Assessment of Validity and Reliability of Plantar Pressure in Smart Insole (스마트 인솔의 족저압 측정 결과에 대한 타당도 및 신뢰도 평가)

  • Purpose: Smart insoles are wearable devices that are inserted into shoes. Smart insoles with built-in pressure and acceleration sensors can measure the plantar pressure, stride length, and walking speed. This study evaluated the validity and reliability of the plantar pressure measurements of smart insoles during walking on flat ground. Materials and Methods: Twenty one subjects were included in this study. After wearing smart insoles, I-SOL® (Gilon, Seongnam, Korea), the subjects walked a 10 m corridor six times at a rate of 100 steps/min, and the middle three steps, free from direction changes, were chosen for data analysis. The same protocol was repeated after wearing Pedar-X (Novel Corporation, Munich, Germany), an insoletype plantar pressure measurement equipment with proven validity. The average maximum pressure (Ppeak, kPa) and the time at which Ppeak appeared (Ptime, %stride) were calculated for each device. The validity of smart insoles was evaluated by using the interclass correlation coefficient (ICC) of Ppeak and Ptime between the two instruments, and Cronbach's alpha was obtained from the Ppeak values to evaluate the reliability. Results: The ICC of Ppeak was 0.651 (good) in the hallux, 0.744 (good) in the medial forefoot, 0.839 (excellent) in the lateral forefoot, and 0.854 (excellent) in the hindfoot. The ICC of Ptime showed 0.868 (excellent) in the hallux, 0.892 (excellent) in the medial forefoot, 0.721 (good) in the lateral forefoot, and 0.832 (excellent) in the hindfoot. All ICC values showed good or excellent results. The Cronbach's alpha of Ppeak measured in the smart insoles was 0.990 in the hallux, 0.961 in the medial forefoot, 0.973 in the lateral forefoot, and 0.995 in the hindfoot; all indicated excellent reliability in all areas. Conclusion: The plantar pressure measurements of smart insoles during walking on a flat ground showed validity compared to Pedar-X, and high reliability after repeated measurements.

Measurement of Streambed Hydraulic Conductivity in Stream Sections in the Anseongcheon Watershed, Korea (안성천 수계 국가하천구간 하상 수리전도도 측정 시험)

  • Field experiments were conducted to estimate streambed hydraulic conductivity at 15 sites in the Anseongcheon watershed, Korea. Seepage meters and piezometers were installed in the streambed at each site to measure the amount of stream water-groundwater exchange and the hydraulic gradient. The vertical hydraulic conductivity was then calculated using Darcy's formula. The measured stream water-groundwater exchange rates were $4.08{\times}10^{-6}$ to $1.49{\times}10^{-5}m/s$, and the vertical hydraulic gradients were 0.005 to 0.145. The data suggest the streambed hydraulic conductivity to be $7.80{\times}10^{-5}$ to $1.58{\times}10^{-3}m/s$. The results show significant differences in connectivity between stream and aquifer. Quantification of the hydraulic interconnection between stream and aquifer, and evaluation of the effects of groundwater development and utilization on the streamflow require hydrogeological investigations of the connection between stream and aquifer, including the hydraulic conductivity of the streambed. Various field testing and analysis methods for hydrogeological assessment also require further improvement.

Quantitative Assessment of Climate Regulating Ecosystem Services Using Carbon Storage in Major Korean Ecosystems (탄소 저장량을 이용한 국내 주요 생태계 기후 조절 서비스 지표 산정)

  • Ecosystems have functions of providing, supporting, regulating and cultural services. In particular, there is an increasing attention to the importance of regulating ecosystem services in carbon sequestration function, since it is closely related to the issue of climate change. In this study, to quantify benefits of climate regulating ecosystem services, the carbon storage was defined as an indicator. Nine major Korean ecosystems were classified and research papers on carbon storage were analyzed. The collected carbon storage data were categorized according to classified ecosystems, methodologies, and carbon storage components. For each category, the mean, standard error and coefficient of variation were calculated. The carbon storage indicator was highest in vegetation biomass of deciduous forest ecosystems. The uncertainty was also estimated by the IPCC 2006 guidelines. The estimations of the uncertainty differed by methodologies and carbon storage components. With exception of forest ecosystems, the limited number of studies were available which might have hindered to conduct accurate estimations. These findings indicate that there are needs for further clarification in the measurement standards by different ecosystems.

Prognostic Impact of Charlson Comorbidity Index Obtained from Medical Records and Claims Data on 1-year Mortality and Length of Stay in Gastric Cancer Patients (위암환자에서 의무기록과 행정자료를 활용한 Charlson Comorbidity Index의 1년 이내 사망 및 재원일수 예측력 연구)

  • Objectives : We tried to evaluate the agreement of the Charlson comorbidity index values(CCI) obtained from different sources(medical records and National Health Insurance claims data) for gastric cancer patients. We also attempted to assess the prognostic value of these data for predicting 1-year mortality and length of the hospital stay(length of stay). Methods : Medical records of 284 gastric cancer patients were reviewed, and their National Health Insurance claims data and death certificates were also investigated. To evaluate agreement, the kappa coefficient was tested. Multiple logistic regression analysis and multiple linear regression analysis were performed to evaluate and compare the prognostic power for predicting 1 year mortality and length of stay. Results : The CCI values for each comorbid condition obtained from 2 different data sources appeared to poorly agree(kappa: 0.00-0.59). It was appeared that the CCI values based on both sources were not valid prognostic indicators of 1-year mortality. Only medical record-based CCI was a valid prognostic indicator of length of stay, even after adjustment of covariables($\beta$ = 0.112, 95% CI = [0.017-1.267]). Conclusions : There was a discrepancy between the data sources with regard to the value of CCI both for the prognostic power and its direction. Therefore, assuming that medical records are the gold standard for the source for CCI measurement, claims data is not an appropriate source for determining the CCI, at least for gastric cancer.

Difference of Functional Outcome and Related Factors in Patients With Stroke (일부 뇌졸중 환자의 기능변화 및 관련요인)

  • The purposes of the present study were to determine the difference of functional outcome, and to identify factors associated with functional difference in patients with stroke. The sample consisted of 56 stroke patients who had received physical therapy at the physical therapy unit of the Dongeui Medical Center in Busan city between January 2000 and June 2002. Stroke patients were evaluated by physical therapists 3 times; The first day in physical therapy (PT) (T1), one month after the first day in PT (T2), and two months after the first day in PT (T3). Functional status was assessed with the Functional Independence Measure (FIM) instrument, a validated instrument for documenting the severity of disability and assessing the outcome of rehabilitation treatment. Functional gain was calculated over T2-T1, T3-T1, and T3-T2. SAS statistical software was used for the analysis. The Student's t-test, paired t-test, analysis of variance (ANOVA/Tukey and Scheffe), and analysis of covariance (ANCOVA) were used to examine the functional difference in variables. Repeated measures ANOVA was also used to analyze the functional difference by time (T1, T2, and T3). Multiple regression analysis was performed to determine the effects of independent variables on the difference of functional outcome as defined by the FIM score. A total of 56 stroke patients were evaluated, their average age${\pm}$standard deviation was $61.6{\pm}9.3$ years (range: 40~81 yr). The functional status of patients who received physical therapy for about 2~3 months was significantly improved (mean FIM scores, $20.5{\pm}1.8$, $28.9{\pm}1.9$, and $8.41{\pm}1.1$ points for each time period, respectively) (p<.0001). Diabetes was significantly associated with the FIM score for T2-T1 (p<.05). The type of diagnosis was significantly associated with the FIM score for T3-T1 (p<.05). Gender, smoking, and the FIM score on admission were significantly associated with the FIM score for T3-T2 (p<.05). In conclusion, gender, smoking, diabetes, the type of diagnosis, and the FIM score on admission were significantly associated with improved FIM scores. We recommend that further research should explore the functional outcome by using larger sample sizes, longer follow-up periods, and more sensitive assessment instruments.

Method Validation for Monitoring of Agricultural Worker Exposure to Insecticide Fenthion (살충제 Fenthion에 대한 농작업자 노출 측정을 위한 분석/시험방법 검증)

  • Exposure measurement of agricultural worker to pesticide is one of important part of health risk assessment of pesticide. Therefore exposure matrices, apparatus, instruments and methods must be validated in advance to field experiment. In this study, method validation with an organophosphorus insecticide fenthion was carried out for exposure monitoring of agricultural worker. LOD and LOQ were 0.01 and 0.05 ng, respectively. Calibration curve linearity ($R^2$ > 0.999) and reproducibility (C.V. < 3%) were also excellent. Recovery at LOQ, 10LOQ and 100LOQ levels from gloves, socks, mask, patch, solid sorbent, glass fiber filter was 76~113% (C.V. < 3%). Trapping efficiency was 95~105% while no breakthrough was observed. Method validation for the exposure monitoring was established successfully through several experiments. Such method validation can be usually performed in laboratory and not much different for each pesticide so that, this techniques will be applied widely in research for pesticide exposure monitoring by combination with body surface area and respiration rates.

A Study of the Fluid Balance of the Patients on Soft Diets (유동식을 섭취하는 대상자의 수분 섭취와 배설의 균형 분석)

  • The purpose of this study was to investigate the fluid balance of the patients who were either on soft fluid diet or total parenteral nutrition. We studied 19 patients with neurologic disorders and 22 patients with oromaxillary surgery who were admitted to either D university hospital in Choognam or S general hospital in Seoul between May and November 1995. The mean age for the patients who had oromaxillary surgery was 24 years and their average hospital stay was 9 days. The mean age of the patients with neurologic disorders was 54 years and they were bedridden for average of 71 days. For the maxillary bone surgery patients we did not limit the range of their activities in the ward during data collection period. The patients with neurological disorders were bedridden and did not move around the ward. They were all either on soft fluid diet, or total parenteral nutrition. The findings of this study are as follows ; 1) The difference of the triceps skinfold thickness between the baseline and the final measurement was 0.4cm for neurologic patient group and 0.5cm for oromaxillary surgery patient group. The difference was not statistically significant in each group. 2) In the oromaxillary surgery patient group, the daily intake of fluid in the form of pure water, other beverages, fluid diet as well as IV fluid was 4581m1 while urine output was 2979ml. The difference between fluid intake and output was statistically significant, indicating that fluid intake was far more than urine output. In neurologic patient group, the daily intake of fluid including water from fluid diet and IV fluid was 2701m1 whereas urine output was 2253m1 and they were statistically significant. 3) For a more accurate assessment we adjusted the fluid balance based on weight changes during data collection period. In the oromaxillary surgery patient group. the difference between fluid intake and output was 1238m1 after weight changes being adjusted. The difference was statistically significant, suggesting fluid overload in this patient group. In neurologic patient group, the difference between fluid intake and output considering weight changes was 124ml. The difference was not statistically significant, suggesting that the fluid intake and output was well balanced in this patient group.

  • Cement is one of the most commonly used materials in the construction and civil engineering industry. However, emissions of carbon dioxide generated during the production of cement have been linked to climate change and environment pollutants. In order to replace cement, many studies have been actively performed research to utilizing Blast Furnace Slag(BFS), which is a byproduct of the steel industry. This study aims to investigate the physiochemical properties of the BFS powder based grout to determine whether it can be used as an environment-friendly grout material. As a fine powder, BSF can be used instead of cement grout due to its potential hydraulic property. BSF has also been known for its ability to strengthen materials long-term and to densify the internal structure of concrete. In order to investigate the physicochemical properties of the BFS powder based grout as a grout material, in this study assessment tests were performed through a gel-time measurement, uniaxial compressive strength, and chemical resistance tests, and heavy-metal leaching test. Characteristics and advantages of the slag were studied by comparing slag and cement in various methods.

Analysing the Use of Test Tools in Science-Related Affective Domain and its Improvements (과학 관련 정의적 영역 검사 도구 활용 및 개선 내용 분석)

  • This study analyzed the current use of science-related test tools in affective area in Korean academic journals for the last 30 years since 1989 and the cases of modification of the original tools related to the tools. This provides practical information on the development and improvement of test tools in the future and offers the basis for developing valid test tools. The study was conducted in two stages. We analyzed the status of 88 domestic science education researches dealing with testing tools related affective characteristics and 57 domestic and foreign resource tools referenced by them. After that, we compared the characteristics of the five cases in which the original tool was revised and the concrete item correction examples. The results were analyzed as follows: topics that have emerged continuously or that have recently begun to appear in domestic research, the research situations according to their purpose and measurement methods used by the tool, and use state of original tools. The background and key points of revision were analyzed for the five tools, TOSRA, SAI, PISA items, STEBI-A/B, SMQ, whose revision process were disclosed. In addition, the changes of the items were analyzed by comparing the four test items with the questions before and after the revision. As a result of item analysis, item improvement was proceeding in the direction of enhancing readability, clarifying meaning, and changing items according to change of sub constructs. The implications of this study are as follows: notes on interpreting the results obtained from the tool with multiple mixed constructs, the possibility of the testing tool that broadens the horizon of the research, error recognition in the subjective self-report, and notes when applying test tools from other disciplines to science.

Assessment of Dosimetric Leaf Gap According to Measuring Active Volume of Detector (검출기 측정 용적에 따른 Dosimetric Leaf Gap 변화와 정확성 검증에 대한 연구)

  • DLG (Dosimetric Leaf Gap) and transmission factor are important parameters of MLC modeling in treatment planning system. In this study, DLG and transmission factor of HD-MLC were measured using detector with different measuring volumes, and the accuracy of the treatment plans was evaluated according to the DLG values. DLG was measured using the dynamic sweeping gap method with Semiflux3D and MicroDiamond detectors. Then, 10 radiation treatment plans were generated to optimize the DLG value and compared with the measurement results. Photon energies 6, 8, 10 MV, the DLG measured by Semiflux3D were 0.76, 0.83, and 0.85 mm, and DLG measured by MicroDiamond were 0.78, 0.86, and 0.9 mm. All plans were measured by portal dosimetry and analyzed using Gamma Evaluation. In the 6 MV photon beams, the average gamma passing rate were 94.3% and 98.4% for DLG 0.78 mm and 1.15 mm. In the 10 MV photon beam, the average gamma passing rate were 91.2% and 97.6% for DLG 0.9 mm and 1.25 mm. HD-MLC needs accurate modeling in the treatment planning system. DLG could be used measured data using small volume detector. However, for better radiation therapy, DLG should be optimized at the commissioning stage of LINAC.
