PURPOSE: The purpose of this study was to investigate the influences of lumbosacral angle, lumbar lordosis, pelvic level and symptoms after standing lumbar traction on HIVD patients. METHOD: For this investigation standing lumbar traction was administered to 22 patient who were diagnosed of HIVD. Standing lumbar traction was given to the subject patients for 3 weeks, times a week and each standng lumbar traction lasted 25 minutes. RESULT: For lumbosacral angle statistically significant different was not found although the lumbosacral angle was normalized. For lumbar lordosis statistically significant different was not found although the lumbar lordosis angle was decreased. For pelvic level statistically significant different was not found although the pelvic level was equalized. Statistically significant improvement in symptoms was found after standing lumbar traction. There was significant correlation between lumbar lordosis and lumbosacral angle. CONCLUSION: This study was found that the influences of standing lumbar traction was to decrease symptoms than lumbosacral angle of patients with HIVD. Therefore, it is necessary that to treat the patients with HIVD applied the method to correct spine angle and pelvic level with standing lumbar traction.
Objectives : The purpose of this study is to find out the relationship between hip internal rotation angle and the level of herniation of lumbar intervertebral disc(HIVD) in single-segment. Methods : We investigated 314 patients (158 male, 156 female) who were diagnosed as herniation of lumbar intervertebral disc(HIVD) in single-segment. We measured 314 patients' hip internal rotation angle and analysed the relationship between the hip internal rotation angle and the level of herniation of lumbar intervertebral disc(HIVD). Results : 1. Among 314 cases, the hip internal rotation angle was different between male and female. Hip internal rotation angle of male was mainly limited and that of female was mainly excessive. 2. Among 314 cases, the normal group, defined as patients who have specific range of hip internal rotation angle(male : $35^{\circ}{\pm}10^{\circ}$, female : $45^{\circ}{\pm}10^{\circ}$), tends to be occurred HIVD at L4/5 level. The limited group, defined as patients who have less angle than normal group, the excessive group, defined as patients who have more angle than normal group, and the complex group, defined as patients who have more angle of one leg and less angle of the other leg than normal group, tend to be occurred HIVD at L5/S1 level(p<0.05). Conclusions : In single-segment lumbar HIVD patients, The normal hip internal rotation angle mainly leads to L4/5 HIVD, while the limited and excessive hip internal rotation angle mainly lead to L5/S1 HIVD.
Objective : This study was designed to analyze the of correlation between narrowing disc space of lumbar X-ray and HIVD of L-spine CT in lumbosacral strain patients. Methods : 63 cases of lumbosacral strain patients who visited Bul-kyooh Oriental Hospital from June 13, 2000 to August 9, 2001 were selected. We performed the radiography by lumbar X-ray and L-spine CT for all cases. Results : 28 of 63 cases revealed narrowing disc space in Lumbar X-ray and these 28 cases were diagnosed as HIVD in L-spine CT at all. 35 of 63 cases revealed normal in Lumbar X-ray and 13 of these 35 cases were diagnosed as HIVD in L-spine CT. Conclusions : We found that narrowing disc space in lumbar X-ray has significant relevance to HIVD in L-spine CT(P-value = 0.001). For accurate diagnosis and treatment of lumbosacral strain patients. combination of clinical symptoms, physical examination and radiography of X-ray and HIVD is needed.
Objectives : The purpose of this study is to report the effect of acupotomy for patients diagnosised Lumbar HIVD. Methods : We treated Three patients who have Lumbar HIVD with acupotomy. Visual analog scores(VAS) was compared before-treatment with after-treatment. Results : The patients's pain, numbness and walking were much improved. Conclusions : This study demonstrate that acupotomy useful effect on Lumbar HIVD . As though we had not wide expience in this treatment, more research is needed.
Objective : The purpose of this study was to suggest (diagnostic) criteria for lumbar disc space and see the correlation between lumbar disc space narrowing and the level of HIVD. Methods : We looked at the 113 patients diagnosed disc space narrowing with lumbar x-ray and MRI films who have visited Jaseng Hospital of Oriental Medicine with low back pain in October,2013. The width of lumbar disc space and the severity of Herniated Intervertebral Disc(HIVD) on MRI films were collected and stastically analyzed using one-way ANOVA. Results : In this study, measured value(%) for lumbar disc space of the group diagnosed serious HIVD, such as protrusion, extrusion and migration was significantly decresed compared to the normal group and the bulging group. Conclusion : There was a significant correlation between lumbar disc space narrowing and the level of HIVD.
Background: Manual traction with a belt is a physiotherapy treatment method that reduces disk pressure and widens the disk space. In clinical settings, it is applied to numerous patients with herniated intervertebral disk (HIVD). This study aimed to identify the effects of manual traction with a belt on the intervertebral space in patients with lumbar HIVDs. Methods: The intervention was performed on 17 patients with lumbar HIVDs who were divided into two groups: one with eight patients having HIVD at L4~L5 and another group with nine patients having HIVD at L5~S1. The participants received manual traction with a belt twice a week for 12 weeks, and radiographic imaging was used to visualize the intervertebral space and compare it before and after treatment. Results: Manual traction with a belt increased the lumbar intervertebral space at L4~L5 and L5~S1 in patients with L4~L5 HIVD. A significant difference was observed in the L4-L5 distance (p<.01); however, no significant difference was observed in the L5~S1 distance (p>.05). The intervertebral space significantly increased at both L4~L5 and L5~S1 in patients with L5~S1 HIVD (p<.05). Conclusion: Thus, manual traction with a belt increased the intervertebral space in patients with L4~L5 and L5~S1 HIVDs. These results are expected to guide studies on manual traction with belts in clinical settings in the future. Further studies using the present research as an objective study method are anticipated.
Objectives : This study aimed to introduce a new traction therapy and proposes that a new Chuna manual therapy technique could be implemented. Methods : The patient is placed in the prone position and their ankle is fixed to the fixing table. Objective : It Is known that depression disorder has been related to chronic pains such as HIVD and physical harm. We propose that chronic cervical and lumbar herniations of intervertebral disc patients have emotional and psychiatric problems, therefore we compared it to Beck's Depression Inventory scores. Method : We divided them into two groups: The groups consisted of cervical and lumbar HIVD patients. We then requested them to fill out BDI research questionnaires, and evaluated patients according to the information and results. Result : The Beck's Depression Inventory mean score for the cervical - lumbar HIVD group was $14.00{\pm}5.80$, $10.83{\pm}$5.64 each. Higher Scores were recorded for cervical HIVD group than the lumbar HIVD group. Conclusion : There is statistical significance among two groups.(p<.05) however, the two groups' BDI score were lower than the defined boundary line of Depression disorder(16 points).
Objectives : The purpose of this study is to report the effect of acupotomy for patients with Lumbar intervertebral disc Herniation. Methods : We treated Four patients who have HIVD of L-spine with acupotomy. Visual analog scores(VAS) and Oswestry Low-back pain Disabiliby Index(ODI) were compared before-treatment with after-tretment. When the patients is discharged, the satisfaction of acupotomy were graded by Five-point Likert scale. Results : VAS and ODI were decreased at all case. Likert scale point were cheked as grade 4 at all case. Conclusions : This study shows acupotomy has useful effect on HIVD of L-spine.
Objectives : The lumbosacral joint is unstable area from an anatomical viewpoint, while it is also a very mobile area in ordinary life, so that clinically major causes of low back pain originate in this joint. The purpose of this study is to assess the difference of the Ferguson's angles, Lumbar gravity lines, Lumbar lordotic angles among Herniated of Intervertebral Disc(HIVD) patients. Methods : We analyzed the lateral view of lumbar spine checked at erect position on 88 patients who had been diagnosed as HIVD by Magnetic Resonance Imaging(MRI). We investigated the Ferguson's angle, Lumbar gravity line, Lumbar lordotic angle on X-ray film. Results and Conclusions : In the acute lumbago group the Ferguson's angle had a tendency to decrease, while in the chronic group it had a tendency to increase. In the acute lumbago group the Lumbar gravity line fell in front of the normal range(sacrum), while in the chronic group it fell behind the normal range(sacrum). In the acute lumbago group the Lumbar lordotic angle usually decreased, while in the chronic group it increased. The Ferguson's angle and the Lumbar gravity line, the Ferguson's angle and the Lumbar lordotic angle, the Lumbar gravity line and Lumbar lordotic angle each had a positive realtionship. The Ferguson's angle, the Lumbar gravity line and the Lumbar lordotic angle was less influenced by the level of HIVD and was more influenced by how long the patient had the pain. The correlationship between each factor was less in the chronic lumbago group than the acute group. In the chronic lumbago group the instability of the lumbosacral joint increased, while in the acute group the compression of the weight on the sacrum increased.
