Radiological Apoplexy and Its Correlation with Acute Clinical Presentation, Angiogenesis and Tumor Microvascular Density in Pituitary Adenomas

  • Objective : Pituitary apoplexy is life-threatening clinical syndrome caused by the rapid enlargement of a pituitary tumor due to hemorrhage and/or infarction. The pathogenesis of pituitary apoplexy is not completely understood. We analyzed the magnetic resonance imaging (MRI) of pituitary tumors and subsequently correlated the radiological findings with the clinical presentation. Additionally, immunohistochemistry was also performed to determine whether certain biomarkers are related to radiological apoplexy. Methods : Thirty-four cases of pituitary adenoma were enrolled for retrospective analysis. In this study, the radiological apoplexy was defined as cases where hemorrhage, infarction or cysts were identified on MRI. Acute clinical presentation was defined as the presence of any of the following symptoms: severe sudden onset headache, decreased visual acuity and/or visual field deficit, and acute mental status changes. Angiogenesis was quantified by immunohistochemical expression of fetal liver kinase 1 (Flk-1), neuropilin (NRP) and vascular endothelial growth factor (VEGF) expression, while microvascular density (MVD) was assessed using Endoglin and CD31. Results : Clinically, fourteen patients presented with acute symptoms and 20 for mild or none clinical symptoms. Radiologically, fifteen patients met the criteria for radiological apoplexy. Of the fifteen patients with radiologic apoplexy, 9 patients presented acute symptoms whereas of the 19 patient without radiologic apoplexy, 5 patients presented acute symptoms. Of the five biomarkers tracked, only VEGF was found to be positively correlated with both radiological and nonradiological apoplexy. Conclusion : While pituitary apoplexy is currently defined in cases where clinical symptoms can be histologically confirmed, we contend that cases of radiologically identified pituitary hemorrhages that present with mild or no symptoms should be designated subacute or subclinical apoplexy. VEGF is believed to have a positive correlation with pituitary hemorrhage. Considering the high rate of symptomatic or asymptomatic pituitary tumor hemorrhage, additional studies are needed to detect predictors of the pituitary hemorrhage.

  • Background and Objectives: Angiogenesis is the process of new blood vessel development from preexisting vessel. Angiogenenesis has been considered to be essential for the growth and expansion of a solid tumor. Vascular endothelial growth factor (VEGF), known as one of the most important vascular permeability factors, induces proliferation of endothelial cells, stiumulates angiogenesis, and increases vascular permeability. Several recents reports have documented that VEGF overexpression is associated with poor clinical outcomes in many maligmancies. The aims of this study were to determine whether microvessel density and VEGF expression are related to clinicopathologic factors such as age, sex, tumor size, tumor stage, and prognostic factors and to evaluate the relationship between VEGF expression and angiogenesis in benign and malignant thyroid tumors. Materials and Methods: The subjects were 65 patients (27 with papillary carcinoma, 27 with adenomatous hyperplasia, 11 with follicular adenoma) who underwent thyroidectomy from 1995 to 2001. Imuunohistochemistry was used to detect VEGF expression and microvessel density (MVD) in paraffin-embedded thryoid tumor specimens. Results: The intensity of the VEGF expression did not show stastically difference between benign and malignant thyroid tumors. There was no apparent correlation between VEGF expression and age, tumor size, T stage or scores of the AGES, AMES and MACIS systems. The neo-microvessel density was higher in the maligant tumor than the benign tumors. Also, higher neo-microvessel density was associated with metastases of the lymph nodes and scores of the AMES and AGES systems. Conclusion: Our results suggest that neo-microvessel vessel density may be a significant prognostic factor in the thyroid papillary carcinoma. But the VEGF expression does not appear to be an significant independent prognostic factor for thyroid papillary carcinoma.

Microvascular Decompression for Hemifacial Spasm Associated with Vertebrobasilar Artery

  • Objective : Hemifacial spasm (HFS) is considered as a reversible pathophysiological condition mainly induced by continuous vascular compression of the facial nerve root exit zone (REZ) at the cerebellopontine angle. As an offending vessel, vertebrobasilar artery tends to compress much more heavily than others. The authors analyzed HFS caused by vertebrobasilar artery and described the relationships between microsurgical findings and clinical courses. Methods : Out of 1,798 cases treated with microvascular decompression (MVD) from Jan. 1980 to Dec. 2004. the causative vessels were either vertebral artery or basilar artery in 87 patients. Seventy-nine patients were enrolled in this study. Preoperatively, computed tomography (CT) or brain magnetic resonance (MR) imaging with 3-dimentional short range MR technique was performed and CT was checked immediately or 2-3 days after anesthetic recovery. The authors retrospectively analyzed the clinical features. the compression patterns of the vessels at the time of surgery and treatment outcomes. Results : There were 47 were male and 32 female patients. HFS developed on the left side in 52 cases and on the right side in 27. The mean age of onset was 52.3 years (range 19-60) and the mean duration of symptoms was 10.7 years. Many patients (39 cases; 49.1%) had past history of hypertension. HFS caused only by the vertebral artery was 8 cases although most of the other cases were caused by vertebral artery (VA) in combination with its branching arteries. Most frequently, the VA and the posterior inferior cerebellar artery (PICA) were the simultaneous causative blood vessels comprising 32 cases (40.5%). and in 27 cases (34.2 %) the VA and the anterior inferior cerebellar artery (AICA) were the offenders. Facial symptoms disappeared in 61 cases (77.2%) immediately after the operation and 68 cases (86.1%) showed good outcome after 6 months. Surgical outcome just after the operation was poor in whom the perforators arose from the offending vessels concurrently (p<0.05). Conclusion : In case where the vertebral artery is a cause of HFS, commonly branching arteries associated with main arterial compression on facial REZ requires more definite treatment for proper decompression because of its relatively poor results compared to the condition caused by other vascular compressive origins.

Phased-in 코드를 이용한 움직임 벡터 예측기의 효율적인 부호화 방법 (Efficient Coding of Motion Vector Predictor using Phased-in Code)

  • H.264/AVC 비디오 압축 표준은 압축 효율을 높이기 위해 다양한 크기의 블록을 사용하여 화면 사이의 움직임 예측을 수행한다. H.264/AVC는 가변적인 블록 크기의 움직임 보상을 통해 세밀한 영역의 움직임까지 예측할 수 있어 잔여 영상을 나타내는 정보량을 효과적으로 줄일 수 있다. 복호를 위해서는 각 블록의 움직임 벡터를 전송해야 하는데, 저비트율 환경에서는 움직임 벡터 정보가 전체 비트스트림의 약 40%를 차지한다. 움직임 벡터 정보량을 줄이기 위해 비디오 부호화 전문가 그룹(VCEG)에서는 다양한 움직임 벡터 예측(Motion Vector Competition) 방법을 제안하였다. 다양한 예측 움직임 벡터를 사용하여 실제 전송해야 할 움직임 벡터 차분값(Motion Vector Difference, MVD)의 크기를 줄이기 때문에 압축 효율을 높일 수 있다. 그러나 다양한 예측 움직임 벡터를 사용하기 때문에 선택된 예측 움직임 벡터의 인덱스 정보를 복호기로 전송해야 한다. 이 논문에서는 인덱스 정보를 효율적으로 전송하기 위해 Phased-in 코드를 기반으로 한 새로운 코드워드 표를 제안했다. 실험을 통해 제안한 방법을 이용하여 동일한 화질에서 평균 약 7.24%의 비트율을 절감할 수 있었고, 동일한 비트율에서는 평균 약 0.36dB의 화질을 향상시킬 수 있었다.

갑상선의 $H\"{u}rthle$씨 세포 종양 ($H\"{u}rthle$ Cell Tumor of the Thyroid)

  • H$\"{u}$rthle cell neoplasm of the thyroid gland is an uncommon, but potentially malignant lesion. However, in many instances, the malignant potential of the H$\"{u}$rthle cell neoplasm is very difficult to judge histologically. For this reason, the biologic behavior of this tumor and its optimal treatment have come under considerable debate in recent years. In order to review the clinicopathologic features of the H$\"{u}$rthle cell neoplasm and to determine its optimal treatment modalities, we studied 26 patients with path logical proof of H$\"{u}$rthle cell tumor from January 1987 to September 1997. We also performed an immunohistochemical study using the monoclonal antibodies against antigen CD34 for the angiogenic activity of this tumor and evaluated the differences of microvessel density(MVD) between benign and malignant tumors. The age of the patients ranged from 1 to 71 years with a mean of 44.2 years. There were 6 males and 20 females(M : F= 1 : 3.3). The accuracies of fine needle aspiration biopsy and frozen section were very low; 6.3% and 34.8%, respectively. There were 20 benign tumors and 6 malignant tumors(23.1%). All the malignant tumors were microinvasive(intermediate) type which had minimal capsular invasion and most of them(5 cases) were diagnosed postoperatively. Any specific clinicopathologic differences were not seen between benign and intermediate groups. Most of the cases had conservative surgeries(15 ipsilateral lobectomy-isthmusectomy, 7 subtotal thyroidectomy) while total thyroidectomy was performed in 4 cases. Of the cases with malignant tumor, 2 had ipsilateral lobectomy-isthmusectomy, 3 had subtotal thyroidectomy and the remaining 1 had total thyroidectomy. Mean size of the tumors was 3.0 cm(0.1- 8.5 cm) in the greatest diameter and multiple tumors were seen in 6 cases(23.1 %). During the follow-up period, only one recurrence(3.8%) of benign tumor occurred but distant metastasis or cause-specific death was seen in the benign or intermediate groups. Mean MVDs of the benign(n=13) and intermediate(n=6) groups were $121.7{\pm}35.3$ and $114.3{\pm}31.7$, respectively and there was no statistical significance between them. In conclusion, because of the low accuracies of fine needle aspiration biopsy and frozen section for the H$\"{u}$rthle cell neoplasm, the extent of surgery could be individualized based on permanent pathologic examination; Conservative surgery would be adequate for patients with benign or intermediate H$\"{u}$rthle cell neoplasm and total or near-total thyroidectomy for those with definite malignancy.

SLD 조건에서 착빙 해석 정확도 개선을 위한 Wall-Droplet Interaction 수치 모델링 및 Deformation 모델 비교 연구 (Wall-Droplet Interaction Modeling and Comparative Study on Deformation Models for the Improvement of Icing Analysis Under SLD Conditions)

  • SLD 조건은 직경이 50㎛ 이상인 과냉각대형액적의 분포가 지배적인 결빙 환경을 뜻한다. SLD 조건에서는 액적의 큰 크기로 인해 wall-droplet interaction, deformation 등의 물리적 현상이 착빙 과정에 중요한 영향을 미친다. 그에 따라 SLD 효과를 수치적으로 모사하기 위한 다양한 연구가 수행되었고, 부착률을 수정하는 후처리 기법을 통해 wall-droplet interaction 현상을 고려하는 방법이 제안되었다. 그러나 이 방법은 액적이 충돌하는 벽면 특성을 제대로 고려하지 않는 반경험식(Wright Model)을 사용하기 때문에 착빙 영역에서 여전히 부착률과 부착 한계를 과대 예측한다. 이 문제를 해결하기 위해 본 연구에서는 착빙 영역을 세 개의 영역으로 구분하였다. 그리고 벽면 특성을 고려하는 Bai and Gosman rebound 모델을 바탕으로 개발한, 새로운 wall-droplet interaction 모델을 후처리 기법에 도입했다. 그리고 액적의 항력계수를 증가시키는 deformation 현상을 반영하기 위해 3가지 deformation 모델을 비교·분석하여 가장 적합한 모델을 선정했다. 앞의 내용을 바탕으로 개발한 SLD 결빙 예측 코드를 검증하기 위해 실험 데이터를 활용하여 해석을 수행했다. 그 결과 수정된 후처리 기법은 착빙 영역에서 rebound 현상에 의한 부착률 감소를 더욱 크게 예측하였고 부착 한계와 부착률 크기를 예측하는 데 있어 향상된 정확성을 보여줬다. 그리고 deformation에 의한 항력계수의 증가를 가장 크게 결정하는 Wiegand model이 실험과 가장 유사한 것을 확인할 수 있었다.

구심로 차단 동통에서의 미세 후근 진입부 절제술 (Microsurgical DREZotomy for Deafferentation Pain)

  • Objective : DREZotomy is effective for the treatment of deafferentation pain as a consequence of root avulsion, postparaplegic pain, posttraumatic syrinx, postherpetic neuralgia, spinal cord injury, and peripheral nerve injury. We performed microsurgical DREZotomy to the patients with deafferentation pain and relieved pain without any serious complication. The purpose of this study is to evaluate the usefulness of the microsurgical DREZotomy for deafferentation pain. Methods : We evaluated 4 patients with deafferntation pain who were intractable to medical therapy. Two of them were brachial plexus injury with root avulsion owing to trauma, one was axillary metastasis of the squamous cell carcinoma of the left forearm, and the last was anesthesia dolorosa after surgical treatment(MVD and rhizotomy) of trigeminal neuralgia. Preoperative evaluation was based on the neurologic examination, radiologic imaging, and electrophysiological study. In the case of anesthesia dolorosa, we produced two parallel lesions in cephalocaudal direction, 2mm in distance, from the C2 dorsal rootlet to the 5mm superior to the obex including nucleus caudalis, after suboccipital craniectomy and C1-2 laminectomy, with use of microelectrode. In the others, we confirmed lesion site with identification of the nerve root after hemilaminectomy. We performed arachnoid dissection along the posterolateral sulcus and made lesion with microsurgical knife and microelectrocoagulation, 2mm in depth, 2mm in distance, to the direction of 30-45 degrees in the medial portion of the Lissauer's tract and the most dorsal layers of the posterior horn at the one root level above and below the lesion. Results : Compared with preoperative state, microsurgical DREZotomy significantly diminished dosage of the drugs and relieved pain meaningfully. One patient showed tansient ipsilateral ataxia, but recovered soon. There was not any serious complication. Conclusion : It may be concluded that microsurgical DREZotomy is very useful and safe therapeutic modality for deafferentation pain, especially segmentally distributed intermittent or evoke pain. Complete preoperative evaluation and proper selection of the patients and lesion making device are needed to improve the result.

Histology and Histochemistry of the Male and Female Reproductive System of the Sesarmid Crab Muradium Tetragonum

  • The sesarmid crab Muradium tetragonum, inhabiting the mangrove, are considered as a key consumer of litter and thereby play an important role in the detritus food chain and energy flow in the mangrove ecosystem. The present investigation was carried out with objectives to enlighten the reproductive system of Muradium tetragonum through histological and histochemical studies. Histological organization of the testis of M. tetragonum revealed that each testis has a lobular structure consisting of several testicular lobules arranged around the collecting duct. Histology of the deferens of M. tetragonum revealed it to be composed of three-layer of tissues along the entire length:the outer connective tissue, the middle muscular and the inner epithelial layer. Based on the histological architecture these three regions are recognized as proximal vas deferens (PVD), middle vas deferens (MVD) and distal vas deferens (DVD). Histological characteristics of the ovary of M. tetragonum during different phases of ovarian development were studied. Based on the colour changes of the ovary and diameter of the oocytes five stages of ovarian development can be pronounced. Histochemical analysis of the male reproductive tissues of M. tetragonum signifies the secretion of a different biomolecule by specifying their origin in the reproductive tissue and their possible transformation into spermatophores. In the female reproductive tissues, histochemical evaluation envisaged the secretory products during different stages of ovarian development The secretory substances of the spermatheca expound on the significance of its secretion in dehiscing the spermatophore wall and in nourishing as well as protecting the spermatozoa.

심장세동의 수술요법 (The Surgical Treatment of Atrial Fibrillation in Patients Undergoing Simultaneous Open Heart Surgery)

  • 심방세동은 가장 흔한 부정맥 질환으로서,특히 승모판막 질환이 있는 경우에는 60%에 이르는높은 빈도의 유병율을 보이는데,심방세동 환자의 약 113에서 혈전 색전증을 일으키고,혈전 색전증이 생긴 환자의 약 60%에서는 사망에 이르거나 심각한 합병증이 초래되므로 심장질환에 대한 수술시,동반 심 방세동에 대한 적극적 인 치료가 고려되어야 한다. 서울대학교병원 흉부외과학교실에서는 1994년 4월부터 1995년 6월까지 심방세동을 동반한 심장질환 을 가진 20명의 환자에서 Maze 술식을 포함한 개심술을 시행하였다 대상환자들의 남녀 성비는 남 '녀 : 6 : 14 이 었으며, 평균연령은 48$\pm$11세 (31 ~66세) 였다. 1년이상 지 속된 만성 심방세동이 14례(70%)였고, 1년미만인 경우가 6례(30%)였으며,심방세동의 과거력은 평균 36$\pm$42개월(1~132개월)이었다. 수술전 혈전전색증의 과거력이 있었던 경우가 7례(35%), 좌심방내에 혈전이 있었던 경우가 9례(45%)였다. 동반 심장질환으로는 판막질환이 19례,심실중격결손증이 1례였 으며, Maze술식과 더불어 승모판막 및 대동맥판막 치환술이 5례, 승모판막 치환술을 시행한 경우가 4 fl, 승모판막 치환술 및 삼첨판\ulcorner 성형술 4례, 승모판막 성형술 3례, 승모판막 성형술 및 삼첨판막 성형 술이 1례, 승모판막 치환술 및 관상동맥 우회술이 1례, 대동맥판막 치환술 1례, 심실중격결손봉합술이 1례 였다. 대동맥차단시간은 평균 175 :41분(116~270분)이었다. 수술과 관련된 사망은 없었으며, 수술 후 심방세동의 재발이 16명(80%)에서 있었으나, 수술후 평균 41일째 규칙적인 심박동 소견을 보였다. 수술후 합병증으로서는 저심박출증을 보였던 경우가 3례 (15%), 술전 존재하였던 반신불수의 악화가 1 례, 그리고 급성 신부전이 1례씩 관찰되었다. 20명의 외래추적 관찰기간은 평균 16.5개월(10.5~24개월) 이었는데, 외래 추적기간 중 모든 환자에서 규칙적인 심박동의 소견을 보였으며, 정상 동방결절리듬을 보인 경우 17례 (85%)중에서 항부정맥제의 투여가 필요 없는 경우가 13례 (76%)이고 나머지 4례에서는 항부정맥제를 투여중이며, 접합부 율동을 보이는 3례 (15%) 중 2례에서는 항부정맥제를 투.i중이고, 1 례는 접합부 서맥으로 인공심박동기 (DDD-R type : AAI mode)의 삽입이 필요했다. 추적기간 중에 심에 코검사는 19명에서 시행하였는데, 우심방 수축력이 보이는 경우가 1 례 (95%) 이었고, 좌심방 수축력은 12례 (63%)에서 명백히 관찰되 었다. 심장질환에 대한 개심술시 Maze술식을 동반시행할 경우심장허혈시간이 길어지는 단점이 있으나, 최근의 발달된 심근보호법의 적용으로 수술에 따른 위험을 최소화할 수 있으므로 심방세동의 적극적 인 치료를 고려하여야 할 것으로 사료된다.

비소세포폐암조직에서 XIAP 발현과 고사지수 및 수술 후 예후와의 관계 (Relationship between Expression of XIAP Protein in Operable Non-small Cell Lung Carcinomas and Apoptosis Index and Postoperative Prognosis)

  • 연구 배경 : 세포자멸사의 장애는 발암, 암의 진행, 화학치료시 내성 등에 중요한 역할을 한다. XIAP는 IAP군 중에 가장 강력한 caspase 억제제로 알려져 있다. 본 연구는 비소세포폐암에서 XIAP의 면역조직화학적 발현이 종양진행이 환자생존율에 미치는 영향을 확인해 보고자 시행되었다. 방 법 : 수술적 절제가 시행된 80예의 비소세포 폐암종의 조직에서 XIAP의 면역 조직학적 발현을 조사하여 임상병리학적 인자들[환자의 연령, TNM 병기, TNMpT, TNM-pN, VEGF, microvessel density(MVD), PCNA index, apoptotic index (AI)]과 생존율과의 연관을 분석하였다. 결 과 : 편평세포암종 46예 중 42예(91.3%)에서, 샘암종 34예 중 21예(61.8%)에서 양성을 보여 종양의 조직형별 비교시 편평세포암종은 샘암종에 비해 유의하게 높은 XIAP의 발현을 보였다(p=0.001). 각 조직형내에서 비교시 샘암종의 경우 XIAP는 58세이상의 고 연령군 및 VEGF의 발현과 유의한 상관관계를 보였지만(p=0.028, p=0.014, respectively) 편평세포암종의 경우 XIAP는 모든 임상병리학적 인자들과 상관관계를 보이지 않았다. TUNEL 염색으로 결정된 AI는 XIAP 양성군이 $2.5{\pm}4.9%$, XIAP 음성군이 $18.5{\pm}28.9%$로서 후자에서 유의하게 높은 수치를 보였다(p=0.001). AI는 XIAP를 제외한 다른 임상병리학적 인자들과는 상관관계를 보이지 않았다. 생존 여부의 확인이 가능했던 72예에서 XIAP 음성군의 중앙 생존기간은 29.89개월, XIAP 양성군의 중앙 생존기간은 42.5개월로서, 후자에서 술 후 생존 기간은 더 길었지만 통계학적 차이를 보이지는 않았다. 결 론 : 비소세포폐암종에서 XIAP는 종양의 조직형, 특히 편평세포암종에서 높은 발현과, 종양의 AI와 역의 상관관계를 보였다. 그러나 XIAP의 발현이 임상병리학적 예후인자들 및 생존율과 유의한 관련성을 보이지 않은 것은 생체 조직에서 XIAP의 생물학적 역할은 매우 복잡할 수 있다는 것을 암시하므로 향후 이의 생물학적 역할과 관련 물질들에 대한 연구가 좀 더 진행되어야 할 것으로 사료된다.
