Numerical Simulation of Flow in a Total Artificial Heart (인공심장내의 혈류유동의 컴퓨터 시뮬레이션)

  • In this paper, a numerical simulation of steady laminar and turbulent flow in a two dimensional model for the total artificial heart is presented. A trileaflet polyurethane valve was simulated at the outflow orifice while the inflow orifice had a trileaflet or a flap valve. The numerical solutions of the simulated model show that regions of relative stasis and trapped vortices were smaller wi thin the ventricular chamber wi th the flap valve at the inflow orifice than that with the trileaflet valve. The predicted Reynolds stresses distal to the inflow valve within the ventricular chamber were also found to be smaller with the flap valve than with the trileaflet valve. Analysis of the numerical solutions suggests that geometries similar to the flap valve(or a tilting disc valve) results in a better flow dynamics within the total artificial heart chamber compared to a trileaflet valve.

  • An analysis of the dynamics of a mechanical monoleaflet heart valve prosthesis in the closing phase is presented. Employing the moment equilibrium principles on the occluder motion and the squeeze film dynamics of the fluid between the occluder and the guiding strut at the instant of impact, the velocity of the occluder tip and the impact force were computed. The dynamics of fluid being squeezed between the occluder and the guiding struts is accounted for by Reynold's equation. The effect of the fluid being squeezed between the occluder and the guiding strut was to reduce the velocity of the occluder tip at the instant of valve closure as well as dampen the fluttering of the occluder before coming to rest in the fully closed position. The squeeze film fluid pressure changed rapidly from a high positive value to a relatively large negative value in less than 1 msec. The results of this study may be extended for the analysis of cavitation inception, mechanical stresses on the formed elements and valve components as well as to estimate the endurance limits of the prosthetic valves.

Open Heart Surgery 110 Cases in One Year(1987) (연간 (1987년) 개심술 110례에 대한 검토)

  • During one year[1987], 110 cases of open heart surgeries were performed in the department of Thoracic and Cardiovascular Surgery, Pusan Paik Hospital, Inje Medical College. And the results were summarized as follows. 1. Among the 110 cases, there were 77 cases of congenital heart diseases and 33 cases of acquired heart diseases. Age range of the congenital patients was 8 months to 30 years with the mean age of 8 years, and acquired patients was 16 to 56 years with the mean age was 32 years. 2. The heart lung machine used for cardiopulmonary bypass was Sarns 7000, 5-head roller pump, and the number and type of oxygenators were 50 of membrane type and 60 of bubble type. For all cases GIK[glucose-insulin-potassium] solution was used as cardioplegic solution for myocardial protection during operation. 3. Among the 77 congenital anomalies, there were 67 cases of acyanotic patients[ASD: 12, VSD: 50, PS: 1, AP window: 1, Gerbode defect: 1, ECD: 2] and 10 cases of cyanotic patients[TOF: 10], and to all of which the appropriate radical operations were applied. 4. Among the 33 acquired diseases, there were one case of left atrial myxoma, one of annuloaortic ectasia, 20 of mitral valve diseases[MS: 2, MSr: 8, MR: 2, MRs: 8], 9 of double valve diseases[MRs+AR: 1, MRs+ARs: 2, MRs+TR: 1, MSr+TR: 3, MSr+ASr: 1, MSr+ARs: 1], 2 of triple valve diseases[MSr+AR+TR: 1, MSr+ASr+TR: 1]. The left atrial myxoma was removed well with right atriotomy and atrial septal approach. And to the annuloaortic ectasia, Bentall operation was applied with good result. Mitral valve replacement[MVR] was applied to 20 cases of mitral valve diseases, double valve replacement[MVR+AVR] was applied to 6 cases of double valve diseases, MVR & tricuspid annuloplasty[TVA] was applied to 3 cases of mitral 5. The number of replaced valve were 39 in 31 cases. In MVR, 5 of mechanical valves[St. Jude Medical Valve] and 26 of tissue valves[Carpentier-Edward valve] were used. In AVR, 3 of mechanical valves and 5 of tissue valves were used. 6. Postoperative complications were occurred in 23 cases, and among them 21 cases were recovered with intensive cares, but 2 cases were expired[mortality: 1.8%].

Dynamic Behavior Analysis of Mechanical Bileaflet Heart Valve Prosthesis (기계식 이엽심장밸브의 동적거동 해석)

  • In this paper, fluttering behavior of mechanical bileaflet heart valve prosthesis was analyzed taking into consideration of the impact between valve plate and stopper Vibration system of the valve was modeled as a rotating system, and equations are induced by moment equilibrium equations. Lift force, drag force, gravity and buoyancy were considered as external forces acting on the valve plate/ The 4th order Runge-Kutta method was used to solve the equations. Valve plate does not come to the static equilibrium position at a stretch, but come to that position after under damping vibration. Damping ratio increases as the cardiac optput increases, and the mean damping ratio is in the range of 0.16~40.25. Fluttering frequency does not have any specific value, but varies as a function of time. It is in the range of 10~40Hz. Valve opening appears to be affected by the orientation of the of the valve relative to gravitational forces.

Changes of Microembolic Signals after Heart Valve Surgery (심장 판막 수술 후 미세색전의 변화)

  • Background: The detection of circulating microemboli by transcranial Doppler ultrasonography (TCD) has the potential to select the patients with high risk for future symptomatic brain embolism. We prospectively evaluated the positive rate and the frequency of microembolic signals (MES) before and after the heart valve surgery (HVS). Material and Method: Fifty in-patients with heart valve disease were enrolled in this study. Patients with history of previous stroke or heart valve surgery were excluded. Two unilateral TCD monitoring sessions were peformed from middle cerebral artery for 1-hour, before and after HVS. Result: Mechanical Heart valves were implanted in 28 patients, tissue valves were implanted in 10 patients, and remaining 12 patients received mitral valve repair. Positive rate of MES was significantly increased after HVS (50%), compared to that of before HVS (8%, p=0.00). There was no relation between MES after HVS and intensity of anticoagulation, cardiac rhythm, patients' age, and history of hypertension. The positive rate of MES after implantation of mechanical heart valve (71.4%) was significantly higher than those after implantation of tissue valve or mitral valve plasty (p=0.002). Conclusion: Positive rate of MES was increased significantly after the implantation of HVS. The changes of MES in those with mechanical prosthesis may be related to the increased risk or embolism after Hvs.

Examination of Cavitation-Induced Surface Erosion Pitting of a Mechanical Heart Valve Using a Solenoid-Actuated Apparatus

  • Several factors, including peak dp/dt of the ventricular pressure and maximum closing velocity of leaflet have been studied as indices of the cavitation threshold. In the present study, just before closing velocity of the leaflet has been studied as indices of the cavitation threshold, and cavitation erosion on the surface of a mechanical valve was examined by focusing on squeeze flow and the water hammer phenomenon during the closing period of the valve. A simple solenoid-actuated test device that can directly control the valve closing velocity was developed, and opening-closing tests of 3,000 and 40,000 cycles were performed at various closing velocities. There was a closing velocity threshold to occur erosion pitting of valve surface, and its value was about 0.4 m/s in this study. Cavitation-induced erosion pits were observed only in regions where squeeze flow occurred immediately before valve closure On the other hand, the number of the pits was found to be closely related to an area of water hammer-induced pressure wave below the critical pressure defined by water vapor pressure. Therefore, it was concluded that cavitation is initiated and augmented by the two pressure drops due to squeeze flow and water hammer phenomenon, respectively.

Status of open heart surgery in Korea (한국의 개심술 현황)

  • Open heart surgery begun at 1959 in Korea. From that time to 1979, the surgery was performed below 100 cases in a year. However,that was performed above, 1,000 cases in a year from 1981. During the recent three years, i.e., 1985, 1986, and 1987, the annual operative cases were 3614,4503, and 4906, and then the mortality rates were progressively decreased to 6.2%, 5.3 %, and 5.0 %. In these time, overall mortality rate above 1 year old was 5.0 % versus that below 1 year old 17.9 9o in congenital heart diseases. These results are statistically different between two groups. Of the valvular heart disease cases, which occupied 97 % of total acquired heart disease, individual incidence was in mitral 68 %, and aortic 28%. The operative method was mainly valve replacement. Operative mortality for valve surgery in total was 4.4 %. Until 1985, bioprosthetic valve was frequently implanted but mechanical valve has been done more frequently in these days. Coronary artery bypass graft was large portion [67 * 75 %] of open heart surgery in western, but was below 1 % in Korea. However our diet patterns have been changed. Therefore we think the incidence of coronary artery disease will be increased. So we will be familiar to this field.

Impact Behavior Analysis of Mechanical Monoleaflet Heart Valve Prostheses in the Opening Phase

  • In this paper, fluttering behavior of mechanical monoleaflet tilting disc heart valve prostheses during the opening phase was analyzed taking into consideration the impact between the occluder and the guiding strut at the fully open position. The motion of the valve occluder was modeled as a rotating system, and equations were derived by employing the moment equilibrium principle. Forces due to lift, drag, gravity and buoyancy were considered as external forces acting on the occluder. The 4th order Runge-Kutta method was used to solve the governing equations. The results iimonstrated that the occludes reaches steady equilibrium position only after damped vibration. Fluttering frequency varies as a function of time after opening and is in the range of 8-84 Hz. Valve opening appears to be affected by the orientation of the valve relative to gravitational force. The opening velocities are in the range of 0.65-1.42m/sec and the dynamic loads by impact of the occludes and the strut are in the range of 90-190 N.

  • For the aritificial heart valve, two types of valves-polymer and mechanical valve- are generally used. The polymer valve is used as a new low-cost artificial valve. Among the several properties of the artificial valve, the low-regurgitation property is important because it can provide better cardiac output characteristic. So in this study we analyzed and compared the regurgitation property of the mechanical valve which is generally used nowdays and the polymer valve which was made in our group. As results, the polymer valve showed the better regurgitation property compared to the mechanical valve approximately by 3 times, and increased the cardiac output by 10%.

A Numerical Analysis on the Motion of Mechanical Heart Valve(MHV) and Characteristics of Blood Flow in an Elastic Blood Vessel (탄성혈관 내 기계식 인공심장판막(MHV)의 거동 및 혈액 유동 특성에 관한 수치해석적 연구)

  • In this study, the leaflet motion of a mechanical heart valve and the characteristics of two-dimensional transient blood flow in an elastic blood vessel have been numerically investigated by using fluid-structure interaction method. Here, blood has been assumed as a Newtonian, incompressible fluid. Pressure profiles have been used as boundary conditions at the ventricle and the aorta. As a result, closing motion of the leaflet is faster than opening one. While opening angles of leaflet grow up, vortex is detected at the sinus and backward of the leaflets. When the leaflet is fully closed, vortex is detected at the ventricle and at that moment maximum displacement of the elastic blood vessel is observed in the vicinity of the sinus region. Maximum displacement is caused in association with the blood flow that is oriented toward the elastic blood vessel.
