Purpose: Pancreatic carcinoma is one of the most malignant tumors of the alimentary system, with relatively high incidence rates. The purpose of this study was to assess the efficacy and safety of two regimens for advanced pancreatic carcinoma: continuous transarterial infusion versus systemic venous chemotherapy with gemcitabine and 5-fluorouracil. Methods: Of the 48 patients with advanced pancreatic carcinoma receiving chemotherapy with gemcitabine and 5-fluorouracil, 24 received the selective transarterial infusion, and 24 the systemic chemotherapy. For the continuous transarterial infusion group (experimental group), all patients received gemcitabine 1000 mg/$m^2$, given by 30-minute transarterial infusion, on day 1 of a 4-week cycle for 2 cycles, and a dose of 600 mg/$m^2$ 5-fluorouracil was infused on days 1~5 of a 4-week cycle for 2 cycles. For the systemic venous group (control group), gemcitabine and 5-fluorouracil were infused through a peripheral vein, a dose of 1000 mg/$m^2$ gemcitabine being administrated over 30 min on days 1 and 8 of a 4-week cycle for 2 cycles, and a dose of 600 mg/$m^2$ 5-fluorouracil was infused on days 1~5 of a 4-week cycle for 2 cycles. The effectiveness and safety were evaluated after 2 cyclesaccording to WHO criteria. Results:The objective effective rate in transarterial group was 33.3% versus 25% in the systemic group, the difference not being significant (P=0.626). Clinical benefit rates(CBR) in the transarterial and systemic groups were 83.3% and 58.3%, respectively (P=0.014). The means and medians for survival time in transarterial group were higher than those of the systemic group (P < 0.005). at the same time, the adverse effects did not significantly differ between the two groups (P > 0.05). Conclusion: Continuous transarterial infusion chemotherapy with gemcitabine and 5-fluorouracil could improve clinical benefit rate and survival time of patients with advanced pancreatic carcinoma, compared with systemic venous chemotherapy. Since adverse effects were limited in the transarterial group, the regimen of continuous transarterial infusion chemotherapy can be used more extensively in clinical practice. A CT and MRI conventional sequence can be used for efficacy evaluation after chemotherapy in pancreatic carcinoma.
최근 들어 심부전증을 치료하기 위한 새로운 방법으로 골수줄기세포를 심근에 이식하여 신생혈관을 생성하거나 새로운 심근 생성을 조장하여 심장기능을 개선시키려는 노력이 활발히 진행되고 있다. 저자들은 심근경색 후 심근의 기능이 저하된 환자에서 심장박동 상태에서 관상동맥우회술 (OPCAB)과 우회술이 불가능한 부위의 골수줄기세포 이식을 동시에 시행한 연구결과를 보고하고자 한다. 대상 및 방법: 관상동맥우회술과 골수줄기세포 이식을 동시에 시행 받았던 4명의 남자 환자를 대상으로 하였다 대상 환자의 평균연령은 58세(48∼73세)로 모두 불안정성 협심증으로 심근경색의 기왕력이 있었다. 환자의 장골에서 골수를 채취한 후 단핵세포만을 분리하였으며(평균 세포 수 1.5 ${\times}$$10^{9}$ 개), 이 중에는 평균 6.7${\times}$$10^{6}$ CD34 + 세포와 3.7 ${\times}$$10^{6}$ AC133 + 세포가 포함되었다. 분리된 단핵세포는 10 cc로 농축하였다. 수술은 관상동맥우회술이 가능한 좌전하행지 부위에는 OPCAB을 시행하고, 나머지 우회로술이 불가능한 심근에는 분리한 단핵세포를 이식하였다. 이식 전 모든 환자는 심초음파, MIBI scan 및 자기공명영상(MRI)으로 심근의 기능을 검사하였다. 수술 후 1개월에 심초음파 및 MIBI scan을 시행하여 수술 전과 비교하였다 결과: 평균 이식편수는 2개였으며, 수술 후 사망이나 부정맥 등 합병증은 없었다. 수술 후 1개월 추적검사에서 모든 환자의 증상은 호전되었으며, 심초음파상에서 좌심실 박출계수는 49%에서 55%로 개선되었고, MIBI scan에서 세포이식 부위의 현저한 관류 개선이 모든 환자에서 있었다. 결론: 심장박동 상태에서 관상동맥우회술(OPCAB)과 우회술이 불가능한 부위에 골수줄기세포 이식을 동시에 시행하는 것은 안전하면서도 심장기능의 개선을 보여주었다. 그러나 심장기능의 개선이 골수줄기세포 단독의 효과라고 단정하기는 어려우며, 따라서 대조군을 이용한 연구가 이어져야 할 것으로 생각된다.
PET과 융합된 MR 영상에서 MR coil은 PET 영상용 감쇄보정에 있어서 위치 정보 수집에 관한 오류를 극복하기 위한 노력이 시도 되어 왔다. 본원에서는 이런 문제점을 보완한 Three modality system (PET/CT +MR)을 이용하고 있다. 이 논문에서는 MR torso coil이 CT 감쇄보정에 미치는 영향을 평가하였다. MR torso coil을 장착하고 CT로 감쇄보정한 PET 영상에서 인공물에 대한 평가를 하였다. 균일도 평가용 팬텀과 1994 NEMA 실린더 팬텀을 사용하여, CT의 관전압과 관전류를 변경해가며 표준화섭취계수에 대한 변화를 추적하였다. MR torso coil안에 있는 금속 물질로 인한 줄무늬 인공물이 관찰되었다. CT 검사에 있어서 관전압과 관전류를 변화 시키면 선 감쇄계수도 일정하게 변화하였다. 관전압과 관전류의 수치가 높아짐에 따라 PET 영상에서의 표준화섭취계수가 증가하는 것을 볼 수 있었다. PET/CT 검사 시, MR torso coil은 금속으로 인한 줄무늬 인공물을 생성하며, 감쇄보정에 있어서 잠재적인 오류를 불러올 수 있다. 이와 같은 오류를 감소시키기 위해서는 첫째, 감쇄보정 알고리즘의 보완, 둘째, MR coil을 사용하지 않은 검사방법의 연구, 셋째, PET/CT 감쇄보정에 영향을 주지 않는 MR coil소재의 개발 등이 요구된다.
이 연구에서는 경막바깥신경성형술 시행에 있어서 중요한 엉치뼈틈새에서 경막주머니종말부 및 척수원뿔 사이의 거리를 자기공명영상에서 계측하여 한국인의 생체 연구에 대한 기초 자료를 제공하고자 한다. 허리통증으로 자기공명영상을 촬영한 환자 중 척추압박골절이나 척추전방전위증 그리고 기형 등이 없는 환자 200명(남자 88명, 여자 112명)을 대상으로 하였으며, 평균 연령은 54.3세(20~84세), 평균 신장은 161.3 cm(135~187 cm)이었다. T2강조 자기공명영상을 이용하여 엉치뼈틈새에서 경막주머니종말부 그리고 척수원뿔 사이 거리를 계측하여 성별 및 신장에 따른 상관관계를 분석하였다. 엉치뼈틈새꼭지에서 경막주머니종말부까지 거리는 $62.8{\pm}9.4mm$, 엉치뼈틈새꼭지에서 척수원뿔까지 거리는 $232.2{\pm}21.8mm$, 엉치뼈틈새꼭지에서 경막주머니종말부 사이 최소 거리는 34.8 mm, 최대 거리는 93.9 mm, 엉치뼈틈새꼭지에서 척수원뿔 사이 최소 거리는 155.0 mm, 최대 거리는 284.0 mm이었다. 엉치뼈틈새꼭지에서 경막주머니종말부까지, 그리고 엉치뼈틈새꼭지에서 척수원뿔까지 거리는 모두 여성이 남성보다 짧게 나타났고(p<0.05), 엉치뼈틈새꼭지에서 경막 종말부 및 척수원뿔 사이 거리는 신장에 대해 유의한 상관관계를 보였다(p<0.01). 이 연구의 결과는 한국 성인의 엉치뼈틈새에서 경막주머니종말부 및 척수원뿔 사이 거리에 대한 생체 계측 자료를 제공하며, 경막바깥공간에 대한 임상적 시술의 안전성 확보와 한국인의 체형에 맞는 카테터 개발에 도움이 될 것이라 생각한다.
중추성 수면 무호흡은 수면 중 상기도 저항이 없을 때 흉부와 복부의 호흡 노력 없이 10초 이상의 호흡부전을 보이는 것이 특징이다. 본 증례에서 뇌자기공명영상 확산 강조 영상검사를 통해 외측 연수 경색의 진단을 받은 환자가 수면다원검사를 통해 중추성 수면 무호흡 진단 기준에 미치지 못하는 호흡부전과 중추성 수면 무호흡이 관찰되었다. 호흡부전과 중추성 수면 무호흡에 대한 일차 치료는 CPAP을 사용하였다. CPAP을 사용한 후 수면다원검사와 이전의 수면다원검사를 비교한 결과 무호흡-저호흡 지수(42.5/hr→82.8/hr)와 각성 지수(21.7/hr→40.8/hr)는 악화되었다. 이에 이차 치료로 ASV를 사용하여 적정압력검사를 시행하였다. 이전의 수면다원검사와 비교한 결과 무호흡-저호흡 지수(42.5/hr→8.6/hr)가 상당히 호전되었다. 이러한 결과로 ASV는 중추성 수면 무호흡과 호흡부전에 대한 좋은 치료방법이 될 수 있다.
목적: 소아 환자에서의 검사 시 필수적인 수면유도를 위해 사용되는 약물이 시술자에 따라 선택이 다양하며, 대상 환자에 따라 그 효과 또한 다양하게 나타난다. 본 연구는 사용된 약물에 따라 약물의 효율성과 안정성을 신경계 질환자와 비 신경계 질환자를 비교 분석하고자 하였다. 방 법: 2007년 2월부터 2007년 8월까지 전북대학교 병원 소아과에서 뇌파 또는 신경 방사선학적 검사를 위해 수면유도를 시행한 172명과 요로감염 환자 등 비 신경계 환자 274명, 총 446명의 소아과 환자를 대상으로 수면 유도 성공여부, 추가 약물, 부작용 발현 등 전향적인 방법으로 연구를 하였다. 결 과: 대상 환자는 신경계 질환자 172명(남: 101명, 여: 71명), 비 신경계 질환자 274명 (남: 211명, 여: 63명)이었다. 수면유도에 사용 된 약물은 chloral hydrate(150/172명, 274/274명으로 각각 87.20%, 100.0% 평균 용량: 50.0 mg/kg), ketamine 17/172 (9.8%), midazolam 5/172 (2.9%) 순서로 사용 되었다. 수면유도의 성공률은 chloral hydrate의 경우 420/424 (99.1%), ketamin은 12/17 (70.6%), midazolam은 3/5 (60%)였다. 실험을 진행하는 동안 총 수면 유도 성공률은 435/446 (97.5%)였으며, 부작용의 발생률은 3.1%로 검사 시행 중에 환자가 깨거나 구토를 하는 것이었으며, 대개는 경미하였고 임상적으로 유의한 부작용은 아니었다. 신경계 환자의 경우 172명이 분석되었는데 뇌파를 위한 수면유도는 136명, DMSA renal scan은 5명, 뇌 전산화 단층 촬영은 3명, 뇌 자기 공명 영상은 28명이었다. 이들 그룹의 성공률은 뇌파의 경우 94.9%, DMSA renal scan 100%, 뇌 전산화 단층 촬영 100 %, 뇌 자기 공명 영상 89.3%였다. 비신경계 환자의 경우 274명을 분석하였고, 모두 DMSA renal scan을 시행하였으며, 성공률은 99.6%였다. 결 론: 수면유도는 영 유아의 경우 소아과 영역에서 술기를 실시하는데 중요한 것으로, 수면유도를 위한 약물에 관하여 여러 논란이 존재한다. 각각의 수면유도 약물사이에 성공률과 부작용 발생률에 따라 술기에 맞게 약물을 선택하여야 할 것으로 여겨지며, 환자 요인 분석에 따르면 신경계 환자의 경우 다른 환자 군에 비교하였을 때 수면유도가 힘든 점으로 미루어보아 일차 약물의 선택에 있어서 다른 기준이 요구될 것으로 보인다.
Background & Objectives: Frameless fractionated stereotactic radiotherapy(FFSRT) is a modification of stereotactic radiosurgery(SRS) with radiobiologic advantage of fractionation without losing mechanical accuracy of SRS. Local recurrence of head and neck cancer at or near skull base benefit from reirradiation. Main barrier to successful palliation is dose limitation secondary to normal tissue tolerance. We try to evaluate the efficacy and safety of FFSRT as a new modality of reirradaton in these challenging patients. Materials & Methods: Seven patients with recurrent head & neck cancer involving at or near skull base received FFSRT from September 1995 to November 1997. Six patients with nasopharyngeal cancer had received induction chemotherapy and curative radiation therapy. One patient with maxillary sinus cancer had received total maxillectomy and postoperative radiation therapy as a initial treatment. Follow-up ranged from 11 to 32 months with median of 24 months. Three of 7 patients received hyperfractionated radiation therapy(1.1-1.2Gy/fraction, bid, total 19.8-24Gy) just before FFSRT. All patients received FFSRT(3-5Gy/fraction, total 15-30Gy/5-10fractions). Chemotherapy(cis-platin $100mg/m^2$) were given concurrently with FFSRT in four patients. Second course of FFSRT were given in 4 patients with progression or recurrence after initial FFSRT. Because IF(irregularity factor; ratio of surface area of target to the surface area of sphere with same volume as a target) is too big to use conventional stereotactic RT using multiple arc method for protection of radiation damage to critical normal tissue, all patients received FFSRT with conformal method using irregular static ports. Results: Five of 7 patients showed complete remission in follow-up CT &/or MRI. Three of these five patients who developed marginal, in-field, and out-field recurrences, respectively. Another one of complete responders has been dead of G-I bleeding without evidence of local recurrence. One partial responder who showed progressive disease 15 months after initial FFSRT has received additional FFSRT, and then he is well-being with symptomatic improvement. One minmal responder who showed progression of locoregional disease 9 months after $1^{st}$ FFSRT has received 2nd FFSRT, and then he is alive with stable disease. Five of 7 case had showed direct invasion to skull base and had complaint headache and various symptoms of cranial nerve involvement. Four of these five case showed improvement of neurologic symptoms after FFSRT. No significant neurologic complicaltion related to FFSRT was observed during follow-up periods. Tumor volumes were ranged from 3.9 to 50.7 cc and surface area ranged from 16.1 to $114.9cm^2$. IF ranged from 1.21 to 1.74. The average ratio of volume of prescription isodose shell to target volume was 1.02 that indicated the improvement of target coverage and dose distribution with FFSRT with conformal method compared to target coverage with FFSRT with multiple arc method. Conclusion: Our initial experience suggests that FFSRT with conformal method was relatively effective and safe modality in the treatment of recurrent head and neck cancer involving at or near skull base. Treatment benefit included good palliation of symptoms and reasonable radiographic response. However, more experience and additional follow-up are needed to better assess its ultimate role in treating these challenging patients.
