교각운동시 엉덩관절 초기 굽힘 각도에 따른 체간 및 하지의 근활성도 분석 (Analysis of the Muscle Activity of the Trunk and the Lower Extremities in Relation to the Initial Bending Angle of the Hip Joint During Bridge Exercise)

  • Background: The present study was conducted with 30 adult males in order to examine the muscle activity of the trunk and the lower extremities at diverse initial bending angles of the hip joint during bridge exercise on a stable surfaces and on an unstable surface that is widely performed for stabilization. Methods: The initial angles of the hip joint used were $0^{\circ}$, $45^{\circ}$ and $90^{\circ}$ and the subjects were divided into a matt experimental group and a balance training group. Results: In maximum values of muscle activity at different exercise methods and angles, the matt experimental group showed statistically significant differences in the muscle activity values of the rectus abdominis muscle, the erector spinae muscle, the rectus femoris muscle and the peroneus muscles between different angles while the balance training group showed significant differences only in the muscle activity values of the erector spinae muscle between different initial angles of the hip joint. The matt experimental group showed significant differences in muscle activity between initial angles $0^{\circ}$ and $90^{\circ}$, between $45^{\circ}$ and $90^{\circ}$ in the rectus abdominis muscle, between $0^{\circ}$ and $90^{\circ}$ in the erector spinae muscle, between $45^{\circ}$ and $90^{\circ}$ in the rectus femoris muscle and between $0^{\circ}$ and $90^{\circ}$ in the peroneus muscles while the balance training group showed significant differences between $0^{\circ}$ and $90^{\circ}$ in the erector spinae muscle. Conclusions: Therefore, it is thought that bridge exercises should be applied to patients using diverse methods.

  • Objective: The purpose of this study was to investigate relations and effectiveness about mountain climbling exercise with different level of support surfaces by analyzing heart rate and EMG data. A total of 10 male college students with no musculoskeltal disorder were recruited for this study. Method: The biomechanical analysis was performed using heart rate monitor (Polar V800, Polar Electro Oy, Finland), step-box, exercise mat, and EMG device (QEMG8, Laxtha Inc. Korea, sampling frequency = 1,024 Hz, gain = 1,000, input impedance > 1012 Ω, CMRR > 100 dB). In this research, step-box were used to create different surface levels on the upper body (flat surface, 10% of subject's height, 20% of subject's height, and 30% of subject's hight). Based on these different conditions, data was collected by performing mountain climbing exercise during 30 seconds. Subjects were given 5 minutes of break to prevent muscular fatigue after each exercise. For each dependent variable, a one-way analysis of variance with repeated measures was conducted to find significant differences and Bonferroni post-hoc test was performed. Results: The results of this study showed that exercise intensity was reduced statistically as increased surface level on the upper body. Muscle activity of the upper rectus abdominis and biceps femoris for 30% of surface level was significantly higher than the corresponding values for flat surface. However, the opposite was found in the rectus femoris. In general, muscle activity of the lower rectus abdominis, erector spinae, external oblique abdominis, and gluteus maximus increased when surface level increased, but the differences were not significant. Conclusion: As a result, the increase in surface level of the body would change muscle activity of the upper body, indicating that different surface level of the upper body may cause significant effect on particular muscles to be more active during mountain climbing exercise. Based on results of this study, it is suggested to set up an appropriate surface level to target particular muscle to expect an effective training. It is also important to set adequate surface levels to create an effective training condition for preventing exercise injuries.

하지근력강화운동이 뇌성마비 아동의 균형과 보행에 미치는 영향 (The Effects of The Lower Extremity Muscle Strengthening Exercise on Walking and Balance of Children with Cerebral Palsy)

  • Purpose: The aim of this study was to ascertain the effects of the lower extremity muscle strengthening exercise on balance and ambulation of children with cerebral palsy. Methods: 10 subjects who participated in this research undertook the 12-week the lower extremity muscle strengthening exercise program, which consisted of a series of mat exercises and sling exercises. The statistical significances were examined by using Wilcoxon signed-rank test, a non-parametric test, for evaluating the improvement of balance and ambulation of the subjects. In order to evaluate the correlation among the variables, Pearson's correlation coefficients were also calculated. In all statistical analyses the significance level was selected as ${\alpha}$=0.05. Results: Berg balance scale(BBS) was significantly increased after the intervention(p<.05). Percentage weight bearing(PWB) was decreased after the intervention, but there was no significant difference. Time up and go test(TUG) value was significantly decreased(p<.05). Gait velocities was increased after the intervention, but there was no significance. Stride length, step lengths of the affected side and the sound side were significantly increased after the intervention(p<.05). In the correlation analyses of the measures before the intervention, TUG had significant negative correlation to BBS and gait velocity(p<.05). Stride length, step lengths of the sound side and the affected side had significant positive correlation between themselves(p<.01). In the correlation analyses of the measures after the intervention, TUG had significant negative correlation to BBS and gait velocity(p<.05). BBS revealed significant positive correlations to stride length, step lengths of the sound side and the affected side(p<.05). Stride length, step lengths of the sound side and the affected side had significant positive correlation between themselves(p<.01). Conclusion: Based upon the outcomes as above, it is likely that the muscle strength exercises have substantial effects on balance and ambulation of children with cerebral palsy. Thus various lower extremity muscle strengthening exercise programs are required to be studied and developed in order to contribute to functional improvements of children with cerebral palsy.

장요근의 신장운동과 근력운동이 만성요통환자의 요부 정적유연성과 통증강도에 미치는 영향 (The effects of the iliopsoas muscle stretching and strengthening exercise on the static flexibility and pain intensity for the chronic low back pain patients)

  • Background: The purpose of this study was to investigate the effect of stretching and strengthening exercise on the static flexibility and pain intensity for the iliopsoas muscle, which is one of the main reasons for the chronic low back pain. Methods: The subjects of this study were 15 male adult patients with showed 6 score or higher in the visual analogue scale(VAS) and complained of low back pain over three months who visited department of the physical therapy, KIA motors Industrial Health Center, from October, 2008 through December, 2008. Fifteen subjects were trained stretching, mat exercises and sling exercises for iliopsoas muscle at 4-5 times a week for 4 weeks. I measured the changes on the extensibility of iliopsoas muscle, static flexibility of low back and VAS between pre- and post exercise treatment. Date were analyzed using the Wilcoxon's signed rank test considering the size of the samples. Results: 1. The angle of the hip joint that showed the extensibility of iliopsoas muscle was relieved, which was significant statistically (p<.05). 2. The static flexibility was statistically significantly improved in the trunk flexion test, trunk extension test and Schober-Test (p<.05). 3. The VAS showed decrease, which is significant statistically (p<.05). Conclusion: It is believed that the exercise treatment of iliopsoas muscle has the significant effects on the improvement of static flexibility and decrease of pain intensity for the chronic low back pain patients.

스팀업(Stim UP)매트 걷기운동 프로그램이 허약노인의 균형능력과 보행기능 및 관절 가동범위에 미치는 효과 (Effects of a Stim up Mat Walking Exercise Program on Balance, Gait Function and Joint Motion Range of the Frail Elderly)

  • Purpose: This study was performed to evaluate effects of a stim-up matt walking exercise program on balance and gait of the frail elderly. Methods: A total of 37 elderly people recruited from S city were randomly assigned to the experimental group (n=22) and control group (n=15). The stim-up matt walking exercise program was offered twice a week for 8 weeks. Data were analyzed by SPSS 21.0. Results: The dynamic balance ability Timed Up and Go test of the experimental group was significantly faster than that of the control group (t=21.72, p<.001). The static balance ability open-eye standing test (t=44.15, p<.001) and close-eye standing test (t=9.01, p=.005) also showed increase in effects of the experimental group. In the walking ability, gait cycle (t=2.48, p=.018), cadence (t=-2.21, p=.034) and gait speed (t=-2.78, p=.009), positive effects were on. However, no statistically significant differences were found in stride length and double support. At the ankle joint range left ankle plantar flexion (t=3.92, p<.001) and left ankle dorsal flexion (t=4.51, p<.001) were higher in the experimental group than in the control group, and also right ankle plantar flexion (t=2.79, p=.008) and right ankle dorsal flexion (t=2.92, p=.006) increased in the experimental group. Conclusion: The significance of this study is that the stim-up matt walking exercise program for the frail elderly proves to be useful for improving balance and walking.

Effects of Lumbar Stabilization Exercises Using a Pilates Reformer on Pain, Function, and Quality of Life in Patients with Chronic Low Back Pain

  • PURPOSE: This study examined the effects of lumbar stabilization exercise using a Pilates reformer on the visual analog scale (VAS), Oswestry disability index (ODI), and quality of life of patients with chronic low back pain. METHODS: The study evaluated individuals aged 30 years and above who had been suffering from persistent back pain exceeding three months. The participants were allocated into two groups: an experimental cohort of 15 individuals utilizing a reformer and a control group of 15 individuals engaging in lumbar stabilization exercises on a mat without a reformer for eight weeks. Each exercise session encompassed a 10-minute warm-up, a 30-minute main exercise, and a 10-minute cool-down, totaling 50 minutes. The lumbar stabilization group using the reformer was also subjected to the same level of resistance provided by the springs. Assessments were conducted before and after the intervention using the VAS, ODI, and quality of life evaluations. RESULTS: Significant disparity existed between the experimental group utilizing the reformer and the control group before and after the intervention in terms of the VAS, ODI change, and quality of life change (p < .05). Furthermore, a significant distinction was observed in the comparison between the experimental and control groups (p < .05). CONCLUSION: Both cohorts showed a decrease in pain, a decrease in the ODI, and an improvement in the quality of life. Furthermore, the experimental group exhibited superior outcomes to the control group regarding pain reduction, reduction in the ODI, and improved quality of life.

  • Purpose : The purpose of this study was to compare the effects of different supporting surfaces on trunk muscle activities during core stabilization exercises. Methods : A total of 20 healthy college students participated in this study for 6 weeks. In this study, we divided participants into a stable surface exercise group and an unstable surface exercise group. Each group performed core stabilization exercises of Curl-ups, Bridges, and Quadrupedal position. Core stabilization exercises were performed 3 times a week for 30 minutes during 6 weeks. The stable surface exercise group used a yoga mat while the unstable surface exercise group used AERO step. This study was designed using pre-test and post-test measurements. We used Surface Electromyograpy (sEMG) to measure for the rectus abdominis, external abdominal oblique, and multifidus muscle of trunk muscle activities. Data was processed using a paired sample t-test on SPSS 18.0. Results : For the stable surface exercise group there was a meaningful improvement in left rectus abdominis, left external abdominal oblique, and right multifidus (p<0.05). For the unstable surface exercise group, a meaningful improvement was seen in the left external abdominal oblique and right multifidus. Conclusion : From the experiment, we concluded that differences in surface can make various degrees of improvement in muscles activities, which suggests patients can choose a better option under their own conditions when planning to have a core stability exercise.

골다공증 노인 환자의 낙상 관련요인에 관한 연구 (Factors Related to Fall in Elderly Patients with Osteoporosis)

  • Purpose: The purpose of this study was to identify risk factors of fall and to examine its results among patients aged 60 and over diagnosed with osteoporosis. Methods: A retrospective descriptive survey was conducted with 91 patients admitted in a university hospital in a city. Data were collected using an interview method with a structured questionnaire, and were analyzed using descriptive statistics and multiple logistic regression. Results: In patients with osteoporosis, the intrinsic risk factors of falls were aging (OR = 3.742), gait disturbance (OR = 12.565), taking one or fewer medicine (OR = 7.873), and having two or more diseases (OR = 5.173). The extrinsic risk factors included the use of a bed (OR = 3.093), slippery floors (OR = 12.130), bathroom mat without anti-slip rubber backing (OR = 3.564), and presence of a night light on the passage from the bedroom to the bathroom (OR = 2.980). Conclusion: For the elderly aged over 70 who are most vulnerable to falls, screening tests such as bone mineral density (BMD) should be conducted in health examinations and the risk of fracture caused by osteoporosis should be communicated to the vulnerable elderly. Besides, development of new exercise programs combining weightbearing exercise is needed to prevent bone loss and increase functional activities.

불안정지지면 훈련에 따른 만성 뇌졸중 환자의 다리 근활성도와 동요속도의 상관성 연구 (Study on the Correlation between Muscle Activity of Lower Extremity and Sway Speed of Chronic Stroke Patients according to Unstable Surface Training)

  • Purpose : This research was conducted to see the correlation between sway speed and muscle activity for lower extremity of stroke patients through unstable surface training. Methods : A total of 60 patients were randomly divided unstable surface group (30 peoples) and stable surface group (30 people). Then they were asked to carry out the same exercise program for 6 weeks. The unstable surface group and stable surface group performed the exercise program on the balance mat and on the hard wood block. We checked the changes of sway speed and the changes in muscle activity for lower extremity. Results : The unstable surface group displayed significantly reduced sway speed, and improved muscle activity of lower extremity. There were significant correlation between change amount of muscle activity and sway velocity in Gastrocnemius, Biceps femoris during unstable surface training(r=.373, p<.05)(r=.369, p<.05). And there were not show significant differences during stable surface training. Conclusion : Judging from this, we can have knowledge that the correlation between increase of muscle activity and decrease of sway velocity for Gastrocnemius, Biceps femoris in the unstable surface training.

고유수용성신경근촉진법이 만성 뇌졸중환자의 하지기능에 미치는 효과 (The Effects of Proprioceptive Neuromuscular Facilitation on Lower Extremity Functions of Chronic Stroke Patients)

  • Purpose: The purpose of this study is to determine the effects of proprioceptive neuromuscular facilitation on the lower extremity function of chronic stroke patients. Methods: The participants consist of 26 chronic stroke patients. They were randomly assigned to either an experimental group (n=13) or a control group (n=13) and engaged in exercise three times per week for eight weeks. The experimental group engaged in proprioceptive neuromuscular facilitation exercises and the control group engaged in general mat exercises. To measure lower extremity strength, the sit to stand test and the static balance test (standing on one leg) were used. To measure the stability index (SI) and weight distribution index (WDI), a Tetrax Portable Multiple System (Tetrax Ltd, Israel) was used in addition to a static balance test (standing on one leg). Results: : Lower extremity strength and static balance showed a significant improvement in the experimental group (p<.05). The SI and WDI were significantly improved in the experimental group (p<.05) for tests of standing with their eyes open, standing with their eyes closed, and standing on a sponge with their eyes open. Conclusion: Proprioceptive neuromuscular facilitation exercises have confirmed that lower extremity function improves after exercise interventions in chronic stroke patients.
