  • Objectives: This study aimed to determine the effects on musculoskeletal symptoms of both social demographic features and detailed characteristics of each task category of care workers who offer visiting home-help services. And to establish the measures that can reduce musculoskeletal symptoms among care workers. Methods: This study was conducted among 192 care workers from welfare centers C and K located in city P. After participants completed the task burden checklist regarding the scale of musculoskeletal symptoms and the details of their duties, the data collected were analyzed using the SPSS 21.0 program. Results: According to the degree of observable musculoskeletal symptoms in care workers, the highest figures were observed for back and shoulder pain. Based on the results of assessing the effect of detailed task category characteristics on musculoskeletal symptoms, "helping patients eat, helping patients move, helping patients bath, and changing body positions" were found to have an effect from the physical care category, "providing physical therapy assistance, treating bedsores" from the health care category, and "cleaning and doing laundry" from the facility management category. Conclusions: Due to the high proportion of patients requiring burdensome physical labor from care workers such as moving patients who have trouble doing so on their own, helping them change positions, and so on, it is highly likely that pain will occur in the low back, which carries most of the physical weight. So, education on human epidemiological positions that can reduce overload on areas prone to pain such as the low back and shoulders is essential. Proper equipment and personnel support must be provided for dangerous tasks. Further, multidimensional social support is required consistently.

  • Objectives: The purpose of this study was to identify the needs and improvement measures of the risk assessment system through a survey of asbestos abatement companies. Methods: This study prepared a questionnaire that includes improvement measures for the risk assessment system(six questions) and the necessity of a risk assessment system for asbestos abatement(one question). The questionnaire was sent to 2,170 asbestos abatement companies and returned by 83 companies(return rate = 3.8%). We conducted frequency analysis, ANOVA, and Chi-squared testing at the 5% significance level. Results: This study analyzed the survey results and identified six main opinions on the risk assessment system and improvement measures. First, giving an advantage to companies with a high grade(S and A grades) in public bidding and/or qualification screening showed a similar preference(agree: 50.6%, disagree: 49.4%). Second, 57.6% of the respondents wanted to allow air showers along with water showers for low risk asbestos work. However, 23.2% of the respondents asserted that only a water shower should be allowed since there is no scientific evidence supporting the removal of asbestos by air shower. Third, in order to prevent missing the enrollment of workers, simply submitting a change report should be allowed when any worker is changed (40.0%). Fourth, 43% of the respondents answered that they did not know how to calculate the proper number of negative pressure units. The reasons given were a lack of guidelines or standards(38.9%), inconvenience (36.1%), and absence of education(25%). Fifth, the respondents who were favorable toward the necessity of a risk assessment system gave higher scores compared to unfavorable respondents on the necessity of the management of workers' work history(favorable respondent = 3.96 out of 5, unfavorable respondent = 2.68) and the necessity of professional training for workers(favorable respondent = 3.71, unfavorable respondent = 2.56). Finally, the respondents favorable toward a risk assessment system showed higher scores on the awareness of calculating the proper number of negative pressure units(4.79) compared to unfavorable respondents(3.3). Conclusions: The opinions of asbestos abatement companies identified through the survey in this study can be usefully utilized as fundamental information to improve the risk assessment system.

  • Objective: This study was performed to identify the related factors of change affecting the stage of change, on the assumption that there are various stages of change in the exercise behavior of workers. Method: The subjects of the study consisted of 138 workers of 10 workplaces located in Daejeon City, and data collected by using questionnaires were analyzed with SPSS/WIN 11.5 program. Results: The subjects were distributed in the stage of exercise behavior change as follows: precontemplation stage(10.9%), contemplation stage(22.5%), preparation stage(41.3%), action stage(8.7%), and maintenance stage(16.7%). Of the process of change depending on the stage of exercise behavior change, the consciousness raising(F=11.50, p=.00), dramatic relief(F=2.82, p=.02) showed significant difference in the cognitive process, and the counter conditioning(F=4.56, p=.00), reinforcement management(F=4.64, p=.00), self-liberation(F=9.46, p=.00), and stimulus control(F=13.28, p=.00) showed significant difference in the behavioral process. For decision-making depending on the stage of exercise behavior change, both the pros of decision-making(F=9.23, p=.00) and the cons of decision-making(F=2.45, p=.04) showed significant difference. Self-efficacy depending on the stage of exercise behavior change showed significant difference as F=11.50, p=.00. The related factors of change affecting the stage of exercise behavior change were the pros of decision-making, the cons of decision-making, and self-efficacy including 34.1% R-square. Conclusion: In order to change the exercise behavior of workers positively, they need to be stimulated to use the cognitive and behavioral process in the process of change properly, and to be induced to make a decision positively. And also exercise programs suitable to various characteristics of each worker as well as those of workplaces to enhance self-efficacy need to be applied after being devised. Through the further longitudinal research, it is necessary to analyze the various aspects, such as groups to select, to maintain, to stop or give up the exercise, and to avoid the change, and to study how the related factors of change affect diversity like the above.

  • This paper has the objectives to define the concept of 'Good Care' which is the service goal we are aiming essentially for the improvement of long-term care service quality, to find out the components for 'Good Care', and to explore the conditions that create a good care. In addition, we tried to find the answer about what is the best way to measure the service quality. For this, I referred the advanced researches which explored the fundamental properties of care and tried to find the answer from the accumulated wisdom of service field through the 5-year long term care service experience. As a result of research, the good care can be defined as helping someone to be able to maintain his own life as maximum as possible with the goal to assure total quality life. The most important condition for good care is making 'a good care relationship'. Without damaging the relationship between care provider and care receiver, the individualized service focusing on the demand of care receiver based on mutual reliability, mutual respect and smooth communication should be provided. For the evaluation system, it is reasonable to set the standard according to the size of each institution for the core quality of facility service and establish the certification system of absolute standard to carry out the quantitative evaluation rather than the relative evaluation in the whole. For the part over the absolute certification standard, it is reasonable for each institution to characterize its own characteristics autonomously and carry out the qualitative evaluation for this. For the evaluation of home visit care service, it is recommended to contain the evaluation contents such as user satisfaction, satisfaction of care worker, how well the case management system of home care service center is operated etc.

  • Background: The poultry industry in Pakistan has flourished since the 1960s; however, there are scarce data regarding the impact of occupational exposure on the pulmonary health of farm workers in terms of years working in the industry. The objective of the present study was to assess the effect of poultry environment on the health of occupationally exposed poultry farmers in countries of warm climatic regions, such as Pakistan. This study will also show the effect of exposure to poultry facilities on the health of poultry farmers in the context of low-income countries with a relatively inadequate occupational exposure risk management. Materials and methods: The lung function capacity of 79 poultry workers was measured using a spirometer. Along with spirometry, a structured questionnaire was also administrated to obtain information about age, height, weight, smokers/nonsmokers, years of working experience, and pulmonary health of farm workers. The workers who were directly involved in the care and handling of birds in these intensive facilities were considered and divided into four groups based on their years of working experience: Group I (3-10 months), Group II (1-5 years), Group III (6-10 years), and Group IV (more than 11 years). The forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and the FEV1/FVC ratio were considered to identify lung function abnormalities. Statistical analysis was carried out using independent sample t test, Chi-square test, Pearson's correlation, and linear regression. Results: Based on the performed spirometry, 68 (86 %) of workers were found normal and healthy, whereas 11 (14 %) had a mild obstruction. Of the 11 workers with mild obstruction, the highest number with respect to the total was in Group IV (more than 11 years of working experience) followed by Group III and Group II. Most of the workers were found healthy, which seems to be because of the healthy survivor effect. For the independent sample t test, a significant difference was noticed between healthy and nonhealthy farmers, whereas Chi-square test showed a significant association with height, drugs, and working experience. Linear regression that was stratified by respiratory symptoms showed for workers with symptoms, regression models for all spirometric parameters (FVC, FEV1, and FEV1/FVC) have better predictive power or R square value than those of workers without symptoms. Conclusion: These findings suggest that lung function capacity was directly related to years of working experience. With increasing number of working years, symptoms of various respiratory problems enhanced in the poultry workers. It should be noted that most of the poultry workers were healthy and young, the rationale being that there is a high turnover rate in this profession. The mobility in this job and our finding of 86% of the healthy workers in the present study also proposed healthy worker survivor effect.

  • The purpose of this study was conducted to investigate the perception and attitude toward physical restraints among elderly care workers. The subjects were 153 care workers who are working at elderly care facilities within the regions. Data were collected by structured questionnaires. Collected data were analyzed using descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test and Pearson Correlation Coefficient. The mean score of the perception for physical restraints was 2.83 out of 5, and the mean score of the attitude for the physical restraints was 3.56 out of 5. There was a statistically significant positive correlation between the perception and attitude about physical restraints (r=.31, p<.001). This research has confirmed that it is necessary for care workers to have education about low score items of recognition and attitude about physical restraints. Therefore, we need to develop proper and efficient educational program about use method for physical restraints, and need to develop guideline for adopting physical restraints. Also, we should make efforts and attentions to reduce or abolish physical restraints in order to protect the elderly human rights.
