Effects of Rituximab Including Long-term Maintenance Therapy in Children with Nephrotic Syndrome in a Single Center of Korea

  • Rituximab (RTX) is a chimeric monoclonal antibody that inhibits CD20-mediated B-cell proliferation and differentiation. Several studies have examined its use in intractable nephrotic syndrome (NS) with some positive results. However, those studies examined such effects for a short-term period of 1 year, and some patients continued to relapse after a lapse in RTX treatment. Our use of RTX as a maintenance therapy (RTX injection when the CD19 cell count exceeded $100-200/{\mu}L$ before relapse) showed some noticeable efficacy. We used RTX in 19 patients with steroid-dependent NS (SDNS). In 12 patients treated with RTX maintenance therapy, only one relapse occurred. The mean treatment period was $23.4{\pm}12.7months$, and the mean number of RTX administrations was $3.9{\pm}1.6$. The relapse rates were decreased (from 2.68/year to 0.04/year), and the drug-free period also increased (from 22.5 days/year to 357.1 days/year) during maintenance therapy. The other seven patients were treated with one cycle of RTX or additional cycles in case of relapse (non-maintenance therapy). Relapse rates were significantly decreased after RTX treatment (from 1.76/year to 0.96/year, P=0.017). The relapse-free period was $15.55{\pm}7.38$ (range, 5.3-30.7) months. No severe side effects of RTX were found except for a hypersensitivity reaction such as fever and chills during its infusion. In conclusion, RTX is considered an effective and safe option to reduce the relapse rate by a single- or maintenance-interval therapy in SDNS.

  • reliability from components reliability. In this case, it assumes that components failure is mutually independent, but it may not true in real systems. In this study, the mean cost per unit time is computed as the ratio of mean life to the mean cost. The mean life is obtained by the reliability function under power rule model. The mean cost is obtained by the mathematical model based on the inspection interval. A heuristic method is proposed to determine the optimal number of redundant units and the optimal inspection interval to minimize the mean cost per unit time. The assumptions of this study are as following : First, in the load-sharing k-out-of-n:G system, total loads are applied to the system and shared by the operating components. Secondly, the number of failed components affects the failure rate of surviving components as a function of the total load applied. Finally, the relation between the load and the failure rate of surviving components is set by the power rule model. For the practical application of the above methods, numerical examples are presented.

An Improved Interval AHP Method for Assessment of Cloud Platform-based Electrical Safety Monitoring System

  • Electrical safety monitoring System (ESMS) is a critical component in modern power systems, which is characterized by large-scale access points, massive users and versatile requirements. For convenience of the information integration and analysis, the software development, maintenance, and application in the system, the cloud platform based ESMS is established and assessed in this paper. Firstly the framework of the system is proposed, and then the assessment scheme with a set of evaluation indices are presented, by which the appropriate cloud product can be chosen to meet the requirements of a specific application. Moreover, to calculate the weights of the evaluation indices under uncertainty, an improved interval AHP method is adopted to take into consideration of the fuzziness of expert scoring, the qualitative consistency test, and the two normalizations in the process of eigenvectors. Case studies have been made to verify the feasibility of the assessment approach for ESMS.

건설용 리프트의 운행정보 및 고장데이터 분석을 통한 주요 부품별 점검주기 산출 연구 (A Study on Optimal Inspection Interval for the Major Components of Construction Lift)

  • 최근 초고층 건축물의 증가로 그에 따른 양중계획 및 양중장비의 효율적 관리에 대한 관심이 증대되고 있다. 건설 현장에서 폭넓게 사용되고 있는 전동 호이스트, 크레인 등과 관련된 재해사고가 빈번히 발생하고 있다. 특히 건설용 리프트는 안전 측면에서 정기점검이나 고장에 대비하는 것이 대단히 중요하나 사용시간에 따른 일률적인 정기검사를 수행하고 있는 실정이다. 리프트는 개별 부품의 사용시간 및 부하조건 등에 따라 그 수명이 달라질 수 있기 때문에 실효성이 매우 떨어지는 유지관리 방법이라고 할 수 있다. 본 연구에서는 이러한 문제점을 개선하고 사용자 편의성과 안전성을 증대시키기 위한 방안으로써 센싱 기술을 활용하여 리프트의 실제 사용시간을 측정하고 그에 따른 고장데이터를 분석하여 리프트의 주요 부품들에 대한 개별 점검주기를 산출하는 것을 목적으로 하였다. 연구의 결과물로 리프트의 주요 부품에 대한 점검주기를 산출하고, 리프트 실 사용시간 40시간, 90시간, 130시간, 400시간으로 나누어 그에 따른 점검 부품들을 구분하여 표로 작성하였다. 이를 통해 안전사고를 미연에 방지하고 작업자의 안전성을 향상시켜 효율적인 유지관리 방안을 제시할 수 있을 것으로 사료된다. 또한 리프트 고장으로 인한 공기지연 요소를 사전에 제거하여 공사의 생산성을 증대시킬 수 있을 것이다.

자주포 경제수명 결정에 관한 연구 (A Study on the Determination for the Economic Life-Time of a Self-Propelled Artillery)

  • This research paper deals with the problem of determining the optimal life time in terms of economical sense for a self-propelled artillery. Equivalent Annual Cost Method(EACM) is used to evaluate the optimal life time, based on the acquisition cost, and the operation and maintenance cost. It is assumed that the operation and maintenance cost includes the costs for spare parts, petroleum and ammunition for training. From the result of this study, the optimal life time for a self-propelled artillery is between 13.9 years and 16.1 years with 95% confidence interval.

The Implementation of Risk-Based Inspection for the Refinery Plant

  • Recently, regulatory bodies quite often encourage to adopt risk-based inspection (RBI) and management programs because they can enhance safety simultaneously with deregulation in Korea. RBI is an integrated methodology that factors risk into inspection and maintenance decision making. This paper describes an example of how to use known risk assessment codes (API 580, API 581 BRD) to address such safety analysis requirements for risk management in the refining industry. Specifically, this paper reports the methodology and the results of implementation to the Crude Distillation Unit(CDU) plant of refinery units using the $KGS-RBI^{TM}$ program, developed by the Korea Gas Safety Corporation in reference of API Codes and ASME PC (Post Construction) with a suitable consideration of Korean situation. The results of the risk and reliability assessment using $KGS-RBI^{TM}$ program are useful in determining whether the detected defects are tolerable or required to be repaired. The subsequent decisions are to manage the future inspection, repair and maintenance planning in the risk reduction control.

  • An ATMS(Assumption-based Truth Maintenance System) has been widely used for maintaining the truth of information by detecting and solving contradictions in nile-based systems. But the ATMS can not correctly maintain the truth of the information in case that the generated information is satisfied within a time interval or includes data about temporal relations of events in time varying situations, because it has no mechanism manipulating temporal data. In this paper, The extended ATMS method is proposed, which can maintain the truth of the information in the inference system using information changing over time or temporal relations of events. In order to maintain contexts generated by relations of events, the label representation method is modified, the disjunction, conjunction simplification method in the label-propagation procedure and nogood handling method of the conventional ATMS are modified, too.

교량의 경험적 손상도 곡선 (Empirical Fragility Curves for Bridge)

  • This paper presents a statistical analysis of empirical fragility curves for bridge. The empirical fragility curves are developed utilizing bridge damage data obtained from the 1995 Hyogoken Nanbu(Kobe) earthquake. Two-parameter lognormal distribution functions are used to represent the fragility curves with the parameters estimated by the maximum likelihood method. This paper also presents methods of testing the goodness of fit of the fragility curves and estimating the confidence intervals of the two parameters(median and log-standard deviation) of the distribution. An analytical interpretation of randomness and uncertainty associated with the median is provided.

철근의 깊이, 굵기 및 간격이 GPR 신호에 미치는 영향 조사 (Examination on the influence of Depth, Size and Interval of Rebar on the Signal of Ground Penetrating Radar)

  • Ground penetrating radar(GPR) was applied for measuring depths, sizes and intervals of rebars embedded in concrete. A concrete wall was constructed for this study and a sand pool and a concrete block were used for simulation. Result of this study shows that GPR can be used for measuring rebar depths and intervals, even though it is limitary, but that measuring sizes is almost impossible. Simulation with the sand pool was helpful for research on the versatile rebar arrays though signal was not clear as real concrete wall. A concrete block with many cylindrical holes for inserting different sized rebars could not be used for simulator due to many unknown reflective waves. Antenna orientation must be perpendicular to rebars for large reflection signal.

  • Queueing system with scheduled arrivals is useful in many fields where both customers' waiting time and servers' operation time (utilization) are important, and arrival time of customers is possible to be controlled. In this paper, we analyzed the operation of ship maintenance with the queueing system with scheduled arrival. Based on the model presented by Pegden and Rosenshine [8], who dealt with exponential service time, we extended the service time distributions to phase-type distribution which is able to include a wide range of real stochastic phenomena. Since most activities in the military are carried out under tight control and schedule, scheduled arrival queue has quite good applicability in this area. In this context, we applied queue with scheduled arrival to the optimal booking time decision for the ship maintenance in the navy.
