A Study on the Spatial Configuration of Type of Health Examination Center (건강검진센터의 공간유형과 구조체계에 관한 연구)

  • Due to development of modern medical services and economics, people raised expectation and demand about medical services from previous disease treatment to comprehensive health care covering prevention and health care. Responses of each medical facility to these social needs and the evolution of concept of medicine rapidly occur. The health examination centers are being operated with the purpose of health examination and this trend is reflected on several aspects such as the size of the facilities, function and configuration of space in health examination centers. Thus, health examination centers consisting of various space systems appear, but this trend and interpretations are lacking. Therefore, the purpose of this study is to draw trends of system through analysis of types and its evolved space systematic analysis and establish it. Analysis targets were classified into small, medium and large groups by sizes based on number of space and a total of 12 health examination centers in four for each category were selected. As research methods, functional relationship of space was examined through analysis of type in which segmentalized type tools were applied in local units. The flow diagram was established based on direction turning point and was classified into sub-flow and main-flow in local units and the systems between local units were derived. Finally, the results of this study can be summarized as the following three results. 1) The space connection system of health examination center showed four systems such as circulation, independence, continuation, and network. 2) Local type indicators and global type indicators which were evolved more from limitation of type analysis tools in existing research were derived so that more systematic analysis could be made. 3) Network system is distributed approach system and space for each function is formed around public space.

Analysis of the Productivity Trend of Public Health Centers in Gangwon-do Using the Malmquist Productivity Index(2006-2013) (맘퀴스트 생산성지수를 활용한 강원도 보건소의 생산성 변화 분석(2006-2013))

  • Objectives : The purpose of this study was to evaluate the productivity changes of 18 public health centers in Gangwon-do from 2006 to 2013 using the Malmquist Productivity Index(MPI). Methods : Data were collected from Statistics Korea from 2006 to 2013. The input variables were the numbers of medical, nursing and administrative personnels. The output variables were the performances of health promotion programs. Along with the traditional input-oriented DEA analysis, the MPI was calculated. Results : First, among the 18 public health centers, the productivity index of 14 public health centers was increased. Second, the annual productivity showed a 6% increase. Third, the productivity improvements were mainly caused by Scale Efficiency Change. Conclusions : Improving the productivity of public health centers requires the support and external policies of the national and local government. Internally, public health centers need to maintain scale optimization of the center. Additionally, efforts should be made to effectively use limited resources.

Health Management Status of Day Care Centers (보육시설 영유아 건강관리 현황)

  • Purpose: This study was conducted to identify the status of health management and to evaluate the need for health professionals in day care centers in the South Korea. Method: Data were obtained from 360 child care providers in 16 provinces from May to August, 2007. Data was analyzed using SPSS WIN 14.0. Results: Safety education (traffic. 83.8% general safety. 71.6%) was provided more often than the other topic of education. All health related education was provided by child care providers. When minor health problems occurred, the most common management strategy was to send the child home (36.1%). During emergency conditions, majority of child care providers preferred taking the children to medical centers without giving any emergency care and 19.4% reported sending the child home without any immediate care. Child care providers recognized the need for health care professionals. Conclusion: The results of this study suggest the need for services of health professionals in day care centers to satisfy the needs and expectations for health management in these centers. As health professionals, public health nurses or pediatric nurse practitioners may play an important role in health management of children in day care centers.

Review on the Enhancement of Accessability to Rehabilitation Therapy -Especially on the Solo Practice of the Physical Therapists (재활치료에 관한 접근성 제고 방안에 관한 단상 -물리치료사의 단독 개업 중심으로)

  • Purpose : The purpose of this study is to check the current status of accessability to rehabilitation therapy in local areas and to review how to improve the accessability. In fact, first, it is very hard for patients to find out a local medical center with rehabilitation therapy capability. Moreover they needs to wait long time to get a treatment they need, because of lack of nearby rehabilitation centers. The best way to enhance the accessability to the rehabilitation therapy service is to allow therapists to set up rehabilitation centers in local areas on their own, physically independent from medical doctors. Basically, the current law does not prohibit therapists' own sole practice. However, it needs to be amended by inserting an explicit legal basis on the setting-up process. If it is legally permissible for the therapists to set up rehabilitation centers to perform a rehabilitation treatment with referrals from of medical doctors (though physically independent from the doctors), it would result in the increase of profits for the doctors and at the same time raise therapists' freedom of occupation, a constitutional right. Furthermore, with their own places to practice, therapists will have to compete with other therapists, that would raise the quality of their treatments, which will in turn benefit patients ultimately. A proposed bill of amendments to the Act on Medical Technicians and etc. is pending for review at the National Assembly. I look forward to vigorous discussion on the bill based on this article, and resulting in revision of the law for the benefits of patients.

Epidemiology and Clinical Characteristics of Enterovirus Infections in Children: A Single Center Analysis from 2006 to 2010 (2006-2010년 단일기관 소아에서의 엔테로바이러스 감염 역학 및 임상 양상에 관한 연구)

  • Purpose : This study was performed to investigate the epidemiology of enterovirus (EV) infections in children at a secondary hospital during recent 5 years. Methods : We collected the cerebrospinal fluid, stool and throat swab samples from the pediatric patients with suspected EV infections in KEPCO Medical Center, Seoul, Korea from July 2006 to September 2010. EV detection and genotype identification were performed by RT-PCR at Korea Centers for Disease Control and Prevention. Results : A total of 386 samples were collected from 277 patients during study period. Ninety-eight patients (35.4%) were diagnosed with EV infections. The RT-PCR positive rate was the highest in throat swab samples (48.3%). The median age of patient was 4.7 years (range, 0.1-12.5 years). Aseptic meningitis (50, 51.0%) was the most common clinical manifestation; herpangina (22, 22.4%) and hand-foot-mouth disease (18, 18.4%). One hundred EVs were isolated from 98 patients and 20 genotypes of EV were identified; Echovirus 30 (28 cases, 28%), Enterovirus 71 (12 cases, 12%), Echovirus 25 (10 cases, 10%), Echovirus 9 (9 cases, 9%) and Coxsackievirus A6 (8 cases, 8%). Aseptic meningitis caused by Echovirus 30 was the most common manifestation in 2008. There was no complicated case caused by Enterovirus 71. Conclusion : This study showed the epidemiology of confirmed EV infection in children from 2006 to 2010. There is a need for continuous surveillance of EV infections and its clinical manifestations.

A Study on Public Health Doctors' Participation in District Public Health Program of Health Sub-centers in Korea (보건지소 공중보건의사의 지역보건사업 참여 실태)

  • Objectives: To investigate the state of medical care around health sub-centers, public health doctors' participation and opinion in the process of district public health programs. Methods: The study included 1,036 public health doctors who worked at health sub-center all over the country. The data were collected for Feb, 2002 using self-administered questionnaire by mail. Results: One or two doctors were working at health sub-center and 33.5% of health sub-centers was located in the region of the separation of prescription and dispensing. There were another medical facilities in 45.9% of the administrative district(eup or myon) where health sub-centers were located. The count of medical utilization went down to 14.8${\pm}$14.8 per a day in Nov, 2001 from 18.0${\pm}$15.6 in May, 2000, and the decline was much more in the region of the separation of prescription and dispensing. Among public health programs in health sub-centers, public health doctors participated mostly in preliminary medical examination for vaccination and least in health education. They participated in implementation rather than planning or evaluation of health program. Over a half of public health doctors were found to be positive that health programs implemented in their health sub-centers would promote the level of health in community people and they were willing to participate in district public health program if community people were in need. Conclusions: Recently health sub-centers are required to turn into health promotion facilities rather than medical practice facilities. Health program in health sub-centers will be advanced in both quality and quantity by turning the role of public health doctors who have provided medical services mainly into managing health program. Persistent education about managing health program and the policy to motivate participation in health program should be provided for public health doctors.

Survey of Doctors' Role Expectation and Patients Satisfaction with Referral Centers (의료협력센터에 대한 개원의 역할기대와 환자 만족도 조사)

  • Purpose: This study focuses on doctors' role expectation and patients satisfaction with the referral centers to get necessary basic Method: There were 495 subjects in this study. 245 of them were doctors working in the 397 clinics and hospitals which have made contracts with the referral center in Y General Hospital located in J city, and 250 were patients who had been treated in it. The data were collected from Sep. 25 to Oct. 21, in 2006. Data analysis was processed with SPSS/WIN 14.0 electronically. Results: Concluding cooperative treaties with other clinics and hospitals, doctors expect the following effects: First, the improvement of the treatment quality of patients. The patients satisfaction ranking order for using referral centers is as follows: kindness of the staff, treatment reservation, doctor's satisfying explanation. providing one-stop service, reduction of waiting time for treatment, evasion of repetitive tests. Conclusion: Referral centers should provide various services totally and actively which reflect the role expectation and satisfaction of doctors and patients altogether.

  • As a functionally complicated facility, it requires rational strategy to accommodate multi-functional needs to plan a hospital. Ambulatory surgery units located in large hospitals have problems such as longer waiting time and congestion for patients due to complicated planning and organization within limited area, which raise needs for independent ambulatory surgery centers to enable smoother medical treatment for patients. To design an independent ambulatory surgery center, understanding of zoning and layout of each unit needs is particularly required. This study focuses on research of efficient zoning/organization of day surgery unit and circulation arrangements of ambulatory surgery centers by analyzing various examples abroad, which will eventually help finding appropriate way of planning domestic ambulatory surgery centers. Specific area of research includes ambulatory surgery process, programmatic requirements, spatial organization of the facility, layout of surgery unit and circulation arrangements of ambulatory surgery centers.

Role of Interventional Radiologists in Trauma Centers (외상센터에서의 인터벤션 영상의학 의사의 역할)

  • Based on statistics available in Korea, trauma centers play a critical role in treatment of patients with trauma. Interventional radiologists in trauma centers perform various procedures, including embolization, which constitutes the basic treatment for control of hemorrhage, although interventions such as stent graft insertion may also be used. Although emergency interventional procedures have been used conventionally, rapid and effective hemorrhage control is important in patients with trauma. Therefore, it is important to accurately understand and implement the concept of damage control interventional radiology, which has gained attention in recent times, to reduce preventable trauma-induced mortality rates.
