제 3형 척수소뇌실조 환자의 한방 치험 1례 (Case Report of a Type 3 Spinocerebellar Ataxia Patient Treated with Traditional Korean Medicine)

  • Objective: This study reports on one clinical case of type 3 spinocerebellar ataxia (SCA) to examine the effectiveness of herbal medicine and traditional Korean treatments.Methods: A patient with type 3 spinocerebellar ataxia was treated using traditional Korean medicine options such as acupuncture and Hyangsayukgunja-tang. The effects on type 3 spinocerebellar ataxia were measured using the numeric rating scale (NRS) and ambulatory status.Results: Improvements in NRS and ambulatory status were observed after treatment.Conclusion: According to this study, traditional Korean medicine may be effective in the treatment of type 3 spinocerebellar ataxia.

아토피 피부염 치료 처방에 대한 방제학적(方劑學的) 고찰(考察) (Study for Treatment of Atopic Dermatitis in Oriental Medical Prescription)

  • In Oriental Medicine, Atopic dermatitis(AD) belongs to the category of the Naesun, Taesun(胎癬), Taeryumchang(胎斂瘡), Eczema(濕疹), Seupchang(濕瘡), Samanpung(四彎風), Chimumchang(浸淫瘡). The Basic Prescriptions which have been used for treatment of Atopic dermatitis are saengryusamultang(生料四物湯), Onchungeum(溫淸飮), Seungmagalgeuntang(升麻葛根湯),Hoichunryangkyuksan(回春凉膈散), Doghengsan(導赤散), Pyungweesan(平胃散), Heungbangpaedoksan(荊防敗毒散), Goomigangharltang(九味羌活湯), Baekhotang(白虎湯), Gaegytang(桂枝湯), Yukmigihyuangtang(六味地黃湯). AD can divide three groups, Acute-Type, Semiacute-Type, Chronic-Type, according to it's condition. This study shows that it can be applicate Seungmagalgeuntang(升麻葛根湯), Hoichunryangkyuksan(回春凉膈散), Doghengsan(導赤散), Heungbangpaedoksan(荊防敗毒散), Goomigangharltang(九味羌活湯), Baekhotang(白虎湯), Gaegytang(桂枝湯) for treatment of Acute-Type, Pyungweesan(平胃散) for treatment of Semiacute-Type, and saengryusamultang(生料四物湯), Onchungeum(溫淸飮) for treatment of Chronic-Type.

Evaluation of a Curtain-Type Radiation Protection Device for Veterinary Interventional Procedures

  • The standard radiation protection method in the angiography suite involves the use of a thyroid shield, a lead apron, and lead glasses. However, exposure to substantial amounts of ionizing radiation can cause cataracts, tumors, and skin erythema. A newly developed curtain-type radiation protection device consists of a curtain drape composed of a five-layer bismuth and lead acrylic head-shielding plate, with both bearing an equivalent 0.25 mm lead thickness. In this study, a quality assurance phantom was used as the patient to create radiation scatter from the radiographic source, and an anthropomorphic mannequin phantom was used as the interventionalist to measure the radiation dose at seven different anatomical locations. Thermoluminescent dosimeters were used to measure the radiation dose. The experimental groups consisted of all-sided or one-sided curtain set-ups, the presence or absence of a conventional shielding system, and the orientation of beam irradiation. Consequently, the curtain-type radiation protection device exhibited better radiation protection range and capabilities than conventional radiation protection systems, especially in safeguarding the forehead, eyes, arms, and feet, with minimal radiation exposure. Moreover, the mean shielding ratios of the conventional shielding system and curtain-type radiation protection device were measured at 51.94% and 93.86%, respectively. Additionally, no significant decrease in the radiation protection range or capability was observed, even with changes in the beam orientation or one-sided protection. Compared with a conventional shielding system, the curtain-type radiation protection device decreased radiation exposure doses and improved comfort. Therefore, it is a potential new radiation protection device for veterinary interventional procedures.

구급대원의 리더십에 대한 인식 유형 (Leadership perception types among 119 emergency medical technicians)

  • Purpose: The purpose of this study was to provide basic data for developing strategic programs based on 119 emergency medical technicians leadership perception types. Methods: The subjects were thirty six 119 emergency medical technicians working for out-of hospital in Jeollanam-Do, Korea. Data were collected from July 20, 2017 to October 30, 2017. Q sample of 40 statements and P sample of 36 persons using PC QUANL software. Results: Three types of leadership styles were identified from the explanation (71.5% of the variables). The relationship oriented progressive type (the most common), the performance and value oriented type (the second most common), and the experience oriented filed participation type (the third most common). Conclusion: The fire department will need to develop and implemented leadership improvement programs based on the leadership types in order to meet the organization's goals.

Assessment of the anterior loop of the inferior alveolar nerve via cone-beam computed tomography

  • Objectives: The aim of this study was to evaluate different anatomical variants of the anterior loop of the inferior alveolar nerve (IAN) via cone-beam computed tomography (CBCT). Materials and Methods: CBCT images of 71 patients (36 males and 35 females) were evaluated. We used the classification described by Solar for IAN evaluation. In this classification, three different types of IAN loops were introduced prior to emerging from the mental foramen. We classified patients according to this system and introduced a new, fourth type. Results: Type I was seen in 15 sites (10.6%), type II in 39 sites (27.5%), and type III in 50 sites (35.2%). We found a new type in 38 sites (26.8%) that constituted a fourth type. Conclusion: We found that type III was the most common variant. In the fourth type, the IAN was not detectable because the main nerve was adjacent to the cortical plate and the incisive branch was thinner than the main branch and alongside it. In this type, more care is needed for surgeries including inferior alveolar and mental nerve transposition.

성폭력피해자를 위한 의료지원에 대한 전문가의견조사: 경남지역 의사의 성폭력에 대한 태도, 진료실태와 의료지원 필요도를 중심으로 (An Exploratory Study of the Effecitve Medical Supports for the Sexual Violence Vvictims: Based on Medical Doctors' Attitudes Toward the Victims, Medical Services Provided and Needs for Medical Supports)

  • 성폭력피해자를 위한 의료지원에 대한 전문가 의견조사로서, 의료서비스 제공 주체인 의사의 성폭력 피해자 및 진료에 대한 태도, 의료서비스 제공실태 및 필요한 의료지원에 대한 의견을 조사하였다. 남녀의사 83명으로부터 수집된 설문조사 자료를 토대로, 피해자 치료경험여부와 해당 진료과여부에 따라 의사유형을 분류하여 비교하였다. 치료경험이 있는 의사(type1)는 성폭력피해자 치료비 보장과 정액반응검사, 다양한 법정증언지원에 대한 필요도가 높았다. 치료경험이 없지만 해당 진료과인 의사(type2)는 피해여성에 대한 부정적 견해, 진료에 따르는 현실적 어려움을 예상하는 정도가 높았다. 특화의료지원 및 사정지원방안에 대한 필요도는 낮은 반면, 법정증언 지원방안에 대한 필요도는 높게 나타났다. 치료경험이 없고 해당진료과가 아닌 의사(type3)는 피해여성에 대한 이해도가 높았고 성폭력관련 법지식과 관련기관에 대한 인지도도 높았다. 반면, 진료시 예상하는 어려움의 정도가 가장 높았으며, 피해여성 진료에 필요한 의료지원방안, 사정지원방안, 법정지원방안에 대한 필요도가 대체로 높게 나타났다. 연구결과를 토대로 의사유형별 차이점에 따라 성폭력피해자 진료가능성을 증진시킬 수 있는 방안과 성폭력피해자를 위한 의료지원방안이 제시되었다.

Diffuse-Type Histology Is Prognostic for All Siewert Types of Gastroesophageal Adenocarcinoma

  • Purpose: The optimal treatment for gastroesophageal junction adenocarcinoma (GEJA) remains controversial. We evaluated the treatment patterns and outcomes of patients with locally advanced GEJA according to the histological type. Materials and Methods: We conducted a single-institution retrospective cohort study of patients with locally advanced GEJA who underwent curative-intent surgical resection between 2010 and 2020. Perioperative therapies as well as clinicopathologic, surgical, and survival data were collected. The results of endoscopy and histopathological examinations were assessed for Siewert and Lauren classifications. Results: Among the 58 patients included in this study, 44 (76%) were clinical stage III, and all received neoadjuvant therapy (72% chemoradiation, 41% chemotherapy, 14% both chemoradiation and chemotherapy). Tumor locations were evenly distributed by Siewert Classification (33% Siewert-I, 40% Siewert-II, and 28% Siewert-III). Esophagogastrectomy (EG) was performed for 47 (81%) patients and total gastrectomy (TG) for 11 (19%) patients. All TG patients received D2 lymphadenectomy compared to 10 (21%) EG patients. Histopathological examination showed the presence of 64% intestinal-type and 36% diffuse-type histology. The frequencies of diffuse-type histology were similar among Siewert groups (37% Siewert-I, 36% Siewert-II, and 33% Siewert-III). Regardless of Siewert type and compared to intestinal-type, diffuse histology was associated with increased intraabdominal recurrence rates (P=0.03) and decreased overall survival (hazard ratio, 2.33; P=0.02). With a median follow-up of 31.2 months, 29 (50%) patients had a recurrence, and the median overall survival was 50.5 months. Conclusions: Present in equal proportions among Siewert types of esophageal and gastric cancer, a diffuse-type histology was associated with high intraabdominal recurrence rates and poor survival. Histopathological evaluation should be considered in addition to anatomic location in the determination of multimodal GEJA treatment strategies.

  • The purpose of this study was to investigate the clinical application of relaxation training and oriental medical treatment to the SimDamHeoGup(心膽虛怯) type of ChilJungGuQiJung(七情九氣證) patient. In this case, a male patient, 31 years old, who lost his job two months ago, complained of the tension of body and anxiety. We treated the patient with relaxation training and oriental medical treatment such as herbal medicine, acupuncture treatment and aroma therapy. And we diagnosed his state SimDamHeoGup(心膽虛怯) type of ChilJungGuQiJung(七情九氣證), so we prescribed HaChuBoSimTang(夏朮補心場) for patient. In result, the patient’s tension of body and anxiety was improved, so he could recover his confidence slowly. Conclusionally, relaxation training and oriental medical treatment could be effective method to treat the patient of adjustment disorder with the tension of body and anxiety.

Long-term complications after extrahepatic cyst excision for type IV-A choledochal cysts

  • Forty-five adults with type IV-A choledochal cysts (CDC) who underwent extrahepatic cyst excision from January 2013 to December 2021 were followed up for a median interval of 25 months (range, 2 to 10 years) to observe the long-term complications in the remaining intrahepatic cyst. Late complications in varying combinations were seen in 10 patients, which included cholangitis and/or intrahepatic stones in 9 patients, intrahepatic bile duct stenosis with stones in 2 patients, anastomotic stricture in 6 patients, and left lobar atrophy with intrahepatic stones in 3 patients. Out of 6 patients who required re-do hepaticojejunostomy (HJ), three patients had left lobe atrophy with patent HJ anastomosis and a recurrent attack of cholangitis on follow-up at 3, 8, and 10 years. Complications occur frequently after extrahepatic cyst excision for type IV-A CDC and require a long-term follow-up.
