Diagnosis of Malignant Fibrous Histiocytoma(MFH) By Histologic Findings (악성 섬유성 조직구종의 조직학적 소견에 의한 진단)

  • Malignant fibrous histiocytoma was classified as fibrosarcoma, liposarcoma, rhabdomyosarcoma in times past and it was first introduced in 1963 to refer to a group of soft tissue tumors and reported in bone in 1972. It was postulated that the origin of tumor cell was derived from histiocytes that could assume the appearance and function of fibroblasts("facultative fibroblasts") at first. But, recently the immunohistochemical studies suggested origin from a primitive mesenchymal cell. The malignant fibrous histiocytoma were commonly misdiagnosed as other tumors such as myxoid liposarcoma, pleomorphic rhabdomyosarcoma, osteogenic sarcoma, leiomyosarcoma, malignant schwannoma, giant cell tumor, hemangiopericytoma, dermatofibrosarcoma protuberans due to the broad morphologic spectrum. The seventeen cases of the malignant fibrous histiocytomas diagnosed at Hanyang University Hospital since 1979 were reviwed, and the suggestions are as follows : 1. No fundamental histologic differences between the MFHs of soft tissues and bones. 2. The other tumors must be excluded in order to diagnose MFH due to wide histologic variety of MFH. 3. The review of the initial tissue slide was necessary for adequate diagnosis because the recurrent MFHs showed different histologic findings from original tumors.

  • Skeletal metastasis are common in terminal malignant tumor patients over 30% of incidence, but hand acrometastasis are very rare under 0.1% in terminal cancer patients. We have reported 5 cases of hand acrometastasis herewith the review of literatures. In the preterminal malignant tumor patients, sudden painful swelling on fingers and osteoporosis on X-rays are thought to be the earliest sign of hand metastasis.

Use of Expandable Prostheses in Malignant Bone Tumors in Children (소아 악성 골종양의 치료에서 확장형 종양대치물의 이용)

  • With the advent of effective chemotherapy and the realization of high economic cost associated with amputation, limb salvage surgery has become the standard of treatment in children with primary malignant bone tumors. Reconstruction after resection of malignant bone tumors of children has to address the leg length inequality and also has to be durable to cope with high functional demands of young patients. Expandable endoprostheses have been used in children for achieving limb length equality with substantial risk of complications. Recently, significant advances in prosthetic designs have reduced the morbidities associated with these prostheses. The purpose of this study was to review the indications, characteristics, complications and recent developments of expandable endoprostheses used for malignant bone tumors in children.

  • Fibrous dysplasia is a developmental benign disorder in which normal bone is replaced by fibrous tissue. Malignant transformation of fibrous dysplasia is a rare complication and more commonly occur in the polyostotic form than monostotic form. We report a case in which osteosarcoma developed in area of fibrous dysplasia of proximal femur after treating with curettage, wedge osteotomy, internal fixation, and bone graft. A review of the literature is presented.

  • The main goals of treatment of malignant bone tumor are the prolongation of life survival and the improvement of quality of life. In growing children, however, leg length discrepancy (LLD) is one of major problem in the treatment of malignant bone tumors. Therefore, the precise understanding of growth in children is essential, and the prediction of LLD is critical in deciding the time and options of surgery. In addition, to use the adequate method of growth expectation, periodic follow-up and collaboration with patient's parents are needed.

  • Malignant melanoma commonly occurs in skin and the incidences are uncommon. Especially, the incidences of subungual melanoma is less than 2% of total numbers of melanoma, and most of the cases tended to be misdiagnosed at first time. Thus, the prognoses are not good. We are going to report our experiences of subungual melanoma and diagnostic criteria in great toe initially misdiagnosed as ingrowing nail with a review of the literature.

Peroneal Nerve Palsy Associated with Musculoskeletal Tumor Resection (종양 절제 후 동반된 비골신경 손상)

  • We retrospectively reviewed the cases of peroneal nerve palsy in seven patients after musculoskeletal tumor resection. Their mean age was 38 years. Three had osteosarcoma in proximal fibula, 2 had chondrosarcoma in proximal fibula and calf and 2 had malignant fibrous histio cytoma in calf. Four of 7 patients had been managed using active dorsiflexion brace. Three patients who underwent tibialis posterior transfer and could walk without brace were able to discontinue the use of the orthosis. Peroneal nerve palsy after wide excision of tumor including peroneal nerve can not resolve spontaneously and results in severe functional disability. To improve the gait function, active surgical treatment should be considered.

  • Pelvic bone is location with the worst prognosis in primary malignant bone tumor. Malignant bone tumor around symphysis pubis is extremely rare, and although small size, it is difficult to excise because of anatomical location. The authors report a case of intra-pelvic chondrosarcoma of the pubic bone with good functional result through resection by both superior & inferior pubic ramus osteotomy.

An Experimental Study on the Changes in the Pasteurized Femoral Vessels of the Dogs (저온열처리한 개의 대퇴 혈관의 변화에 대한 실험적 연구)

  • The pasteurization of bone tumor shows necrosis of tumor tissue and it is used widely as one of the options of limb salvage operation. However malignant tumors of the extremities commonly involve major neurovascular structures and pasteurization of this structure will make limb salvage operation much easier and safer than autogenous vein graft or artificial vessel graft. So the purpose of this study is to evaluate that the pasteurization can be applied in the limb salvage surgery of malignant tumor involving major vessels by means of studying the patency of pasteurized femoral vessels of the dogs. The right femoral arteries of 5 to 7 mm in diameters and veins of 7 to 10 mm in diameters of five dogs were pasteurized with sterile $60^{\circ}C$ saline for 30 minutes. Contralateral femoral vessels were evaluated for the control study. After one month, the changes in the pasteurized femoral vessels were evaluated by physical examinations, femoral angiography, gross findings, and pathologic findings on the each side. One month after pasteurization, the pulse of the femoral and popliteal arteries was palpated with normal tone on the each side of the all five experimental animals, and there was no gross swelling or necrotic changes in the legs. Femoral angiography showed a good patency of femoral and popliteal arteries. On the gross examinations at time of sampling of the specimen for the pathologic examinations, there was a good patency of femoral artery and vein, and mild fibrous adhesion was noted around the pasteurized femoral vessels. On the pathologic examinations, the more fibrotic adhesion and neocapillarization were noted in the outer layer of adventitia of the pasteurized femoral arteries and veins than the control sides. The vascular lumina were also patent in all cases. With these results, we suggest that the malignant tumor of the extremity involving major vessels is possibly treated by the limb salvage operation using the pasteurization of the involved vessels.

Osteosarcoma Arising from Desmoplastic Fibroma of Proximal Tibia - A Case Report - (근위 경골의 결합조직형성 섬유종 치료 후 발생한 골육종 - 1예 보고 -)

  • Desmoplastic fibroma of bone is rare, involves usually long bones, and histologically resemblance to the desmoid tumor of soft tissue. It is benign, but aggressive locally. Recurrence rate is high when treated with simple curettage and bone graft. Moreover, malignant transformation of desmoplastic fibroma with recurrence has been reported very rarely, and then it make us review again whether previous tumor probably was low grade malignant tumor. We report a surgically proven case of osteosarcoma arising from desmoplastic fibroma. A 17 years old male had been treated for a desmoplastic fibroma of the proximal tibia with curettage and bone graft. But after 11 years, the patient showed recurrence accompanying malignant transformation with osteosarcoma. This is very rare case of an osteosarcoma arising in a desmoplatic fibroma of the proximal tibia.
