Paradigm Shift in Prostate Cancer Diagnosis: Pre-Biopsy Prostate Magnetic Resonance Imaging and Targeted Biopsy

  • With regard to the indolent clinical characteristics of prostate cancer (PCa), the more selective detection of clinically significant PCa (CSC) has been emphasized in its diagnosis and management. Magnetic resonance imaging (MRI) has advanced technically, and recent international cooperation has provided a standardized imaging and reporting system for prostate MRI. Accordingly, prostate MRI has recently been investigated and utilized as a triage tool before biopsy to guide tissue sampling to increase the detection rate of CSC beyond the staging tool for patients in whom PCa was already confirmed on conventional systematic biopsy. Radiologists must understand the current paradigm shift for better PCa diagnosis and management. This article reviewed the recent literature, demonstrating the diagnostic value of pre-biopsy prostate MRI with targeted biopsy and discussed unsolved issues regarding the paradigm shift in the diagnosis of PCa.

  • 상완신경총의 영상 진단은 복잡한 해부학적인 구조와 다양하게 침범하는 병들로 인해 어렵고 도전적이다. 상완신경총 영상 진단의 일차적인 목표는 외상성 손상과 비외상성 손상의 구별이다. MRI의 급속경사와 전용표면코일을 이용한 적절한 공간분해능을 가진 영상기기의 발전과 이를 이용한 다양한 펄스열의 개발이 상완신경총의 외상성 손상과 비외상성 손상에 대한 구별에 많은 도움을 주어 정확한 진단으로 수술 여부의 결정에 이용된다. MRI는 비외상성 상완신경총 병변의 진단에 가장 좋은 영상진단이며, CT 척수강조영술은 상완신경총을 침범하는 신경근견열 병변의 검사에 많이 이용된다. 그리고 일반적으로 고화질, 고해상력의 MRI 가 상완신경총 비외상성병변의 진단에 일차적인 영상검사로 중요한 역할을 하고 있다.

Current Practices in Breast Magnetic Resonance Imaging: a Survey Involving the Korean Society of Breast Imaging

  • Purpose: To report on the current practices in breast magnetic resonance imaging (MRI) in Korea. Materials and Methods: We invited the 68 members of the Korean Society of Breast Imaging who were working in hospitals with available breast MRI to participate in a survey on how they performed and interpreted breast MRI. We asked one member from each hospital to respond to the survey. A total of 22 surveys from 22 hospitals were analyzed. Results: Out of 22 hospitals, 13 (59.1%) performed at least 300 breast MRI examinations per year, and 5 out of 22 (22.7%) performed > 1200 per year. Out of 31 machines, 14 (45.2%) machines were 1.5-T scanners and 17 (54.8%) were 3.0-T scanners. All hospitals did contrast-enhanced breast MRI. Full-time breast radiologists supervised the performance and interpreted breast MRI in 19 of 22 (86.4%) of hospitals. All hospitals used BI-RADS for MRI interpretation. For computer-aided detection (CAD), 13 (59.1%) hospitals sometimes or always use it and 9 (40.9%) hospitals did not use CAD. Two (9.1%) and twelve (54.5%) hospitals never and rarely interpreted breast MRI without correlating the mammography or ultrasound, respectively. The majority of respondents rarely (13/21, 61.9%) or never (5/21, 23.8%) interpreted breast MRI performed at an outside facility. Of the hospitals performing contrast-enhanced examinations, 15 of 22 (68.2%) did not perform MRI-guided interventional procedures. Conclusion: Breast MRI is extensively performed in Korea. The indication and practical patterns are diverse. The information from this survey would provide the basis for the development of Korean breast MRI practice guidelines.

  • 무결석 출혈성 담낭염은 높은 사망률을 보이는 급성 담낭염의 드문 합병증이다. 저자는 담낭의 내강에 혈종을 동반한 무결석 출혈성 담남염의 자기공명영상소견을 보고하고자 한다. 자기공명영상에서 팽대된 담낭과 담낭 내강의 혈종이 관찰되었고, 혈종은 T1과 T2강조영상에서 담낭 내강의 저신호강도의 종괴로 보였다. 자기공명영상은 우수한 조직 대조성을 가지므로 출혈성 담낭염에서 담낭내강에 혈종을 검출하는데 유용하다.

The Present Status of Cell Tracking Methods in Animal Models Using Magnetic Resonance Imaging Technology

  • With the advance of stem cell transplantation research, in vivo cell tracking techniques have become increasingly important in recent years. Magnetic resonance imaging (MRI) may provide a unique tool for non-invasive tracking of transplanted cells. Since the initial findings on the stem cell migration by MRI several years ago, there have been numerous studies using various animal models, notably in heart or brain disease models. In order to develop more reliable and clinically applicable methodologies, multiple aspects should be taken into consideration. In this review, we will summarize the current status and future perspectives of in vivo cell tracking technologies using MRI. In particular, use of different MR contrast agents and their detection methods using MRI will be described in much detail. In addition, various cell labeling methods to increase the sensitivity of signals will be extensively discussed. We will also review several key experiments, in which MRI techniques were utilized to detect the presence and/or migration of transplanted stem cells in various animal models. Finally, we will discuss the current problems and future directions of cell tracking methods using MRI.

  • MRI(magnetic resonance imaging) is very useful to visualize the anterior cruciate ligament (ACL) injury of the knee. Differential features of ACL injury at MRI according to acute and chronic stage should be evaluated to enable accurate assessment of the integrity or ACL. Distinguishing features of normal anatomy. acute and chronic injuries or ACL at MRI will be reviewed.

Non-Invasive in vivo Loss Tangent Imaging: Thermal Sensitivity Estimation at the Larmor Frequency

  • Visualization of the tissue loss tangent property can provide distinct contrast and offer new information related to tissue electrical properties. A method for non-invasive imaging of the electrical loss tangent of tissue using magnetic resonance imaging (MRI) was demonstrated, and the effect of loss tangent was observed through simulations assuming a hyperthermia procedure. For measurement of tissue loss tangent, radiofrequency field maps ($B_1{^+}$ complex map) were acquired using a double-angle actual flip angle imaging MRI sequence. The conductivity and permittivity were estimated from the complex valued $B_1{^+}$ map using Helmholtz equations. Phantom and ex-vivo experiments were then performed. Electromagnetic simulations of hyperthermia were carried out for observation of temperature elevation with respect to loss tangent. Non-invasive imaging of tissue loss tangent via complex valued $B_1{^+}$ mapping using MRI was successfully conducted. Simulation results indicated that loss tangent is a dominant factor in temperature elevation in the high frequency range during hyperthermia. Knowledge of the tissue loss tangent value can be a useful marker for thermotherapy applications.

Diagnostic value of magnetic resonance imaging using superparamagnetic iron oxide for axillary node metastasis in patients with breast cancer: a meta-analysis

  • Objectives: Identification of axillary metastases in breast cancer is important for staging disease and planning treatment, but current techniques are associated with a number of adverse events. This report evaluates the diagnostic accuracy of superparamagnetic iron oxide (SPIO)-enhanced magnetic resonance imaging (MRI) techniques for identification of axillary metastases in breast cancer patients. Methods: We performed a meta-analysis of previous studies that compared SPIO enhanced MRI with histological diagnosis after surgery or biopsy. We searched PubMed, Ovid, Springer Link, and Cochrane library to identify studies reporting data for SPIO enhanced MRI for detection of axillary lymph node metastases in breast cancer until December 2013. The following keywords were used: "magnetic resonance imaging AND axilla" and "superparamagnetic iron oxide AND axilla". Eligible studies were those that compared SPIO enhanced MRI with histological diagnosis. Sensitivity and specificity were calculated for every study; summary receiver operating characteristic and subgroup analyses were done. Study quality and heterogeneity were also assessed. Results: There were 7 publications that met the criteria for inclusion in our meta-analysis. SROC curve analysis for per patient data showed an overall sensitivity of 0.83 (95% Confidence interval (CI): 0.75-0.89) and overall specificity of 0.97 (95% CI: 0.94-0.98). Overall weighted area under the curve was 0.9563. Conclusions: SPIO enhanced MRI showed a trend toward high diagnostic accuracy in detection of lymph node metastases for breast cancer. So, when the breast cancer patients has axillary metastases histologically, SPIO enhanced MRI may be effective diagnostic imaging modality for axillary metastases.

A Review on the RF Coil Designs and Trends for Ultra High Field Magnetic Resonance Imaging

  • In this article, we evaluated the performance of radiofrequency (RF) coils in terms of the signal-to-noise ratio (S/N) and homogeneity of magnetic resonance images when used for ultrahigh-frequency (UHF) 7T magnetic resonance imaging (MRI). High-quality MRI can be obtained when these two basic requirements are met. However, because of the dielectric effect, 7T magnetic resonance imaging still produces essentially a non-uniform magnetic flux (|B1|) density distribution. In general, heterogeneous and homogeneous RF coils may be designed using electromagnetic (EM) modeling. Heterogeneous coils, which are surface coils, are used in consideration of scalability in the |B1| region with a high S/N as multichannel loop coils rather than selecting a single loop. Loop coils are considered state of the art for their simplicity yet effective |B1|-field distribution and intensity. In addition, combining multiple loop coils allows phase arrays (PA). PA coils have gained great interest for use in receiving signals because of parallel imaging (PI) techniques, such as sensitivity encoding (SENSE) and generalized autocalibrating partial parallel acquisition (GRAPPA), which drastically reduce the acquisition time. With the introduction of a parallel transmit coil (pTx) system, a form of transceiver loop arrays has also been proposed. In this article, we discussed the applications and proposed designs of loop coils. RF homogeneous coils for volume imaging include Alderman-Grant resonators, birdcage coils, saddle coils, traveling wave coils, transmission line arrays, composite right-/left-handed arrays, and fusion coils. In this article, we also discussed the basic operation, design, and applications of these coils.
