Lung Perfusion Imaging and $Tc^{99m}-Macroaggregated$ Human Serum Albumin

  • Lung perfusion scanning, invariably combined with ventilation studies provides a reliable and non-invasive mean to diagnose lung related pathologies despite the availability of modern techniques such as angiography, magnetic resonance imaging, magnetic resonance angiography, and helical (spiral) computed tomography. The technique involves the generation of images by radiations emitted from radioisotopes introduced in to the lungs. Various radiopharmaceuticals have been proposed and designed to incorporate $Tc^{99m}$ in to macroparticulate form for lung perfusion imaging. However, most of these have associated difficulties such as reproducibility of the product with regards to particle size distribution and poor elimination from the lung capillary bed. $Tc^{99m}$ macroaggregated albumin $(Tc^{99m}-MAA)$ is used extensively for clinical lung perfusion imaging and is considered as the radiopharmaceutical of choice. It is non-toxic, safe, and being biodegradable, is easily eliminated from the lung capillary bed by proteolytic enzyme metabolism and by mechanical forces due to lung movement.

심근관류 스캔중에 나타난 Thallium-201의 심장외 국소적 섭취 (Extracardiac Uptake of Thallium-201 during Myocardial Perfusion Imaging with Pharmaeologic Vasodilation)

  • Myocardial perfusion imaging with $^{201}Tl-chloride$ following exercise or vasodilator-induced hyperemia has been effective in detecting the presence of coronary artery disease. An increased lung uptake of thallium has been reported as a sensitive marker of severe and extensive coronary artery disease and associated with poor prognosis. Thallium has also been noted to concentrate in a variety of malignant lesions. We report 5 cases of extracardiac uptake of thallium during myocardial perfusion scan with pharmacologic vasodilation. Accumulation of thallium was found in the lesions of a breast cancer, a lung cancer, a Castleman's disease and 2 cases of thymoma. We believe that the presence of focal extracardiac uptake of thallium during myocardial perfusion scan should suggest the need for further clinical evaluation to detect the tumor and must differentiate the increased lung uptake of thallium due to left ventricular dysfunction in coronary artery disease.

  • Magnetic Resonance Imaging (MRI) is one of the most advanced imaging techniques in clinical and research medicine. However, clinical application of MRI to the lung or thorax has been limited due to various drawbacks. Low signal intensity of the lung and cardiac and respiratory movements are the most serious problems with MRI in thorax. Nevertheless, MRI is superior to CT in some selected patients with thoracic diseases. The role of clinical MRI in thoracic disease has been widened with improvement of MR equipments and development of new pulse sequences. Otherwise, functional assessment of lung by MRI has been studied for the last decade. These include perfusion MRI with or without contrast enhancement and ventilation MRI with oxygen-enhancement or hyperpolarized noble gas, $^3He$ and $^{129}Xe$.

Human Recombinant Endostatin Combined with Cisplatin Based Doublets in Treating Patients with Advanced NSCLC and Evaluation by CT Perfusion Imaging

  • Aims: To study the effectiveness of human recombinant endostatin injection (Endostar(R)) combined with cisplatin doublets in treating advanced non-small cell lung cancer (NSCLC), and to evaluate outcome by CT perfusion imaging. Methods: From April 2011 to September 2014, 76 patients with advanced NSCLC who were treated with platinum-based doublets were divided into group A (36 patients) and group B (40 patients). Endostar(R) 15mg/day was administered 4 days before chemotherapy and combined with chemotherapy from day 5 in group A, and combined with chemotherapy from the first day in Group B. Endostar(R) in the two groups was injected intravenously for 14 days. Results: Treatment effectiveness in the two groups differed with statistical significance (p<0.05). Effectiveness evaluated by CT perfusion imaging, BF, BV, MTT and PS also demonstrated significant differences (all p<0.05). Adverse reactions in the two groups did not significantly vary (p> 0.05). Conclusions: The response rate with Endostar(R) administered 4 days before chemotherapy and combined with chemotherapy from day 5 in group A was better than Endostar(R) combined with chemotherapy from the first day, and CT perfusion imaging could be a reasonable method for evaluation of patient outcomes.

폐기종의 연무흡입 폐환기스캔 소견 (Radioaerosol Inhalation Lung Scan in Pulmonary Emphysema)

  • Perfusion and ventilaion imagings of the lung are well established procedure for diagnosing pulmonary embolism, differentiation it from chronic obstructive lung disease, and making an early detection of chronic obstructive lung disease. To evaluate the usefulness of radioaerosol inhalation imaging (RII) in chronic obstructive lung disease, especially pulmonary emphysema, we analyzed RIIs of five normal adult non-smokers, five asymptomatic smokers (age 25-42 years with the mean 36), and 21 patients with pulmonry emphysema (age 59-78 years with the mean 67). Scintigrams were obtained with radioaerosol produced by a BARC nebulizer with 15 mCi of Tc-99m-phytate. Scanning was performed in the anterior, posterior, and lateral projections after five to 10-minute inhalation of the radioaerosol on sitting position. The scans were analyzed and correlated with the results of pulmonary function studies and chest radiographs. Also lung perfusion scan with $^{99m}Tc-MAA$ was performed in 12 patients. In five patients, we performed follow-up scans for the evaluation of the effects of a bronchodilator. Based on the X-ray findings and clinical symptoms, pulmonary emphysema was classified into four types: centrilobular (3 patients), panlobular (4 patients), intermediate (10 patients), and combined (4 patients). RII findings were patternized according to the type, extent, and intensity of the aerosol deposition in the central bronchial and bronchopulmonary system and lung parenchyma. 10 controls, normal five non-smokers and three asymptomatic smokers revealed homogeneous parenchymal deposition in the entire lung fields without central bronchial deposition. The remaining two of asymptomatic smokers revealed mild central airway deposition. The great majority of the patients showed either central (9/21) or combined type (10/21) of bronchopulmonary deposition and the remaining two patients peripheral bronchopulmonary deposition. Parenchymal aerosol deposition in pulmonary emphysema was diffuse (6/21), discrete(6/21), intermediate (3/21), or combined (6/21). In 12 patients studied also with perfusion scans, perfusion defects matched closely with ventilation defects in location and configuration. But the size of the ventilation defects was generally larger than the perfusion defects. In all four patients treated with bronchodilators, the follow-up study demonstrated decrease in abnormal of radioaerosol deposition in the central airway with improvement of ventilation defects. RII was useful technique for the evaluation of regional ventilatory abnormality and the effects of treatment with bronchodilators in pulmonary emphysema.

일상적인 운동 부하 게이트 심근 관류 SPECT에서 $^{99m}Tc-sestamibi$ 폐섭취 : 좌심실 구혈률과 관류 결손 정도와의 비교 (Lung Uptake of $^{99m}Tc-sestamibi$ during Routine Gated Exercise SPECT Imaging : Comparison with Left Ventricular Ejection Fraction and Severity of Perfusion Defect)

  • 배경 및 목적: $^{201}Tl$ 심근 관류 SPECT에서, 폐-심장 섭취비(lung-to-heart uptake ratio; LHR)의 정량적 측정은 관동맥 질환자의 예후를 평가할 수 있는 지표이다. 그러나 최근$^{99m}Tc-sestamibi$ (MIBI)를 이용한 심근 관류 SPECT가 많이 이용되고 있으나 MIBI의 폐섭취가 관동맥 질환자에 $^{201}Tl$ LHR와 같은 의미를 가지는 지는 의문이 있어왔다. 본 연구는 MIBI 주사후 1시간에 촬영한 전면상을 이용하여 LHR을 구하여 좌심실 구혈률, 관류 결손 정도와의 상관 관계를 알아보고자 하였다. 대상 및 방법: Bruce법으로 운동 부하를 실시한 241명(남성 130명, 평균 연령 55세)을 대상으로 하였다. 부하검사와 휴식기 검사를 같은 날에 실시하는 프로토콜(1일 영상법)은 152명, 다른 날에 실시하는 프로토콜(2일 영상법)은 89명에서 시행하였다. 운동 부하 SPECT는 MIBI를 주사한 후 1시간에 이중헤드 감마 카메라를 이용하여 얻었다. 폐와 심장의 MIBI 섭취 정도는 20초간 얻은 전면상에서 폐와 심장에 관심영역을 설정하여 구하고, 이를 이용하여 섭취비를 계산하였다. AutoQUANT 소프트 웨어를 이용한 자동화 방법으로 운동부하시 좌심실 구혈률(LVEF)을 구하였고 운동부하시 관류 결손 정도(Summed Stress Score, SSS)를 구하였다. LVEF에 따라 3군(35%이하, 36-50%, 50%초과), SSS에 따라 4군(4미만, 4-8, 9-13, 13초과)으로 나누었다. 증가된 LHR의 정의는 관류 결손이 없는 남, 여 각 군에서 구한 LHR의 평균값에 표준편차의 2배값을 더한 값을 초과하는 것으로 판단하였고 각 군에서 증가된 LHR을 보인 환자의 분포를 구하여 비교하였다. 결과: 정상 관류 소견을 보였던 130명의 분석에서, 여성에 비해 남성에서 LHR이 유의하게 높았고 정상 관류 소견을 보였던 130명의 남성과 여성 모두에서, 1일 영상법와 2일 영상법을 실시한 군 사이의 LHR은 의미 있는 차이가 관찰되지 않았다. 정상 관류 소견을 보인 군과 관류 결손을 보인 군 사이의 비교에서는 관류 결손이 있는 남성과 여성에서 관류 결손이 없는 남성과 여성에 비해 LHR이 유의하게 높았다. 관류 결손이 있는 111명의 분석에서는 LHR과 EF는 역상관 관계, LHR과 SSS 사이에는 양의 상관 관계가 관찰되었고, 남성에서는 통계학적 의의가 있었으나 여성에서는 통계학적 의의는 없었다. EF군에 따른 분석에서는 좌심실 구혈률이 낮을수록 LHR이 높았고, SSS군에 따른 분석에서 관류 결손이 높은 군에서 유의하게 LHR가 높았다. 결론: 일상적인 운동부하 심근 관류 SPECT상 관류 결손이 있는 환자에서 폐심장 섭취비는 좌심실 구혈률과 심근 부하시 관류 결손 정도가 클수록 증가된 폐-심장 섭취비를 보인 환자가 많았으며, 좌심실 구혈률이 낮을수록 그리고 관류 결손의 정도가 클수록 폐-심장 섭취비가 증가하였다 이런 결과로 보아 주사후 1시간 $^{99m}Tc-MIBI$의 LHR은 좌심실 부전과 큰 관류 결손을 나타내는 간접적인 지표로 생각되어 진다. 그러나 관류 결손의 정도나 좌심실 구혈률 감소 정도가 미약한 군에서는 폐-심장 섭취비가 정상군과 비슷하여, 저위험군의 감별에는 제하점이 있다.

Dynamic Chest X-Ray Using a Flat-Panel Detector System: Technique and Applications

  • Dynamic X-ray (DXR) is a functional imaging technique that uses sequential images obtained by a flat-panel detector (FPD). This article aims to describe the mechanism of DXR and the analysis methods used as well as review the clinical evidence for its use. DXR analyzes dynamic changes on the basis of X-ray translucency and can be used for analysis of diaphragmatic kinetics, ventilation, and lung perfusion. It offers many advantages such as a high temporal resolution and flexibility in body positioning. Many clinical studies have reported the feasibility of DXR and its characteristic findings in pulmonary diseases. DXR may serve as an alternative to pulmonary function tests in patients requiring contact inhibition, including patients with suspected or confirmed coronavirus disease 2019 or other infectious diseases. Thus, DXR has a great potential to play an important role in the clinical setting. Further investigations are needed to utilize DXR more effectively and to establish it as a valuable diagnostic tool.

새로운 심관관류 영상 화합물로서 $^{99m}Tc$-Ethyl-3-Isocyano-butyrate의 합성, 표지 및 체내동태에 대한 연구 (Synthesis Characterization and Biodistribution of $^{99m}Tc$-Ethyl-3-Isocyanobutyrate as a New Myocardial Perfusion Agent)

  • Technetium labeled isonitrile analogues are widely used as myocardial perfusion imaging agents. We synthesized and characterized a new isonitrile compound, ethyl 3-isocyanobutyrate(EIB). Proton and $^{13}C$ NMR spectroscopy and thin layer chromatography with a $C_{18}$ coat was performed. EIB was easily labeled with $^{99m}TcO_4^-$- with sodium dithionite. The labeling efficiency measured by RP-HPLC was over 95%. The labeled product was stable with dilution in normal saline and with prolonged incubation at room temperature. There was no formation of secondary products or free $^{99m}TcO_4^-$. In vivo kinetics study of $^{99m}Tc$ (I) labeled EIB in rabbits showed adequate myocardial uptake, good contrast against lung background, and relatively rapid liver clearance. The heart to lung ratio was over 2.5 and the heart to liver ratio was approximately from 0.4 to 5 at 60 minutes post injection. Hepatic clearance of $^{99m}Tc-MIBI$ was faster ($t_{1/2}$=6 minutes) than that of $^{99m}Tc-MIBI$. In vivo kinetics observed in dog was similar to that in rabbit but there was faster gallbladder filling, and thus lower liver background. SPECT imaging of the canine myocardium showed favorable imaging characteristics. However, biodistribution in mice demonstrated a myocardial % injected dose/organ of less than 0.1%. This was thought to be due to interspecies difference in plasma esterase activity. In human plasma, $^{99m}Tc$ ( I ) labeled EIB was stable for at least 2 hours, without production of secondary products by HPLC. We conclude that ethyl 3-isocyanobutyrate may be a potential new myocardial perfusion imaging agent and deserves further investigation as to its usefulness for clinical use.

심근 저산소증 영상 (Imaging Hypoxic Myocardium)

  • Hypoxia (decreased tissue oxygen tension) is a component of many diseases such as tumors, cerebrovascular diseases and ischemic heart diseases. Although hypoxia can be secondary to a low inspired $pO_2$ or a variety of lung disorders, the most common cause is ischemia due to an oxygen demand greater than the local oxygen supply. In the heart tissue, hypoxia is often observed in persistent low-flow states, such as hibernating myocardium. Direct "hot spot" imaging of myocardial tissue hypoxia is potentially of great clinical importance because it may provide a means of identifying dysfunctional chronically ischemic but viable hibernating myocardium. A series of radiopharmaceuticals that incorporate nitroimidazole moieties have been synthesized to detect decreased local tissue pO2. In contrast to agents that localize in proportion to perfusion, these agents concentrate in hypoxic tissue. However, the ideal agents are not developed yet and the progress is very slow. Furthermore, the research focus is on tumor hypoxia nowadays. This review introduces the myocardial hypoxia imaging with summarizing the development of radiopharmaceuticals.

간 혈관종의 Tc-99m 표지 적혈구 혈액풀 스캔 (Evaluation of Hepatic Hemangioma by Tc-99m Red Blood Cell Hepatic Blood Pool Scan)

  • Hemangioma is the most common benign tumor of the liver, with a prevalence estimated as high as 7%. Tc-99m red blood cell (RBC) hepatic blood pool scan with single photon omission computed tomography (SPECT) imaging is extremely useful for the confirmation or exclusion of hepatic hemangiomas. The classic finding of absent or decreased perfusion and increased blood pooling ("perfusion/blood pool mismatch") is the key diagnostic element in the diagnosis of hemangiomas. The combination of early arterial flow and delayed blood pooling ("perfusion/blood pool match") is shown uncommonly. In giant hemangioma, filling with radioactivity appears first in the periphery, with progressive central fill-in on sequential RBC blood pool scan. However, the reverse filling pattern, which begins first in the center with progressive peripheral filling, is also rarely seen. Studies with false-positive blood pooling have been reported infrequently in nonhemangiomas, including hemangiosarcoma, hepatocellular carcinoma, hepatic adenoma, and metastatic carcinomas (adenocarcinoma of the colon, small cell carcinoma of the lung, neruroendocrine carcinoma). False-negative results have been also reported rarely except for small hemagniomas that are below the limits of spatial resolution of gamma camera.
