Objective : Magnetic resonance imaging (MRI) grading systems using sagittal images are useful for evaluation of lumbar foraminal stenosis. We evaluated whether such a grading system is useful as a diagnostic tool for surgery. Methods : Between July 2014 and June 2015, 99 consecutive patients underwent unilateral lumbar foraminotomy for lumbar foraminal stenosis. Surgically confirmed foraminal stenosis and the contralateral, asymptomatic neuroforamen were assessed based on a 4-point MRI grading system. Two experienced researchers independently evaluated the MR sagittal images. Interobserver agreement and intraobserver agreement were analyzed using ${\kappa}$ statistics. Results : The mean age of patients (54 women, 45 men) was 62.5 years. A total of 101 levels (202 neuroforamens) were evaluated. MRI grades for operated neuroforamens were as follows : Grade 0 in 0.99%, Grade 1 in 5.28%, Grade 2 in 14.85%, and Grade 3 in 78.88%. Interobserver agreement was moderate for operated neuroforamens (${\kappa}=0.511$) and good for asymptomatic neuroforamens (${\kappa}=0.696$). Intraobserver agreement by reader 1 for operated neuroforamens was good (${\kappa}=0.776$) and that for asymptomatic neuroforamens was very good (${\kappa}=0.831$). In terms of lumbar level, interobserver agreement for L5-S1 (${\kappa}=0.313$, fair) was relatively lower than the other level (${\kappa}=0.804$, very good). Conclusion : MRI grading system for lumbar foraminal stenosis is thought to be useful as a diagnostic tool for surgery in the lumbar spine; however, it is less reliable for symptomatic L5-S1 foraminal stenosis than for other levels. Thus, various clinical factors as well as the MRI grading system are required for surgical decision-making.
Objective : To investigate the potential risk of approach-related complications at different access angles in minimally invasive lateral lumbar interbody fusion. Methods : Eighty-six axial magnetic resonance images were obtained to analyze the risk of approach-related complications. The access corridor were simulated at different access angles and the potential risk of neurovascular structure injury was evaluated when the access corridor touching or overlapping the corresponding structures at each angle. Furthermore, the safe corridor length was measured when the corridor width was 18 and 22 mm. Results : When access angle was $0^{\circ}$, the potential risk of ipsilateral nerve roots injury was 54.7% at L4-L5. When access angle was $45^{\circ}$, the potential risk of abdominal aorta, contralateral nerve roots or central canal injury at L4-L5 was 79.1%, 74.4%, and 30.2%, respectively. The length of the 18 mm-wide access corridor was largest at $0^{\circ}$ and it could reach 44.5 mm at L3-L4 and 46.4 mm at L4-L5. While the length of the 22 mm-wide access corridor was 42.3 mm at L3-L4 and 44.1 mm at L4-L5 at $0^{\circ}$. Conclusion : Changes in the access angle would not only affect the ipsilateral neurovascular structures, but also might adversely influence the contralateral neural elements. It should be also noted to surgeons that alteration of the access angle changed the corridor length.

Objective : Preoperative prediction of the arachnoid membrane descent in pituitary surgery is useful for achieving gross total removal and avoiding cerebrospinal fluid leakage resulting from tearing of the arachnoid membrane in the chiasmatic cistern. In this study, we analyzed the patterns of arachnoid membrane descent during or after pituitary tumor surgery and identified the factors related to this descent. Methods : Analysis was restricted to pituitary macroadenomas not extending into the third ventricle or over the internal carotid artery. To minimize confounding factors, patients who underwent revision surgery, those who had a torn arachnoid during operation or small medial diaphragma sellae (DS) opening, and subtotal resections were excluded. We enrolled 41 consecutive patients in this retrospective analysis. The degree of arachnoid descent was categorized using intraoperative videos. Preoperative magnetic resonance findings, including tumor height, suprasellar extension, and variables including DS area and medial opening size, tumor composition, and displacement of the pituitary stalk and gland were evaluated to determine their correlations with arachnoid membrane descent. Results : Arachnoid membrane descent was significantly correlated with DS area and medial opening size. Based on T2-weighted images (T2WI) magnetic resonance (MR) images, tumor composition was significantly associated with arachnoid membrane descent. Other factors were not significantly correlated with arachnoid membrane descent. Conclusion : T2WI of tumor composition and preoperative MR imaging of DS area and medial opening provided valuable information regarding arachnoid membrane descent. These parameters may serve as fundamental measures to facilitate complete resection of pituitary macroadenomas.
Purpose: The aim of this study was to assess the artefacts of 12 fixed orthodontic appliances in magnetic resonance images obtained using 1.5-T and 3-T scanners, and to evaluate different imaging sequences designed to suppress metal artefacts. Materials and Methods: In vitro, study casts of 1 adult with normal occlusion were used. Twelve orthodontic appliances were attached to the study casts and scanned. Turbo spin echo (TSE), TSE with high readout bandwidth, and TSE with view angle tilting and slice encoding for metal artefact correction were used to suppress metal artefacts. Artefacts were measured. In vivo, 6 appliances were scanned: 1) conventional stainless-steel brackets; 2) nickelfree brackets; 3) titanium brackets; 4) a Herbst appliance; 5) a fixed retainer; and 6) a rapid maxillary expander. The maxilla, mandible, nasopharynx, tongue, temporomandibular joints, and cranial base/eye globes were assessed. Scores of 0, 1, 2, and 3 indicated no artefacts and minor, moderate, and major artefacts, respectively. Results: In vitro, titanium brackets and the fixed retainer created minor artefacts. In vivo, titanium brackets caused minor artefacts. Conventional stainless-steel and nickel free brackets, the fixed retainer, and the rapid maxillary expander caused major artefacts in the maxilla and mandible. Conventional stainless-steel and nickel-free brackets caused major artefacts in the eye globe (3-T). TSE with high readout bandwidth reduced image artefacts in both scanners. Conclusion: Titanium brackets, the Herbst appliance, and the fixed retainer caused minor artefacts in images of neurocranial structures(1.5-T and 3-T) when using TSE with high readout bandwidth.
Purpose: To overcome the difficulty in building a large data set with a high-quality in medical imaging, a concept of 'blended-transfer learning' (BTL) using a combination of both source data and target data is proposed for the target task. Materials and Methods: Source and target tasks were defined as training of the source and target networks to reconstruct cardiac CINE images from undersampled data, respectively. In transfer learning (TL), the entire neural network (NN) or some parts of the NN after conducting a source task using an open data set was adopted in the target network as the initial network to improve the learning speed and the performance of the target task. Using BTL, an NN effectively learned the target data while preserving knowledge from the source data to the maximum extent possible. The ratio of the source data to the target data was reduced stepwise from 1 in the initial stage to 0 in the final stage. Results: NN that performed BTL showed an improved performance compared to those that performed TL or standalone learning (SL). Generalization of NN was also better achieved. The learning curve was evaluated using normalized mean square error (NMSE) of reconstructed images for both target data and source data. BTL reduced the learning time by 1.25 to 100 times and provided better image quality. Its NMSE was 3% to 8% lower than with SL. Conclusion: The NN that performed the proposed BTL showed the best performance in terms of learning speed and learning curve. It also showed the highest reconstructed-image quality with the lowest NMSE for the test data set. Thus, BTL is an effective way of learning for NNs in the medical-imaging domain where both quality and quantity of data are always limited.
요추 후관절 주위 낭종은 신경근을 누름으로써 요통과 방사통, 신경성 파행을 일으킬 수 있는 드문 질환이다. 34세 남자가 심한 요통 및 우측 하지 방사통을 호소하였다. 자기공명영상에서 척추관 내에서 제3-4번 요추 추간판과 제4번 요추체 후면까지 우측으로 위치한 낭종이 보였다. 영상증폭기 감시하에서 주사 바늘 흡인을 시행하여 1.5 ml의 장액성의 노란 액체가 흡인되었다. 흡인 치료 후, 증상들이 즉각적으로 소실되었다. 자기공명영상 추시 촬영에서 낭종의 재발소견이 보이지 않았다. 저자들이 아는 한 국내에서는 요추 후관절 주위 낭종에 대한 직접적 바늘 흡인 치료에 대한 보고가 전혀 없기에 이 증례를 문헌 고찰과 함께 보고하는 바이다.

Alzheimer's disease (AD) is a neurological condition that is recognized as one of the primary causes of memory loss. AD currently has no cure. Therefore, the need to develop an efficient model with high precision for timely detection of the disease is very essential. When AD is detected early, treatment would be most likely successful. The most often utilized indicators for AD identification are the Mini-mental state examination (MMSE), and the clinical dementia. However, the use of these indicators as ground truth marking could be imprecise for AD detection. Researchers have proposed several computer-aided frameworks and lately, the supervised model is mostly used. In this study, we propose a novel 3D Convolutional Neural Network Multilayer Perceptron (3D CNN-MLP) based model for AD classification. The model uses Attention Mechanism to automatically extract relevant features from Magnetic Resonance Images (MRI) to generate probability maps which serves as input for the MLP classifier. Three MRI scan categories were considered, thus AD dementia patients, Mild Cognitive Impairment patients (MCI), and Normal Control (NC) or healthy patients. The performance of the model is assessed by comparing basic CNN, VGG16, DenseNet models, and other state of the art works. The models were adjusted to fit the 3D images before the comparison was done. Our model exhibited excellent classification performance, with an accuracy of 91.27% for AD and NC, 80.85% for MCI and NC, and 87.34% for AD and MCI.
Purpose: There is an ongoing search for a stent material that produces a reduced susceptibility artifact. This study evaluated the effect of manganese (Mn) content on the MRI susceptibility artifact of ferrous-manganese (Fe-Mn) alloys, and investigated the correlation between MRI findings and measurements of Fe-Mn microstructure on X-ray diffraction (XRD). Materials and Methods: Fe-Mn binary alloys were prepared with Mn contents varying from 10% to 35% by weight (i.e., 10%, 15%, 20%, 25%, 30%, and 35%; designated as Fe-10Mn, Fe-15Mn, Fe-20Mn, Fe-25Mn, Fe-30Mn, and Fe-35Mn, respectively), and their microstructure was evaluated using XRD. Three-dimensional spoiled gradient echo sequences of cylindrical specimens were obtained in parallel and perpendicular to the static magnetic field (B0). In addition, T1-weighted spin echo, T2-weighted fast spin echo, and $T2^*$weighted gradient echo images were obtained. The size of the low-intensity area on MRI was measured for each of the Fe-Mn binary alloys prepared. Results: Three phases of ${\alpha}^{\prime}$-martensite, ${\gamma}$-austenite, and ${\varepsilon}$-martensite were seen on XRD, and their composition changed from ${\alpha}^{\prime}$-martensite to ${\gamma}$-austenite and/or ${\varepsilon}$-martensite, with increasing Mn content. The Fe-10Mn and Fe-15Mn specimens comprised ${\alpha}^{\prime}$-martensite, the Fe-20Mn and Fe-25Mn specimens comprised ${\gamma}+{\varepsilon}$ phases, and the Fe-30Mn and Fe-35Mn specimens exhibited a single ${\gamma}$ phase. The size of the low-intensity areas of Fe-Mn on MRI decreased relative to its microstructure on XRD with increasing Mn content. Conclusion: Based on these findings, proper conditioning of the Mn content in Fe-Mn alloys will improve its visibility on MR angiography, and a Mn content of more than 25% is recommended to reduce the magnetic susceptibility artifacts on MRI. A reduced artifact of Fe-Mn alloys on MRI is closely related to the paramagnetic constitution of ${\gamma}$-austenite and/or ${\varepsilon}$-martensite.
Magnetic nanoparticles were synthesized by using the sonochemical method with oleic acid as a surfactant. The average size of the magnetite nanoparticles was controlled by varying the ratio R=[$H_2O$]/[surfactant] in the range of 2 to 9 nm. To prepare chitosan-coated magnetite nanoparticles, chitosan solution was added to a magnetite colloid suspension under ultrasonication at room temperature for 20 min. The chitosan-coated magnetite nanoparticles were characterized by several techniques. Atomic force microscopy (AFM) was used to image the chitosan-coated nanoparticles. Magnetic hysteresis measurement was performed by using a superconducting quantum interference device (SQUID) magnetometer to investigate the magnetic properties of the magnetite nanoparticles and the chitosan-coated magnetite nanoparticles. The SQUID measurements revealed the superparamagnetism of both nanoparticles. The T1- and T2-weighted MR images of these chitosan-coated magnetite colloidal suspensions were obtained with a 4.7 T magnetic resonance imaging (MRI) system. The chitosancoated magnetite colloidal suspensions exhibited enhanced MRI contrasts in vitro.

Purpose: Little is known regarding the accuracy of clinical magnetic resonance imaging (MRI) protocols with acceptable scan times in sinus graft assessment. The aim of this study was to evaluate the correlations between MRI and cone-beam computed tomographic (CBCT) measurements of maxillary sinus grafts using 2 different clinical MRI imaging protocols. Materials and Methods: A total of 15 patients who underwent unilateral sinus lift surgery with biphasic calcium phosphate were included in this study. CBCT, T1-weighted MRI, and T2-weighted MRI scans were taken 6 months after sinus lift surgery. Linear measurements of the maximum height and buccolingual width in coronal images, as well as the maximum anteroposterior depth in sagittal images, were performed by 2 trained observers using CBCT and MRI Digital Imaging and Communication in Medicine files. Micro-computed tomography (micro-CT) was also performed to confirm the presence of bone tissue in the grafted area. Correlations between MRI and CBCT measurements were assessed with the Pearson test. Results: Significant correlations between CBCT and MRI were found for sinus graft height (T1-weighted, r=0.711 and P<0.05; T2-weighted, r=0.713 and P<0.05), buccolingual width (T1-weighted, r=0.892 and P<0.05; T2-weighted, r=0.956 and P<0.05), and anteroposterior depth (T1-weighted, r=0.731 and P<0.05; T2-weighted, r=0.873 and P<0.05). The presence of bone tissue in the grafted areas was confirmed via micro-CT. Conclusion: Both MRI pulse sequences tested can be used for sinus graft measurements, as strong correlations with CBCT were found. However, correlations between T2-weighted MRI and CBCT were slightly higher than those between T1-weighted MRI and CBCT.
