Clinical Outcome in Gamma Knife Radiosurgery for Metastatic Brain Tumors from the Primary Breast Cancer : Prognostic Factors in Local Treatment Failure and Survival

  • Objective : Brain metastases in primary breast cancer patients are considerable sources of morbidity and mortality. Gamma knife radiosurgery (GKRS) has gained popularity as an up-front therapy in treating such metastases over traditional radiation therapy due to better neurocognitive function preservation. The aim of this study was to clarify the prognostic factors for local tumor control and survival in radiosurgery for brain metastases from primary breast cancer. Methods : From March 2001 to May 2011, 124 women with metastatic brain lesions originating from a primary breast cancer underwent GKRS at a tertiary medical center in Seoul, Korea. All patients had radiosurgery as a primary treatment or salvage therapy. We retrospectively reviewed their clinical outcomes and radiological responses. The end point of this study was the date of patient's death or the last follow-up examination. Results : In total, 106 patients (268 lesions) were available for follow-up imaging. The median follow-up time was 7.5 months. The mean treated tumor volume at the time of GKRS was 6273 $mm^3$ (range, 4.5-27745 mm3) and the median dose delivered to the tumor margin was 22 Gy (range, 20-25 Gy). Local recurrence was assessed in 86 patients (216 lesions) and found to have occurred in 36 patients (83 lesions, 38.6%) with a median time of 6 months (range, 4-16 months). A treated tumor volume >5000 $mm^3$ was significantly correlated with poor local tumor control through a multivariate analysis (hazard risk=7.091, p=0.01). Overall survival was 79.9%, 48.3%, and 15.3% at 6, 12, and 24 months, respectively. The median overall survival was 11 months after GKRS (range, 6 days-113 months). Multivariate analysis showed that the pre-GKRS Karnofsky performance status, leptomeningeal seeding prior to initial GKRS, and multiple metastatic lesions were significant prognostic factors for reduced overall survival (hazard risk=1.94, p=0.001, hazard risk=7.13, p<0.001, and hazard risk=1.46, p=0.046, respectively). Conclusion : GKRS has shown to be an effective and safe treatment modality for treating brain metastases of primary breast cancer. Most metastatic brain lesions initially respond to GKRS, though, many patients have further CNS progression in subsequent periods. Patients with poor Karnofsky performance status and multiple metastatic lesions are at risk of CNS progression and poor survival, and a more frequent and strict surveillance protocol is suggested in such high-risk groups.

  • Cupping therapy has been used to extensive diseases. However, the research was yet to meridian and acupoint's change of state caused by Cupping. This study was considered a human-ripple effect by the anlaysis of meridian & acupoint's change of state. We developed the system which is keeps 30kPa Negative-pressure and stimulated on the Left Lung Meridian(LU) a halfway point between LU4 and LU5. We checked moisture and elasticity change on left/right acupoints & non-acupoints, then measured EMG Signal. We observed that moisture had little change(12.49%) and the better part of the elasticity on both sides had identified the change(64.42%). The number of the provoked acupoiunt elasticity change was more than non-acupoint. Also, elasticity change of Right side was more than Left side. We confirmed that RMS and Median Power change patterns were lower in proportional to the elastin change, but there was no significance about Median Frequency. Therefore inducing the change of elasticity and EMG signal on meridian-acupoints. Therefore, We checked the human-ripple effect Negative-pressure by the Cupping therapy.

Comparison of the Efficacy of Diluted Polyethylene Glycol and Low-Density (0.1% w/v) Barium Sulfate Suspension for CT Enterography (전산화단층촬영 소장조영술을 위한 희석된 폴리에틸렌 글리콜과 저밀도(0.1% w/v) 바륨 현탁액의 유용성 비교)

  • Purpose To compare small bowel distension and side effects between a diluted polyethylene glycol (PEG) solution and a low-density (0.1% w/v) barium sulfate suspension (LDBSS) for CT enterography (CTE) preparation. Materials and Methods Total 173 consecutive patients who underwent CTE were enrolled in this study. The LDBSS (1 L) was used in 50 patients, and the diluted iso-osmotic PEG solution (1 L) was used in 123 patients. Two blinded radiologists independently scored jejunal and ileal distensions on a 5-point scale. To compare side effects between the two groups, the patients reported whether they had immediate complications after the administration of the oral contrast media. Results For ileal and jejunal distension, the diluted PEG solution showed no difference from the LDBSS for either reader (ileum: reader 1, median, 4; 4, interquartile range, 3-4; 3-4, p = 0.997; reader 2, median, 4; 4, interquartile range, 3.3-4.0; 3-4, p = 0.064; jejunum: reader 1, median, 2; 2, interquartile range, 2-3; 2-3, p = 0.560; reader 2, median, 3; 2, interquartile range, 2-3; 2-3, p = 0.192). None of the patients complained of immediate complications following administration of either of the oral contrast media. Conclusion The diluted PEG solution showed comparable bowel distension compared to LDBSS and no immediate side effects; thus, it can be a useful alternative.

Autonomous Battle Tank Detection and Aiming Point Search Using Imagery (영상정보에 기초한 전차 자율탐지 및 조준점탐색 연구)

  • This paper presents an autonomous detection and aiming point computation of a battle tank by using RGB images. Maximally stable extremal regions algorithm was implemented to find features of the tank, which are matched with images extracted from streaming video to figure out the region of interest where the tank is present. The median filter was applied to remove noises in the region of interest and decrease camouflage effects of the tank. For the tank segmentation, k-mean clustering was used to autonomously distinguish the tank from its background. Also, both erosion and dilation algorithms of morphology techniques were applied to extract the tank shape without noises and generate the binary image with 1 for the tank and 0 for the background. After that, Sobel's edge detection was used to measure the outline of the tank by which the aiming point at the center of the tank was calculated. For performance measurement, accuracy, precision, recall, and F-measure were analyzed by confusion matrix, resulting in 91.6%, 90.4%, 85.8%, and 88.1%, respectively.

Pragmatic Combinations of Acupuncture Points for Lateral Epicondylalgia are Unreliable in the Physiotherapy Setting

  • This study describes the reliability of pragmatic combinations of acupuncture points for lateral epicondylalgia (LE) as prescribed by physiotherapists who were experts in acupuncture. Raters (n = 14; 33-59 years) independently prescribed acupuncture points for 30 simulated human patients with LE who were surveyed via a printed questionnaire. The frequency and cooccurrence of acupuncture points prescribed for patients with lateral epicondylitis were assessed. Absolute agreement and Light's kappa (${\kappa}_{Light}$) with 95% confidence interval (CI) were used to quantify the interrater agreement. Raters prescribed 103 unique acupuncture points in different combinations with a median (min-max) of 5 (0-11) acupuncture points. The most prescribed acupuncture point was LI-11 (297 of 420; 71%), and the most common cooccurring acupuncture points were LI-11 and LI-4 (160 of 420; 38%). The absolute agreement for prescribing the acupuncture points ranged from 70% (point GB-20) to 0% (points LI-10, SP-6, LI-11, GB-34, LI-12, and LI-4). Point LR-3 showed the highest interrater reliability for prescribing the acupuncture points [${\kappa}_{Light}=0.112$, 95% CI = (0.055-0.194)], whereas point LI-4 showed the lowest reliability [${\kappa}_{Light}=-0.003$, 95% CI = (-0.024 to 0.024)]. These findings suggest that pragmatic prescriptions of acupuncture points for LE are unreliable among physiotherapists who are experts in acupuncture. Explicit, high-level evidence-based rules for prescribing and teaching combinations of acupuncture points for LE are warranted.

A Study on the Visual Odometer using Ground Feature Point (지면 특징점을 이용한 영상 주행기록계에 관한 연구)

  • Odometry is the critical factor to estimate the location of the robot. In the mobile robot with wheels, odometry can be performed using the information from the encoder. However, the information of location in the encoder is inaccurate because of the errors caused by the wheel's alignment or slip. In general, visual odometer has been used to compensate for the kinetic errors of robot. In case of using the visual odometry under some robot system, the kinetic analysis is required for compensation of errors, which means that the conventional visual odometry cannot be easily applied to the implementation of the other type of the robot system. In this paper, the novel visual odometry, which employs only the single camera toward the ground, is proposed. The camera is mounted at the center of the bottom of the mobile robot. Feature points of the ground image are extracted by using median filter and color contrast filter. In addition, the linear and angular vectors of the mobile robot are calculated with feature points matching, and the visual odometry is performed by using these linear and angular vectors. The proposed odometry is verified through the experimental results of driving tests using the encoder and the new visual odometry.

Determination of Probable Rainfall Intensity Formulas for Designing Storm Sewer Systems at Incheon District (우수거 설계를 위한 인천지방에서의 확률강우강도식의 산정)

  • This paper presents a procedure for determining the design rainfall depth and the design rainfall intensity at Incheon city area in Korea. In this study the eight probability distributions are considered to estimate the probable rainfall depths for 11 different durations. The Kolmogorov - Smirnov test and the Chi-square test are adopted to test each distribution. The probable rainfall intensity formulas are then determined by i) the least squares (LS) method, ii) the least median squares (LMS) method, iii) the reweighted least squares method based on the LMS (RLS), and iv) the constrained regression (CR) model. The Talbot, the Sherman, the Japanese, and the Unified type are considered to determine the best type for the Incheon station. The root mean squared (RMS) errors are computed to test the formulas derived by four methods. It is found that the Unified type is the most reliable and that all methods presented herein are acceptable for determining the coefficients of rainfall intensity formulas from an engineering point of view.

  • The purpose of this study was to examine whether social support buffers the deleterious effects of strain on life satisfaction of the aged and to provide information for finding out ways of increasing their life satisfaction. The subjects of this study were 252 aged persons living in Kwangju and Joun-nam. The statistics used for data analysis were frequency percentage mean standard deviation reliablity Pearson's correlation multiple regression analysis through the SAS package program. 1) The support score from family was higher than the support from friends and the total point of life satisfaction of the aged was 28.4 which was above the median 24. 2) Social support especially the support from family buffer the negative effect of strain on life satisfaction of the aged. The result indicates that the family remains a supportive system for the aged parents Therefore A program in necessary for aged parents to promote the emotional solidarity between the aged parents and their children.

  • The least informative distributions minimizing Fisher information for location are obtained in the classes of continuously differentiable and piece-wise continuously differentiable densities with the additional restrictions on their values at the median and mode of population in the point and interval forms. The structure of these optimal solutions depends both on the assumptions of smoothness and form of characterizing restrictions of the class of distributions: in the class of continuously differentiable densities, the least informative distributions are finite and have the cosine-type form, and, in the class of piece-wise continuously differentiable densities, the least informative densities have exponential-type tails, the Laplace density in particular. The dependence of optimal solutions on the assumptions of symmetry is also analyzed.

  • This study purpose to know the degree of adolescent consumer's conspicuous consumption how difference do adolescent consumers' conspicuous consumption have by socio demographic variables socio-psychological variables consumer ability. Therefore we suggest the adolescent consumer's sound consumption to consumer education. To approach to above purpose the research question are as the followings. 1) The average level of conspicuous consumption a little low median point. This implies that comsumer education is necessary for the adolescent consumer in order to reduce the level of conspicuous consumption. 2) Theadolescent consumers' consumption differs significantly according to the sex(p<0.05) monthly mean income of family(P<0.01) degree of taking in massmedia (P<0.01) subjective status on oneself(P<0.01) consumer socialization(P<0.01) 3) According to the results of the regression analysis examining the relative influences of variables affecting for conspicuous consumption the rela ive importance of the variables ; degree of taking in massmedia sex consumer knowledge communication with peers consumer attitude and the amount of allowance.
