Adjuvant Therapy Efficacy of Herbal Medicine Zeo Lyung Tang (Zhu Ling Decoction) for Primary Glomerulonephritis: Systematic Review and Meta-Analysis (일차성 사구체신염에 대한 저령탕 병용요법의 효능 : 체계적 문헌고찰 및 메타 분석)

  • Objectives: The aim of this study was to systematically evaluate the clinical therapeutic effects and safety of the Zeo lyung tang (ZLT) on primary glomerulonephritis (PGN). Methods: The MEDLINE, EMBASE, PubMed, CENTRAL, CNKI, RISS, NDSL, KISS, and OASIS databases were searched for randomized controlled trials (RCTs) testing the effects of ZLT on PGN. The Cochrane collaboration bias risk assessment scale was used to evaluate the methodological quality of the included studies. RevMan 5.3 software was used for data analysis. Results: Ten RCTs involving 781 patients were included in the review. Compared with conventional Western medicine (WM) therapy alone, a combination treatment of ZLT and WM improved the total effective rate (RR=1.24; 95%CI [1.16, 1.33]; p<0.00001), reduce the blood urea nitrogen (BUN; MD=-1.05; 95%CI [-1.32, -0.78]; p<0.00001) and the 24-hour urinary protein (MD=-0.38; 95%CI [-0.46, -0.29]; p<0.00001). Conclusions: The combination of ZLT with WM has therapeutic effects on PGN, and it has advantages over WM treatment alone in reducing BUN and 24-hour urinary protein. However, due to the low quality of the included studies and the small sample sizes, additional research is needed in this area.

The Effect of Increased Dilution Volume and Prolonged Infusion Time of Vancomycin on Incidence of Adverse Reactions through Peripheral Venous Cannulae (말초정맥을 통한 반코마이신희석과 주입시간연장이 부작용발생에 미치는 영향)

  • The purpose of the study was to explore the effect of 2 hour infusion of vancomycin(1g) in 200ml of isotonic saline every 12 hour on the frequency of "red man syndrome", phlebitis and length of peripheral catheter placement of infected patients, in order to provide safe infusion method for reducing vancomycinin-duced RMS and phlebitis. The subjects of the study consisted of 16 hospitalized patients; 3 oncology and gastro-intestinal patients, 1 neurological patient, 6 thoracic surgical patients and 6 orthopedic patients, who had received vancomycin from July to October in 1999 at S-hospital. The dependent variables were the incidence of RMS, phlebitis and the length of peripheral catheter placement. The incidence of RMS was checked by an inspector at the first night whenever the infusion method of vancomycin was changed. RMS was observed every 15 minutes during an hour for symptoms of RMS such as itching, erythema, chest pain and systolic blood pressure. Incidence of phlebitis was assessed by inspector twice a day from the insertion of peripheral catheter to the removal of the catheter. The data were analyzed by percentage, mean, $X^2$-test, t-test, repeated ANOVA, and logistic regression analysis using the SPSSWIN program. The results are summarized as follows; 1. No significant difference was identified in frequency of RMS between the experimental group and control group. 2. There was no significant difference in the change of systolic blood pressure as the time goes on between the experimental group and control group. 3. The incidence of phlebitis was significantly lower in the experimental group than in the control group. 4. The length of peripheral catheter placement was significantly longer in the experimental group than in the control group. 5. Other drugs administrated with vancomycin didn't influence the occurrence of phlebitis. However, the infusion method of vancomycin influenced the occurrence of phlebitis. The results suggest that 2 hour infusion of vancomycin(1g) in 200ml of isotonic saline every 12 hours may decrease the incidence of phlebitis and increase the length of peripheral catheter placement compared to 1 hour infusion of vancomycin(1g) in 100ml of isotonic saline every 12 hours. However, it does not reduce the incidence of RMS.

  • The main idea of this report, which deals with the process analysis in outfitting fields, is to improve working methods and further to secure higher productivity by removing unreasonable work factors in the workshop with the economical point of view. For this purpose, a series of techniques has been applied to some parts of chronically less productive outfitting works, expecting less man hour consumption and better working conditions to go with it. That is to make full use of process chart flow diagram(man & machine chart gang process chart). The text gives brief introductions of the charts respectively and the appendix shows actual practice directly applicable for the guidance of utilizing these charts. As the result of this, some of unreasonable work factors are removed from the total cycle and finally standard working method & time can be come out by reasonable working time and gradually improved working method.

  • This paper explores the possibility of improving R&D management system through researchers' satisfaction analysis. The relationship between job satisfaction and performance has traditionally been one of the engaging topics in organization psychology. However, the research results of the past showed the relatively low level of correlation between satisfaction and performance at the individual level. In contrast to these past research results, recent research results on this relationship at the organizational level revealed the higher correlation between these two factors, The present study extends this 'satisfaction and performance' hypothesis to the development and improvement of R&D management system. That is, we used the results of researchers' satisfaction analysis to devise appropriate criteria for the design and implementation of more effective R&D management systems. This paper seeks to show that we can make fruitful use of researchers' satisfaction analysis in order to choose a more effective one among alternative R&D management systems.

  • Taguchi parameter design is an approach to reducing performance variation of quality characteristic value in products and processes. Taguchi has used SN (Signal to Noise) ratio to achieve the appropriate set of operating conditions where variability around target is low in the Taguchi parameter design. This paper describes the prediction technique of vehicle performance using parameter analysis to reduce man hour and test development period as well as to achieve stable NVH performance. Design engineer could efficiently decide the design variable using parameter analysis database in early design stage. These improvements can reduce the time needed to develop better vehicles.

The Study on the Relationships Between Inhalation Volume and Exposure Duration and Biological Indices of Mercury among Workers Exposed to Mercury (수은 폭로 근로자들의 생물학적 폭로지표와 호흡량 및 폭로 기간과의 상관성에 관한 연구)

  • This study was conducted to evaluate the relationships between the environmental exposure and biological monitoring among workers exposed to metallic mercury We interviewed each workers to get the medical history including previous hazardous occupational history. We measured the respiration rate and tidal volume of each worker in order to calculate the 8-hour inhaled mercury of workers. And we wafted to evaluate the effect of exposure duration to mercury concentrations in blood and urine as biologic exposure indices of metallic mercury. The regression and correlation analysis were done to the relationships of 8-hour inhaled mercury and mercury in blood and urine. The results were as follows; 1. The subjects were 35 fluorescent lamp manufacturing workers. The mean age of subjects was .24.8 years old, and the mean work careers of workers was 1.19 years. 89% of the total was consisted man. 2. The correlation coefficients between 8-hour inhaled mercury and mercury in blood and urine were higher than that of only considered air mercury concentration. 3. The correlation coefficients of 8-hour inhaled mercury and mercury in blood and urine were above 0.9 in workers who had exposed to mercury more than 1 year 4. The R-square value and -value of regression analysis between the 8-hour inhaled mercury and mercury in blood and urine was also higher in workers who had exposed to mercury over 1 year than in workers who had less than 1 year working experience. The important results of this study were that relationships between the 8hr-inhaled mercury and mercury in blood and urine was very high than that with air mercury concentration only. And the results were very apparent when considering workers 1 year or more. Therefore we concluded that the work career and respiratory volume of each individuals should be considered in evaluation the, results of biological monitoring of workers exposed to metallic mercury.

  • In the field of nuclear power construction, management and the systematic construction is very necessary. For more efficiently managing, analysis of influence factors and productivity is indispensable now. On this research, we collected the data from the nuclear power plant which is located in Korea. We analyzed the productivity and influence factors. As a result, productivity was 0.75(㎡/man·hour).On one hand, the productivity should be personnel, weather and whether there is a night duty or not. On the other hand, work crew depends upon operators, formwork amount, the presence or absence of night duty.

  • For the design of welded bogie frame, the concept of multidisciplinary engineering activities, i.e. static, fatigue and dynamic analysis, has been applied, in which the sharing of design parameters related with each analysis and the collaboration of the working parts in charge should be fulfilled. However, in spite of these necessities, the multi-disciplinary engineering activities couldn't be performed in practice due to tack of the automation of the required analysis. In this paper, an automation of fatigue durability analysis of welded bogie frame according to UIC-Code was proposed by using the Model Center, which enables to integrate the several tools for the fatigue durability analysis, i.e. I-DEAS, ANSYS and BFAP, and to perform iterative analysis works in relation to the geometrical change of transom support bracket. Besides, the wrapping programs to control I/O-data and interfaces of these tools were developed. The developed automation technique brings not only significant decreasing man-hour required in the durability analysis, but also providing a platform of the multidisciplinary engineering activities.

  • The finite element modeling of a ship for hull structural analysis on the basis of new harmonized common structural rules (CSR-H) is to be extended to the cargo holds in fore and after body of a ship. Unlike the parallel middle-body where the external and internal features of hull are equal along to the longitudinal direction of a ship, in fore and after body, the external and internal features of hull vary linearly or even irregularly in forms of a surface or a curve along to the longitudinal direction of a ship. Thus, it needs lots of design man-hours for the modeling for structural analysis. In order to save man-hours in initial structural design phase of a ship, the specified 3D CAD system has been adopted in shipbuilding industry. Through the interface between CAD and CAE (rule scantling and direct strength assessment), design man-hour in initial design phase can be saved even under the environment of CSR-H.

Differenting Effects of Cerebral Hemispheric Lateralization and Gender by Power Spectral Analysis of Heart Rate Variability in Cerebrovascular disease (뇌졸중(腦卒中) 환자(患者)의 성별(性別)과 부위(部位)에 대한 Heart Rate Variability의 변화(變化))

  • Background and Purpose The increasing evidence for neurally mediated cardiac damage and sudden death has focused attention on the central autonomic control of cardiac function. Power spectral analysis of heart rate variability(HRV) can detect autonomic consequences of stroke. We performed power spectral analysis of heart rate variability from 24-hour holter recording to identify cerebral hemispheric lateralization and gender effect in cardiac autonomic control. Methods Data were obtained from 24-hour holter recordings in 30 consecutive patients with hemispheric brain infarction in the subacute phase. We analysed the time domain and frequency domain measures of HRV and ST segment by hemispheric lateralization and Gender. Results ST segment was depressed in women compared with man. There was no statistically significant differences between right and left hemisphere stroke patients in any standard index of HRV and ST segment changes. Conclusions These data are partial consistent with evidence from the recent literature that two cerebral hemispheres and gender have a differential influence on the nature and severity of cardiac dysfunction. We think the prospective and definite study was necessary.
