Diagnostic performance of stitched and non-stitched cross-sectional cone-beam computed tomography images of a non-displaced fracture of ovine mandibular bone

  • Purpose: This study assessed the diagnostic performance of stitched and non-stitched cross-sectional cone-beam computed tomography (CBCT) images of non-displaced ovine mandibular fractures. Materials and Methods: In this ex vivo study, non-displaced fractures were artificially created in 10 ovine mandibles (20 hemi-mandibles) using a hammer. The control group comprised 8 hemi-mandibles. The non-displaced fracture lines were oblique or vertical, <0.5 mm wide, 10-20 mm long, and only in the buccal or lingual cortex. Fracture lines in the ramus and posterior mandible were created to be at the interface or borders of the 2 stitched images. CBCT images were obtained from the specimens with an 80 mm×80 mm field of view before and after fracture induction. OnDemand software (Cybermed, Seoul, Korea) was used for stitching the CBCT images. Four observers evaluated 56 (28 stitched and 28 non-stitched) images to detect fracture lines. The diagnostic performance of stitched and non-stitched images was assessed by calculating the area under the receiver operating characteristic curve (AUC). Sensitivity and specificity values were also calculated (alpha=0.05). Results: The AUC was calculated to be 0.862 and 0.825 for the stitched and non-stitched images, respectively (P=0.747). The sensitivity and specificity were 90% and 75% for the non-stitched images and 85% and 87% for the stitched images, respectively. The inter-observer reliability was shown by a Fleiss kappa coefficient of 0.79, indicating good agreement. Conclusion: No significant difference was found in the diagnostic performance of stitched and non-stitched cross-sectional CBCT images of non-displaced fractures of the ovine mandible.

  • 관골 골절을 분류할 때 주위의 해부학적인 구조물과 연관해서 기능적인 면에 중점을 두어야 하므로 복시나 내안증같은 합병증이 나타날 수 있는 안와골절을 포함하는 분류를 해야한다. 관골 골절의 변위에 따라 나타나는 합병증의 양상이 매우 다양하므로 진단에 도움이 될 수 있도록 지속적인 연구가 필요할 것으로 사료되며, 초창기에 이러한 임상적인 합병증이 나타나지 않더라도 추후에 발생할 가능성이 있기 때문에 이를 간과해서는 안될 것으로 사료된다.

  • 하악골 과두돌기 골절은 하악골에 있어서 자주 일어나는 골절중의 하나이며, 이 부위는 해부학적 구조로 이상적인 Reposition ( )이 통례적으로 불가능 하다고 할 수 있을 정도로 어렵기 때문에 치료방법이 선택적으로 이용된다. 수술후 충분한 악관절기능을 발휘하는데 문제가 있을수도 있기 때문에 (Arthropathia deformans) 치료 계획을 세우는데 있어서는 우선 관절기능이 정상적으로 회복되어져야 하며, 또한 악관절 강직에 따른 안면골의 발육 부전으로 인한 악골기형을 고려하여야 한다는데 치료의 촛점이 두어져야 한다. 본인은 1981년 부터 1987년까지 인제대학부속부산백병원에서 치료한 바 있는 73명을 대상으로 (88골절선) 임상조사와 문헌적 고찰을 통하여 악관절 과두돌기 골절에 대한 치료방법의 선택기준 및 합병증 방지에 그 일익을 하고자 이 연구를 시행하였다.

Epidemiology of Facial Bone Fractures During the Coronavirus Disease 2019 Pandemic: A Single Korean Level I Trauma Center Study

  • Purpose: The medical community has been heavily impacted by the coronavirus disease 2019 pandemic. The management of facial trauma patients has been affected by the patient capacity of emergency rooms. In this study, we share our experiences of facial trauma management during the social lockdown period and investigate the epidemiological changes in facial bone fractures. Methods: A total of 997 patients who presented to Ajou University Hospital Emergency Center and were evaluated by plastic or maxillofacial surgeons for facial trauma were included in this retrospective study. Our study design was a comparative study of two groups: the 2019 group (control) and the 2020 group (the experimental group that experienced social lockdown). Results: The total number of emergency room inpatients reflected the national pandemic trends with three peaks in patient numbers. According to these trends, facial bone fractures had two different low points in August 2020 and December 2020. A comparison of the 2019 and 2020 facial bone fractures did not show a statistically significant difference in the total number of patients. An analysis of the causes of trauma showed that domestic accidents increased in 2020 (30.92%; p<0.001). Among the anatomical sites of facial injury in surgical patients, the frontozygomatic complex fracture increased the most in 2020 (p=0.018). Facial injuries with two separate sites of injury or with three or more involved sites also showed a significant increase in 2020 (p<0.001). Conclusions: We demonstrated that the incidence of facial trauma patients correlated with the incidence of patients presenting to the emergency department and that facial trauma is inextricably related to multi-trauma cases. Domestic accidents and facial trauma with multiple anatomically involved sites are increasing trends that need more attention.

악안면 골절에 관한 임상적 고찰 (Clinical Study of facial bone fracture)

  • The purpose of this study was to investigate the fractures of facial skeleton ; mandible, maxilla, zygoma Clinically, we observed 413 patients with facial bone fractures treated at the department of dentistry, Koryo General Hospital from Jan. l989 to Dec. I991. This results ere as follows : 1. The most common fracture was occured in the mandible (63.7%) and the symphysis was occured most freguently(30.4%) 2. The most prevalent age was twenties(37.5%) 3. Main causes was traffic accident(21.7% ) 4. The ratio of male to female was about 9.3 : 1 5. Most combined injuries was head injuries. (54.6%) 6. The most frequent job was a company employee. 7. There were the highest frequency in March, and the lowest frequency in February.

  • As panfacial fractures are involved multiple fracture, there are possibility of many pre-operative & postoperative complications. It is necessary to do exact pre-operative evaluation, appropriate operation and care, for preventing and treating these complications, especially related to optic nerve injury. The complication occurs rarely after periorbital facial bone fracture, and indirect injuries may occur as a result of impact shearing force transmitted into the optic nerve axons or to the nutrient vessels of the optic nerve. Also indirect injuries may occur after the force of impact because of vasospasm and swelling of the optic nerve within the confines of the nonexpansile optic canal. It is necessary to active evaluation and treatments involving decompression of the orbit surgically and high dose steroid therapy in relation to panfacial fracture. But sometimes this treatments are limited due to severe swelling of the face and related multiple bone fractures in the body. This case showed the delayed neuropathy, at last visual loss, in spite of megadose methylprednisolone administration. The purpose of this article is to present indirect traumatic optic neuropathy that is one of many complications in panfacial bone fracture.

  • Compound comminuted mandibular fracture is defined as the presence of multiple fracture lines with open wound resulting in many small pieces within the same area. The incidence of mandibular comminution is difficult to determine but reported as 2.7~18.6 % incidence among mandibular fractures. There are controversies in the treatment of mandibular comminuted fractures. Treatment of comminuted mandibular fracture has traditionally involved closed reduction in an effort to avoid stripping periostcum from the bony segments, but rigid internal rigid fixation is used more popular at present. The extent of comminution, displacement of bony fragments and patient general conditions are important factors in decision of the treatment methods. When significant bone displacement is present, it is necessary to reduce these comminuted fragments to an anatomic, pretraumatic relationship to restore facial form and function. In these cases, ORIF allows anantomic reduction of comminuted segments as well as pretraumatic occlusion. Gentle handling of the soft tissue, rigid fixation of bony fragments and adequate immobilization are essential for reducing the complications. This is the report the incidence, causes, complications and treatment of the patients who visited our department for compound comminuted mandibular fractures.

  • Orbital floor blowout fractures can result in a variety of signs and symptoms depending on the severity of the bone defect. Large defects often result in enophthalmos and restriction of ocular movement; yet the timing of surgery can be delayed up to two weeks with good functional outcomes. In contrast, an orbital trapdoor defect with entrapment of the inferior rectus muscle usually elicits pain with marked restriction of the upward gaze and activation of the oculocardiac reflex without significant dystopia or enophthalmos. When autonomic cardiac derangement is diagnosed along with an orbital floor fracture, it has been suggested that the fracture should be treated immediately. Otherwise, it will result in continued hemodynamic instability and muscular injury and may require a second surgery. This article reports the management of an unusual presentation of a trapdoor blowout orbital floor fracture surgery with oculocardiac response in an adult, with emphasis on its pathophysiology, management, and differential diagnosis.

A comparative study of quality of life of patients with maxillofacial fracture and healthy controls at two tertiary healthcare institutions

  • Objectives: Despite treatment, the pre-traumatic facial appearance of patients with maxillofacial fractures might not be able to be restored, and this difference can affect the person's quality of life (QoL). This study was designed to evaluate changes in QoL of people with maxillofacial fractures. Materials and Methods: The study population was comprised of participants with maxillofacial fracture and age- and sex-matched healthy controls without history of such fracture. QoL was measured using the World Health Organization Quality of Life BREF (WHOQOL-BREF) questionnaire, which was administered to the patients before treatment (Time 1), at 6 weeks post-treatment (Time 2), and at 12 weeks postoperatively (Time 3). The values were compared with those of healthy controls. The QoL was compared between closed reduction group and open reduction and internal fixation group. Results: The QoL scores of people with maxillofacial fracture before treatment were significantly lower (P=0.001) than those of healthy controls in all domains of the WHOQOL-BREF. The QoL scores in the psychological and social domains of patients with maxillofacial fracture at Time 3 were still lower than those of healthy controls (P=0.001). Conclusion: The QoL of patients with maxillofacial fracture was significantly reduced before treatment in all domains and remained reduced in both psychological and social domains weeks after treatment. Therefore, clinicians must be aware of and manage the residual psychosocial issues that can accompany the post-treatment period of maxillofacial injury.

안와 골절의 치료 전략 (Treatment strategy for orbital fractures)

  • Orbital fractures have a significant portion in facial bone trauma. The important thing in treatment of orbital fractures is variable depending on the patient. Reconstruction of orbital wall demands an understanding of the anatomy and function of the orbit, including the orbital tissues, and the approacheds, materials, and methods available.
