33% were injected by anterior and 67% by lateral approach. Of the inaccurate injections (21%), 75% were injected anterior and 25% lateral. Conclusions: Manual infiltration without ultrasound guidance for distal biceps pathology lacks accuracy. We therefore recommend ultrasound guidance for more accurate infiltration.

Influence of biliary stents on the diagnostic outcome of endoscopic ultrasound-guided tissue acquisition from solid pancreatic lesions: a systematic review and meta-analysis

  • Background/Aims: This meta-analysis analyzed the effect of an indwelling biliary stent on endoscopic ultrasound (EUS)-guided tissue acquisition from pancreatic lesions. Methods: A literature search was performed to identify studies published between 2000 and July 2022 comparing the diagnostic outcomes of EUS-tissue acquisition (TA) in patients with or without biliary stents. For non-strict criteria, samples reported as malignant or suspicious for malignancy were included, whereas for strict criteria, only samples reported as malignant were included in the analysis. Results: Nine studies were included in this analysis. The odds of an accurate diagnosis were significantly lower in patients with indwelling stents using both non-strict (odds ratio [OR], 0.68; 95% confidence interval [CI], 0.52-0.90) and strict criteria (OR, 0.58; 95% CI, 0.46-0.74). The pooled sensitivity with and without stents were similar (87% vs. 91%) using non-strict criteria. However, patients with stents had a lower pooled sensitivity (79% vs. 88%) when using strict criteria. The sample inadequacy rate was comparable between groups (OR, 1.12; 95% CI, 0.76-1.65). The diagnostic accuracy and sample inadequacy were comparable between plastic and metal biliary stents. Conclusions: The presence of a biliary stent may negatively affect the diagnostic outcome of EUS-TA for pancreatic lesions.

Low-Intensity Pulsed Ultrasound Promotes BMP9 Induced Osteoblastic Differentiation in Rat Dedifferentiated Fat Cells

  • Dedifferentiated fat cells (DFATs) isolated from mature adipocytes have a multilineage differentiation capacity similar to mesenchymal stem cells and are considered as promising source of cells for tissue engineering. Bone morphogenetic protein 9 (BMP9) and low-intensity pulsed ultrasound (LIPUS) have been reported to stimulate bone formation both in vitro and in vivo. However, the combined effect of BMP9 and LIPUS on osteoblastic differentiation of DFATs has not been studied. After preparing DFATs from mature adipose tissue from rats, DFATs were treated with different doses of BMP9 and/or LIPUS. The effects on osteoblastic differentiation were assessed by changes in alkaline phosphatase (ALP) activity, mineralization/calcium deposition, and expression of bone related genes; Runx2, osterix, osteopontin. No significant differences for ALP activity, mineralization deposition, as well as expression for bone related genes were observed by LIPUS treatment alone while treatment with BMP9 induced osteoblastic differentiation of DFATs in a dose dependent manner. Further, co-treatment with BMP9 and LIPUS significantly increased osteoblastic differentiation of DFATs compared to those treated with BMP9 alone. In addition, upregulation for BMP9-receptor genes was observed by LIPUS treatment. Indomethacin, an inhibitor of prostaglandin synthesis, significantly inhibited the synergistic effect of BMP9 and LIPUS co-stimulation on osteoblastic differentiation of DFATs. LIPUS promotes BMP9 induced osteoblastic differentiation of DFATs in vitro and prostaglandins may be involved in this mechanism.

Modulation in Action Potentials of Rat Hippocampal Neurons Measured on Multi-Channel Electrodes During Ultrasound Stimulation (다채널 전극을 이용한 초음파 자극 시 쥐 해마 신경 세포의 활동 전위 검출)

  • It is known that ultrasound affects action potentials in neurons, but the underlying principles of ultrasonic neural stimulation are not clearly elucidated yet. In this study, we measured the action potentials of rat hippocampal neurons cultured on multi-electrode arrays during ultrasound stimulation. From most of electrodes, it was observed that the ultrasound stimulation increased the frequencies of action potentials (i.e., spikes) during ultrasound stimulation.

  • This paper proposes a calibration method for a freehand 3-D ultrasound system in medical robotic research. The calibration block with six wires was designed to set the fixed target points. The positions of the ultrasound probe and calibration block were measured using an optical tracker. The relationship between the position of the ultrasound probe and the pixel coordinates in the images was derived using a calibration process. The scaling matrix was also calculated. The experimental results showed that the proposed method could find solutions using a simple least square method from one or multiple ultrasound images.

Extraction Method of Ultrasound Spectral Information using Phase-Compensation and Weighted Averaging Techniques (위상 보상과 가중치 평균을 이용한 의료 초음파 신호의 주파수 특성 추출 방법)

  • Quantitative ultrasound analysis provides fundamental information of various ultrasound parameters using spectral information of the short-gated radiofrequency(RF) data. Therefore, accurate extraction of spectral information from backscattered RF signal is crucial for further analysis of medical ultrasound parameters. In this paper, we propose two techniques for calculating a more accurate power spectrum which are based on the phase-compensation using the normalized cross-correlation to minimize estimation errors due to phase variations, and the weighted averaging technique to maximize the signal-to-noise ratio(SNR). The simulation results demonstrate that the proposed method estimates better results with 10% smaller estimation variances compared to the conventional methods.

International Health Project for Improving the Level of Mother and Child Health in Developing Countries: Focusing on KOICA CTS Cases in Vietnam (개발도상국 모자보건 수준 향상을 위한 국제보건사업: 베트남 KOICA CTS 사례를 중심으로)

  • We developed the world's first wireless ultrasound diagnostic device. For the smooth delivery of pregnant women, the WHO requires four prior examinations, and the use of ultrasound diagnostic devices is essential for this purpose. However, income levels and medical facilities in developing countries are falling short. We conducted KOICA's CTS program in Vietnam using a self-developed radio ultrasound diagnostic device. We supplied Sonon300C, a portable wireless ultrasound diagnostic device, to health centers and hospitals in Huong-Hoa district, Vietnam, and conducted an outreach program in an effort to further enhance business performance. As a result, the rate of ultrasound diagnostics in the region reached 100% and the percentage of trained graduates increased to 59%.
