의료 3D 프린팅 기술의 전망 및 소아치과분야에서의 활용 (Prospect for 3D Printing Technology in Medical, Dental, and Pediatric Dental Field)

  • 3D 프린팅 기술이 가장 많이 활용될 수 있는 분야의 하나가 의학분야이다. 3D 프린팅 기술은 최근들어 더욱 상업화되고 프린팅에 사용되는 재료 또한 생체친화성, 생분해성 고분자를 이용 가능하게 됨에 따라 생체의료분야에서의 활용성이 점차적으로 높아지고 있는 경향이다. 생체의료분야에서는 수술 모형을 제작하고 절제범위와 시술 후의 형태를 시술 전에 미리 확인하여 시술시간을 단축하고 부작용을 최소화하고 있으며 인공 골과 장기를 생산함으로써 이식에 따른 부작용을 감소시키고 있다. 또한 보청기, 의족 등 맞춤형 의료 보조용품을 생산하고 있다. 치의학 분야에도 크라운, 덴쳐 등의 보철 수복물 제작, 교정 장치 및 모델 제작, 임플란트 식립이나 외과 수술을 위한 수술용 가이드 제작 등 치과 의료기술을 한 차원 더 높일 수 있을 것으로 전망된다. 그러나 아직은 프린팅 재료(소재), 조형기술, CAD 관련 소프트웨어 기술, 생체안정성과 유효성 검증, 호환성과 표준화 등 해결해야 할 과제가 산적해 있있어 앞으로 이에 대한 지속적인 연구, 개발이 이루어져야 할 것으로 사료된다.

Factors affecting treatment outcomes in patients with oral lichen planus lesions: a retrospective study of 113 cases

  • Purpose: Oral lichen planus (OLP) is a chronic oral mucosal disease that has been recognized as an immune condition. The purpose of this study was to evaluate factors affecting the clinical outcomes of topical corticosteroid application on OLP lesions using dexamethasone gargle and ointment. Methods: The charts of patients who were clinically diagnosed with OLP and treated with dexamethasone from July 2003 to August 2017 at the Section of Dentistry of Seoul National University Bundang Hospital were thoroughly evaluated to identify subjects who were suitable for this retrospective study. For each patient, age at the index date, gender, medical history, and dental records related to OLP lesions and dexamethasone treatment were reviewed. Results: In total, 113 of the 225 patients were included in the present study. Among them, 79 patients were female (69.9%) and 34 were male (30.1%), with a mean age of 57.6 years. The average duration of dexamethasone treatment was 4.7 months and the mean follow-up period was 2.24 years. Improvements were observed within 1 year after dexamethasone treatment in most cases, and 17.7% of patients had a new OLP lesion after treatment. New OLP lesions were more frequently gingival than mucosal, although mucosal OLP lesions were more common than gingival OLP lesions in all age groups. In age- and gender-adjusted multivariate logistic regression, a history of malignant disease was found to be a significant factor affecting the formation of new lesions. Gingival OLP lesions and intermittent use of dexamethasone showed near-significant associations. In Kaplan-Meier failure analysis, history of malignancy, menopausal status, age, and the site of the OLP lesion were significant factors affecting clinical outcomes. Conclusions: The treatment outcomes of OLP were significantly influenced by age, history of malignancy, menopausal status, and the site of the OLP lesion, but not by factors related to dexamethasone treatment.

Impact of iron deficiency anemia on chronic periodontitis and superoxide dismutase activity: a cross-sectional study

  • Purpose: Both chronic periodontitis (CP) and iron deficiency anemia (IDA) induce oxidative stress in the body and cause an imbalance between reactive oxygen species and antioxidants, such as superoxide dismutase (SOD). This study explored the SOD enzyme activity of saliva and serum in CP patients with and without IDA and analyzed the impact of IDA on CP. Methods: A total of 82 patients were divided into four groups: control group (CG, 22), periodontally healthy IDA patients (IDA-PH, 20), CP patients (CP, 20), and IDA patients with CP (IDA-CP, 20). After clinical measurements and samplings, serum and salivary SOD levels were determined using an SOD assay kit. Results: IDA-CP patients exhibited a higher gingival index, bleeding on probing, probing pocket depth, and percentage (%) of sites with a clinical attachment loss (CAL) of ${\geq}6mm$ (P<0.008) than CP patients. The mean salivary and serum SOD levels were significantly lower in the IDA-PH, CP, and IDA-CP patients than in the CG group (P<0.008). A significant positive correlation between salivary and serum SOD activity was observed in IDA (P<0.05). Furthermore, serum and salivary SOD levels were significantly and negatively correlated with all periodontal parameters including the percentage of sites with CAL of 4-5 and ${\geq}6mm$ (P<0.05) except the significant correlation between salivary SOD activity and mean CAL and the percentage of sites with CAL of 4-5 mm (P>0.05) in these patients. Conclusions: Within the limits of this study, it may be suggested that IDA patients with chronic periodontitis have more periodontal breakdowns than patients with chronic periodontitis. Serum and salivary SOD activity levels were lower in the IDA-PH, CP and IDA-CP groups than in the CG. Iron deficiency anemia influenced the serum SOD activity but did not seem to affect the salivary SOD activity in these patients.

MICS 대역과 ISM 대역에서 인체 전기적 상수를 갖는 준(準) 고체형 플랫 팬텀 제작 (The Study on Implementation of a Semi-Solid Flat Phantom with Equivalent Electrical Properties to Whole Human Body at MICS and ISM Band)

  • MICS 및 ISM 대역의 무선기기 설계 시에 인체 팬텀을 이용한 성능검증이 필수적으로 요구되고 있으나, 인체팬텀에 관한 연구 대부분이 휴대폰에 의한 생체 영향에 국한되어 있는 실정이다. 본 논문에서는 FCC에서 제안하고 있는 MICS 대역과 ISM 대역의 인체 팬텀에 대한 전기적 상수를 이용하여 준고체형 팬텀들을 제작하였다. 제작된 준 고체형 팬텀들은 각 대역에서 FCC에서 제시한 전기적 상수(MICS 대역에서 $\varepsilon_r=56.7$, $\sigma=0.94$, ISM 대역에서 $\varepsilon_r=52.7$, $\sigma=1.95$)들을 만족하였다. 또한, 제작된 준 고체형 팬텀의 재료들은 쉽게 구매 가능한 재료들로 구성되었으며, 팬텀 재료의 다양성을 위해서 각 대역에서 polyethylene과 TX-151을 이용한 방법과 글리세린을 이용한 두 가지 방법을 제시하였다. 제작된 준 고체형 팬텀은 평탄한 형태이며, 유전율 측정기로 제작후 1일 (24 시간)이 경과한 후 팬텀의 전기적 특성을 측정하였다.

성견 치계줄기세포 및 골수줄기세포 특성에 관한 연구 (Investigation of postnatal stem cells from canine dental tissue and bone marrow)

  • Purpose: The aim of this study was to evaluate the stemness of cells from canine dental tissues and bone marrow. Methods: Canine periodontal ligament stem cells (PDLSC), alveolar bone stem cells (ABSC) and bone marrow stem cells(BMSC) were isolated and cultured. Cell differentiations (osteogenic, adipogenic and chondrogenic) and surface antigens (CD146, STRO-1, CD44, CD90, CD45, CD34) were evaluated in vitro. The cells were transplanted into the subcutaneous space of nude mice to assess capacity for ectopic bone formation at 8 weeks after implantation. Results: PDLSC, ABSC and BMSC differentiated into osteoblasts, adipocytes and chondrocytes under defined condition. The cells expressed the mesenchymal stem cell markers differently. When transplanted into athymic nude mice, these three kinds of cells with hydroxyapatite /${\beta}$- tricalcium phosphate (HA/TCP) carrier showed ectopic bone formation. Conclusions: This study demonstrated that canine dental stem cells have stemness like bone marrow stem cells. Transplantation of these cells might be used as a therapeutic approach for dental stem cell-mediated periodontal tissue regeneration.

천연추출물 Curcuma xanthorriza oil 함유치약의 치태 및 치은염 억제효과 (Suppressive effect of Curcuma xanthorrhiza oil on plaque and gingivitis)

  • To find out the suppressive effect of natural extract Curcuma xanthorrhiza on $IL-1{\beta}$ and MMP-2 derived from periodontal ligament cells through in vitro study and to confirm its effect on plaque and gingivitis through clinical study, Curcuma xanthorrhiza containing toothpaste was used and following results were produced. 1. In vitro study, type IV collagenase MMP-2 production was inhibited dose-dependently in the group treated with Curcuma xanthorrhiza compared to the control group. 2. In vitro study, the production of $IL-l{\beta}$ which is one of the inflammatory mediators associated with periodontitis was inhibited dose-dependently in the group treated with Curcuma xanthorrhiza. 3. On the third week, the plaque index of the groups treated with or without Curcuma xanthorrhiza containing toothpastes were both increased significantly compared to the baseline(p<0.05). 4. On the third week, the gingival index of the group treated with Curcuma xanthorrhiza containing toothpaste was not significantly different from baseline. However, the group treated without Curcuma xanthorrhiza containing toothpaste showed a significant increase of gingival index at shielded area(p<0.05). 5. The gingival index of the group without Curcuma xanthorrhiza containing toothpaste showed a significant increase in the sites without tooth brushing when compared to sites with tooth brushing(p<0.05). However. there was no significant difference for the group with Curcuma xanthorrhiza containing toothpaste in sites either with or without tooth brushing. 6. The Bleeding on probing for the group without Curcuma xanthorrhiza containing toothpaste showed no significant difference even when tooth brushing was done. However, for the group with Curcuma xanthorrhiza containing toothpaste, bleeding on probing was significantly reduced compared to baseline when tooth brushing was done(p<0.05).

The effect of local application of thymoquinone, Nigella sativa's bioactive component, on bone healing in experimental bone defects infected with Porphyromonas gingivalis

  • Purpose: This study was performed to evaluate the influence of local application of thymoquinone (TQ) on bone healing in experimental bone defects infected with Porphyromonas gingivalis (PG). Methods: Forty-two female rats were randomly divided into 6 groups. A bone defect was created on the right tibia of all animals. The PG, PG/collagen membrane (COL) and PG/TQ/COL groups were infected with PG. In the COL and PG/COL groups, the defects were covered with a COL; in the TQ/COL and PG/TQ/COL groups, the defects were covered with a TQ-containing COL. After 28 days, all animals were sacrificed. Quantitative measurements of new bone formation and osteoblast lining, as well as semiquantitative measurements of capillary density and tissue response, were analyzed. Furthermore, the presence of bacterial infections in defect areas was evaluated. Results: The new bone formation, osteoblast number, and capillary density were significantly higher in the TQ groups than in the control groups (P<0.001, P<0.001, and P<0.01, respectively). In a comparison between the TQ/COL group, with a TQ-containing COL (TQ/COL), and the PG-infected TQ-containing COL (PG/TQ/COL) group, the newly formed bone and capillary density were higher in the TQ/COL group (P<0.01). When the control group was compared to the PG, PG/COL, and PG/TQ/COL groups in terms of tissue response, the differences were statistically significant (P<0.001, P=0.02, and P=0.041, respectively). The intensity of the inflammatory cell reaction was higher in the PG, PG/COL, and PG/TQ/COL groups (P<0.05). Conclusions: Within the limitations of this study, the local application of a TQ-containing COL positively affected bone healing even if the bone defects were infected. The results suggest that TQ increased angiogenesis and showed promise for accelerating bone defect healing. Further research is warranted to support these findings and reach more definitive conclusions.

Repeated irradiation by light-emitting diodes may impede the spontaneous progression of experimental periodontitis: a preclinical study

  • Purpose: We investigated whether repeated irradiation with light-emitting diodes (LEDs) at a combination of 470 nm and 525 nm could suppress the progression of experimental periodontitis. Methods: A experimental periodontitis model was established in the second, third, and fourth premolars of the mandible in beagle dogs for 2 months. The spontaneous progression of periodontitis was monitored under the specified treatment regimen for 3 months. During this period, the animals were subjected to treatments of either plaque control only (control) or plaque control with LED application (test) at 2-week intervals. The clinical parameters included the probing pocket depth (PPD), gingival recession (GR), and the clinical attachment level (CAL). Histomorphometric analysis was performed using measurements of the length of the junctional epithelium, connective tissue (CT) zone, and total soft tissue (ST). Results: There were significant differences in PPD between the control and test groups at baseline and 12 weeks. When the change in PPD was stratified based on time intervals, it was shown that greater differences occurred in the test group, with statistical significance for baseline to 12 weeks, 6 to 12 weeks, and baseline to 6 weeks. There was no significant difference in GR between the control and test groups at any time points. Likewise, no statistically significant differences were found in GR at any time intervals. CAL showed a statistically significant difference between the control and test groups at baseline only, although significant differences in CAL were observed between baseline and 12 weeks and between 6 and 12 weeks. The proportion of CT to ST was smaller for both buccal and lingual areas in the control group than in the test group. Conclusions: Repeated LED irradiation with a combination of 470-nm and 525-nm wavelengths may help suppress the progression of periodontal disease.

심한 교모와 구치부 상실을 보이는 환자의 전악 수복: Jaw motion tracking과 digital workflow를 활용한 증례 보고 (Full-mouth rehabilitation of severely attrited dentition with missing posterior teeth: a case report using digital workflow with jaw motion tracking)

  • 최근 다양한 증례 보고에서 소개되고 있는 jaw motion tracking은 환자의 안궁 이전 및 개별화된 하악 운동 경로를 기록한 후, 이를 computer-aided-design/computer-aided-manufacturing (CAD-CAM) 소프트웨어의 가상공간상에 재현하는 방법이다. 본 증례의 환자는 오랜 기간 구치부의 상실로 인해 교합평면의 붕괴가 관찰되었기에, 수직 교합 고경의 증가를 동반한 완전 구강 회복술을 계획하였다. 우선 jaw motion tracking을 진행하여 새로운 중심위 상에서 환자의 하악 운동을 기록한 후, 이 정보를 환자의 초진 구내 데이터 및 3차원 안면 스캔 데이터와 조합하여 가상 환자를 생성하였다. 가상 환자 상에서 진행한 디지털 왁스업과 임플란트 식립 계획을 바탕으로 임시 보철물을 제작하였다. 새롭게 설정된 수직 교합 고경 상에서 적절한 견치 유도를 보이는 임시 보철물의 검증을 통해, 최종 보철물로 이행하였다. 이처럼 디지털 치의학 기술의 장점을 활용하여 환자는 저작능과 심미성의 개선에 만족하였기에 본 증례를 보고하고자 한다.

Adjunctive effect of 470-nm and 630-nm light-emitting diode irradiation in experimental periodontitis treatment: a preclinical study

  • Purpose: This study investigated the adjunctive effect of light-emitting diodes (LEDs) in the treatment of experimental periodontitis. Methods: Experimental periodontitis was induced by placing ligatures around the mandibular second, third, and fourth premolars of 6 beagles for 3 months. After ligature removal, periodontitis progressed spontaneously for 2 months. The animals' hemimandibles were allocated among the following 3 groups: 1) no treatment (control), 2) scaling and root planing (SRP), and 3) SRP with LED irradiation at 470-nm and 630-nm wavelengths (SRP/LED). The probing pocket depth (PPD) and gingival recession (GR) were measured at baseline, 6 weeks, and 12 weeks. The clinical attachment level (CAL) was calculated. After 12 weeks, histological and histomorphometric assessments were performed. The distances from the gingival margin to the apical extent of the junctional epithelium (E) and to the connective tissue (CT) attachment were measured, as was the total length of soft tissue (ST). Results: PPD and CAL increased at 12 weeks compared with baseline in the control group (6.31±0.43 mm to 6.93±0.50 mm, and 6.46±0.60 mm to 7.61±0.78 mm, respectively). PPD and CAL decreased at 12 weeks compared with baseline in the SRP group (6.01±0.59 to 4.81±0.65 mm, and 6.51±0.98 to 5.39±0.93 mm, respectively). PPD and CAL decreased at 12 weeks compared with baseline in the SRP/LED group (6.03±0.39 to 4.46±0.47 mm, and 6.11±0.47 to 4.78±0.57 mm, respectively). The E/ST and CT/ST ratios significantly differed among the 3 groups (P<0.05). The clinical parameters and histologic findings demonstrated that 470-nm and 630-nm wavelength LED irradiation accompanying SRP could improve treatment results. Conclusions: Within the study limitations, 470 nm and 630 nm wavelength LED irradiation might provide additional benefits for periodontitis treatment.
