관절 연골 손상의 병태 생리 (Pathophysiology of Articular Cartilage Injury)

  • 관절 연골 손상은 급성 손상과 만성 손상인 퇴행성 관절염으로 구분되며, 급성 손상은 연골 손상의 깊이에 따라서 미세 손상, 연골 골절, 골연골 골절의 세 종류로 구분할 수 있고 각 손상의 종류별로 서로 다른 양상의 치유 반응과 예후를 보인다. 만성 관절 연골 손상은 다양한 원인으로 인해서 발생하며 관절 연골의 전반적인 퇴행성 관절염의 형태를 보인다. 관절 연골 손상 시 초기 손상의 정도가 예후를 결정짓는데 가장 중요하고 이외에도 손상의 크기, 부위, 나이, 활동성, 비만 정도, 하지 정렬 상태등도 예후를 결정하는 중요한 요소들이다. 본 논문에서는 관절 연골 손상 시 발생하는 관절 연골 내의 병태 생리적 변화에 대하여 기술하고 비수술적인 치료 방법을 생역학적인 측면과 생물학적인 측면으로 구분하여 그 효과를 알아보고자 한다.

가토 하악골에서 신연 골형성술시 제 I형 및 II형 교원질의 발현 (EXPRESSION OF TYPE I, TYPE II COLLAGEN ON DISTRACTION OSTEOGENESIS IN THE RABBIT MANDIBLE)

  • The purpose of this experiment was to examine the histological changes and the pattern of expression of type I, II collagen in the elongated area by distraction osteogenesis in the rabbit mandible. Sixteen rabbits weighing 2.5kg-3kg were used for this experiment. Experimental group was distracted at the rate of 0.7mm, twice/day for 7days, and control group was only osteotomized. After 5 days latency, osteotomic site is distracted for 7days. Consolidation period is 28days. The animal was sacrificed at the 3rd, 7th, 14th, 28th day after the operation. The distracted bone was examined by histological analysis and RT-PCR analysis. The results were summarized as follows: 1. Experimental group was observed that the gaps between the distracted bone edges were occupied by new bone. 2. Expression of Type I collagen were detected throughout the experiment in both groups and Expression of Type I collagen were markedly increased during distraction and consolidation period in experimental group than control group. 3. Expression of Type II collagen were detected throughout the experiment in both groups and expression of Type II collagen were maintained at high level during distraction and consolidation period in experimental group than control group. From these results, in contrast to type II collagen, type I collagen seemed to be more expressed by mechanical stimuli during distraction and consolidation period. The predominent mechanism of new bone formation in the distraction gap was intramembranous bone formation, but some of the regenerated bone was formed by endochondral ossification.

Effects of continuous force application for extrusive tipping movement on periapical root resorption in the rat mandibular first molar

  • Objective: The purpose of this study was to clarify the effects of continuous force application for extrusive tipping movement and occlusal interference on periapical root resorption in the rat mandibular first molar. Methods: We constructed an appliance comprising a titanium screw implant with a cobalt-chromium post as the anchorage unit and a nickel-titanium closed coil spring (50 cN) as the active unit. Force was applied on the mandibular left first molar of rats for 8 (n = 10) and 15 days (n = 10; experimental groups), with the tooth in occlusion. Five rats were included as a non-treated control group to examine the body effect of the appliance. Active root resorption lacunae, identified using tartrate-resistant acid phosphatase, were evaluated in terms of the length, depth, and area. Results: The rat mandibular first molars were mesially tipped and extruded in the occlusal direction. This mesio-occlusal tipping movement and occlusion resulted in the formation of a compression zone and active root resorption lacunae in the distoapical third of the distal roots. However, there was no significant difference in the amount of root resorption between the two experimental groups. The control group did not exhibit any active root resorption lacunae. Conclusions: Periapical root resorption was induced by continuous extrusive tipping force and occlusal interference in rat mandibular molars. These data suggest that we orthodontists had better take care not to induce occlusal interference during our orthodontic treatment.

고전압맥동전류에 의한 일차 및 이차통각과민대의 RIII 반사의 변화 (Change of RIII Reflex of Primary and Secondary Hyperalgesia Site by High Voltage Pulsed Current)

  • This study conducted RIII reflex measurement to examine degree of pain depending on polarity of high voltage pulsed current of primary and secondary hyperalgesia site in hyperalgesia rat by local thermal injury. Hind paw which was injury site was taken as primary hyperalgeisa site, sole which was injury adjacent site was taken as secondary hyperalgesia site, and mechanical pain threshold, thermal pain threshold and root mean square of RIII reflex were measured. This study was conducted with control group I of hyperalgesia rat at hind paw by thermal injury and experimental groups divided into cathodal high voltage treatment group II, anodal high voltage treatment group III and alternate high voltage treatment group IV, applied active electrode of high voltage pulsed current to hind paw directly, placed reference electrode on the sole of injury adjacent site and applied pulse frequency. It measured RIII reflex and obtained the following results: Root mean square of RIII reflex at primary hyperalgeisa site was significantly reduced in group II after 2 days of hyperalgesia. Group II showed significant decrease after 5 and 6 days of hyperalgesia. Root mean square of RIII reflex at secondary hyperalgesia site showed significant reduction in group II after 6 days of hyperalgesia. Consequently it was found that application of high voltage pulsed current of hyperalgesia site reduced RIII reflex at primary hyperalgeisa site and secondary hyperalgesia site by electrical stimuli. Effects by polarity of high voltage pulsed current showed the greatest reduction of pain threshold when cathodal active electrode was used.

Sensor Applications of Microporous Conjugated Polymers

  • In 1991, Prof. Toshio Masuda of Kyoto University for the first time synthesized a representative of diphenylacetylene polymer derivatives, poly[1-phenyl-2-(p-trimethylsilyl)phenylacetylene] [PTMSDPA]. This polymer is highly soluble nevertheless a ultra-high molecular weight (Mw) of > $1.0{\times}10^6$ which showed excellent chemical, physical, mechanical properties [1]. As one of the most interesting features of PTMSDPA, Prof. Katsumi Yoshino of Osaka Univ. reported that this polymer emits an intense fluorescence (FL) in a visible region because of the effective exciton confinement within the resonant structure between the polyene pi-conjugated chain and side phenyl full-aromatic bulky groups [2]. Very recently, Prof. Ben-Zhong Tang of Hong-Kong Institute of Science and Technology clarified the idea that the FL emission of disubstituted acetylene polymer derivatives originates from intramolecular excimer due to the face-to-face stacking of the side phenyl groups [3]. Thus, to know what influence to intramolecular excimer emission in the film as well as to further understand how the intramolecular excimer forms in the film became more crucial in order to further precisely design the optimized molecular structure for highly emissive, substituted acetylene polymers in the solid state. In recent studies, we have focused our interests on the origin of the FL emission in order to expand our knowledge to developments of novel sensor applications. It was found that the intramolecular phenyl-pheyl stack structure of PTMSDPA in film was variable in response to various external chemical stimuli. Using PTMSDPA and its derivatives, we have developed various potential applications such as latent fingerprint identification, viscosity sensor, chemical-responsive actuator, gum-like soft conjugated polymer, and bioimaging. The details will be presented in the 49th KVS Symposium held in Pyong Chang city.

흰쥐의 성별, 체중 및 주령에 따른 족삼리(足三里) 전침자극의 면화가 소장수송능(小腸輸送能)에 미치는 영향 (Study on Relation of Effects of Electro-acupuncture with Sex, Age, and Weight in Rats)

  • Electroacupuncture is the combined treatment of mechanical and electric stimuli on the needle head, on which electricity runs after acupuncturing at the acupuncture points. It is reported that its treatment effect depends on the parameter of stimulus, which is wave of stimulus, frequency, voltage, and duration of electric current. To get the effective treatment, stimulus intensity over threshold is needed. We planned to experiment whether acupuncture effect is affected by sex, age, and weight. After we conducted electric stimulus at Zusanli(ST 36) which is known to increase small intestinal motility in normal rats, we observed the effects and got the following results. Though the increased effects of small intestinal motility of electro-acupuncture at Zusanli(ST 36) did not show the difference of sex, this effect was observed to have a tendency to decrease according to weight increase without distinction of sex. But the effect of electro-acupuncture shown in 4, 5, 7 weeks old without distinction of sex was observed to have a tendency to decrease with the growing of weeks-old in the male rat group. And the significant change without specific tendency was observed in the 4, 5 weeks old female rat group. This result suggested that sex, weight, and age should be considered to get the efficient electro-acupuncture. The more experimental study should be advanced to investigate the correlation between the factors that can affect the acupuncture treatment, and, afterward, the interconnection into the clinical use.

간헐적 정수압이 세포의 부착에 미치는 영향성 평가 (Effects of Intermittent Hydrostatic Pressure on the Cell Adhesive force on the substrate)

  • In this study, effects of IHPs with various resting times to cell adhesion were investigated through the measurements of cell adhesive force, number and area of focal contacts (stained vinculin spots), and projected cell area, perimeter and circularity. In addition correlation tests and curve estimations using the experimental results were performed fur the finding an essential factor for increment of cell adhesive force. Tn the results, immediately after mechanical stimuli (150 minutes after seeding) and one hour later (210 minutes after seeding), the average adhesive force of experimental group 5 (resting time: 15min) compared with that of control group at same culture time was increased significantly (p<0.05). Average projected area and perimeter of cells at Group 5 were increased significantly (p<0.05), while average circularity of cells at Group 5 incubated fur 210 minutes was decreased significantly (p<0.05). In the digital image analysis of focal contacts containing vinculins, area and numbers of focal contacts per cell at Group 5 were higher than those of the other groups. This study indicated that IHP with appropriate resting time could contribute in improving cell adhesive force, cell spreading, development of cytoskeleton and formation of focal contacts. And cell adhesive force was correlated to the morphological aspects of cell and development of focal contacts. Particularly, area of focal contacts was closely related to cell adhesive force.

Synergistic interaction between acetaminophen and L-carnosine improved neuropathic pain via NF-κB pathway and antioxidant properties in chronic constriction injury model

  • Background: Inflammation is known to underlie the pathogenesis in neuropathic pain. This study investigated the anti-inflammatory and neuroprotective mechanisms involved in antinociceptive effects of co-administration of acetaminophen and L-carnosine in chronic constriction injury (CCI)-induced peripheral neuropathy in male Wistar rats. Methods: Fifty-six male Wistar rats were randomly divided into seven experimental groups (n = 8) treated with normal saline/acetaminophen/acetaminophen + L-carnosine. CCI was used to induce neuropathic pain in rats. Hyperalgesia and allodynia were assessed using hotplate and von Frey tests, respectively. Investigation of spinal proinflammatory cytokines and antioxidant system were carried out after twenty-one days of treatment. Results: The results showed that the co-administration of acetaminophen and L-carnosine significantly (P < 0.001) increased the paw withdrawal threshold to thermal and mechanical stimuli in ligated rats compared to the ligated naïve group. There was a significant (P < 0.001) decrease in the levels of nuclear factor kappa light chain enhancer B cell inhibitor, calcium ion, interleukin-1-beta, and tumour necrotic factor-alpha in the spinal cord of the group coadministered with acetaminophen and L-carnosine compared to the ligated control group. Co-administration with acetaminophen and L-carnosine increased the antioxidant enzymatic activities and reduced the lipid peroxidation in the spinal cord. Conclusions: Co-administration of acetaminophen and L-carnosine has anti-inflammatory effects as a mechanism that mediate its antinociceptive effects in CCI-induced peripheral neuropathy in Wistar rat.

비디오 에스노그래피를 이용한 서비스 로봇의 대기상태 행동패턴 연구 (The Behavioral Patterns of Neutral Affective State for Service Robot Using Video Ethnography)

  • 현재 진행되고 있는 대부분의 로봇 연구는 특정한 목적을 수행함에 있어서 기능적으로 안정적이고 원활한 움직임을 보이는 것과 더불어 사용자가 원하는 정보를 적절하게 전달하는 것에 초점을 맞춰 진행하고 있다. 본 논문에서는 로봇의 작업수행 상태가 아닌 외부로부터 입력 값이 없는 대기상태의 행동패턴을 제시하여 보다 자연스러운 인간-로봇 상호작용을 제시하고자 하였다. 로봇의 대기상태 행동패턴을 디자인함에 있어서 비디오 판독 방법을 선택하였고, 실제 서비스업에 종사하는 10명의 사람들을 비디오로 녹화하여 사람들과 상호작용이 없는 대기상태의 반복적인 행동패턴을 관찰하였다. 각각의 비디오 데이터로부터 총 21개의 반복적 행동을 기록하였고, 비슷한 항목들을 통합하여 최종적으로 11개의 행동패턴을 추출하였다. 추출된 패턴들 가운데 6개의 대표적인 행동들을 EEEX라는 로봇에 적합하도록 인코딩 작업을 하였고, 이것들이 사람들에게 올바르게 인식되는지를 확인하기 위하여 검증실험을 수행하였다. 사람들은 대부분의 로봇 행동패턴을 실험에서 의도한 바와 같이 인식하였으나 로봇의 하드웨어 특성상 몸과 팔의 움직임에서 약간의 혼동 요소가 있었다. 추후 실험을 통해 EEEX를 대형 마트의 입구에 실제로 배치하여 대기 중에 중립행동을 보일 때와 보이지 않을 때의 손님의 관심도 차이를 조사해보고자 한다.

관절 가동운동(mobilization)이 관절 감수기(joint receptors)에 미치는 영향 (Effects of Joint Mobilization Techniques on the Joint Receptors)

  • 지금까지의 내용을 요약하면 먼저, 인체의 각 관절 주위에는 관절의 움직임과 위치를 파악하는 감지기 역할을 하는 기계적 감수기가 있다. 이러한 감수기는 크게 네가지로 구분되어지는데 대개 I형 감수기, II형 감수기 등의 용어를 사용한다. 각각의 감수기의 특성(표 2.)을 알아보면, 루피니(Ruffini)감각기와 유사한 모양을 하고 있는 I형 관절감수기는 주로 운동의 속도(speed)와 방향(direction)을 감지(detection) 하며, 파시니안(Pacinian)감각기와 유사한 형태를 하고 있는 II형 관절감수기는 매우 작은 움직임에 대한 감지를 하는데, 예를 들어 속도가 가속(acceleration)되는 것과 같은 움직임 등을 알아낸다. 이외에 골지건과 비슷한 모습을 하고 있는 III형 관절감수기는 운동의 자세(position)와 방향에 대한 정보를 전달하게 된다 (Carpanter, 1990). 관절의 자세 유지 혹은 안정성 즉, 정적(static) 운동에 주로 관여하는 I형 관절감수기와, 역동적(dynamic)인 운동에 감지를 주로 하는 II형 관절감수기는 진동(oscillation)자극에 잘 촉진된다. 이러한 자극은 동통이 심하게 발생된 관절에 나타나는 근육의 과긴장(hypertonicity)과 동통을 경감시킬 수 있다. III형 관절감수기는 억제 (inhibitory)감수기라 할 수 있는데, 골지건(golgi tendon organ)의 기능과 매우 관련이 깊다. 이 감수기는 관절의 가동범위를 기계적인 자극으로 증가시키기 위해 관절의 가동 끝범위에서 강한 신장(strong strectch)이나 thrust 기술로 감수기를 촉진할 수 있다. 이러한 자극은 결국 근육에 이완을 얻게 할 수 있다. 위에서 언급한 관절 주위의 조직에 분포한 특별한 기계적 감지를 환자들의 임상 증상에 따라 선택적으로 자극하는 치료를 적용할 수 있다면, 현재 물리치료사들이 적용하고 있는 관절 가동운동을 좀더 전문적인 수준으로 수행할 수 있을 것이다.
