전방순환 뇌경색의 중재적 재관류 치료 이후 출혈 변환으로 의식 및 인지장애를 동반한 편마비환자의 한방치험 1례 (The Effect of Korean Medicine Treatment for Hemorrhagic Transformation after Interventional Reperfusion Therapy of an Anterior Circulation Infarction in a Patient with Hemiplegia and Conscious and Cognitive Disorders: A Case Report)

  • Objectives: The study investigated the effect of Korean medicine treatment on a hemiplegic patient with conscious and cognitive disorders due to hemorrhagic transformation after interventional reperfusion therapy of anterior circulation infarction. Case presentation: The patient was treated with acupuncture, moxibustion, cupping, and herbal medicine in combination with Western medicine and physical therapy. The effects on clinical symptoms were evaluated using the Manual Muscle Test (MMT), Glasgow Coma Scale (GCS), Korean Mini-Mental State Examination (K-MMSE), Korean Nursing Delirium Screening Scale (Korean Nu-DESC), and Modified Bathel Index (MBI). After the treatment, the MMT grade increased from Gr.0-1 to Gr.0-3, the GCS score increased from 10 to 15, the K-MMSE score increased from 8 to 15, the Korean Nu-DESC score decreased from 3 to 1, night delirium disappeared, and the MBI score increased from 13 to 26. Conclusions: Complex Korean medicine treatments were effective for improving the clinical symptoms of hemorrhagic transformation after interventional reperfusion therapy for anterior circulation infarction in a patient with hemiplegia and conscious and cognitive disorders. However, further studies are needed.

  • Objectives: The aim of this study is to describe the effects of traditional Korean medicine treatment on a patient with vascular dementia caused by an intracerebral hemorrhage. Methods: The patient was treated with herbal medicine (modified Guibi-tang) and acupuncture. The effects of these treatments were evaluated using the Korean Mini Mental Status Examination (K-MMSE-2), Cognitive Dementia Rating (CDR), Manual Muscle Test (MMT), Korean version of Modified Barthel Index (K-MBI), and Videofluoroscopic Swallow Study (VFSS). Results: After treatment, the K-MMSE-2 score, CDR score, MMT, K-MBI, and VFSS improved. Conclusion: The results suggest that traditional Korean medicine may be effective for the symptoms of vascular dementia caused by intracerebral hemorrhage.

  • Objectives: This case study examined the effects of treatment with traditional Korean medicine on a central post-stroke pain patient, who had impingement syndrome of the right shoulder. Methods: The patient received traditional Korean medical treatments, including acupuncture, warm-needling, moxibustion, and cupping, throughout the treatment period, and extracorporeal shock-wave therapy and physical therapy for part of the treatment period. The effects of treatments were measured with a manual muscle test (MMT), numerical rating scale (NRS), range of motion (ROM), and gait staging index (self-invented). Results: The clinical symptoms of the patient were improved considerably after the complete treatment. Conclusions: Traditional Korean medicine may be effective for treatment of central post-stroke pain in patients with impingement syndrome.

  • Objective: This study is a report on a case of a Guillain-Barre syndrome patient with quadriplegia and paresthesia, whose condition was improved by Korean medicine treatment. Methods: A 71-year-old woman diagnosed with Guillain-Barre Syndrome was treated by acupuncture, electroacupuncture, Sambi-tang-gami, cupping, moxibustion, and rehabilitation. Clinical symptoms were measured with the Manual Muscle Test (MMT), the Numeric Rating Scale (NRS), the Modified Barthel Index (MBI), and the Functional Independence Measurement (FIM). Result: After treatment, clinical symptoms showed improvement. Conclusion: Korean medicine treatment could be effective in the treatment of Guillain-Barre syndrome patients with quadriplegia and paresthesia.

  • Objectives We report 1 treatment case with Onbaek-won which is based on Sasang constitutional medicine for specific symptoms that leukoaraiosis Soeumin patient has. Methods We treated leukoaraiosis Soeumin patient suffering from specific symptoms. We prescribed Onbaek-won for her physical symptoms. The improvement of her symptoms were evaluated by Manual Muscle Test(MMT) and Visual Analogue Scale(VAS). Results After the Onbaek-won was given, the patient's most of symptoms were improved. And there was not any adverse effect. Conclusions This study shows that using Onbaek-won can be effective treatment for leukoaraiosis Soeumin patient's specific symptoms.

  • 본 증례보고는 GBS를 진단받고 IVIg치료 후 호중구 감소증이 동반된 환자를 위증(痿證)으로 변증하여 한약, 침구치료, 물리치료를 통해 환자의 증상 호전을 보였기에 보고하는 바이다.

일산화탄소 중독 지연성 뇌손상 소양인 환자 치험 1례 (Case Study of Soyangin Patient with Delayed Encephalopathy after Carbon Monoxide Intoxication)

  • Objective This case report is to show the effectiveness of Sasang Constitutional Medicine treatment on the patient with delayed encephalopathy after acute carbon monoxide intoxication. Methods A 40-year-old female patient diagnosed with delayed encephalopathy after carbon monoxide poisoning 2 weeks ago before visiting our hospital. The patient suffered gait disturbance and cognitive disorder. She was identified as Soyangin and treated with Sasang constitutional Herbal Medicine(Yangkyuksanwha-tang and Hyungbangjihwang-tang). Improvement of symptoms was evaluated through Korean Version of Mini Mental Status Examination score(MMSE-K), Modified Barthel Index(MBI), Manual Muscle Testing(MMT), Range of Motion(ROM) and Hand Function Test. Results After 7 weeks of treatment, total score of MMSE-K increased from 0 to 26, the patient could walk by herself and had no trouble carrying out her daily life. Conclusions This case showed the effectiveness of Sasang constitutional medicine treatment for delayed encephalopathy after acute carbon monoxide intoxication.

  • ■ Objectives The goal of this study is to find the correlation between the motor function and gait pattern of stroke patients. ■ Methods We measured Manual muscle test(MMT), Motricity index(MI) and Spatiotemporal gait parameters of admitted hemiplegic patients with stroke. The gait parameters were measured using a Treadmill gait system. ■ Results There is a significant correlation between motor function and spatiotemporal parameters such as step length, stride length, step time, stride time, total double support or cadence, in stroke patients. ■ Conclusion The better motor function of stroke was, the more gait improved.

A comparison of ankle function between adults with and without Down syndrome

  • Objective: The purpose of this study was to compare ankle function between adults with and without Down syndrome (DS). Design: Cross-sectional study. Methods: Ten adults with DS and 18 without participated in this study and underwent manual muscle test (MMT), range of motion (ROM) assessment, star excursion balance test (SEBT), and functional movement screen (FMS). The tests were demonstrated to increase their accuracy and the actual measurements were assessed after one or two demonstrations. To minimize the standby time and fatigue, the travelled distance and measuring order were adjusted. To remove the influence of shoes on the measurements, the shoes were taken off and only socks were worn. Results: Dorsal and plantar flexion MMTs of both ankles were significantly weaker and plantar flexion ROM of both ankles were significantly lower in adults with DS compared with those without (p<0.05). However, dorsal flexion ROM of both ankles were not significantly different between them. There were significant differences in distances measured in all the directions (anterior, anterolateral, lateral, posterolateral, posterior, posteromedial, medial, and anteromedial directions) of SEBT (p<0.05). Significant differences were also demonstrated in the scores of hurdle step, inline lunge, shoulder mobility, and rotary stability among the seven items of FMS (p<0.05). Conclusions: To enhance the dynamic stability of adults with DS, it is necessary to improve ankle stability by strengthening the ankle dorsal and plantar flexors.

Infusion Methods for Continuous Interscalene Brachial Plexus Block for Postoperative Pain Control after Arthroscopic Rotator Cuff Repair

  • Background: Infusion methods during regional analgesia using perineural catheters may influence the quality of postoperative analgesia. This study was conducted to compare the effects of combined or bolus-only infusion of 0.2% ropivacaine on the postoperative analgesia in interscalene brachial plexus block (ISBPB) with perineural catheterization. Methods: Patients scheduled for arthroscopic rotator cuff repair were divided into two groups, one that would receive a combined infusion (group C, n = 32), and one that would receive intermittent infusion (group I, n = 32). A perineural catheter was inserted into the interscalene brachial plexus (ISBP) using ultrasound (US) and nerve stimulation, and 10 ml of 0.2% ropivacaine was administered. After the operation, group C received a continuous infusion of 4 ml/h, and a 4 ml bolus with a lockout interval of 60 min. Group I received only a 4 ml bolus, and the lockout interval was 30 min. Postoperative pain by the numeric rating scale (NRS) and the forearm muscle tone by the manual muscle test (MMT) were checked and evaluated at the following timepoints: preoperative, and postoperative 1, 4, 12, 24, 36, and 48 h. Supplemental opioid requirements, total consumed dose of local anesthetic, and adverse effects were compared between the two groups. Results: Sixty-four patients completed the study and the postoperative values such as operation time, time to discharge, and operation site were comparable. There were no differences in NRS scores and supplemental opioid requirements between the two groups. The MMT scores of group I at 4 and 12 h after surgery were significantly higher than those of group C (P < 0.05). The total consumed dose of local anesthetic was significantly lower in group I than in group C (P < 0.05). The adverse effects were not different between the groups. Conclusions: The bolus-only administration of 0.2% ropivacaine provided a similar analgesic effect with a lower total volume of local anesthetic and decreased motor weakness compared to combined infusion. Therefore, bolus-only administration is an effective postoperative analgesic method in ISBPB with perineural catheterization after rotator cuff repair.
