모-태아상호작용 신념과 모-태아상호작용 (Maternal-Fetal Interaction Belief and Maternal-Fetal Interaction)

  • Purposes: The purposes of this descriptive survey study were to describe levels of Maternal-Fetal Interaction Belief and Maternal-Fetal Interaction, and to define their correlation. Method: Data were collected from 273 pregnant women who visited one public health center and OB/GY clinic in Gangneung city. The instrument used for this study was a self-report questionnaire that included the Maternal-Fetal Interaction Belief scale(MFIBS) and maternal-fetal interaction. Results: The mean scores for maternal-fetal interaction belief and maternal-fetal interaction were $107.41{\pm}15.67$ and $31.75{\pm}5.92$ respectively. For maternal-fetal interaction belief, there were significant differences according to education, religion, income, feeding plan, marriage satisfaction, family support, and husband's love. For maternal-fetal interaction, there were significant differences according to mother's age, period of pregnancy, marriage satisfaction, family support, husband's love. There was a correlation between maternal-fetal interaction belief and maternal-fetal interaction. Conclusion: Maternal-fetal interaction belief is related to increase in maternal-fetal interaction and fetal development. It is essential to develop a maternal-fetal interaction program that includes maternal-fetal interaction belief.

  • Purpose: The purposes of this descriptive survey study were to describe Maternal-Fetal Interaction aspect according to period of pregnancy. Method: Data were collected from 273 pregnant women who visited one public health center and OBGY clinic in Gangneung city. The instrument used for this study was a self-report questionnaire. Results: During maternal-fetal interaction, there were significant differences in place, feeling, touch type, voice tone, sensitivity according to period of pregnancy. Conclusion: Maternal-fetal interaction aspect according to period of pregnancy is different. Thus, it is essential to develop maternal-fetal interaction program that includes maternal-fetal interaction aspect according to period of pregnancy.

  • Pregnancy is a task of creation in which a women mobilizes her self and the resources available to her in the generation of a new person. Through the pregnancy, a mother has formed the new human relationship with a fetus. Maternal-fetal relationship is considered one of mechanism making the relationship of mother and child. It is important to well-being of mother and fetus, too. The earliest interaction between a mother and her child is during prenatal period. Maternal-fetal dyad is unique and perceived interactions with the fetus make the pregnancy real for the mother. Maternal behavior is "instinctive" and is formed in early childhood by copy of the mother. But, Rubin argues that this behavior is an open intellectual system rather than a prepackaged bundle of traits. There is openness to new learning and a high value placed on knowing which occurs with silent organization in thought. Thus, nurses and other health professionals provide prenatal care that optimally is part of the environment in which the maternal-fetal dyad develops. Thus it is appropriate for nurses to increases their understanding of the dyad and to explore ways to enhance its development. This study focusses on the interaction ability and response of fetus, and the maternal-fetal interaction. The research of fetal responses that involve physiological changes and motor movement have been shown to coccur to both external sensory stimuli and to maternal emotional states. The fetus does also have sensory capacity to be aware of some maternal behaviors, and the motor ability to respond in a way the mother can notice. Thus, very rudimentary interactions appear to be possible. Maternal awareness of fetal activity was supported by several studies. More interesting to the present study are description of maternal-fetal interaction and the finding that there appear to be levels of sensitivity to the fetus involved in maternal-fetal interactions. First, recognition comes that the fetus is separate from the maternal self. Next, the fetus engages in. Lastly, the parent may describe active interaction with the fetus, believing that mother and fetus are communicating on a meaningful level. Several interventions, developed to promote more active interaction between mother and fetus, have been reviewed. In general, the parents were taught to stimulate the fetus and to notice the fetus' responses. This type of intervention might increase the mother's sensitivity to her unborn baby, and she may have a head start toward learning how to res pond sensitivity to the newborn infant. Research In the area of maternal-fetal interaction is scarce. Sensitive behavior is construed as an appropriate and timely response to a signal of need from another person, but no such signal of need can be claimed regarding the fetus. The highest level of maternal-fetal interaction, therefore, might be based more on maternal representations of the imagined fetus than on factual evidence of fetal participation.

모-태아상호작용신념 도구 개발 (Development of the Maternal-Fetal Interaction Belief Scale)

  • Purpose: The purpose of this study was to develop a Maternal-Fetal Interaction Belief Scale (MFIBS). Method: The research design was a methodological study. The items generated for this scale were drawn from a comprehensive literature review. Content validity was established for the MFIBS. The 26-item scale was developed to measure the construct of the MFIBS during pregnancy and tested on 186 pregnant women. Results: The final tool consist of 20 items which were sorted into 6 factors by factor analysis. The factors were identified as 'influence of pregnancy (5 items)', 'ability of fetus (3 items)', 'maternal-fetal interaction behavior (3 items)', 'practice of taegyo (3 items)', 'ability of infant (3 items)', and 'taemong (3 items)'. The six factors explained 62.37% of the total variance in the MFIBS. The Cronbach's alpha coefficient for internal consistency was .81 and the reliability of the subscales ranged from .60-.85 Guttman split-half coefficient was .66. Conclusions: This scale proves to be a effective, useful tool and suitable in Korean women for measuring the Maternal-Fetal Interaction Belief. Future studies are required for further refinement of the scale.

간호이론개발을 위한 개념 분석 : 어머니됨 (The Concept Analysis of Motherhood)

  • The characteristics of health behavior related pregnancy and childbirth have reflected on the cultural belief and value in the society. The efforts for women's health promotion through the current illumination of the traditional health care are the prompting assignment to be in nursing. The process of motherhood already progress before the motherhood actually. The functional state as the expectant mother can be the important predicting factor of the postpartum state, the quality of a married life. Motherhood was analyzed by Walker and Avant's method to clarify the concept 'to be a mother' using the various concepts like Koreans' Taekyo, transition to motherhood, maternal identity, maternal role attainment, maternal fetal attachment, and maternal fetal interaction. Upon the concept analysis, naturalness, responsibility, attachment, readiness, controllability were identified as the defining characteristics of motherhood. The antecedents of motherhood were consist of maternal affection, positive self esteem, pregnancy acceptance, fetus recognition and the consequences of motherhood were consist of positive maternal identity, maternal fetal attachment, confidence about the maternal role, the healthy mother and the healthy baby. The empirical referents of motherhood were consists of recognition of motherhood, expectation about motherhood, fetal recognition with ultrasonography and fetal movement, experience of unification between mother and fetus, expression of affection to the fetus, concern about fetal health, concern and practice about Taekyo, adaptation behavior about physical change and discomfort due to pregnancy. Therefore it is necessary to develop the instruction program of motherhood including the defining attributes identified in this study.

Effect of Taegyo-focused Prenatal Education on Maternal-fetal Attachment and Self-efficacy Related to Childbirth

  • Purpose. To examine the effect of Taegyo-focused prenatal classes on maternal-fetal attachment and self-efficacy related to childbirth. Methods. Over 4 weeks, 49 women, 20 to 36 weeks of gestation participated in a prenatal program led by the nurse who developed it. In addition to Lamaze content it included; understanding ability of fetus to respond, sharing motivation, purpose of pregnancy, and preconceptions of experiencing childbirth, training in maternal-fetal interaction, writing letters and making a declaration of love to unborn baby. Using a pre-experimental design, data were collected by self-report, before and after program, using Cranley's Maternal-Fetal Attachment Scale (1981), and Shin's(1997) Labor Self-Efficacy Measurement. Results and Conclusion. Paired t-test showed significant changes in scores of maternal-fetal attachment (t=6.91. p<.001) and self-efficacy related to childbirth (t=10.19, p<.001). Taegyo opens the possibility of integrating Western ideas with Korean traditional health behavior. Incorporation of Taegyo into existing prenatal classes is recommended.

태교관점 임부교실 참여 전후 임부 태아애착과 분만자신감의 변화 (Changes of Maternal-fetal Attachment and Self Efficacy for Delivery after the Taekyo-perspective Prenatal Class)

  • The purpose of this study was to evaluate the effects on maternal-fetal attachment and self efficacy for delivery using the Taekyo-oriented prenatal class. This class is for 2 hours/week for 4 weeks. The program covers the contents of fetal growth and development including their responding ability, the importance of the uterine environment, sharing the motive and purpose of pregnancy, sharing experiences about pregnancy, sharing of prejudices against delivery, training of maternal-fetal interaction, understanding delivery, relaxation breathing techniques, maternity exercises, writing letters or prayers to the baby, and declaration of loving the baby. This study took place from March 4th to June 15th, 2000, in a university hospital and community care center, and was done by with a pretest-posttest design, with 55 pregnant women who were within 32-36 weeks pregnant and who agreed to participate in this study. Data was measured twice by self-report by the Cranley's Maternal-fetal Attachment Scale(MFAS, 1981), and the Shin's(1997) Self Efficacy for Delivery Scale at the beginning and at the completion of the class. Data was analyzed by SAS. The study results were: 1. The score of maternal-fetal attachment was significantly increased after the Taekyo perspective prenatal class than before the class. (t=7.389, p=0.000) 2. The score of self efficacy for delivery was significantly increased after the Taekyo perspective prenatal class than before the class. (t=8.885, p=0.000) The above results proved that the present Taekyo perspective prenatal education program was effective in increasing maternal-infant attachment and self efficacy for delivery. Therefore, it is concluded that the existing prenatal class should include Taekyo perspective elements. However, further study is needed to compare the effects with preexisted prenatal class.

Factors influencing happiness and depression in high-risk pregnant women: a cross-sectional study using the ecological systems approach

  • Purpose: The increasing number of high-risk pregnancies has led to a greater emphasis on psychological well-being in nursing care. However, reducing depression does not automatically equate to increasing happiness. This study aimed to systematically examine the factors influencing happiness and depression among high-risk pregnant women in South Korea. Methods: This correlational, cross-sectional study was based on the ecological systems theory. In total, 152 high-risk pregnant women completed a self-report survey questionnaire available online or offline. Data were analyzed using hierarchical regression analysis. Results: The first model (individual system) identified pregnancy stress and mindfulness as significant factors influencing both happiness and depression. The second model (microsystem) identified medical status at the time of the survey, maternal-fetal interaction, marital intimacy, and social support as additional significant factors influencing either happiness or depression. In the third model (mesosystem), maternal-fetal interaction and paternal-fetal attachment were no longer identified as significant factors. Although the fourth model (exosystem) did not identify community service as a significant factor, individual (pregnancy stress, mindfulness) and microsystem (marital intimacy) factors were found to influence happiness and depression. Medical status at the time of survey and social support were additional factors that influenced happiness, but not depression. These factors explained 51.2% and 55.5% of the variance in happiness and depression, respectively, among high-risk pregnant women. Conclusion: Different factors at the individual and microsystem levels affected happiness and depression among high-risk pregnant women. Hence, efforts to reduce depression among these women should be accompanied by efforts to actively promote happiness.

임부의 모성 정체성 측정을 위한 척도개발 (Development of a Maternal Identity Scale for Pregnant Women)

  • This study was done to develop a Maternal Identity Scale for Pregnant Women and to test the validity and reliability of the scale. A convenience sample of 161 pregnant women were asked to complete the MISP questionnaire which consisted of 45 item, this was done from December 20, 1995 to January 15, 1996. The research procedure were as follows. The first step was to identify a conceptual definition of maternal identity using Robin(1984)'s maternal identity and maternal experience during pregnancy. The second step was to operationalize the maternal identity, that is, perception of image possible of selves as mother, maternal role play by imagination, and the experiences of various emotional responses which are embedded in the mother-fetus dyad. The third step was item development which resulted in 45 items as appropriate measurement of maternal identity are except for the perception of image possible of selves as mother. The result findings were as follows : 1) Four factors for MISP (finally 40 items) were extracted through the principal component analysis and varimax rotation, and these contributed 49.3% of the variance in the total score. All 40 items in the scale loaded above .43 on one of 4 factors. 2) Each factor was named : factor 1 was named maternal role imagery and has 10 items, factor 2 was named happiness and has 11 items, factor 3 was named maternal fetal interaction and has 10 items, and the last factor 4 was named negative emotion and has 9 items. 3) Cronbach's -alpha coefficient for internal consistsncy was .92 for the total 40 items and .89, .90, .86, .78 for the four subscales in that order. Recommendations are suggested below : 1) The developed MISP be used to assess maternal readiness in pregnancy. 2) Replication study be done to test validity and relaibility. 3) For the overall measure of Maternal Identity in Pregnancy, scale for the perception of image possible of selves as mother, and cognitive domain be reorganized for the maternal identity in pregnancy. 4) It is necessary to identify variables that influences maternal pregnancy. 5) It Is necessary to identify that maternal identity in pregnancy is a reliable index of motherhood, to do correlation studies on maternal identity and major maternal variables in maternal transition period, to reoperationalize the maternal identity in postpartum, and finally to designate a longitudinal study of the maternal identity changes or stabilities.

태담과 촉각 자극의 모-태아 상호작용 증진 프로그램이 모-태아 애착에 미치는 효과 (The Effect of Mother-fetus Interaction Promotion Program of Talking and Tactile Stimulation on Maternal-fetal Attachment)

  • The purpose of this study is to investigate the effect on mother-fetus attachment through mother-fetus interaction promotion program of talking and tactual stimulation aimed at enhancing sensitivity of primipara Non-equivalent control group posttest design was used. Fifty primiparas (26 mothers for intervention group and 24 mothers for control group) were recruited from three general hospital and an OBGY clinic located in Gang Nung city. Data was collected from January 30th to December 20th in 2001. For the intervention group, programed education which focused on mother-fetus interaction promotion of talking and tactile stimulation in the 1st trimester. Telephone counselling was provided with interval of two weeks. For two groups, home visiting for data collection of mother-fetus attachment was conducted at 36 to 38 weeks of gestational age. The data were analyzed using chi-square test and Fisher' Exact test to test the equivalence of two groups, and the effect of intervention program was determined with t- test. The result was as follow: The significant difference was found in mother-fetus attachment between two groups(t= 2.772, P= 0.0079). It indicated that intervention program was effective in improving mother-fetus attachment. In conclusion, this study has shown that the applied nursing intervention aimed at enhancing sensitivity of primiparas to fetus's movement promoted mother-fetus attachment. Therefore, this study suggests that this nursing intervention to increase maternal sensitivity to the fetus's movement should be broadly applied to primiparas, which can be beneficial for formation of mother-infant relationship, and for promotion of the social, affective, and cognitive developments of their children.
