Association of Bone Mineral Density with Airway Obstruction and Emphysema

  • Background: Airway obstruction and the extent of emphysema are reported to be responsible for reduced bone mineral density (BMD). Corresponding to different phenotypes of a pulmonary disease, different severity in extra pulmonary features may exist. We compared BMDs of subjects with or without airway obstruction and/or emphysema and investigated the relationships among BMD, the severity of airway obstruction, and the extent of emphysema. Methods: Using a university hospital database, we reviewed patients over 40 years old who performed spirometry, computed tomography of chest, and measurement of BMD of the lumbar (L) spine. According to the presence or absence of airway obstruction and/or emphysema, four groups were classified. Results: Among a total of 59 subjects, 33 (56%) had osteoporosis. The prevalence of osteoporosis in subjects with no airway obstruction and no emphysema, those with only emphysema, those with only airway obstruction, and those with both airway obstruction and emphysema were 42%, 57%, 64%, and 73%, respectively (p=0.047 by linear-by-linear association). The mean T-scores of BMD of L1 (p=0.032) and L1-4 spines were different among the four groups (p=0.034). Although the T-score of L1 BMD negatively correlated with the extent of emphysema (r=-0.275, p=0.035) and positively with each of body mass index (BMI) (r=0.520, p<0.001), forced expiratory volume in one second ($FEV_1$) (r=0.330, p=0.011), $FEV_1$/forced vital capacity (r=0.409, p=0.001), and forced expiratory flow at 25~75% of FVC ($FEF_{25-75%}$) (r=0.438, p=0.0001), respectively, multiple linear regression analysis indicated that BMI (p<0.001) and $FEF_{25-75%}$ were predictive of BMD (p=0.012). Conclusion: Low BMI and airway obstruction were strongly associated with reduced bone density rather than the extent of emphysema.

The Effect of Postural Correction and Subsequent Balloon Inflation in Deformity Correction of Acute Osteoporotic Vertebral Fractures

  • Objective : To determine deformity correction by postural correction and subsequent balloon inflation in acute vertebral compression fractures (OVCFs) and to examine the effect of bone mineral density on deformity correction. Methods : A totol of 50 acute OVCFs received balloon kyphoplasty. Lateral radiographs were taken and analyzed at five different time points : 1) preoperative, 2) after placing the patient in prone hyperextended position, 3) after balloon inflation, 4) after deposition of the cement, and 5) postoperative. All fractures were analyzed for height restoration of anterior (Ha), middle (Hm) and posterior (Hp) vertebra as well as Cobb angle and Kyphotic angle. The bone mineral density (BMD) of lumbar spine was measured by dual-energy X-ray absorptiometry. According to the T-score, the patients were divided into two groups which were osteoporosis group and osteopenia group. Results : Postoperative measurements of Ha, Hm and the Cobb angle demonstrated significant reduction of 4.62 mm, 3.66 mm and $5.34^{\circ}$ compared with the preoperative measurements, respectively (each p<0.05). Postural correction significantly increased Ha by 5.51 mm, Hm by 4.35 mm and improved the Cobb angle by $8.32^{\circ}$ (each p<0.05). Balloon inflation did not demonstrate a significant improvement of Ha, Hm or the Cobb angle compared with baseline prone hyperextended. Postural correction led to greater improvements of Ha, Hm and Cobb angle in osteoporosis group than osteopenia group (each p<0.05). Conclusion : In acute OVCFs, the height restoration was mainly attributed to postural correction rather than deformity correction by balloon inflation. BMD affected deformity correction in the process of postural correction.

이소플라본 보충이 폐경 후 여성에서 골밀도와 성호르몬에 미치는 영향 (Effects of Isoflavones Supplementation on Bone Mineral Density and Sex Hormones in Postmenopausal Women)

  • Soy isoflavones have been hypothesized to exert hormonal effects and to enhance bone mineral density in postmenopausal women. To test this hypothesis, we studied the effects of soy isoflavones supplements on bone mineral density and sex hormones (serum estradiol, sex hormone-binding globulin and testosterone) in 47 postmenopausal women. There were 24 participants in the treatment group and 23 in the control group. The treatment group consumed the isoftavones extract capsule daily (which contained 90 mg of soy isoflavones) for 12 weeks. The study compared pre- and post-isoflavones intake in the following areas: physical examination, diet survey, bone mineral density and serum sex hormone levels. The average age of the treatment group was 64.63 years and that of the control group was 66.48 years. There were no significant differences between the two groups in terms of height, weight, and body mass index. Both groups maintained regular diet patterns in terms of their average daily nutrient intake. There was no significant difference between the treatment group (18.49 mg) and the control group (21.27 mg) in terms of daily isoflavones intake based on diet. The 12-week analysis of bone mineral density change after taking isoflavones supplements demonstrated no significant differences in the following : lumbar spine BMD (0.82 g/$\textrm{cm}^2$ in pre versus 0.81 g/$\textrm{cm}^2$ in post), femoral neck BMD (0.58 g/$\textrm{cm}^2$ in pre versus 0.57 g/$\textrm{cm}^2$ in post) in the treatment group. There was no significant difference in serum estradiol in the isoflavones treatment group. The subjects indicated no significant difference in serum testosterone in the isoflavones treatment group. But the subjects indicated a significant difference in sex hormone-binding globulin (60.04 nmol/L in pre versus 52.39 nmol/L in post) in the isoflavones treatment group at the levels of p < 0.05. The significant decrease in sex hormone-binding globulin did indicate the need for long-term study on isoflavones supplementation as well as its positive effect on bone mineral density.

Efficacy and safety of denosumab treatment for Korean patients with Stage 3b-4 chronic kidney disease and osteoporosis

  • Background/Aims: We evaluated the efficacy and safety of denosumab treatment in severe chronic kidney disease (CKD) patients with osteoporosis. We also investigated whether the treatment affects the coronary artery calcifications. Methods: Twenty-seven postmenopausal women with Stage 3b-4 CKD and osteoporosis were enrolled. Twenty patients received denosumab plus calcium carbonate and vitamin D, and seven controls received calcium carbonate and vitamin D for 1 year. Dual-energy X-ray absorptiometry and coronary artery calcium (CAC) scoring computed tomography were performed before and after treatment. Hypocalcemic symptoms and serum calcium levels were evaluated. Results: After 1 year of treatment, the percent changes of femur neck (3.6 ± 3.2% vs. -0.7 ± 4.4%, p = 0.033) and total hip (3.4 ± 3.8% vs. -1.9 ± 2.1%, p = 0.001) bone mineral density (BMD) were significantly increased in the denosumab treated group compared to the control group. However, the percent change of lumbar spine BMD did not differ between two groups (5.6 ± 5.9% vs. 2.7 ± 3.9%, p = 0.273). The percent change of bone alkaline phosphatase was significantly different in the denosumab-treated group and control group (-31.1 ± 30.0% vs. 0.5 ± 32.0%, p = 0.027). CAC scores did not differ between groups. No hypocalcemic events occurred in both groups. Conclusions: If carefully monitored and supplemented with calcium and vitamin D, denosumab treatment for 1 year provides significant benefits in patients with Stage 3b-4 CKD and osteoporosis. However, denosumab treatment did not affect coronary artery calcifications in these patients.

원위 요골 골밀도의 측정 자세 및 요추 골밀도의 상관관계에 관한 연구 (The Position for Measuring BMD of the Distal Radius and The Study of the Correlation Between the Distal Radius and Lumba)

  • 본 연구는 원위요골의 회전에 따른 골밀도의 변화를 알아보고 원위요골 골밀도와 요추 골밀도를 비교 분석하였다. 골밀도 측정시 정확한 측정 자세를 알아보고자 자체 제작한 보조기구를 이용하여 수술, 기형이나 외상력이 없는 20, 30대 남성 11명을 대상으로 전완부를 DR(digital diagnostic IR-1100-150 ; Philips)로 촬영하였고 남성 21명을 DXA(QDR 4500W ; HOLOGIC, USA)로 골밀도 측정을 시행하였다. 실험은 두 가지로 수행하였다. 첫째, 보조기구를 이용하여 11명을 대상으로 전완부를 DR로 중립위(0도)를 기준으로 내회전 및 외회전 각각 3, 6, 9도에서 촬영하였고 둘째, 요추 및 전완부(Forearm)의 골밀도를 21명을 대상으로 DXA 장비를 이용해 중립위(0도)를 기준으로 내회전 및 외회전 각각 5, 10도에서 측정하였고 통계프로그램 SPSS 12.0v으로 비교 분석하였다. 그 결과 DR로 촬영한 11명의 원위요골 회전각은 내회전인 경우가 82%(n = 9)로 가장 많았으며, 평균 내회전 각도는 7도였다. 외회전 6도와 중립위(0도)에서도 1명(9%)씩 나타났다. 전체평균 회전각은 내회전 5.1도였다. DXA로 측정한 21명의 원위요골 회전각은 내회전인 경우가 43%(n = 9)로 가장 많았으며, 평균 내회전 각도는 7.2도였다. 외회전은 24%(n = 5)이고 평균 외회전 각은 6도이며 중립위(0도)은 33%(n = 7)로 나타났다. 전체평균 회전각은 내회전 4.1도였다. 요추와 원위요골의 상관 계수는 r = 0.30, p = 0.18로 통계적 유의성은 없었다. 요추의 골밀도가 원위요골의 골밀도를 대변하지 못하기 때문에 원위요골 골밀도의 평가는 원위요골에서 측정해야 한다. 전완부 골밀도 측정시 중립위는 원위요골이 외회전이 대부분이기 때문에 최저 골밀도의 회전각에서 일정하게 골밀도를 측정하기 위해서는 중립위 보다 내회전이 필요하며 내회전 각은 약 5도가 추천된다.

치조 골소주 모폴로지 특성과 골다공증의 연관관계에 대한 연구 (Relationship between the morphologic features of alveolar trabecular bone and systemic osteoporosis)

  • Purpose : The purpose of this study was to investigate the preliminary use of morphologic operation (MO) in analyzing trabecular pattern of alveolar bone for the predicting systemic osteoporosis. Materials and Methods : Study subjects consisted of 35 females (average age 48.5 years) and 25 males (average age 25.8 years). Bone mineral density BMD $(grams/cm^2)$ of lumbar spine and proximal femur of these subjects were measured by a dual energy X-ray absorptiometry (DEXA). Regions of interest (ROIs) were selected from the digitized periapical radiographs of subjects' posterior jaw. A custom computer program processed morphology operations of ROIs. We compared mean values of 11 MO variables according to the osteoporotic group divided by the T-scores of DEXA. We also studied correlation between radiographic density and these MO variables. Results : The mean radiographic densities insignificantly correlated with MO variables. There were statistically significant differences among the values of 9 MO variables according to the osteoporotic group. Conclusion Morphologic operation can be effective in analyzing trabecular pattern of alveolar bone for the predicting osteoporosis.

한국 폐경 여성의 골다공증 위험 인자에 관한 연구 (Risk Factors for Postmenopausal Osteoporosis in Korea)

  • Objective : To identify the risk factors for postmenopausal osteoporosis in Korea Materials and methods : Bone mineral density (BMD) at the lumbar spine and femoral neck was measured by dual energy X-ray absorptiometry in 808 apparently normal postmenopausal Korean women. Questions about life style, demographic parameters, medical history and social habits etc. were asked on these women; 2ll women with normal bone mineral density, and 597 women with osteopenia or osteoporosis. Results 'Age of >50 years, low body mass index (BMI; <18.5 kg/m2), long duration of menopause(>10 years), and previous history of fracture were associated with increased prevalence of osteopenia or osteoporosis. Women without the outside activity also showed a higher frequency of low bone mass, Risk for osteopenia or osteoporosis was low in women with high BMI (>23 kg/m2) and women with job. The prevalence of low bone mass appeared to be independent of the following parameters: socioeconomic status, familial history, smoking, drinking, exercise, previous use of oral contraceptive, coffee or milk consumption, and degree of sunlight exposure. Conclusion 'Age, BMI, duration of menopause, previous history of fracture and degree of outside activity are the risk factors for postmenopausal osteoporosis in Korea.

골밀도 향상을 위한 대체 요법으로 칼슘/마그네슘 복합체의 효능 (Ionized Cal/MagTM Complex as an Alternative Supplement for Enhancing Bone Mineral Density - Preliminary Results from Primary Care Centers -)

  • The medicines for treating osteoporosis currently in use have minor to severe side effects, and can be financially burdensome. Thus, there is a need for prevention and alternative supplement that is relatively inexpensive, and can be easily consumed daily as an alternative dietary therapy. In this study, bone marrow density of the spine and femur of osteoporosis patients were checked before and after consuming complex composed of calcium and magnesium, considered to be the core of bone mineral content. November 2017-November 2021, patients with T-score of less than -2.5 or -1.0 < T-score < -2.5 with history of fractures or recent fractures were enrolled. The data of 60 patients who orally administered Ionized Cal/MagTM Complex were reviewed retrospectively, and it was significantly confirmed that the average value of T-score was up-regulated by 0.5. Additionally, the cumulative dose was observed to have a positive effect, on the improvement of BMD in the 2nd Lumbar and Femur neck. It is expected that better results will be achieved if use of the supplement is continued.

생애전환기 건강진단 골밀도 검사시 측정도구에 따른 진단범위의 유효성 평가 (Estimation of Diagnostic Range for Measurement Tools, while BMD Testing to Health Examination in Transitional Ages)

  • 본 연구의 목적은 생애전환기 건강진단 골밀도 검사시 측정도구(DEXA, QUS, RA)에 따른 진단범위의 유효성을 평가하는 것이다. DEXA를 이용한 골밀도 검사 결과 T-score -2.5를 기준으로 할 때 QUS의 cutoff 값은 -1.733이며, 이때 민감도는 70.4%, 특이도는 59.5%이었고 T-score -3.0을 기준으로 할 때 cutoff 값은 -2.323으로 이때 민감도는 70.4%, 특이도는 56.8%이었다. DEXA를 이용한 골밀도 검사 결과 T-score -2.5를 기준으로 할 때 RA의 cutoff 값은 -1.675이며, 이때 민감도는 70.0%, 특이도는 63.7%이었고 T-score -3.0을 기준으로 할 때 cutoff 값은 -2.325로 이때 민감도는 70.0%, 특이도는 42.9%로 QUS와 RA의 측정도구 간에 큰 차이를 보이지 않았다. 골밀도 측정도구의 상관관계와 재현성에 대한 평가에서는 일부 차이를 제외하고는 측정도구 간 및 측정도구의 반복 측정값 간에 유의한 차이를 보이지 않았다. ROC 분석에서 측정도구별 평균골밀도 측정결과는 DEXA를 기준으로 할 때 RA와 QUS는 모두 유효한 것으로 판단된다. 다만, 생애전환기 골다공증 진단에 보다 정확한 결과를 얻기 위해서는 성별이나 연령층, 다양한 측정부위별 정량화된 결과를 진단기준의 보정지표로 고려하는 것이 필요하다.

여대생의 우유와 유제품 섭취실태 및 골밀도와의 관계 연구 (Relation between Milk Consumption and Bone Mineral Density of Female College Students in Korea)

  • The purpose of our study was to evaluate an association between milk consumption and bone mineral density (BMD) of female college students in Korea. The dietary data of 1,410 Korean college students living in the vicinity of Seoul area were collected through the Food Frequency Questionnaire. The mean age, height, weight and 8MI of the subjects were 20.4 years, 162.1 cm, 52.7 kg, and $20.0\;kg/m^2$ respectively. The mean intakes of energy, protein, and phosphorus were 2,398.7 kcal, 95.2 g, and 1,302.9 mg respectively, which were above their EER or Recommended Intake (RI), but the mean intake of calcium was 657.8 mg which was below its RI, and 44.0% of the subjects did not meet Estimated Average Requirement (EAR) of calcium. The daily milk consumption was 131.1 g, and the total consumption of milk and dairy product was 228.7 g. Forty five percent of the subjects consumed $50{\sim}200\;g$ of milk or dairy products. After the analysis of their dietary intakes, the subjects were divided into quintiles of total consumption of milk and dairy product, which were considered as major source of dietary calcium. Ninety two subjects from the upper quintile (UQ) and 85 subjects from the lower quintile (LQ) were selected and their bone mineral densities (BMD) of lumbar spine (L1-L4) and the femur were examined by the dual energy X-ray absorptiometry. The BMD $(g/cm^2)$of L1, L2, L3, and average of femur were 1.065, 1.150, 1.187, and 0.988 for the UQ subjects, which were significantly higher than those of the LQ subjects, 1.010, 1.090, 1.146, and 0.942 respectively. Moreover, the T-scores of the UQ were significantly higher than those of the LQ. The prevalence rates of osteopenia classified according to WHO's criterion were 12.0% in the UQ. and 20.0% in the LQ group which showed non significant. Blood levels of calcium, and phosphorus of UQ and LQ group were both within the normal range, and were not significantly different. Our results showed that UQ group presented the better status of bone mineral density and bone health. Therfore our results suggest that continuous consumption of more than one and a half cups of milk per day, which corresponds to the least amount of UQ group's consumption, would affect positively on the bone health of Korean adults.
