만성폐쇄성폐질환 환자 사망 원인 - 한 3차 병원 연구 (Cause of Death in COPD Patients of a Referral Hospital)

  • 연구 배경 : 만성폐쇄성폐질환(COPD)은 45세 이상의 성인에서 국내 유병률 17.2%로 주요 질환이다. 하지만, 국내 COPD 환자의 사망원인에 대한 연구는 불충분한 상황이다. 이에 국내 COPD 환자 사망원인에 대해서 알아보고자 서울아산병원 의무기록을 후향적으로 조사하였다. 방 법 : 2003년 1년간 서울아산병원에서 COPD로 진료한 1,078명의 사망여부를 통계청에 의뢰하여 총 사망자 88명을 얻었고 이중 폐결핵 후유증, 기관지확장증, 폐암 등 암 환자를 제외한 후 남은 28명의 COPD 환자 대상으로 사망원인을 분석하였다. 결 과 : COPD 환자의 사망원인은 폐렴 등 호흡기 원인이 16명 (57%), 심장 원인 5명 (18%), 급사 3명 (11%), 기타 4명 (14%) 등이었다. 서울아산병원 내에서 사망한 환자와 외에서 사망한 환자의 호흡기 관련 사망이 각각 83%(10명/12명)과 38%(6명/16명)이었다 (P=0.05) $FEV_1$이 50%예측치보다 큰 환자와 작은 환자의 호흡기 관련 사망은 각각 43%과 55%이었다 (P=0.89). 결 론 : 국내 3차 병원에서 진료하는 COPD 환자의 사망 원인은 폐렴 등 호흡기 원인 다수를 차지한다.

The Comparison of Clinical Variables in Two Classifications: GOLD 2017 Combined Assessment and Spirometric Stage of Chronic Obstructive Pulmonary Disease

  • Background: There are limited number of studies that investigate clinical variables instead of chronic obstructive lung disease (COPD) management according to Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2017 classification. The aim of the study was to investigate whether there was a difference between GOLD 2017 classification and spirometric stage in clinical variables in patients with COPD. The data of 427 male patients with stable COPD were investigated retrospectively. Methods: Patients were allocated into combined assessment of GOLD 2017 and spirometric stage. Age, amount of smoking, pulmonary function, modified Medical Research Council (mMRC), incremental shuttle walk test (ISWT), Hospital Anxiety-Depression Scale (HADS), St. George's Respiratory Questionnaire (SGRQ), body mass index (BMI), and fat free mass index (FFMI) were recorded. Results: Seventy-three (17%) patients were in group A, 103 (24%) constituted group B, 38 (9%) were included in group C, and 213 (50%) comprised group D according to the combined assessment of GOLD 2017. Twenty-three patients (5%) were in stage 1, 95 (22%) were in stage 2, 149 (35%) were in stage 3, and 160 (38%) were in stage 4 according to spirometric stage. According to GOLD 2017, age, amount of smoking, mMRC, BMI, FFMI, SGRQ, HADS, forced vital capacity, forced expiratory volume in 1 second ($FEV_1$), and ISWT were significantly different between groups. Ages, amount of smoking, FFMI, BMI, HADS of group A were different from B and D. Smiliar values of $FEV_1$ were found in A-C and B-D. A and C had smiliar ISWT. According to spirometric stage, BMI, FFMI of stage 4 were statistically different. mMRC, ISWT, and SGRQ of stages 3 and 4 were different from other stages, amongst themselves. $FEV_1$ was correlated with mMRC, SGRQ, anxiety scores, BMI, FFMI, and ISWT. Conclusion: This study showed that the GOLD ABCD classification might not represent the severity of COPD sufficiently well in terms of lung function or exercise capacity. The combination of both spirometric stage and combined assessment of GOLD 2017 is important, especially for estimating clinical variables.

우리나라 성인 폐용적 및 폐확산능 정상예측식의 선정 (Selection of Reference Equations for Lung Volumes and Diffusing Capacity in Korea)

  • 연구 배경: 폐용적과 폐확산능의 정상예측식은 국가나 인종 등에 영향을 받는 것으로 알려져 있으나 우리나라 성인 대상으로 한 정상예측식이 폐용적은 없고, 폐확산능의 정상예측식은 있으나 많이 사용하지 않고 있다. 이에 기존에 사용하고 있는 여러 정상예측식 중 우리나라 성인에 가장 적합한 식을 선정하고자 본 연구를 수행하였다. 방 법: 비흡연, 건강한 성인을 대상으로 한양대학교 구리병원에서 모집한 남자 30명, 여자 33명 그리고 서울아산병원에서 모집한 남자 27명, 여자 34명을 대상으로 하였다. 이 대상자의 키와 몸무게를 측정하였고 전신체적용적기록(whole body plethysmography)로 폐용적을 측정하였으며 단회호흡법(single breath mothod)를 이용하여 폐확산능을 측정하였다. 가장 적합한 정상예측식은 실측치와 예측치의 차이인 잔차의 합이 최소가 되는 식으로 선정하였다. 결 과: 전폐용량의 잔차의 합이 최소가 되는 정상예측식은 한양대병원에서 모집한 대상자의 경우 남자는 ECSC 식 (잔차의 평균: 0.04 L), 여자는 Crapo/Morris 식 (-1.04)이었고 서울아산병원에서 모집한 대상자의 경우 남자는 Goldman/Becklake식 (-2.35), 여자는 ECSC 식 (-4.49)이었다. 폐확산능은 잔차의 평균이 최소가 되는 식은 한양대병원 남자는 Roca 식 (잔차의 평균: -13.66 ml/min/mmHg, 여자는 박 등의 식 (25.08)이었고 서울아산병원 남녀는 모두 박 등의 식(남자: -1.65, 여자: -6.46)이었다.

Asthma-COPD Overlap Syndrome: What We Know and What We Don't

  • Approximately one in four patients with chronic obstructive pulmonary disease (COPD) have asthmatic features consisting of wheezing, airway hyper-responsiveness or atopy. The Global initiative for Asthma/Globalinitiative for chronic Obstructive Lung Disease committee recently labelled these patients as having asthma-COPD overlap syndrome or ACOS. ACOS also encompasses patients with asthma, ${\geq}40$ years of age, who have been cigarette smokers (more than 5-10 pack years) or have had significant biomass exposure, and demonstrate persistent airflow limitation defined as a post-bronchodilator forced expiratory volume in 1 second ($FEV_1$)/forced vital capacity of <70%. Data over the past 30 years indicate that patients with ACOS have greater burden of symptoms including dyspnea and cough and show higher risk of COPD exacerbations and hospitalizations than those with pure COPD or pure asthma. Patients with ACOS also have increased risk of rapid $FEV_1$ decline and COPD mortality. Paradoxically, experimental evidence to support therapeutic decisions in ACOS patients is lacking because traditionally, patients with ACOS have been systematically excluded from therapeutic COPD and asthma trials to maintain homogeneity of the study population. In this study, we summarize the current understanding of ACOS, focusing on definitions, epidemiology and patient prognosis.

만성폐쇄성폐질환 유소견 성인의 폐쇄성 기류제한 상태 비인지 영향요인 (Risk Factors for Unawareness of Obstructive Airflow Limitation among Adults with Chronic Obstructive Pulmonary Disease)

  • Purpose: The purpose of the present study is to examine risk factors for unawareness of obstructive airflow limitation among adults with chronic obstructive pulmonary disease. Methods: Secondary data analysis was performed with the data from the 6th Korea National Health and Nutrition Examination Survey (KNHANES; 2013-2014). The data were analyzed with the IBM SPSS 22.0 version using frequency, percentage, odds ratio, and logistic regression. Results: Ninety-eight percent of subject with a pulmonary function test score of $FEV_1/FVC$<0.7 (N=833) did not recognize that their lung function was impaired. The heavy drink, absence of tuberculosis or asthma diagnosis, and no symptom of expelling phlegm were identified as major risk factors for unawareness of airflow limitation. Conclusion: In order to increase awareness of airflow limitation and to prevent the worsening of the condition, the pulmonary function screening test should be provided to community residents including those who do not show symptoms of respiratory illness.

Correlation between Physical Activity and Lung Function in Dusty Areas: Results from the Chronic Obstructive Pulmonary Disease in Dusty Areas (CODA) Cohort

  • Background: Although physical activity is known to be beneficial to lung function, few studies have been conducted to investigate the correlation between physical activity and lung function in dusty areas. Therefore, the purpose of this study is to investigate the correlation between physical activity and lung function in a Korean cohort including normal and COPD-diagnosed participants. Methods: Data obtained from the COPD in dusty areas (CODA) cohort was analyzed for the following factors: lung function, symptoms, and information about physical activity. Information on physical activity was valuated using questionnaires, and participants were categorized into two groups: active and inactive. The evaluation of the mean lung function, modified Medical Research Council dyspnea grade scores, and COPD assessment test scores was done based on the participant physical activity using a general linear model after adjusting for age, sex, smoking status, pack-years, height, and weight. In addition, a stratification analysis was performed based on the smoking status and COPD. Results: Physical activity had a correlation with high forced expiratory volume in 1 second ($FEV_1$) among CODA cohort (p=0.03). While the active group exhibited significantly higher $FEV_1$ compared to one exhibited by the inactive group among past smokers (p=0.02), no such correlation existed among current smokers. There was no significant difference observed in lung function after it was stratified by COPD. Conclusion: This study established a positive correlation between regular physical activity in dusty areas and lung function in participants.

The Serum Concentrations of YKL-40, IL-6, and TNF-${\alpha}$ in Retired Workers Exposed to Inorganic Dusts

  • Occupational long-term exposure to inorganic dusts may cause a variety of lung diseases such as pneumoconiosis and chronic obstructive pulmonary disease (COPD). Diagnosis of pneumoconiosis and COPD, however, is currently dependent on radiological findings and pulmonary test, which are both late diagnostic tools. Therefore, there is a need to identify novel biomarkers in pneumoconiosis and COPD. Hence, in this current study we investigated the serum concentrations of YKL-40, interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-${\alpha}$) as biomarkers for pneumoconiosis and COPD in 161 retired male workers exposed to inorganic dusts. The serum concentration of YKL-40 was significantly increased with age, pneumoconiosis, and airflow limitation. The serum concentration of IL-6 was significantly higher in airflow limitation. These results suggest that serum concentration of YKL-40 is associated with age, pneumoconiosis, and airflow limitation. Also, serum concentration of IL-6 is associated with airflow limitation.

Blood Eosinophil Counts in Chronic Obstructive Pulmonary Disease: A Biomarker of Inhaled Corticosteroid Effects

  • Blood eosinophil counts have emerged as a chronic obstructive pulmonary disease (COPD) biomarker that predict the effects of inhaled corticosteroids (ICS) in clinical practice. Post-hoc and prospective analysis of randomized control trials have shown that higher blood eosinophil counts at the start of the study predict a greater response to ICS. COPD patients with frequent exacerbations (2 or more moderate exacerbations/yr) or a history of hospitalization have a greater response to ICS. Ex-smokers also appear to have a greater ICS response. Blood eosinophil counts can be combined with clinical information such as exacerbation history and smoking status to enable a precision medicine approach to the use of ICS. Higher blood eosinophil counts are associated with increased eosinophilic lung inflammation, and other biological features that may contribute to the increased ICS response observed. Emerging data indicates that lower blood eosinophil counts are associated with an increased risk of bacterial infection, suggesting complex relationships between eosinophils, ICS response, and the airway microbiome.

Status of Studies Investigating Asthma-Chronic Obstructive Pulmonary Disease Overlap in Korea: A Review

  • There is a considerable number of individuals who exhibit features of both asthma and chronic obstructive pulmonary disease (COPD), defined as asthma-COPD overlap (ACO). Many studies have reported that these patients have a greater burden of symptoms, including cough and dyspnea, and experience more exacerbations and hospitalizations than those with non-ACO COPD or asthma. Although diagnostic criteria for ACO have not yet been clearly established, their clinical significance remains to be determined. As interest in ACO grows, related studies have been conducted in South Korea as well. The present review summarizes ACO-related studies in South Korea to better understand Korean ACO patients and guide further research. Several cohort studies of asthma and COPD and population-based studies for ACO were reviewed and the key results from demographics, clinical features, lung function, biomarkers, treatment, and prognosis were summarized.

  • Chronic obstructive pulmonary disease (COPD) affects close to 400 million people worldwide. COPD is characterized by significant airflow limitation on spirometry. Most patients with COPD are diagnosed in their fifth or sixth decades of life. However, the disease begins much earlier. By the time airflow limitation is detected on spirometry, patients with COPD have lost close to 50% of their small airways. Thus, identification of patients with early COPD, defined as persons with preserved spirometry, who demonstrate pathologic or functional hallmarks of COPD, is essential for disease modification and ultimately disease elimination. This paper provides an up-to-date overview of the current case definition of early COPD, its importance, the novel technologies required for its detection in young adults and future directions in therapeutics for treatment.
