단측에 발생한 사구체낭성신질환 1례 (A Case Report Unilaterally Involved Glomerulocystic Kidney Disease)

  • Glomerulocystic kidney disease(GCKD) is a rare form of renal cystic disease defined histopathologically by containing dilated Bowman's space with variable atrophy of glomerular tufts, which may occur as sporadically or as familial cases and can be presented as a major component of heritable syndromes. It has not been recognized in Korean children but only one report of adult case has been reported having GCKD. We experienced a case of GCKD in a 10-year-10-month-old boy, who was admitted for hypertension. Abdominal ultrasonography and computed tomography revealed clustered numerous small cysts in left kidney and renal biopsy findings was consistent with the GCKD showing cystic dilatation of Bowman's space with intact glomerular structure.

한국 30세 이상 성인의 심혈관계 질환의 위험 요인 (Risk Factors for Cardiovascular Disease in Adults Aged 30 Years and Older)

  • Purpose : Cardiovascular disease is major factor of mortality in worldwide. Previous studies shown that the socioeconomic factors, nutrition factors, health behavior factors, biological factors and co-morbidity are increasing a prevalence of cardiovascular disease. Method : This study examined the risk factors for cardiovascular disease among adults aged 30 years and older using the data from the 2012 to 2014 Korean National Health and Nutrition Examination Survey (KNHANES). The study participants were 7,555 Cardiovascular disease includes hypertension, stroke, angina pactoris, and myocardial infarction. Descriptive statistic and multivariates logistic regression were calculated. Result : The overall prevalence of cardiovascular disease was 31.16% in the participants. Cardiovascular disease was significantly associated with gender, age, income, education, marital status as socioeconomic factors in unadjusted model. After adjusting socioeconomic status variables, past smoker (OR 1.27, 95% CI 1.05-1.55), obesity (OR 7.14, 95% CI 4.21-12.11), skipping a meal (OR 2.79, 95% CI 2.46-3.16), HDL-C (OR 0.99, 95% CI 0.98-0.99) and WC (OR 1.06, 95% CI 1.05-1.07) were associated with cardiovascular disease. Conclusion : The results marked the importance of finding high risk groups and an early management of cardiovascular disease.

노인의 사중고(四重苦)가 우울을 매개로 자살생각에 미치는 영향: 경로당 이용자를 중심으로 (Effects of Korean Elder's Four Major Pains on Suicidal Thought Mediated by Depression: Focused on Gyungrodang Users)

  • 본 연구는 노인의 사중고(四重苦) 개념을 확인하고 이를 구성하는 빈곤, 질병, 역할상실, 고독이 우울을 매개로 자살생각에 미치는 영향을 검증했다. 연구를 위해서 전주시 경로당을 지역별로 안배한 후 선정된 경로당에서 응답자를 편의 추출했다. 회수된 응답은 309사례였으며 이중에서 결측치, 이상치, 정규분포, 다중공선성의 조건을 만족한 291사례가 분석대상이 되었다. 분석은 빈도분석, 요인분석, 내적일관성신뢰도분석, 구조방정식을 이용한 확인요인분석과 구조모형분석을 실시했다. 구조모형분석을 통해서 각각의 가설에 해당하는 경로계수를 확인하였으며 이들의 유의성에 따라서 가설의 지지여부를 판정했다. 또한 요인들의 매개효과를 확인하기 위해서 모수추정치를 직접효과와 간접효과로 분해하고, 매개효과의 크기와 유의성을 확인했다. 주요 연구결과는 다음과 같다. 첫째, 사중고의 구성요인들인 빈곤, 질병, 역할상실, 고독은 우울을 매개로 자살생각에 영향을 미쳤다. 둘째, 사중고의 구성요인 중에서 가장 비중이 큰 빈곤은 다른 구성요인에 영향을 미치며, 역할상실은 고독에 영향을 미치는 것으로 확인되었다. 셋째, 노인의 빈곤, 질병, 역할상실, 고독으로 구성된 사중고의 모든 구성요인이 우울의 선행요인으로 확인되었다. 다만 빈곤이 우울에 미치는 영향은 직접적인 것보다는 간접적인 것으로 확인되었다. 본 연구는 노인의 사중고 개념을 확인했고 사중고를 통해서 급증하는 노인자살의 원인을 검증했다. 따라서 노인의 자살예방계획을 수립할 때 자살의 선행변수인 우울이나 사중고의 구성요인에 대한 포괄적인 중재계획이 필요하다.

심층 신경망 기반의 앙상블 방식을 이용한 토마토 작물의 질병 식별 (Tomato Crop Disease Classification Using an Ensemble Approach Based on a Deep Neural Network)

  • The early detection of diseases is important in agriculture because diseases are major threats of reducing crop yield for farmers. The shape and color of plant leaf are changed differently according to the disease. So we can detect and estimate the disease by inspecting the visual feature in leaf. This study presents a vision-based leaf classification method for detecting the diseases of tomato crop. ResNet-50 model was used to extract the visual feature in leaf and classify the disease of tomato crop, since the model showed the higher accuracy than the other ResNet models with different depths. We propose a new ensemble approach using several DCNN classifiers that have the same structure but have been trained at different ranges in the DCNN layers. Experimental result achieved accuracy of 97.19% for PlantVillage dataset. It validates that the proposed method effectively classify the disease of tomato crop.

  • Chronic obstructive pulmonary disease (COPD) is characterized by a diverse array of pulmonary and nonpulmonary manifestations, but our understanding of COPD pathogenesis and the factors that influence its heterogeneity in disease presentation is poor. Despite this heterogeneity, treatment algorithms are primarily driven by a single measurement, forced expiratory volume in 1 second ($FEV_1$) as a percentage of its predicted value ($FEV_1%$). In 2011, a major shift in Global Initiative for Chronic Obstructive Lung Disease (GOLD) treatment recommendations was proposed that stratifies patients with COPD on the basis of symptoms and exacerbation history. This article reviews the work reported in 2013 that enlightens our understanding of COPD with respect to COPD classification systems, phenotype, biomarker, exacerbation, and management for patients with COPD.

  • PET of the cerebral metabolic rate of glucose is increasingly used to support the clinical diagnosis in the examination of patients with suspected major neurodegenerative disorders, such as Alzheimer's disease. $^{18}F-FDG$ PET has been reported to have high diagnostic performance, especially, very high sensitivity in the diagnosis and clinical assessment of therapeutic efficacy. According to clinical research data hitherto, $^{18}F-FDG$ PET is expected to be an effective diagnostic tool in early and differential diagnosis of Alzheimer's disease. Since 2004, Medicare covers $^{18}F-FDG$ PET scans for the differential diagnosis of fronto-temporal dementia (FTD) and Alzheimer's disease (AD) under specific requirements; or, its use in a CMS approved practical clinical trial focused on the utility of $^{18}F-FDG$ PET in the diagnosis or treatment of dementing neurodegenerative diseases.

  • Polycystic kidney disease is defined as a heritable disorder with diffuse involvement of both kidneys without dysplasia other than cysts. The major clinical entities of autosomal recessive polycystic kidney disease and autosomal dominant polycystic kidney disease have a considerable overlap in clinical presentations and radiographic features in the pediatric population. We experienced three cases of autosomal recessive polycystic kidney disease of neonate who expired within 24 hours due to respiratory difficulty and the other case was detected by gross hematuria. So we report four cases with brief review of literatures.

크론병에서 복잡성 항문주위 샛길의 수술적 치료 (Surgical treatment of perianal fistula in Crohn's disease)

  • Perianal Crohn's disease is a major problem that impair quality of life. This article reviews the current surgical treatment of Crohn's perianal fistula. Fistulotomy and loose seton are commonly used surgical methods for treatment of perianal Crohn's disease. Mucosal advancement flap and fibrin glue are used in this treatment, despite a lake of controlled trials. Fecal diversion is disturbingly high in complicated complex perianal fistula in Crohn's disease. Ligation of intersphincteric fistula and autologous or allogenic stem cells are new surgical procedures for treatment of Crohn's disease that need further studies. Treatment success might be improved by multimodal treatment and new surgical and medical treatment options.

Catheter를 이용한 지속적 성상신경절 차단 경험 -증례 보고- (Continuous Stellate Ganglion Block for Raynaud'S Disease -A case report-)

  • Stellate ganglion block has been used to treat diseases such as peripheral vascular disease, sympathetic dystrophy, and various pain syndromes involving the head or arm. Raynaud's disease is a syndrome manifested by attacks of pallor, cyanosis, numbness and pain of the digits in response to cold or emotional change. I report one case who was given Stellate ganglion block using 18G teflon Catheter(4.5 cm in length) for Raynaud's disease. Continuous stellate ganglion block is more convinient to inpatient than repeated needle punctures and may reduce major complications and more useful to patient who needs continuous sympathetic block about one week duration.

Serological Survey of Major Avian Viral Diseases Related with Egg Production in Commercial Chicken Flocks in Korea

  • While use of mass rearing systems improved poultry production, chances of exposing to contagious diseases have been increased, making flocks more vulnerable to diseases. Diseases of interest which affects egg production adversely include Low pathogenic avian influenza (LPAI), Infectious bronchitis (IB), Avian meta-pneumoviral infection (aMPV) and Egg drop syndrome'76 (EDS'76). This report collected and analyzed 5,385 serum samples, which were collected from 1,330 different chicken flock, provided by Chungbuk National University, Avian Disease Laboratory at 2009. Serums were analyzed based on rearing stages; 0~1.3weeks (wks) (maternal antibody period), >1.3~3 wks (starting period), >3~10 wks (growing period), >10~22 wks (developing period), >22~40 wks (peak laying period), >40~60 wks (late laying period) and over 60 wks (post-molting period). Results showed the 99.7% of the tested flocks were immunized against ND and73.8%, 97.1%, 78,2% and 78% of the flocks were immunized against other 4 agents (LPAI, IB, EDS'76, aMPV). Maternal antibody was transferred to enough quantity for NDV. Generally, antibody titers which were developed at 22 weeks were stabilized permanently for life. In case of IB and aMPV, infection titer emerged as early as 10 weeks and the titer was increased from 99.4% to 100% for life. EDS76 showed increase in titers, reflecting decreased frequency of vaccination programs. Overall, this study displayed general trends of major viral disease in layers, but considering the trend of development of preventive measures and evolution of pathogens, conducting serological surveys on a regular basis is important.
