A Study on the case of Application of Women's Personnel in the Australian Defence Force (호주군 여성인력의 활용과 우리군에 주는 시사점)

  • After participating in the Second Boer War in 1899, the Australian Army participated in world wars such as World War I, World War II, the Korean War, and the Vietnam War. To overcome the decline in the social recognition of soldiers and the continuous shortage of troops caused by the protracted war, the status and role of female personnel were expanded. The use of female manpower, which started as medical support during the Second Boer War, expanded to combat support missions such as communications, maintenance, driving, secret document management, and radar surveillance during World War II. After the Vietnam War, the Chiefs of Staff of the Australian Army, Navy and Air Force established a committee to expand women's participation in the military, improved service conditions for female personnel and supplemented regulations so that they were treated as soldiers, not women, and reached a turning point in expanding the role of female personnel in the Australian military. As a result, all combat positions, including special forces, were opened to women in 2014, and a plan was established to increase the proportion of women in each service by 2030. Like the Australian military, South Korea is also experiencing a demographic cliff and shortage of troops due to the continuing low birthrate problem. Transfer Learning based DNN-SVM Hybrid Model for Breast Cancer Classification

  • Breast cancer is the disease that affects women the most worldwide. Due to the development of computer technology, the efficiency of machine learning has increased, and thus plays an important role in cancer detection and diagnosis. Deep learning is a field of machine learning technology based on an artificial neural network, and its performance has been rapidly improved in recent years, and its application range is expanding. In this paper, we propose a DNN-SVM hybrid model that combines the structure of a deep neural network (DNN) based on transfer learning and a support vector machine (SVM) for breast cancer classification. The transfer learning-based proposed model is effective for small training data, has a fast learning speed, and can improve model performance by combining all the advantages of a single model, that is, DNN and SVM. To evaluate the performance of the proposed DNN-SVM Hybrid model, the performance test results with WOBC and WDBC breast cancer data provided by the UCI machine learning repository showed that the proposed model is superior to single models such as logistic regression, DNN, and SVM, and ensemble models such as random forest in various performance measures.

Standards for Applying Reasonable Receive Bandwidth to Suppress Metal Artifacts in MRI (MRI 검사 시 금속 인공물 억제를 위한 합리적인 수신대역폭 적용 기준)

  • This study aimed to present reasonable reception bandwidth application standards for the purpose of suppressing metal objects during MRI examinations. For this purpose, T2 contrast images were acquired using high-speed spin echo technology on a phantom made of screws for spinal surgery, and metal objects were detected. In addition, images were obtained by increasing the reception bandwidth from 100 Hz/PX to 800 Hz/PX by 100 Hz/PX. The metal artifacts were determined as the sum of the areas of the signal attenuation area and the signal accumulation area. In addition, Pearson correlation analysis was performed to analyze the pattern of metal artifacts according to imaging variables. As a result, the signal accumulation area did not change significantly as the reception bandwidth increased (p>0.05), but the signal loss area and the area of metal artifacts decreased as the reception bandwidth increased (p<0.05). Interestingly, the area of metal objects decreased to a maximum in the section where the reception bandwidth was increased from 100 Hz/PX to 200 Hz/PX, consistent with the section where the echo spacing was reduced to a maximum due to the increase in reception bandwidth. In addition, the correlation analysis results also showed that the eco spacing was more related to the signal attenuation area and the area of metal objects than to the reception bandwidth. Therefore, if the reception bandwidth is increased for the purpose of reducing metal objects, it is reasonable to set it based on a value that minimizes the echo spacing in consideration of image quality factors.

Customer Voices in Telehealth: Constructing Positioning Maps from App Reviews (고객 리뷰를 통한 모바일 앱 서비스 포지셔닝 분석: 비대면 진료 앱을 중심으로)

  • The purpose of this study is to evaluate the service attributes and consumer reactions of telemedicine apps in South Korea and visualize their differentiation by constructing positioning maps. We crawled 23,219 user reviews of 6 major telemedicine apps in Korea from the Google Play store. Topics were derived by BERTopic modeling, and sentiment scores for each topic were calculated through KoBERT sentiment analysis. As a result, five service characteristics in the application attribute category and three in the medical service category were derived. Based on this, a two-dimensional positioning map was constructed through principal component analysis. This study proposes an objective service evaluation method based on text mining, which has implications. In sum, this study combines empirical statistical methods and text mining techniques based on user review texts of telemedicine apps. It presents a system of service attribute elicitation, sentiment analysis, and product positioning. This can serve as an effective way to objectively diagnose the service quality and consumer responses of telemedicine applications.

Pediatric Radiation Examination by Development of Bismuth Shield Research on Radiation Exposure (비스무스 차폐체 개발을 통한 소아 방사선검사의 피폭에 관한 연구)

  • Currently, with the development of technologies, X-ray examinations for medical examinations at hospital is increasing. This study was conducted to help reduce radiation exposure by measuring the exposure dose received by pediatric patients and the spatial dose of the X-ray room. Dosimeters were installed in the eyeball, thyroid gland, breast, gonads and 4 directions at a distance of 30 cm, 40 cm, 50 cm from the phantom. The dose was measured ten times each, before, and after the application of the bismuth shield under the examination conditions of the head, chest, and abdomen of pediatric patients. Under the condition of head examination, when a shielding was applied, the dose reduction rate was 68.58% for the eyeball, 72.88% for the thyroid, 84.2% for the breast, and 72.36% for the gonad. The chest examination showed reductions of 19.56% eyeball, 56.98% thyroid, 1.21% breast, and 0.68% gonad. The abdominal examination showed reduction rates of 2.6% eyeball, 10.67% thyroid, 19.85% breast, and 82.02% gonad. Spatial dose decreased by 62.25% at 30 cm, 61.16% at 40 cm, and 68.68% at 50 cm. When the bismuth shield was applied, there was a decrease in dose across all examinations, as well as a reduction in spatial dose. Continued research on the use of bismuth shields will help radiological technologists achieve their goal of dose reduction.

Recent Trends in the Withdrawal of Life-Sustaining Treatment in Patients with Acute Cerebrovascular Disease : 2017-2021

  • Objective : The Act on Life-Sustaining Treatment (LST) decisions for end-of-life patients has been effective since February 2018. An increasing number of patients and their families want to withhold or withdraw from LST when medical futility is expected. This study aimed to investigate the status of the Act on LST decisions for patients with acute cerebrovascular disease at a single hospital. Methods : Between January 2017 and December 2021, 227 patients with acute cerebrovascular diseases, including hemorrhagic stroke (n=184) and ischemic stroke (n=43), died at the hospital. The study period was divided into the periods before and after the Act. Results : The duration of hospitalization decreased after the Act was implemented compared to before (15.9±16.1 vs. 11.2±18.6 days, p=0.127). The rate of obtaining consent for the LST plan tended to increase after the Act (139/183 [76.0%] vs. 27/44 [61.4%], p=0.077). Notably, none of the patients made an LST decision independently. Ventilator withdrawal was more frequently performed after the Act than before (52/183 [28.4%] vs. 0/44 [0%], p<0.001). Conversely, the rate of organ donation decreased after the Act was implemented (5/183 [2.7%] vs. 6/44 [13.6%], p=0.008). Refusal to undergo surgery was more common after the Act was implemented than before (87/149 [58.4%] vs. 15/41 [36.6%], p=0.021) among the 190 patients who required surgery. Conclusion : After the Act on LST decisions was implemented, the rate of LST withdrawal increased in patients with acute cerebrovascular disease. However, the decision to withdraw LST was made by the patient's family rather than the patient themselves. After the execution of the Act, we also observed an increased rate of refusal to undergo surgery and a decreased rate of organ donation. The Act on LST decisions may reduce unnecessary treatments that prolong end-of-life processes without a curative effect. However, the widespread application of this law may also reduce beneficial treatments and contribute to a decline in organ donation.

Cigarette Smoke Extract-Treated Mouse Airway Epithelial Cells-Derived Exosomal LncRNA MEG3 Promotes M1 Macrophage Polarization and Pyroptosis in Chronic Obstructive Pulmonary Disease by Upregulating TREM-1 via m6A Methylation

  • Cigarette smoke extract (CSE)-treated mouse airway epithelial cells (MAECs)-derived exosomes accelerate the progression of chronic obstructive pulmonary disease (COPD) by upregulating triggering receptor expressed on myeloid cells 1 (TREM-1); however, the specific mechanism remains unclear. We aimed to explore the potential mechanisms of CSE-treated MAECs-derived exosomes on M1 macrophage polarization and pyroptosis in COPD. In vitro, exosomes were extracted from CSE-treated MAECs, followed by co-culture with macrophages. In vivo, mice exposed to cigarette smoke (CS) to induce COPD, followed by injection or/and intranasal instillation with oe-TREM-1 lentivirus. Lung function and pathological changes were evaluated. CD68+ cell number and the levels of iNOS, TNF-α, IL-1β (M1 macrophage marker), and pyroptosis-related proteins (NOD-like receptor family pyrin domain containing 3, apoptosis-associated speck-like protein containing a caspase-1 recruitment domain, caspase-1, cleaved-caspase-1, gasdermin D [GSDMD], and GSDMD-N) were examined. The expression of maternally expressed gene 3 (MEG3), spleen focus forming virus proviral integration oncogene (SPI1), methyltransferase 3 (METTL3), and TREM-1 was detected and the binding relationships among them were verified. MEG3 increased N6-methyladenosine methylation of TREM-1 by recruiting SPI1 to activate METTL3. Overexpression of TREM-1 or METTL3 negated the alleviative effects of MEG3 inhibition on M1 polarization and pyroptosis. In mice exposed to CS, EXO-CSE further aggravated lung injury, M1 polarization, and pyroptosis, which were reversed by MEG3 inhibition. TREM-1 overexpression negated the palliative effects of MEG3 inhibition on COPD mouse lung injury. Collectively, CSE-treated MAECs-derived exosomal long non-coding RNA MEG3 may expedite M1 macrophage polarization and pyroptosis in COPD via the SPI1/METTL3/TREM-1 axis.

Electrochemical Characteristics of Pencil Graphite Electrode Through Surface Modification and its Application of Non-enzymatic Glucose Sensor (표면 개질된 샤프심 전극의 전기화학적 특성 고찰 및 비효소적 글루코스 센서 활용)

  • Most medical sensors are disposable products. In order to reduce inspection and diagnosis costs, it is more important to develop the inexpensive electrode materials. We fabricated the CuO NPs/PANI/E-PGE as an electrode material for disposable electrochemical sensors and applied it to a non-enzymatic glucose sensor. For surface activation of PGE, pretreatment was performed using chemical and electrochemical methods, respectively. Electrochemical properties according to the pretreatment method were analyzed through chronoamperometry (CA), cyclic voltammetry (CV) and electrochemical impedance (EIS). From these analytical results, the electrochemically pretreated PGE (E-PGE) was finally adopted. The non-enzymatic glucose sensor based on CuO NPs/PANI/E-PGE shows sensitivity of 239.18 mA/mM×cm2 (in a linear range of 0.282~2.112 mM) and 36.99 mA/mM×cm2 (3.75423~50 mM), detection limit of 17.6 μM and good selectivity. Based on the results of this study, it was confirmed that the modified PGE is a high-performance electrode material. Therefore, these electrodes can be applied to a variety of disposable sensors.

Effects of Hwangryunhaedok-Tang and Geongangbuja-Tang on the Change of Interleukin-6 and $TNF-{\alpha}$ Level Induced by LPS I.C.V. Injection in Mice (황연해독탕(黃連解毒湯)과 건강부자탕(乾薑附子湯)이 LPS유도에 의한 마우스 혈중 IL-6와 $TNF-{\alpha}$ 변화에 미치는 영향)

  • Objective : This study was conducted to investigate the effects of Hwangryunhaedok-Tang and Geongangbuja-Tang on the change of interleukin-6 (IL-6) and tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) level induced by LPS I.C.V. injection in mice. Method : We devided group into 6 mice and 6 mice were assingned to each group. In the normal group only saline was administered intragastrically, and in the control group LPS was injected intracerebroventricularly 1 hr after intragastric administration of saline. In the experiment groups Hwangryunhaedok-Tang(0.5g/kg, 1.0g/kg, 3.0g/kg) was administered intragastrically to mice 1 hr prior to LPS(100mg/mouse) I.C.V. injection.. Also Geongangbuja-Tang (0.5g/kg, 1.0g/kg, 3.0g/kg) was administered intragastrically to mice 1 hr prior to LPS(100mg/mouse) I.C.V. injection. To measure the plasma IL-6 and $TNF-{\alpha}$ level of mice, their blood samples were collected from retro-orbital plexus, immediately centrifuged at $4^{\circ}C$, and plasma was removed and stored frozen at $-83^{\circ}C$ for later determination of IL-6 and $TNF-{\alpha}$. The level of IL-6 and $TNF-{\alpha}$ production was measured by enzyme-linked immunosorbent assay in the plasma. Result : Regarding IL-6 level, The 0.5g/kg and the 1g/kg groups of Geongangbuja-Tang decreased IL-6 level. Especially the 3g/kg control group decreased IL-6 level significantly than the normal group(p<0.01). Regarding $TNF-{\alpha}$ level, the 3g/kg group of Geongangbuja-Tang decreased it significantly(p<0.05). Conclusion : These data revealed that Hwangryunhaedok-Tang might not have the anti imflammatory effect and Geongangbuja-Tang(3g/kg)might have the anti imflammatory effect by reducing the plasma IL-6 and $TNF-{\alpha}$ level in mice LPS Injection.EIM (Eighteen Incompatible Medicaments) is an important component in Oriental pharmacology and is directly related to clinical prescriptions. Medical practitioners argued that the definite cause and meaning of EIM was ambiguous and therefore debated the issue of clinical application of the EIM. This study conducted an in-depth literary research on the origin, meaning and contents of EIM with the purpose to contribute in its efforts to be used clinically. Even after thousands of years have past since establishment of Oriental medicine, EIM is still tabooed and was an obstacle that hindered ideologies. Modern herbal medicine texts claim that the use of EIM can reduce treatment effects and promote poisoning and side effects. However, since long ago, there has been medical practitioners who reject this as false. Recently, poisoning caused by EIM has been claimed to be from the toxicity of the drug itself, rather than the result of interaction between the drugs, and therefore they suggest that EIM is not a forbidden domain. In addition, EIM showed a difference in number depending on the era. However, this can be understood not as a definite number, but instead as a warning to be careful during combination of drugs for use as clinical medicine. Historically, there were very few cases in which EIM was used for clinical tests and thus, the clinical value is not, while others applied EIM directly to their bodies, which showed signs for the usefulness and potential of EIM for us. A more concrete and in-depth study must be made on EIM.

Target dose study of effects of changes in the AAA Calculation resolution on Lung SABR plan (Lung SABR plan시 AAA의 Calculation resolution 변화에 의한 Target dose 영향 연구)

  • Purpose : Changing the calculation grid of AAA in Lung SABR plan and to analyze the changes in target dose, and investigated the effects associated with it, and considered a suitable method of application. Materials and Methods : 4D CT image that was used to plan all been taken with Brilliance Big Bore CT (Philips, Netherlands) and in Lung SABR plan($Eclipse^{TM}$ ver10.0.42, Varian, the USA), use anisotropic analytic algorithm(AAA, ver.10, Varian Medical Systems, Palo Alto, CA, USA) and, was calculated by the calculation grid 1.0, 3.0, 5.0 mm in each Lung SABR plan. Results : Lung SABR plan of 10 cases are using each of 1.0 mm, 3.0 mm, 5.0 mm calculation grid, and in case of use a 1.0 mm calculation grid $V_{98}$. of the prescribed dose is about $99.5%{\pm}1.5%$, $D_{min}$ of the prescribed dose is about $92.5{\pm}1.5%$ and Homogeneity Index(HI) is $1.0489{\pm}0.0025$. In the case of use a 3.0 mm calculation grid $V_{98}$ dose of the prescribed dose is about $90{\pm}4.5%$, $D_{min}$ of the prescribed dose is about $87.5{\pm}3%$ and HI is about $1.07{\pm}1$. In the case of use a 5.0 mm calculation grid $V_{98}$ dose of the prescribed dose is about $63{\pm}15%$, $D_{min}$ of the prescribed dose is about $83{\pm}4%$ and HI is about $1.13{\pm}0.2$, respectively. Conclusion : The calculation grid of 1.0 mm is better improves the accuracy of dose calculation than using 3.0 mm and 5.0 mm, although calculation times increase in the case of smaller PTV relatively. As lung, spread relatively large and low density and small PTV, it is considered and good to use a calculation grid of 1.0 mm.
