Evaluation of Vancomycin Use before and after the Computerized Restricted Antibiotic Control Program (제한항균제 전산 관리 프로그램 도입 전과 후의 반코마이신 사용 평가)

  • Since 2006, the Computerized Restricted Antibiotic Control Program (CRACP) has been incorporated to facilitate a more efficient approval process of vancomycin use at the Chonbuk National University Hospital. The purposes of the study were to evaluate proper use of vancomycin and to examine if there is any improvement when administering the CRACP. The use of vancomycin was retrospectively reviewed by a medical record review based on the American Society of Hospital Pharmacists (ASHP) criteria (i.e., indication, use of critical process indicators and use of outcome measurements) and compared before (Group I) and after the CRACP (Group II). Two hundred fifty six patient records were evaluated (138 in Group I and 118 in Group II). There was a statistically significant improvement in the report of justification for vancomycin use after CRACP (42.0% vs. 62.7%, p=0.001). Out of eight critical process indicators, two indicators (appropriate cultures prior to medication [68.8% vs. 85.6%, p=0.002] and serum vancomycin level measurement [0% vs. 7.6%, p=0.001]) were significantly improved after CRACP. The total incidence of adverse effects was decreased from 14.5% to 6.8%. In addition, there was a statistically significant difference in WBC count reduction within normal range (52.8% vs. 73.1%, p=0.024). The CRACP appears to be a promising approach to improve use of vacomycin in a hospital setting. However, further evaluation for the long-term period should be performed to confirm the performance of the program.

A Study on the Index of Social Integration of Person with Disabilities and its Measurement (장애인 사회통합의 지수개발과 측정에 관한 연구)

  • The purpose of this paper is to suggest a scientific evaluation criteria for integration of person with disabilities by developing and measuring the index and indicator of disability integration. Disability integration index will be expressed in a simple number to represent the current situation which will summarize the level of integration of persons with disabilities from the perspective of outcome. Disability integration index is composed of four indicators-social attitude toward person disability, physical environment, participation in society expressed by employment and education, and social institution. Observation research was conducted to measure first two indicators of social attitude and physical environment while existing statistics and data were analyzed for other two of participation in society and social institution. The result of this study showed that disability integration index of Korea was 0.4832. Therefore this paper comes to conclude that the level of integration in Korea approaches to 48 percent which is half of ideal integration. However, this study has its limitations in selection of indicators and index formula which in turn must be followed by next studies.

Development of Clinical Performance Indicators for Establishing a Connecting System between Hospital Performance Management and Quality Improvement (의료기관의 성과관리와 질 향상 연계체계 구축을 위한 임상적 성과지표 개발)

  • Purpose: This study aimed at developing integrated clinical performance indicators(CPIs) through the analysis of quality improvement(QI) activities of a hospital and literature review about performance measures. Method: The CPIs were developed through the following three stages; 1)Identifying preliminary CPIs 2)A staff validity test in preliminary CPIs 3)Developing final CPIs. Result: One hundred twenty-three preliminary CPIs were developed through QI activities of the target hospital for 8 years and literature review. The results of the validity test for the preliminary CPIs supported ninety-one items. Sixty-two CPIs were selected through integration, reclassification and renaming. Then, eighteen items were deleted on account of an imprecise calculation method. Finally, forty-four CPIs were confirmed. They consisted of twenty-six items at the hospital level and eighteen items at the department level. Conclusion: CPIs can be used as criteria to evaluate the performance of healthcare organizations, and to decide the quality of healthcare for customers. This study may contribute to establishing an integrated system between QI activities and performance measurement of healthcare organizations.

  • All medical information system stakeholders and the environment exists. Medical information systems for development in these environments and non-functional requirements, functional requirements and quality goals are to be met. In order to achieve these goals in a variety of ways currently being made to develop information systems and various applications are emerging. However, the process of developing these health information systems meet the basic requirements and does not consider that from the point of view should not be separate. This study of the development of health information systems related to quality measurement indicators for the analysis software architectures, and medical information, information quality evaluation of service quality information associated indicators evaluation are offered. This way of associated indicators for the quality of the output sum and analyze the trends in software architecture u-Healthcare should be available for assessment. Quality score compared with pre-set goals for achievement and satisfaction levels of analysis further support the cause excerpt field use in analysis and improvement is possible.

  • Enterprises should set up realizable goals and continually control the goals from apprehending whether they can be realized or not so that the enterprises grow consistently. Furthermore, from supplementing parts to be improved, goals are reset and the result should be controlled in order to fit to them. Lately, a lot of discussions about the results of organizations, projects, and companies are being realized in construction field. Particularly, in construction management, effective and professional control activities throughout the life cycle of facilities as well as the stage of business implementation of construction work are needed. About the result measurement of the construction management, lots of most domestic researches stay in the level of establishing basic concepts. Also, since characteristics of the construction field and the construction management were not reflected properly, it is in a situation that the result control system is not applied to on-site real works. Hence, this study intends to suggest fundamental data for measuring objective results from extracting result areas and key performance indicators of the construction management using Balanced Score Card.

A Correlation Study of the Body Mass Index and the Indicators of Oral Health (체질량지수와 구강건강지표 사이의 상관성에 대한 연구)

  • Objectives: We aim to observe the relation of body mass index (BMI) and the indicators of oral health. Methods: 400 subjects participated in the study. The BMI values are calculated from the height and weight. For the tongue diagnosis, we used the tongue imaging device to analyze the color, tongue coating, and tooth marks. We measured the concentration of hydrogen sulfide (H2S) and methyl mercaptan (CH3SH) to evaluate the halitosis. The dry mouth was evaluated through the measurement of saliva secretion and with the questionnaire asking the frequency of dry mouth. Results: The BMI values were significantly higher in the group with light-white and blue-purple colored tongue, and significantly lower for lightly-coated tongue. However, the correlation of BMI and the amounts of saliva secretion was not significant as well as in the correlation of BMI and the concentration of H2S, CH3SH. In tongue diagnosis, the subjects who had blue-purple colored tongue also had significantly higher H2S and CH3SH, but tendency of lower saliva secretion. Conclusion: We obtain data showing that BMI value and the indicators of oral health including tongue diagnosis have meaningful correlation.

  • A SPICE (Software Process Improvement and Capability dEtermination) and CMM (Capability Maturity Model) assessment can be considered as representative software process assessment models since assessors assign ratings to indicators and metrics to measure the capability of software precesses. But ail of process assessment models don't provide a subjective measurement procedure and metrics. In this paper we define basic quality process metrics and propose standard metrics for each process based on SPICE and CMM. This study also proposed a guideline for a subjective measurement procedure and the metrics are effectively use to SPI.

A Laboratory Study on Low Frequency Noise Assessment based on Noise Acceptability Limit (소음 수응 한계를 고려한 저주파 소음평가에 대한 실험적 연구)

  • A laboratory study on low frequency noise assessment has been carried out to evaluate the relevance of the weighting curve. Especially, the A-weighting curve which is used in most noise assessments has been evaluated using the acceptability limit in this study. The acceptability limit is one of the indicators in which the subjective responses were well-reflected. For the measurement of the acceptability limit, pure tone stimuli were used in the frequency range between 20 and 200 Hz. The measurement was proceeded in the anechoic chamber to minimize the background noise level. A total of 29 test subjects, who were aged between 19 to 33 years, participated in this study. They had been exposed to various stimuli for about 1 hour by supra-aural earphone. The measurement consisted of two listening sessions: hearing threshold and the acceptability limit session. The results showed that the tendency of the acceptability limit curve was approximately equal to C-weighting curve which had been found to be superior to A-weighting curve in assessment of low frequencies.

In Vivo Measurement of Plant Vitality by the Fluorescence Transient

  • The chlorophyll fluorescence combined with the O-J-I-P transients were examined in the leaves of the crinum plants (Crinum asiaticum var. japonicum BAK.), in order to satisfy the demand for rapid in vivo measurement of vitality, and to apply easily to approach questions of economical interest concerning the plant vitality. The photosynthetic efficiency, Fv/Fm, of crinum plants dramatically decreased depending on temperature drop in winter. In summer, the Fv/Fm values was lower in day time than at dawn and night, suggesting that photosynthetic efficiency is chronically photoinhibited in day time. In winter, there was no prominent diurnal fluctuations of Fv/Fm values. However, based on the O-J-I-P transient, PI$\_$NO/ and SFI$\_$NO/ dramatically increased at noon in summer, and $\psi$o/(1-$\psi$o) diurnally fluctuated in winter. These results indicated that vitality indexes such as PI$\_$NO/, SFI$\_$NO/ and $\psi$o/(1-$\psi$o) can be used as the indicators for in vivo measurement of environmental stresses.
