심장수술시 Myeloperoxidase 생성의 증가와 심장 및 폐에 대한 영향 (Increase of Myeloperoxidase Production and Effect on The Heart and Lung during Cardiac Surgery)

  • 심장수술에 따른 백혈구의 활성화는 여러 염증성 물질을 생성시켜 수술 후 병태생리에 깊이 관여한다. 본 연구는 심장수술 동안 백혈구의 과립성분인 myeloperoxidase의 농도 변화와 심장 및 폐에 대한 영향을 규명하기 위해 시행되었다. 심장수술 전후로 말초혈액 총 백혈구 수 및 분획세포 수와 관상정맥 혈액의 myeloperoxidase 농도 (MPO) 및 troponin-T 농도 (TnT), 그리고 폐혈관 저항을 측정하였다. 수술 직후, 호중구 수의 상승에 기인한 총 백혈구 수의 증가가 있었고 (p<0.01), MPO 및 TnT 역시 증가하였으나 (p<0.05), 폐혈관 저항은 감소하였다(p<0.05). 수술 후, MPO는 TnT, 총 백혈구 수, 호중구 수, 그리고 심장수술 소요시간과 각각 양의 상관관계가 있었다(p<0.05). 수술 후 폐혈관 저항은 총 백혈구 수 및 호중구 수와 각각 음의 상관성이 있었다(p<0.05). 본 연구의 결과들은 심장수술이 호중구로 부터 MPO 분비를 증가시키며, 유리된 MPO는 폐보다는 심장에 유해한 효과를 미칠 수 있고 호중구의 또 다른 기전이 수술 후 폐 손상에 관여할 수 있음을 시사하고 있다.

Association of the Myeloperoxidase $^{-463}G{\to}A$ Polymorphism with Helicobacter pylori-induced Atrophic Gastritis

  • Although only a minority of the infected individuals develops atrophic gastritis and the malignancy, factors governing clinical outcomes subsequent to Helicobacter pylori (H. pylori) infection have not yet been defined. H. pylori infection is characterized by extensive infiltration of neutrophils. Myeloperoxidase (MPO) in neutrophils amplifies the oxidative potential of hydrogen peroxides that induce gastric mucosal damage, thus MPO is suspected to play a role in H. pylori-induced gastric injury. Therefore, we explored the association of host MPO genetic polymorphism with atrophic gastritis upon H. pylori infection. Biopsy specimens taken from the gastric mucosa were examined histologically in 87 patients. The PCR-RFLP assay was used to characterize MPO genotypes. The distributions of MPO genotypes were MPO (G/G) 82% and MPO (G/A) 18%. None of MPO (A/A) genotype was observed. A strong positive correlation between the levels of neutrophil infiltration and gastric atrophy found only in MPO (G/G) but not in MPO (G/A) genotype. These results suggest that MPO genotype is a critical determinant in the pathogenesis of atrophic gastritis subsequent to H. pylori infection. Further works need to clarify the functional relevance of MPO genetic polymorphisms on gastric cell injury.

보양환오탕(補陽還五湯)이 흰쥐 뇌출혈 손상에 미치는 영향에 대한 면역조직화학 연구 (Neuroprotective Effects of Boyanghwano-tang on Intracerebral Hemorrhage-Induced Rats Using Immunohistochemistry)

  • This study was performed in order to evaluate the neuroprotective effect of Boyanghwano-tang (BYT) water extract on intracerebral hemorrhage (ICH). ICH was induced by the stereotaxic intrastriatal injection of bacterial collagenase type IV in Sprague-Dawley rats. BYT was orally given once a day for 3 days after ICH. Hematoma volume and percentage edema were examined. As imflammatory markers, myeloperoxidase (MPO)-positive neutrophils infiltration and iNOS expression in the peri-ICH regions were examined using immunohistochemistry. As cellular damage markers, c-Fos, Bax, and HSP72 positive cells in the peri-ICH regions were measured also. BYT significantly reduced the hematoma volume and percentage edema of the ICH-induced rat brain. In the peri-hematoma regions, BYT significantly reduced MPO-positive neutrophil infiltration and iNOS expression of the ICH-induced rat brain. Additionally, BYT significantly reduced c-Fos, Bax, and HSP72 positive cells in the peri-hematoma regions of the ICH-induced rat brain. These results suggest that BYT plays a neuroprotective role against ICH through suppression of inflammatory responses, apoptosis and cellular damage.

천궁(川芎)의 뇌조직출혈 흰쥐 힝염증반응에 대한 연구 (Anti-inflammatory Effects of Cnidium Rhizoma against Intracerebral Hemorrhage in Rats)

  • Objectives : Inflammation is mediated by cellular components, such as leukocytes and microglia, and molecular components, including cytokines, extracellular proteases, and reactive oxygen species. Cnidium Rhizoma effects the anti-inflammatory, antioxidant, suppression of the microglia activation and protection of the nerve cell injury. For this reason, we investigated the anti-inflammatory effects of water extracts of Cnidium Rhizoma on intracerebral hemorrhage (ICH). Method : ICH was induced by the stereotaxic intracerebral injection of bacterial collagenase type IV (0.23 $U/{\mu}{\ell}$, 0.1 ${\mu}{\ell}/min$) in Sprague-Dawley rats. We orally administrated once 3 hours after ICH, then 2 times at 24-hour intervals the water extracts of Cnidium Rhizoma (500 mg/kg), myeloperoxidase (MPO) was observed by using immunofluorescense and expression of inducible nitric oxide synthase (iNOS), tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) and microglia were observed by using immunohistochemistry. Results : Infiltration of MPO expressing neutrophil, expression of iNOS and TNF-${\alpha}$ and activated microglia were significantly reduced in peri-hematoma of the rats fed with water extracts of Cnidium Rhizoma. Conclusion : These results demonstrated that water extracts of Cnidium Rhizoma suppressed an inflammatory reaction through inhibition of MPO, iNOS and TNF-${\alpha}$ positive cell and activated microglia number in peri-hematoma of ICH-induced rats.

척수압박손상 흰쥐의 척수조직 염증반응에 황금(黃芩)이 미치는 영향 (Effects of Root of Scutellariae Radix against Inflammatory Response in the Spinal Cord Contusion Injury in Rats)

  • Objectives : This study was performed to evaluate the effects of root of Scutellariae Radix(SR) water extract against inflammatory response in the spinal cord injury(SCI). Methods : SCI was induced by mechanical contusion following laminectomy of 10th thoracic vertebra in Sprague-Dawley rat. SR was orally given once a day for 7days after SCI. Myeloperoxidase(MPO) positive neutrophils infiltration was examined. Inducible nitric oxide synthase(iNOS) and tumor necrosis factor-${\alpha}$(TNF-${\alpha}$) expressions were observed with immunohistochemistry. Glial fibrillary acidic protein(GFAP) positive astrocytes were examined using immuno-fluorescence. Results : 1. SR reduced MPO-positive neutrophils infiltration in peri-damage regions of the contusive SCI-induced rats. 2. SR reduced iNOS positive cells in the white matter of the contusive SCI-induced rats. 3. SR reduced TNF-${\alpha}$ positive cells in the gray and white matter of the contusive SCI-induced rats. 4. SR reduced cell number and size of astrocytes in peri-damage regions of the contusive SCI-induced rats. Conclusions : These results suggest that SR plays an inhibitory role against inflammatory response in the SCI.

황련해독탕(黃連解毒湯)이 Collagenase에 의한 흰쥐의 뇌출혈 및 뇌부종에 미치는 영향 (Effects of Hwangnyeonhaedok-tang on Cerebral Hemorrhage and Edema in Collagenase Induced-ICH Rats)

  • Objectives: Brain edema is brain swelling that occurs due to the accumulation of excess water in the brain parenchyma. AQP4 and AQP9 are water-channel proteins expressed strongly in the brain and control water fluxes into and out of the brain parenchyma. This study was conducted to evaluate effect of Hwangnyeonhaedok-tang on brain edema and intracerebral hemorrhage. Methods: Intracerebral hemorrhage was induced by intrastriatal injection of type IV collagenase(0.23 U/${\mu}l$, 0.1 ${\mu}l$/min) into Sprague-Dawley rat brains. Hwangnyeonhaedok-tang water extract(1,000 mg/kg) was administered orally three times every 20 hours from 4 hours after ICH operation. Then hematoma volume, brain edema percentage, and water content of brain tissue were measured. Immuno-histochemistry was performed for AQP4 and AQP9 expressions in the brain sections and area % of immuno-labeling was analyzed with image analyzing system. Results: 1. Water extract of Hwangnyeonhaedok-tang reduced hematoma volume of ICH induced rat. 2. Water extract of Hwangnyeonhaedok-tang reduced MPO positive neutrophils in the perihematoma of the ICH induced rat. 3. Water extract of Hwangnyeonhaedok-tang reduced brain edema percentage and water content of brain tissue of ICH induced rat. 4. Water extract of Hwangnyeonhaedok-tang reduced AQP4 immuno-positive cells in the perihematoma of the ICH induced rat. 5. Water extract of Hwangnyeonhaedok-tang reduced AQP9 immuno-positive cells in the perihematoma of the ICH induced rat. Conclusions: These results suggest that Hwangnyeonhaedok-tang decreases intracerebral hemorrhage and brain edema by means of downregulating AQP4 and AQP9 expressions in the brain.

Platelet-activating Factor에 의한 급성폐손상에서 호중구성 산화성 스트레스에 관여하는 Cytosolic Phospholipase A2 활성도의 변화 (Cytosolic Phospholipase A2 Activity in Neutrophilic Oxidative Stress of Platelet-activating Factor-induced Acute Lung Injury)

  • 연구배경: 급성호흡곤란증후군의 병인론에 관여하는 PAF의 역할이 다양하고 중요하므로 본 연구에서는 PAF의 또 다른 작용의 가능성, 즉 $cPLA_2$의 활성화(retrograde activation of $cPLA_2$ by PAF)의 가능성을 검사하고자 하였다. 즉, $cPLA_2$의 활성화에 따른 염증성 지질분자의 생성이 산소기의 생성과정을 증폭시키고 이 때 생성된 PAF가 역으로 $cPLA_2$를 활성화시키는지를 확인하기 위하여 본 연구는 고안되었다. 방 법: 흰쥐에서 급성폐손상을 유도하기 위하여 $5{\mu}g$의 PAF를 0.5 ml의 0.25% bovine serum albumin 용액과 혼합한 뒤 기도 내로 직접 분무하거나 0.5 ml의 4.5 mM의 과산화수소를 기도 내로 분무하였다. 대조군의 경우는 0.5 ml의 생리적 식염수를 기도 내로 분무하였다. 5 시간 후에 단백누출지수 측정, 폐장의 MPO 활성도 측정, 폐포 세척액 내의 호중구 산정, CINC 측정, NBT 및 cytochrome-c 환원검사를 시행하였다. 또한 폐장 및 호중구에 서의 $cPLA_2$ 활성도의 측정 및 광학현미경과 전자현미경을 이용하여 형태학적 관찰을 시행하였다. 결 과: PAF투여 후 단백누출지수, MPO, BAL내의 호중 구의 수 및 CINC의 농도가 대조군에 비하여 유의하게 증가하였다. NBT및 cytochrome-c환원검사의 결과 PAF는 호중구의 respiratory burst를 현저히 증가시키고, 분리된 사람의 호중구에서도 산소기의 생성을 현저히 증가시켰 다. 동시에 PAF는 분리된 호중구 및 폐장의 $cPLA_2$의 활성도도 증가 시켰다. 폐장 내로 투여한 과산화수소는 폐장의 $cPLA_2$활성도를 대조군에 비하여 현저히 증가시켰다. 결 론: $cPLA_2$의 활성화에 따라 생성된 PAF는 호중구의 산소기 생성을 증가시켜 폐장 내의 산화성스트레스를 유발하고 동시에 이때 생성된 산화기는 $cPLA_2$를 활성화시키며 PAF 또한 $cPLA_2$의 활성도를 증가시켜 PAF가 급성호흡 곤란증후군의 병인론에 관여하는 것으로 생각된다.
