The first KREDOS-EPR intercomparison exercise using alanine pellet dosimeter in South Korea

  • This paper presents the results of the first intercomparison exercise performed by the Korea retrospective dosimetry (KREDOS) working group using electron paramagnetic resonance (EPR) spectroscopy. The intercomparison employed the alanine dosimeter, which is commonly used as the standard dosimeter in EPR methods. Four laboratories participated in the dose assessment of blind samples, and one laboratory carried out irradiation of blind samples. Two types of alanine dosimeters (Bruker and Magnettech) with different geometries were used. Both dosimeters were blindly irradiated at three dose levels (0.60, 2.70, and 8.00 Gy) and four samples per dose were distributed to the participating laboratories. Assessments of blind doses by the laboratories were performed using their own measurement protocols. One laboratory did not participate in the measurements of Magnettech alanine dosimeter samples. Intercomparison results were analyzed by calculating the relative bias, En value, and z-score. The results reported by participating laboratories were overall satisfactory for doses of 2.70 and 8.00 Gy but were considerably overestimated with a relative bias range of 10-95% for 0.60 Gy, which is lower than the minimum detectable dose (MDD) of the alanine dosimeter. After the first intercomparison, participating laboratories are working to improve their alanine-EPR dosimetry systems through continuous meetings and are preparing a second intercomparison exercise for other materials.

The Effectiveness and Safety of Acupuncture for Recovery of Patients with Vertebral Compression Fractures: A Systematic Review and Meta-analysis (척추 압박 골절 환자의 회복에 대한 침 치료의 유효성 및 안전성 평가: 체계적 문헌 고찰과 메타분석)

  • Objectives : The purpose of this study was to evaluate the effectiveness and safety of acupuncture treatment for recovery of patients with vertebral compression fracture(VCF). Methods : We searched ten English and Chinese and seven Korean database up to April 2018. Randomised controlled trials(RCTs), quasi-RCTs, non-radomised Controlled Trials(CCTs) were eligible. Quasi-RCTs and CCTs were assessed only for safety assessment. Pain and adverse events were primary outcome of this review. Quality of life, dysfunction, patient satisfaction, incidence of new vertebral compression fracture were regarded as secondary outcomes. The risk of bias was assessed by two independent authors using the Cochrane risk of bias tool. Level of evidence was tabulated using the GRADE methods. Results : Of 1656 screened, 15 RCTs, 1 quasi-RCT and 3 CCTs were included. Number of participants per study ranged from 45 to 135. Most of the studies had unclear or high risk of bias and considerable heterogeneity in terms of type of intervention, comparison and time-points for outcome measurement. Compared to usual care alone, acupuncture combined with usual care showed short-term favorable results for pain relief in patients with VCF(5 studies, n=252, MD -1.05 point on a 0 to 10 point scale, 95% CI -1.45 to -0.65, $I^2=74%$). Four studies reported mild and temporary adverse events, and no serious adverse events were reported. One study descriptively reported that acupuncture was effective for improving quality of life without providing numerical outcomes. There were no reports of patient satisfaction and incidence of new VCF. Conclusions : Level of evidence is very low for the effectiveness and safety of acupuncture for pain, harms and other clinical outcomes in patients with VCF. Included studies suffered from incomplete reporting, high or unclear risk of bias and substantial heterogeneity between studies. Future high-quality RCTs are needed to assess whether acupuncture is beneficial for recovery of patients with VCF.

A Study on the Predictability of Eastern Winter Storm Waves Using Operational Wind Forecasts of KMA (기상청 현업 예보 바람자료를 이용한 동해안 동계 파랑 예측 재현도 연구)

  • The predictability of winter storm waves using KMA's operational wind forecasts has been studied to predict wind waves and swells in the East coast of Korea using SWAN. The nested model were employed along the East coast of Korea to simulate the wave transformation in the coastal area and wave dissipation term of whitecapping is optimized to improve swell prediction accuracy. In this study, KMA's operational meteorological models (RDAPS and LDAPS) are used as input wind fields. In order to evaluate model accuracy, we also simulate wind waves and swells using ECMWF reanalysis and KIOST WRF wind and they are compared with the KMA's operational wave model and the wave measurement data on the offshore and onshore stations. As a result, it has the lowest RMSE and the highest correlation coefficient in the onshore when the input wind fields are KMA's operational meteorological forecasts. In the offshore, all of the simulate results shows good agreements with similar error statistics. It means that it is very feasible to use SWAN model with the modified whitecapping factor and KMA's operational meteorological forecasts for predicting the wind waves and swells in the East coast of Korea.

A Study on Body Image Recognition, Food Habits, Food Behaviors and Nutrient Intake according to the Obesity Index of Elementary Children in Changwon (창원시 일부 초등학교 고학년의 비만도에 따른 체형인식도, 식습관, 식행동 및 영양섭취상태에 대한 비교연구)

  • This study was carried out to investigate the difference of the body image recognition, food habits, food behaviors and nutrient intake according to the obesity index(underweight-, normal-, obese group) in children(aged 10.8 ys, 47 boys, 33 girls). Subjects were eva1uated based an anthrofometric measurement and questionnaries including food record diary in Changwon. The results are summarized as follows. The weight, BMI, body fat, waist circumference, hip circumference and birth weight except height showed a significant difference between the three groups(p<0.001). The waist and hip circumference showed the possibility of being a predictor of obesity in children. The children's obesity was related to mother's weight(p<0.01), but not to father's. Ninety percent of the obese group, 47.8% of the normal group, and 37.8% of the underweight group preferred a thinner figure than their current status. The underweight group did more exercise regularly than the other groups. The underweight and obese groups had more irregular meal times and foster eating habits compared to the normal group. Most of subjects(89.9%) had an overeating habit, and tole ratio of the overeating habit increased with obesity. Fifty five percent of the subjects clad the habit of skipping meals, mainly breakfast. 46.8% of the subjects ate snacks more than twice per a day, the underweight and obese groups had snacks more of than the normal group. The preferred snack was 'cookies'and 'fruits & juices'. 53.8% of the subjects had a prejudice for special food(especially 'pulses'(37.9%) and 'vegetables'(31.0%)). The assessment scores of food behaviors was relatively low in most of the subjects. Most nutrient intakes, except vitamin B$_1$, C and phosphate, were lower than those of Korean RDA. The nutrient intake of the normal group was higher than the underweight and obese groups. The results of this study showed some nutritional problems, which indicates the need for nutritional management for the children. To educate children, who are able to change their food habits and lifestyle, each means to help healthy growth and to help them become healthy adults.

Relationships between the Nutrient Intake Status, Dietary Habits, Academic Stress and Academic Achievement in the Elementary School Children in Bucheon-si (부천시 일부 초등학생의 영양소 섭취상태, 식습관, 학업스트레스와 학업성취도와의 관련성 연구)

  • This study examined the influence of the nutrient intake status, dietary habit, and academic stress of elementary school children on their academic achievement. Two-hundred and twenty-four fifth-graders in Bucheon-si, Gyeonggido were surveyed. The study included the 24 hr-recall, anthropometric measurement, assessment of stress level and academic achievement. The subjects were normal in height, weight and Rohrer index, but higher percentage of underweight was seen in girls and vice versa in boys. The overall nutrient intake and dietary habits were fairly good, but Ca and folate intake was less than 75% KDRIs and dietary habits of boys were inferior. Academic stress level of all the subjects was not high. In terms of academic performance and its relations with nutrient intake, the more the amount of nutrient intake, the higher the academic performance. Especially, it was true for the energy, protein, phosphorus, potassium, zinc, polyunsaturated fatty acids, and n-6 fatty acid intakes (p < 0.05). The overall academic performance was higher for those who eating-out less frequently. Children with higher comprehensive dietary habit scores have shown better academic performance (p < 0.05). Less stress implied to those shown higher academic achievement while those with relatively poor academic performance showed high stress level since their grade did not improve as much as they anticipated. In conclusion, the academic achievement was higher for those who have a better nutritional status, better dietary habits, and lower stress levels. Therefore, it is critical for nutritionists, parents, and teachers to improve the nutritional status and dietary habits as well as to help them manage their stress levels, which will eventually contribute to an enhanced academic performance.

Evaluation of Stability of Double Threaded Implant-Emphasis on Initial Stability Using Osstell MentorTM; Part I (이중나사산 임플란트의 안정성에 대한 평가 - 오스텔 멘토를 이용한 초기 안정성 ; PART I)

  • Purpose This study was planned to compare and evaluate the stability of implant using $Osstell^{TM}$ and Osstell $Mentor^{TM}$. Material and methods Artificial bone and RBM(resorbable blasting media) surface blasted implants(Osstem US II, SS II implants - diameter: 4mm, length: 13mm) were used. To measure the stability of installed implants, $Osstell^{TM}$ and Osstell $Mentor^{TM}$ were used. In the first experiment, five implants were installed in D1(external type implants) and D3(internal type implants). In the second experiment, 4 internal type implants were divided in two groups and installed in D1 artificial bone with different depth. In the third experiment, two external implants were installed in D1 and D3 artificial bone each and two internal implants were installed in D1 and D3 artificial bone. In all groups, their stability were measured by $Osstell^{TM}$ and Osstell $Mentor^{TM}$. Results In all groups, $Osstell^{TM}$ and Osstell $Mentor^{TM}$ both showed reliable measurement values. The value difference between $Osstell^{TM}$ and Osstell $Mentor^{TM}$ was observed but the difference was small and clinically acceptable. Conclusion These results suggest that the use of Osstell $Mentor^{TM}$ has clinical relevance in the assessment of implant stability.

Precision evaluation of crown prosthesis manufactured by two bur and three bur (2종류의 버와 3종류의 버를 이용해 제작된 크라운 보철물의 정밀도 평가)

  • Purpose: The purpose of this study was to assess precision of crown prostheses that were fabricated by using 2 kinds of bur or 3 kinds of bur. Methods: The crowns were fabricated by using the first molar of the right maxillary in this study. The abutments that were prepared were scanned by using a scanner and designed by using CAD software. Based on the crown design, NC data were created with CAM software. The created NC data were used while fabricating the crown prostheses by using 5-axis milling machine. Scanning was done for the internal and external surface of the completed crown prostheses and 3-dimensional measurement was conducted for precision assessment. Results: The $RMS{\pm}SD$ value for the external surface of the crown prostheses that was fabricated by using two burs and three burs were $28.5{\pm}4.1{\mu}m$ and $19.1{\pm}2.8{\mu}m$, respectively; and the value for two burs were bigger than that for three burs with statistical significance (p<0.001). The $RMS{\pm}SD$ value for the internal surface of the crown prostheses that was fabricated by using two burs and three burs were $14.9{\pm}1.9{\mu}m$ and $13.3{\pm}2.5{\mu}m$, respectively; and the value for two burs were bigger than that for three burs but with no statistical significance. Conclusion: Based on this study, the prostheses that were fabricated by using 3 bur presented better stability compared to those that were fabricated by using 2 bur and statistically significant difference was found only in the external surface.

An Assessment of Iron Nutritional Status of Korean Women in Seoul and Puyo Areas (서울 및 부여지역 여성들의 철분 영양상태에 관한 비교 연구)

  • The iron nutritional status of 328(20-70 years old) women in Seoul and Puyo areas was evaluated using a dietary information and a measurement of hematological indices. The serum iron was measured by Red Blood Cell(RBC), Hemoglobin(Hb), Hematocrit(Hct), Serum Iron(S-Fe), Total Iron Binding Capacity (TIBC), Transferrin Saturation(TS) and Serum Ferritin(SF) and was analyzed with the information obtained by interviews which included socio-demographic variables and the dietary pattern of the subjects. The mean Hb was 12.6 $\pm$ 1.21g/dl(Seoul : 12.4 $\pm$ 1.14g/dl, Puyo : 13.2 $\pm$ 1.28g/dl), Hct was 38.7 $\pm$ 3.84%(Seoul: 37.5 $\pm$ 3.28%, Puyo: 4.19 $\pm$ 3.48%), S-Fe was 76.8 $\pm$ 31.49ug/dl(Seoul: 68.8 $\pm$ 27.3ug/dl, Puyo: 78.8 $\pm$ 32.3ug/dl) and TIBC was 277.7 $\pm$ 86.15ug/dl(Seoul: 354.1 $\pm$ 129.8ug/dl, Puyo: 259.0 $\pm$ 59.55ug/dl). The mean Ts(%) was 30.9 $\pm$ 17.9% (Seoul: 21.7 $\pm$ 10.52%, Puyo: 33.2 $\pm$ 18.68%) and the level was significantly higher in Puyo women(p<0.05). The mean SF was 45.4 $\pm$ 46.21ng/ml(Seoul: 53.6 $\pm$ 50.21ng/ml, Puyo: 36.1 $\pm$ 39.83ng/ml). The Red Blood Cell(RBC) showed a negative correlation with age and Hb, Hct showed a negative correlation with education and income level. However TIBC showed a and total energy expenditure per day. Energy, protein, carbohydrate, iron and vit.C intake showed a positive correlation with Hb and Hct. In particular, heme iron showed a positive correlation with RBC and nonheme iron of Hb, income level and iron intake affected on the level of Hct. In Seoul women, the age of menarche affected on the level of TIBC, total energy expenditure affected on the level of Hct. In Seoul women, the age of menarche affected on the level of TIBC, total energy expenditure affected on the level of S-Fe.

Studies on Motor Activity Log-28 and Actual Amount of Use Test, Actual Amount of Use Test Inter-rater Reliability in Healthy Individuals: Age Dependence and Handedness (연령과 손잡이에 따른 정상인들의 Motor Activity Log-28과 Actual Amount of Use Test 연구 및 Actual Amount of Use Test의 평가자간 신뢰도연구)

  • Background: Spontaneous use of the upper extremities on the affected side of patients with stroke is a meaningful indicator of recovery and may vary by the age or dominant hand of patients. No prior study has reported changes in actual amount of use test (AAUT) and motor activity log (MAL)-28 according to age and handedness in healthy adults, and AAUT inter-rater reliability for assessment of healthy adults. Objects: This study aimed to (1) research the differences in AAUT and MAL-28 according to age and handedness in healthy adults, and (2) determine the inter-rater reliability of the AAUT. Methods: Seventy healthy adults participated in this study. The MAL-28 was assessed by dividing 61 subjects into young right-handed ($n_1=20$), young left-handed ($n_2=21$), and older right-handed ($n_3=20$) groups. The AAUT was assessed by dividing 63 subjects into young right-handed ($n_1=25$), young left-handed ($n_2=18$), and older right-handed ($n_3=20$) groups. Student's t-test and the Wilcoxon signedrank test were used for statistical analysis. Results: The Amount of Use (AOU) scale values for each group showed no significant differences between age groups and handedness groups in the MAL-28 (p>.05). The AAUT AOU scale value showed significant differences regarding dominant handedness in the AAUT (p<.05), but no significant differences according to age (p>.05). (2) Inter-rater reliability of the AAUT was excellent, except few items (item 9, 11, and 12). Conclusion: Although both the MAL-28 and the AAUT measured how much participants used their dominant arms in healthy subjects, the AAUT only showed significantly higher dominant arm use in left hander than the right hander. In addition, the inter-rater reliability of the AAUT was excellent. Current results can be utilized as a basic information when clinicians develop rehabilitation strategies, and AAUT was shown to be a reliable evaluation tool for measurement of upper extremity use in Korean adults, based on the reliability demonstrated by this study.

  • The purpose of this study was to identify the professional identity levels and characteristics of occupational therapists through Rasch analysis and to use this as the basis for further assessment and research on the same subject. An online survey was conducted, and the response of 88 people working as occupational therapists between May and June 2020 was analyzed. The collected data included the frequency, analyzed by SPSS 22.0 and technical statistics, Winsteps 3.8.0 target and item suitability, item difficulty, level scale, and separation reliability. According to the analysis, 17 (16.3%) of the subjects and 5 (20%) out of 25 items were unsuitable. Item difficulty was the highest in the category "I systematically read professional publications on occupational therapy", and the lowest level of difficulty in "I make my own decisions about what to do." The three-point scale was suitable for the equilibrium scale, and the separation reliability was between .86 and .95. Thus, identity levels and characteristics of occupational therapists, the suitability of items and subjects, and difficulty levels can be identified through Rasch analysis. In the future, research should be conducted on various sample groups, to develop identity measurement tools specifically for occupational therapists, and to present items appropriate to the characteristics of their work.
