Purpose: The aim of our study was to determine the prevalence and degree of lingual concavities in the first molar region of the mandible to reduce the risk of perforating the lingual cortical bone during dental implant insertion. Methods: A total of 163 suitable cross-sectional cone-beam computed tomography images of edentulous mandibular first molar regions were evaluated. The mandibular morphology was classified as a U-configuration (undercut), a P-configuration (parallel), or a C-configuration (convex), depending on the shape of the alveolar ridge. The characteristics of lingual concavities, including their depth, angle, vertical location, and additional parameters, were measured. Results: Lingual undercuts had a prevalence of 32.5% in the first molar region. The mean concavity angle was 63.34°±8.26°, and the mean linear concavity depth (LCD) was 3.03±0.99 mm. The mean vertical distances of point P from the alveolar crest (Vc) and from the inferior mandibular border were 9.39±3.39 and 16.25±2.44, respectively. Men displayed a larger vertical height from the alveolar crest to 2 mm coronal to the inferior alveolar nerve (Vcb) and a wider LCD than women (P<0.05). Negative correlations were found between age and buccolingual width at 2 mm apical to the alveolar crest, between age and Vcb, between age and Vc, and between age and LCD (P<0.05). Conclusions: The prevalence of lingual concavities was 32.5% in this study. Age and gender had statistically significant effects on the lingual morphology. The risk of lingual perforation was higher in young men than in the other groups analyzed.

Objective: The purpose of this study was to investigate the relationship between the facial skeletal patterns and the shape of the mandibular symphysis in adults with malocclusion by using a structural equation model (SEM). Methods: Ninety adults who had malocclusion and had records of facial skeletal measurements performed using cone-beam computed tomography were selected for this study. The skeletal measurements were classified into three groups (vertical, anteroposterior, and transverse). Cross-sectional images of the mandibular symphysis were analyzed using generalized Procrustes and principal component (PC) analyses. A SEM was constructed after the factors were extracted via factor analysis. Results: Two factors were extracted from the transverse, vertical, and anteroposterior skeletal measurements. Latent variables were extracted for each factor. PC1, PC2, and PC3 were selected to analyze the variations of the mandibular symphyseal shape. The SEM was constructed using the skeletal variables, PCs, and latent variables. The SEM showed that the vertical latent variable exerted the most influence on the mandibular symphyseal shape. Conclusions: The relationship between the skeletal pattern and the mandibular symphysis was analyzed using a SEM, which showed that the vertical facial skeletal pattern had the highest effect on the shape of the mandibular symphysis.

Purpose: To determine the effects of botulinum toxin (BoNT) injection into the human masseter muscle on the morphology of the mandibular condyle bone using cone-beam computed tomography (CBCT). Methods: Twenty volunteers were randomly assigned to one of two groups. Group I received a single BoNT injection; Group II received two injections, with the second being administered 4 months after the first. CBCT scans of both temporomandibular joints (TMJs) were performed before and 6 months after the first injection. Bony changes in the cortical layers of the condyle heads were evaluated and the long and short axes of both mandibular condyles were measured. The thickness at the thinnest part of glenoid fossa was also quantitatively measured. Results: There was no significant difference between pre- and post-injection CBCT images. Furthermore, no changes in the cortical layers of the condyle heads were observed among the subjects who exhibited mild degenerative TMJ changes. The quantitative measurements (long axis, short axis, and the thickness of thinnest part of glenoid fossa roof) did not differ significantly between pre- and post-injection except for the long axis in Group I. Conclusions: Within the limitations of this study, it appears that BoNT injections into human masseter muscles do not alter the morphology of the mandibular condyle bone in healthy adults.

Purpose: This study aimed to broaden our understanding of the predisposing factor and treatment of dislocation by analyzing and evaluating the morphology of the articular eminence (AE) in subjects with temporomandibular joint (TMJ) dislocation using cone beam computed tomography (CBCT). Methods: The subjects were divided into two groups: dislocation (31 subjects) and control (32 subjects). CBCT was used to examine 126 TMJs in 63 subjects (26 males, 37 females). The height, width, and posterior slope of the AE were measured in the parasagittal plane. The posterior slope was measured using the "top-roof line angle (TR angle)" method and the "best-fit line angle (BF angle)" method. The AE on the left side (AEL) and the AE on the right side (AER) of the subjects in the dislocation group were separately analyzed and compared with the control group after taking measurements. The average value of both sides was used when comparing with subjects with bilateral dislocation. Results: Dislocations were more frequent in females (67.7%) than in males (32.3%). The dislocation group showed a gentler TR angle than the control group in the AER and in the average of AE on the both sides (AEB). The same group also showed a wider AE in the AEL and the AER (p<0.05). In subjects with unilateral dislocation, the width of the AE with dislocation was narrower and the TR angle and BF angle was steeper than the other side without dislocation (p<0.05). Conclusions: In subjects with unilateral TMJ dislocation, the posterior slope of the AE is steeper, and the width is narrower at the site of dislocation compared to the site without dislocation. However, in subjects with bilateral TMJ dislocation, AEB were wider, and the mean value of the posterior slope of AEB was gentler than that of the control group.

Purpose: This study aimed to quantify alveolar bone morphology, demonstrate the relationship between tooth angulation and alveolar bone thickness, and introduce a new classification for anterior mandibular teeth related to immediate implant placement (IIP). Methods: Cone-beam computed tomography (CBCT) images of 211 anterior mandibular teeth were analyzed in sagittal slices to measure the thickness of the facial alveolar bone crest (FAB1) and apex (FAB2), and the lingual alveolar bone crest (LAB1) and apex (LAB2). Tooth angulation was classified as 1°-10°, 11°-20°, and >20° according to the tooth's long axis and alveolar bone wall. Spearman correlation coefficients were used to evaluate correlations between the variables. Results: FAB1 and LAB1 were predominantly thin (<1 mm) (84.4% and 73.4%, respectively), with the lateral incisors being thinnest. At the apical level, FAB2 and LAB2 were thick in 99.5% and 99.1% of cases, respectively. Significant differences were documented in FAB2 (P=0.004), LAB1 (P=0.001), and LAB2 (P=0.001) of all mandibular teeth. At all apical levels of the inspected teeth, a significant negative correlation existed between TA and FAB2. Meanwhile, TA showed a significant positive correlation with LAB2 of the lateral incisors and canines. These patterns were then divided into class I (thick facial and lingual alveolar bone), class II (facially inclined teeth) with subtype A (1°-10°) and subtype B (11°-20°), and class III (lingually inclined teeth) with subtype A (1°-10°) and subtype B (11°-20°). Conclusions: Mandibular anterior teeth have predominantly thin facial and lingual crests, making the lingual bone apical thickness crucial for IIP. Although anchorage can be obtained from lingual bone, tooth angulation and tooth types had an impact on IIP planning. Hence, the new classification based on TA and alveolar bone wall may enable rational clinical planning for IIP treatment.
Radiographic diagnosis of periapical lesions is based on many factors, including anatomical limitations such as thickness of the cortical bone; positioning of the apical abscess to the cortical bone; and is complicated by proximity to other anatomical structures and neighboring teeth. With conventional radiographs, these structures are often superimposed. Dental CBCT with its associated geometric accuracy offers accurate visualizations of the complex relationships and boundaries between teeth, related anatomical features, and their associated pathology. Its images also provide us internal tooth morphology, periodontal ligament space, the presence or absence of periapicl lesions in association wi th critical anatomical structures and maxillary sinus involement. Using 3 D imaging makes it easier for clinicians to detect, diagnosis, and develop highly effective treatment plans. Now, 4 cases of periapical and periodontal pathosis with CBCT images are to be presented including periapical abscess, furcation involvement, periapical pathosis involving maxillary sinus, and osteomyelitis. CBCT analyze specific area of interest and provides the highly detailed anatomical information. It also facilitates earlier and more accurate diagnosis, and treatment planning decisions and more predictable outcome.

Purpose: The aim of this study was to evaluate the ossification and maturation of the midpalatal suture in young Koreans using cone-beam computed tomography (CBCT). Materials and Methods: The study sample consisted of 40 patients with ages from 8.2 to 23.6 years who visited the Department of Orthodontics, Chung-Ang University Dental Hospital. CBCT images were taken for diagnosis. From the CBCT image, morphological stages and Hounsfield units (HU) of midpalatal suture were obtained to evaluate the midpalatal suture maturation. Spearman's correlation coefficients were calculated to analyze relationships between chronological age, morphological stage, and HU. Result: There was a wide variation in the morphology of the midpalatal suture in each age group. It showed a modest relationship between the chronological age and the degree of morphological stages. Also there was a modest relationship between the chronological age and HU. In contrast, there was a significant relationship between morphological stages and HU of midpalatal suture (P<0.01). Conclusion: CBCT images can be used for the evaluation of midpalatal suture maturation. For the estimation of the prognosis of rapid maxillary expansion, CBCT may be reliable for the assessment of the maturation of the midpalatal suture.
To recognize the problems in malocclusion by roentgenocephalograms, the author designed a new pentagonal frame based on maxillary and mandibular bones. The subjects consisted of 44 normal occlusions (20 male and 24 female), 44 Class II division 1 malocclusions (15 male and 29 female) and 67 Class III malocclusions (31 male and 36 female). The results are as follows; 1. In normal group, the maxillary and mandibular skeletons of female we placed more closely to FH plane, so more anteriorly and upward than those of male. 2. In normal group, the posterior vertical height is longer in male than in female and the upper anterior teeth of female are in more labioversion than those of male. 3. By the X, Y coordinate values in pentagonal frame, it is helpful to recognize certain problems in malocclusions. 4. The posterior vertical height is a good indicator in detecting Angle's Class III malocclusion. 5. The maxillary and mandibular body length, the anterior point of maxillary and mandibular body length and the axial inclination of upper and lower anterior teeth can be useful in discerning Angle's Class II & Class III malocclusion.

Silk fibroin membrane was prepared and examined to know the feasibility of SF membrane as guided bone regeneration. The morphology of silk membrane was flat and smooth surface. The conformation of silk fibroin was $\beta$-sheet structure. When the silk membrane was applied on the rat calvarial defect model, it showed significantly higher new bone formation than uncovered control in histomorphometric analysis. The silk membrane was covered by thin fibrotic tissue and there was not observed any inflammatory cells infiltration. In conclusion, silk fibroin membrane could be useful materials for guided bone regeneration.

Three-dimensional (3D) reconstruction of cone-beam computed tomography (CBCT) scans appears to be a valuable method for assessing pulp canal configuration. The aim of this report is to describe endodontic treatment of a mandibular second premolar with aberrant pulp canal morphology detected by CBCT and confirmed by 3D modeling. An accessory canal was suspected during endodontic treatment of the mandibular left second premolar in a 21 year old woman with a chief complaint of pulsating pain. Axial cross-sectional CBCT scans revealed that the pulp canal divided into mesiobuccal, lingual, and buccal canals in the middle third and ended as four separate foramina. 3D modeling confirmed the anomalous configuration of the fused root with a deep lingual groove. Endodontic treatment of the tooth was completed in two appointments. The root canals were obturated using lateral compaction of gutta-percha and root canal sealer. The tooth remained asymptomatic and did not develop periapical pathology until 12 months postoperatively. CBCT and 3D modeling enable preoperative evaluation of aberrant root canal systems and facilitate endodontic treatment.
