Purpose : Pneumococcal protein vaccine based on pneumococcal surface protein A (PspA) is in development with the potential to offer a broad range of protection against different strains. PspA elicits protection in mice against fatal sepsis as well as carriage and lung infection. This study was performed to investigate the frequency of PspA families among Streptococcus pneumoniae recovered from Korean children and adults. Methods : A total of 89 pneumococcal isolates was included in the study. They were capsule serotyped by the slide agglutination assay with commercial antisera. PspA families were determined with polymerase chain reaction using the pair of primers for family 1 and family 2. Results : Seventeen pneumococcal serotypes were found in a total of 89 isolates. PspA typing was able to ascertain 79 of the 89 isolates (88.8 percent). Among these, 20 (22.5 percent) isolates were family 1 PspA, 59 (66.3 percent) were family 2. Moreover, because 9 (10.1 percent) isolates were of positive reactions for both, families 1 and 2 primers, the potential coverage of PspA vaccine was 98.9 percent. PspA families were not associated with age group, source of isolates, or penicillin susceptibility. However, the relative distribution of family 1 isolates to family 2 isolates was significantly different over capsular serotypes. Conclusion : The finding that 98.9 percent of Korean isolates belonging to PspA families 1 and 2 support the hypothesis that a human PspA vaccine covering a few PspA families could be broadly effective. The monitoring of the PspA families derived from large population-based isolates will be necessary in the context of vaccine development.
The purpose of this study was to investigate quantitative data on the difference in breast entrance surface dose with changes in focus-film distance, patient posture (anteroposterior-posteroanterior), thoracic wall thickness, rib bone thickness, lung tissue thickness, tube voltage, and high-voltage rectification method in Whole Spine Scanography, which is necessary for the treatment of scoliosis patients. Given a tube voltage of 90 kVp, kerma of 0.1 mGy, focus-film distance of 260 cm, tube voltage ripple rate of 0, filter thickness of 3.5 mm, and thickness of patient's thoracic wall of 120 mm as an X-ray exposure condition, from the simulation results using the Simulation of X-ray Spectra program to confirm the reduction effect of breast entrance surface dose according to the patient's posture (AP and PA), there was a dose reduction effect in aluminum filter thickness of 2.6 times at 3.5 mm, 25.7 times the thoracic wall thickness at 120 mm, 1.43 times higher tube voltage, and 0 to 1.14 times the tube voltage ripple rate. The total dose reduction effect was about 109 times. In order to confirm the dose reduction effect of RANDO phantom posture (AP and PA), from the results of the measurements taken under the conditions that the focus-film distance was 260 cm, the tube voltage was 90 kVp, the tube current was 270 mA, the exposure time was 0.31 sec, and the tube voltage ripple rate of X-ray generators was 0, the entrance surface dose reduction effect of the breast in the PA position was found to be 20.56 times lower than that of the AP position.
Air pollution levels of gases and aerosol particles inside the Jeongneung and Hongjimun tunnels of the Naebu express way in Seoul were investigated through on-road measurement using a mobile emission laboratory (MEL) on February 8, 2011. The concentrations of $NO_x$, $CO_2$, number concentration of particles ranging 21-560 nm, and surface area of particles deposited on a human lung almost linearly increased with increasing distance from the tunnel entrance, and decreased rapidly before the tunnel exit. This trend was observed regardless of tunnel length and driving directions, which thought to be caused by semi-transverse ventilation facilities of the tunnels. The concentration increments per 1-m distance for $NO_x$, $CO_2$, deposited particle surface area, and number of particles ranging 21-560 nm were 0.61~0.80 ppb, 0.16~0.21 ppm, $0.20{\sim}0.29{\mu}m^2/cm^3$, and 117~192 particles/$cm^3$, respectively. Average pollution levels inside the two tunnels for $CO_2$, deposited particle surface area, and number of particles >5.6 nm ranged 681~748 ppm, $246{\sim}381{\mu}m^2/cm^3$, and $2.4{\sim}6.7{\times}10^5$ particles/$cm^3$, respectively. In case of $NO_x$, the maximum concentration exceeded 1 ppm. These pollution levels inside the tunnels are much higher than those at urban background sites. This result can be utilized as basic data to evaluate the effectiveness of present ventilation system for reducing the pollution level caused by vehicles inside the tunnels.
PURPOSE. The objective of this study was to conduct an in vitro comparative evaluation of polished and laser-dimpled titanium (Ti) surfaces to determine whether either surface has an advantage in promoting the attachment of epithelial-like cells and fibroblast to Ti. MATERIALS AND METHODS. Forty-eight coin-shaped samples of commercially pure, grade 4 Ti plates were used in this study. These discs were cleaned to a surface roughness (Ra: roughness centerline average) of 180 nm by polishing and were divided into three groups: SM (n=16) had no dimples and served as the control, SM15 (n=16) had $5-{\mu}m$ dimples at $10-{\mu}m$ intervals, and SM30 (n=16) had $5-{\mu}m$ dimples at $25-{\mu}m$ intervals in a $2{\times}4mm^2$ area at the center of the disc. Human gingival squamous cell carcinoma cells (YD-38) and human lung fibroblasts (MRC-5) were cultured and used in cell proliferation assays, adhesion assays, immunofluorescent staining of adhesion proteins, and morphological analysis by SEM. The data were analyzed statistically to determine the significance of differences. RESULTS. The adhesion strength of epithelial cells was higher on Ti surfaces with $5-{\mu}m$ laser dimples than on polished Ti surfaces, while the adhesion of fibroblasts was not significantly changed by laser treatment of implant surfaces. However, epithelial cells and fibroblasts around the laser dimples appeared larger and showed increased expression of adhesion proteins. CONCLUSION. These findings demonstrate that laser dimpling may contribute to improving the peri-implant soft tissue barrier. This study provided helpful information for developing the transmucosal surface of the abutment.

Purpose : The purpose of this study was to evaluate the surface and superficial dose for patients requiring postmastectomy radiation therapy(PMRT) with different treatment techniques. Materials and Methods : Computed tomography images were acquired for the phantom(I'mRT, IBA) consisting of tissue equivalent material. Hypothetical chestwall and lung were outlined and modified. Five treatment techniques(Wedged Tangential; WT, 4-field IMRT, 7-field IMRT, TOMO DIRECT, TOMO HELICAL) were evaluated using only 6MV photon beam. GafChromic EBT3 film was used for dose measurements at the surface and superficial dose. Surface dose profiles around the phantom were obtained for each treatment technique. For superficial dose measurements, film were used inside the phantom and analyzed superficial region for depth from 1-6mm. Results : TOMO DIRECT showed the highest surface dose by 47~70% of prescribed dose, while 7-field IMRT showed the lowest by 35~46% of prescribed dose. For the WT, 4-field IMRT and 7-field IMRT, superficial dose were measured over 60%, 70%, and 80% for 1mm, 2mm, and 5mm depth, respectively. In case of TOMO DIRECT and TOMO HELICAL, over 75%, 80%, and 90% of prescribed dose was measured, respectively. Surface and superficial dose range were uniform in overall chestwall for the 7-field IMRT and TOMO HELICAL. In contrast, Because of the dose enhancement effect with oblique incidence, The dose was gradually increased toward the obliquely tangential angle for the WT and TOMO DIRECT. Conclusion : For PMRT, TOMO DIRECT and TOMO HELICAL deliver the higher surface and superficial doses than treatment techniques based linear accelerator. It showed adequate dose(over 75% of prescribed dose) at 1mm depth in skin region.
From Jan. 1996 to Oct. 1997, 29 pigs with 40-70 days old showing dyspnea inappetite and polyserositis were collected and carried out necropsy, bacterial culture, histopathology, avidin biotin complex(ABC) stain, fluorescent antibody(FA) test, and electron microscopy. In the study, 4 strains from 3 pigs were isolated from meninges, pleura and synovial fluid and also were identified as Haemophilus parasuis serovar 5. Main histopathological lesions of 29 pigs with polyserositis were frequently composed of fibrinous peritonitis(27), pleurisy(22), interstitial pneumonia(21), fibrinous epicarditis(20), fibrinopurulent meningitis(8) and synovitis(4). By ABC stain, 11/29(37.9%) pigs with polyserositis were confirmed to have H parasuis serovar 5 in the cytoplasm of macrophages and neutrophils in cerebral meninges, epicardium, pleura surface of lung or serosa of spleen. ABC stain(20.8~40.0%) to detect H parasuis serovar 5 in tissues was more sensitive than bacterial culture(10.3%), but less sensitive than FA test(62.5%) using frozen tissues even though the result of 8 cases. By electron microscopy, a bacterium was also detected in the cytoplasm of macrophages in purulent exudate of cerebral meninges. Consequently, we confirmed that H parasuis serovar 5 has been involving to cause pigs with polyserositis and can be detected by FA and ABC stain as reliable diagnostic tools.
Purpose : The purpose of this study is the magnification rates depending on the area of patient dose (DAP) and glass dosimeter see the change of the dose according to the dose characteristics of low-magnification aims to raise standards. Materials and Method : Direct DR equipment Sonialvision DAR-8000f, Shimadzu was used, the patient entrance dose measurements to the surface of the Rando Phantom of the neck and the abdomen was placed on the Xi unfors. glass dosimeter for measuring organ doses at the same time the Rando Phantom of the major organs in place by inserting a 9 ", 12", 15 ", 17" and 30 seconds for each magnification were measured according in fluoroscopy. DAP meter area of the patient dose was measured. Result : Esophagography at 17" 143% than 9"magnification the average area dose was increased. Organ dose of Esophagography at 17" was decreased 25.32% than 9" magnification. UGI at 17" was increased 129.73% DAP than 9" magnification. Organ dose of UGI at 17" was decreased 23.32% than 9" magnification. Where the major organs of magnification at 17" were decreased(lung -25.96%, stomach -33.09%, spleen -27.81%, liver -4.92%) than 9" magnification. Conclusion : Expected to get better quality image While using the proper magnification, and have recognition that difference Organ doses and DAP meter in fluoroscopy.
In this paper, we investigate the potential for retrieval of morphometric data from three dimensional images of conducting bronchus obtained by X-ray Computerized Tomography (CT) and to explore the potential for the use of rapid prototype machine to produce physical hollow bronchus casts for mathematical modeling and experimental verification of particle deposition models. We segment the bronchus of lung by mathematical morphology method from obtained images by CT. The surface data representing volumetric bronchus data in three dimensions are converted to STL(streolithography) file and three dimensional solid model is created by using input STL file and rapid prototype machine. Two physical hollow cast models are created from the CT images of bronchial tree phantom and living human bronchus. We evaluate the usefulness of the rapid prototype model of bronchial tree by comparing diameters of the cross sectional area bronchus segments of the original CT images and the rapid prototyping-derived models imaged by X-ray CT.

Purpose Carcinoembryonic antigen (CEA) is cell-surface 180-200 kDa glycoprotein that is overexpressed in breast, stomach pancreas, lung, and colorectal cancers. CEA was first described in 1965 by Gold and Freedman and then serum CEA of colorectal cancer patients was first measured in 1969 by radioimmunoassay by Thomson. CEA is currently most widely used tumor marker in the clinic for management of colorectal cancer. Various CEA test kits have been developed and commercialized. CEA kits from different manufacturers might have different test results because of different reagents and protocol. The purpose of this study was to compare results of four commercial available CEA kits. Materials and Methods This study was designed to evaluate four commercially available CEA kits using serum samples acquired from 120 patients who visited our clinic. Test results were compared and analyzed according to the respective test methods. High concentration samples were diluted with saline and diluted solution. Results All of the four kits showed a significant correlation within the reference value. However, three of the four kits used for the dilution test using high concentration samples showed the hook effect. Conclusion Results of the present study showed that It is important to establish the standardized dilution standards for the high-concentration specimens to manage the error of the test result by the hook effect.
The trachea is defined as the airway from the inferior border of the cricoid cartilage to the top of the carinal spur. This paper would confirm the normal tracheal length of Korean adults through the actual measurement using the fiberoptic bronchoscopy. The subjects of this study were 25 patients, 13 males and 12 females between the age of 20 to 69 without abnormality on the neck, trachea, mediastinum and lung pharenchyme on the preoperative chest X-ray, who received the operations from the period of July to September, 1994. For those patients who had heart diseases, the cardiothoracic ratio was below 50%. The measurement was performed on the patients with endotracheal intubation under the general anesthesia in supine and neutral position. The tracheal length was calculated by the difference between the length from the tip of the endotracheal tube [E-tube to carina and to the needle which was inserted into the E-tube at the lower border of the palpated cricoid cartilage, by inserting the broncoscopy through the E-tube. The result was as follow : 1 The measured tracheal length for men was 11.8 0.2 cm[mean standard deviation and women was 10.5 0.3 cm, and that was longer than this [p<0.01 . The average was 11.2 1.0 cm and the standard error was 0.20 cm. 2 According to the correlation between the tracheal length to weight, height[Ht , age, and body surface area[BSA respectively, the Ht [p=0.003 , age [p=0.055 , and the BSA[p=0.017 were significant, while weight was not [p=0.314 . 3 From the regression analysis of the tracheal length[T.L. to the Ht, Age, and the BSA which were significant, the following equation was derived.i Ht : T.L.= -1.29 + 0.076 x Ht [P=0.003 ii Age: T.L.= 10.04 + 0.028 x Age [P=0.055 iii BSA : T.L.= 5.60 + 3.48 x BSA [P=0.017 iv In multi-regression : T.L. = -4.15 + 0.034 x Age + 0.085 x Ht [P=0.0002]
